The Salmon Sperm Facial: What Are Polynucleotides, and Is This Trending Skincare Treatment Worth All the Hype?

A treatment with no needles, no downtime, and with sessions that take less time than a mani / pedi… count us in. EMFACE is a new, non-invasive facial rejuvenation treatment by BTL (most famous for EMSCULPT) that everyone in the skincare world has been buzzing about. The innovative device, which was FDA-approved in 2022, uses a combination of high-intensity focused electrical stimulation (HIFES) and synchronized radiofrequency (RF). The HIFES acts as a “facial workout” that tones and lifts the facial muscles while the RF heats the skin and stimulates collagen and elastin production. 

Doctors are particularly excited about this new technology because it addresses two of the most common signs of aging: facial volume loss and wrinkles. “It is exciting because it targets the facial muscles and helps tone, tighten, and build muscle for a more youthful-looking appearance,” says [David Shafer, MD](https://aedit.com/provider/david-shafer-md-153 "David Shafer, MD"), a double board-certified plastic and reconstructive surgeon and founder of Shafer Clinic in New York City.  

So, how exactly does EMFACE work and does it really live up to all of the hype? Read on. 

## What does the treatment entail?
Getting EMFACE could not be easier; the treatment takes a little over 20 minutes and virtually has no downtime. The only “downside” (and we put that in quotes because it’s really not that big of a deal) is that, in order to get optimized results, the treatment calls for four sessions that are scheduled about a week apart. 

Before getting the EMFACE treatment, your provider will take about 10 minutes to prep your skin and the equipment. First, they’ll wipe your face with an alcohol cleanser to help the sticky pads adhere properly. From there, they’ll place a grounding pad on your upper back, which attaches to the machine and is responsible for safely channeling the energy to the facial pads. As a last step, they’ll place three pads on your face: one on your forehead and one on each of your cheeks. Now, you’re ready to go. Your provider will turn the machine on and it’ll start to work its magic; for the next 20 minutes, the pads will send signals that contract the facial muscles, making you involuntarily smile and twitch. You may feel a little bit of heat from the RF and a tickling sensation from the HIFES, but most people report that it is a fairly comfortable treatment. 

While the actual EMFACE session is quite quick, it’s important to note that it can take time before you see a noticeable difference. Dr. Shafer says you can expect peak results around one to three months post-treatment. It’s also worth mentioning that you’ll likely need to go in for maintenance sessions a few times a year after those four initial treatments.

## How does EMFACE compare to other facial rejuvenation treatments?
While EMFACE uses unmatched technology, experts say its best used in combination with other rejuvenation treatments. For instance, Santa Monica based Board-Certified Dermatologist [Dr. Ava Shamban, MD](https://aedit.com/provider/ava-shamban-md-2325), views EMFACE as complementary to treatments like Botox and filler. Since Botox and other neuromodulators work by relaxing the muscles that pull down the skin, Dr. Shamban says that ideally, a patient would have a neuromodulator treatment first, followed by an Emface treatment. 

Compared to filler, Dr. Shamban says EMFACE is completely different. “Emface can give a mild lift to fat pads that overly the muscles that are lifting, but it doesn’t address lip volume or cheek volume changes. It can improve facial contour and appearance of the jawline,” she tells us.

The same goes for Ultherapy, another non-invasive rejuvenation treatment; the two procedures treat different signs of aging. According to Dr. Shafer, Ultherapy tightens tissue by targeting loose or saggy skin whereas EMFACE lifts and tones the underlying muscle through muscle stimulation. 

And, in case you were wondering, while EMFACE is a total game-changer, it’s not going to replace a facelift… sorry! With the current technology, sadly no non-invasive treatment can deliver comparable results. That being said, you can combine EMFACE with other in-office treatments to achieve results that have never been possible until now.

## Bottom line: is EMFACE worth it?
EMFACE is a great option for those who are needle-phobic, are too young for injectables, or who want efficacious but natural-looking results. Since the treatment doesn’t have any downtime, Dr. Shafer says it’s particularly attractive for his patients looking for pain-free quick solutions with subtle results. 

Aside from its price point, the biggest limitation to EMFACE is that you can’t have metal plates or implants in your mouth, since it can interfere with the energy currents. But, as always, it’s best to discuss with your board-certified dermatologist to see if EMFACE is a good fit for you. 

For those who aren’t getting the desired results from their existing skincare treatments or find that needles are a barrier, EMFACE could be a great fit for them -- if their budget allows. Depending on the provider and location, the treatment costs around $4,000 for four sessions.

