Salicylic acid (SA) is a beta hydroxy acid that is naturally derived from willow bark, sweet birch, and wintergreen leaves. SA has been observed to decrease sebum secretion, exfoliate the skin, and reduce inflammation. In lower concentrations, typically .5-10%, SA can be found in many beauty and skincare products designed to reduce acne breakouts and facial redness. Salicylic acid in concentrations of 20-30% is effective as a superficial chemical peeling agent to improve the appearance of dry, dull, and uneven complexions. SA peels are considered superficial, meaning they do not penetrate deeper levels of the skin like medium and deep chemical peels. Multiple treatments are necessary to achieve desired results and are commonly combined with other procedures such as microneedling, microdermabrasion, and facials to further rejuvenate the skin. 

The patient's skin will be analyzed to determine candidacy for the chemical peel. Once approved and any pretreatment skincare courses have been completed, the patient will return to their provider's office for treatment. The skin will be cleansed and prepped. No anesthesia or topical numbing cream is needed as the SA solution will act as its own anesthetic. The solution will be applied to the skin is several coats and left on the skin for 3-5 minutes. Some patients notice a numbing affect to the treated areas 1-3 minutes and may experience a mild stinging sensation, all of which is normal.

Unlike a TCA peel, when frosting occurs during a salicylic acid peel it indicates a crystallization of the SA, not a change in the skin proteins. If the skin begins to turn a white color during a salicylic peel, the patient then may experience more peeling and crusting post procedure. Depending on cause for treatment frosting may or may not be acceptable, which is why it is important to seek a skincare professional to perform chemical exfoliating treatments. After 3-5 minutes the applied solution will be removed with water or a gentle cleanser, no neutralizing agent is necessary. A moisturizing cream will then be applied. Most patients are then able to resume normal activity with little to no downtime.

Salicylic Acid Peel

A skin resurfacing technique that causes the skin to exfoliate and peel to enhance its appearance.

The ideal chemical is chosen for the desired depth of penetration. The skin is thoroughly cleansed prior to the administration of the solution. The chemical solution is then applied to the desired areas of treatment. The candidate is closely observed for changes to the skin. Whitening of the skin is monitored to assess epidermal damage and is a sign of epidermolysis. As the peel progresses to a frosted appearance this indicates a deeper depth of penetration into the dermis.

Once the appropriate amount of time has elapsed, the solution is then neutralized with another agent such as sodium bicarbonate. Some peels do not require neutralization, as the skin naturally neutralizes them. The skin is then cleansed. Various dressings such as silicone may be applied, ointments or emollients can be utilized after the peel has completed to form a protective barrier. This is more common for deep peels and is typically not necessary for more superficial peels. Keep in mind that the depth of peel is determined not just by the chemical composition or strength of ingredients, but also by the length of time it is left on the skin.

Chemical Peels

A chemical peel is a skin resurfacing technique to improve the appearance of the skin. A solution is applied to the skin which causes it to exfoliate and "peel off". There are many chemical peels available that range from superficial to deep. The choice depends on the desired depth of penetration, treatment effect, and recovery time. After surgery, patients are concerned with having a swift, healthy recovery and whether or not a scar may develop. Raised scars occur when an overproduction of collagen elevates the skin. Collagen is responsible for creating youthful volume, but when there is too much during the healing process, bumpy, keloid scars can form. By decreasing the amount of tension on the incision, collagen production is better controlled. 

The embrace scar therapy kit includes applicators and silicone dressings. Once staples and stitches have been removed and the incision is closed and dry, the dressing is applied directly over the incision, drawing together both sides and relieving tension. Scars come in a variety of shapes, sizes, and colors. Depending on the healing conditions, scars can become raised, depressed, or discolored. When an incision or injury heals and appears too prominently or is in a conspicuous location, a revision surgery may be needed to better conceal the scar. A Z-plasty scar revision surgery can correct and diminish the appearance of a visible, linear scar by interrupting the straight line and creating a zig-zag shape instead. The Z shape to the scar not only allows for less tension on the incisions (which creates a lesser likelihood for keloid scarring), but makes it harder for the human eye to detect. The goal of a scar revision is to also reposition the new centerline of the scar at a natural skin fold, further softening the appearance of the scar.