In a world where anti-aging skincare trends change each season, it can be really tough to keep up with the newest ‘it’ ingredients - and even tougher to know if there’s any merit to including them in your daily routine. One of these buzz-worthy ingredients are neuropeptides: small chains of amino acids are responsible for acting as chemical messengers that can reportedly reduce signs of aging without the need for injectables. But the real question is, do they really work? We’ve spoken to dermatologists Dr. Marisa Garshick [Dr. Marisa Garshick](https://aedit.com/provider/marisa-garshick-md-313) and Dr. Amy Spizuoco [Dr. Amy Spizuoco](https://aedit.com/provider/amy-spizuoco-19341)and Dr. William A. Kennedy III  to get the inside track on neuropeptides in skincare.


### **What are neuropeptides?**

“Neuropeptides are an innovative skincare ingredient due to their unique ability to interact with the skin at a cellular level, offering a range of potential benefits” says Dr. Kennedy.  From enhancing overall skin health and functionality, to promoting collagen synthesis and enhancing the skin's barrier function, vital for retaining moisture and protecting against environmental stressors neuropeptides have a dramatic effect on promoting healthy looking skin” Dr. Kennedy explains.  “Neuropeptides are a topical treatment that can sometimes be called ‘Nature's Botox’” Dr. Spizuoco explains. “They are a topical treatment that can help reduce wrinkles, inflammation, and promote younger-looking skin.” These protein-like molecules, that can give various signals to your body. In skincare, “they provide a signal to the body that an action needs to take place. In the case of neuropeptides, the peptides block the release of chemicals that are responsible for muscle movement that leads to fine lines and wrinkles,” Dr. Garshick tells us. Indeed, research carried out by scientists at Granada University suggests that certain neuropeptides may reduce the appearance of common signs of aging by targeting specific cellular processes. 

### **Where do neuropeptides get their names from?** 

The naming of neuropeptides can be confusing, so finding them in skincare labeling certainly isn’t always straightforward! While their scientific name is generally made up of the sequence of different amino acids that it is composed of, many of the most effective peptides that have been discovered are not marketed under brand names. For example, hexapeptide-8, one of the most frequently used neuropeptides in skincare, is marketed under the brand name Argireline®.

### **Which neuropeptide skincare formulations are most effective?** 

“As with many skincare products, serums tend to be most effective as serums often have a higher concentration of the active ingredient and may offer better penetration. Neuropeptides may also be combined with certain other ingredients like hydrating ingredients such as hyaluronic acid or glycerin to help plump the skin as well as other peptides that can help to firm the skin,” Dr. Gashick told us. When you’re shopping for skincare, look for products that include one of multiple of the following neuropeptides in their ingredient list:

- **Acetyl hexapeptide-8:** marketed under the brand name Argireline® is a synthetic peptide that is believed to work by inhibiting the release of neurotransmitters called catecholamines. It is understood that by blocking catecholamines Argireline may help reduce the appearance of fine lines that are associated with frequent repetitive movements such as crows feet or frown lines.

- **Matrixyl 3000:** the peptide is understood to work by gradually slowing down the glycation process, where sugars reduce the quality of the dermis (the second layer of the three-layer system that makes up your skin). The neuropeptide has also been found to reduce muscle contractions that lead to fine lines.

- **Palmitoyl Tripeptide-1 (Pal-GHK):** aka Matrixyl 3000, is used in plumping anti-aging products, the peptide “stimulates collagen production and improves skin elasticity” [Dr. Spizuoco](https://aedit.com/provider/amy-spizuoco-19341) tells us. 

- **Palmitoyl Tetrapeptide-7 (Pal-GQPR):** “reduces inflammation and supports skin’s firmness and elasticity”, [Dr. Spizuoco](https://aedit.com/provider/amy-spizuoco-19341) explains. You can generally find it in serums alongside other active ingredients that have been curated to reduce signs of aging.

- **Pentapeptides:** these peptides actually have five amino acids, and they can help improve the appearance of skin by boosting moisture absorption. They are often associated with hydrating ingredients to help maintain the healthy, youthful glow that we all seek. 

- **SNAP-8:** this elongation of acetyl hexapeptide-8 is reportedly up to 30% more effective than its predecessor.

### **What skin concerns can neuropeptides address?**

The primary benefit of adding neuropeptides to your skincare routine is addressing signs of aging at their root cause. By sending messages to the muscles that contract and accentuate fine lines, peptides are an excellent ally in fighting wrinkles. They could be of particular interest to those who are sensitive to retinol but who still want to take steps to prevent early signs of aging. [Dr. Garshick](https://aedit.com/provider/marisa-garshick-md-313) explains: “Neuropeptides may be used as an alternative to retinol to help improve the appearance of fine lines and wrinkles in someone unable to tolerate retinol, even in their 20s and 30s.” But patients with more advanced signs of aging could still benefit from using them too, she says. “They can also be incorporated as individuals get older and lines may become deeper to help make the lines appear softer and smoother.”