During a Z-plasty scar revision surgery, either general, local, or local anesthesia with sedation will be used depending on the size and location of the original scar. Once the patient is thoroughly anesthetized, an incision will be made on top of the original scar along the entire length, cutting it in half. At the ends of either side of the original scar, additional incisions will be made of the same length of the original scar, forming a Z-shape. If the scar is long, several Z-shaped cuts may be made along the length of the original scar, sometimes referred to as a W-plasty. 

Next, the triangular flaps of the Z-shaped incision will be undermined, allowing for free movement of the two flaps. Then, the two triangular flaps will be transposed, moving the original scar from the middle portion of the Z-shape to the top and bottom of the Z-shape.  The incisions will then be closed with sutures. Initial wound healing typically takes 1-2 weeks while the scar will continue to refine over a 1 year period.

Z-Plasty Scar Revision

A Z-Plasty scar revision reduces the appearance of scars by altering the linear scar appearance into a jagged shape less perceptible to the human eye.

Scars form as a result of skin injuries. They can be major accidental cuts, minor intentional surgical scars, or unwanted marks caused by severe acne. Many different types of scars exist. As such, there are a variety of ways to treat scars. Telangiectasias, more commonly referred to as spider veins, are clusters of blood vessels that are close to the surface of the skin. Although the exact cause of spider veins is unknown, it is believed that certain heredity preconditions, pregnancy, trauma, aging, sun damage, and hormonal influences can contribute to their development. Sclerotherapy is an in-office procedure that “dissolves” spider veins almost instantly. This procedure is ideal for patients who do not have bulging varicose veins. Varicose veins will require a more in-depth procedure such as ambulatory phlebotomy or endovenous laser treatment. During sclerotherapy treatment, a solution is injected into the vein causing it to collapse and eventually die. The most common agents used in sclerotherapy are hypertonic saline, sodium tetradecyl sulfate, polidocanol, and chromated glycerin. While results can be visually seen almost instantly, final results are determined after several weeks, once the veins have died.

Once it has been determined that the spider veins are not an indicator of a more serious health concern and any larger varicose veins have been successfully treated, sclerotherapy can be performed on smaller surface veins. The patient will lay comfortably on a table while the treatment area is cleansed. Next, a tiny needle will be used to inject the veins with the sclerosing agent. Injections are made every 2-3cm until the entire vein cluster has been cleared. The provider may apply manual pressure to the treatment area to spread out the sclerosing agent. When the treatment is complete, injection sites may be covered with bandages or compression stockings may be worn.

- Stop taking blood thinning medications two weeks prior to surgery. Blood thinners may include, Advil, Tylenol, Aspirin, and prescription anticoagulants. 
- Stop smoking and drinking alcohol two weeks prior to the procedure and continue cessation for two weeks post op

Before Surgery

- Incisions are made and soft tissue is retracted towards the top of the nose to expose the internal nasal structures
- Alterations to the nasal septum, nasal bone, and nasal cartilage
- Sutures are used to close the wounds

During Surgery

A septorhinoplasty is a hybrid surgical procedure combining the aesthetic benefits of rhinoplasty with functional alterations to the nasal cavity to improve airflow and breathing (septoplasty). The plastic surgeon will straighten and refine the septum to improve air flow. Moles are collections of pigmented cells within the skin. While some may consider moles a part of their signature look, such as Marilyn Monroe or Cindy Crawford, others find these growths unsightly. Superficial moles can be removed in a simple, in office procedure by a dermatologist. Larger moles that extend deeper into the dermis may require a more invasive punch removal or surgical excision. Birthmarks that are vascular such as Port Wine stains, cannot be as easily cut off and would need treatment with either laser or light therapies. 

Once a skincare professional has determined the mole to be noncancerous, the growth can be removed. Before the mole is excised the doctor will inject a numbing anesthetic beneath the mole to not only reduce the amount of discomfort experienced by the patient, but also raise the mole for easier removal. Next, the doctor will use a sharp razor to expertly remove the growth. Patients may feel a pushing sensation or pressure in the area of the mole but typically do not feel pain. After the mole is completely removed the doctor will apply an antibiotic ointment and sterile bandage.