### **What is the difference between neuropeptides and neuromodulators?**

If you’re looking into antiaging skincare, chances are you have probably come across both terms. While they sound similar, their mechanisms are quite different. Neuromodulators ([Botox®](https://aedit.com/procedure/botox), [Dysport®](https://aedit.com/procedure/dysportr), [Daxxify®](https://www.daxxify.com/), etc.) are injectables that work by blocking certain signals that allow muscles to contract. The mechanism is similar, though not the same, for neuropeptides. “Neuropeptides are peptides that are released as chemical messengers from nerve cells,” [Dr. Spizuoco](https://aedit.com/provider/amy-spizuoco-19341) explained. “They act either in an endocrine manner, where they reach their target cells via the bloodstream, or a paracrine manner, as co-transmitters modulating the function of neurotransmitters”, she added. Neuropeptides are incorporated into topical skincare products to have a similar result as injectables, but the results aren’t as pronounced or as long-lasting. “Neuropeptides offer a great option for someone who isn’t yet ready for injectables or prefers to avoid injectables,” says [Dr. Garshick](https://aedit.com/provider/marisa-garshick-md-313). “While they will not deliver the same results as neuromodulators like Botox, they can be a good option and can be helpful to reduce the appearance of fine lines and wrinkles. Neuropeptides also have a role to be used in conjunction with neuromodulators as a complementary treatment and may be considered a form of maintenance,” she added. Says Dr. Kennedy, “while both neuropeptides and neuromodulators play crucial roles in the nervous system, neuropeptides are more specialized in direct neural communication and specific responses, whereas neuromodulators broadly adjust neural activity and functional states.”

### **What makes the skin on your scalp different?**

Before delving into the merits of having a scalp care routine, let’s start by looking at what makes the skin on your scalp special, and what that means for how you should be taking care of it. While the scalp is an extension of the skin on our bodies, it also acts as a base for thousands of hair follicles and glands. These unique conditions mean that traditional skincare products won’t be the right fit for your scalp.”The weather, stress, age, and scalp-stripping hair products can all trigger irritation and dryness that present as tiny flakes. Hydrating shampoos and conditioners made with humectant ingredients like aloe or glycerin will help the scalp retain its much-needed moisture,” Dr. Spizuoco explains.

### **What are the benefits of caring for your scalp?** 

Caring for your scalp offers numerous benefits beyond just looking after your skin. Scalp care promotes hair growth by providing a healthy environment in which your hair follicles will thrive, while also preventing common issues like dandruff and excess oiliness. Massaging your scalp can improve circulation, reducing stress and promoting relaxation. Additionally, scalp care supports overall skin health, protecting against environmental damage and premature aging.  Just as aging skin on other parts of your body lose moisture, older scalp hair follicles also lose nutrients and moisture and this causes your hair to lose its luxurious, youthful appearance, [Dr. Kennedy](https://aedit.com/provider/william-kennedy-md-26292) tells us. 

### **How many steps should you include in a scalp skincare routine?**

Scalp care doesn’t need to be complicated, [Dr. Garshick](https://aedit.com/provider/marisa-garshick-md-313) tells us: “In general, at a minimum caring for your scalp should include proper cleansing with a shampoo to remove buildup without drying out the scalp.” But for some people, a more complex routine could be advisable: “Exfoliating can be helpful to get rid of dead skin cells and in other cases, properly nourishing the scalp and moisturizing the scalp may be needed.” Generally speaking, approaching scalp care as you would skincare is advisable, adjusting the steps you include depending on specific concerns or goals. ”Some scalp ingredients can be too harsh to be on facial skin,” [Dr. Spizuoco](https://aedit.com/provider/amy-spizuoco-19341) points out, so be sure to use specific products for your face and scalp.  [Dr. Kennedy](https://aedit.com/provider/william-kennedy-md-26292) says not to forget that scalp care is skin care; and everyones skin care should include a healthy mix of nondrying cleaning exfoliation, moisturizing, and frequent gentle massage to encourage blood flow and hair follicle nutrition 

### **Which steps should be included in a scalp care routine?**

Much like skincare, scalp care routines can be adjusted over time as the condition of your skin evolves. Dr. Spizuoco shared her ideal scalp care routine with us:

- **Start with a pre-shampoo treatment:** these oils, balms, and butters target specific scalp and hair concerns. They are typically applied 10-15 minutes before you shower to allow them time to get to work on your scalp.

- **Use an exfoliating shampoo:** this step is probably the most straightforward to add to your routine simply by switching out your regular shampoo for something kinder to your scalp. To take your scalp cleanse to the next level, look for a shampoo with skincare ingredients such as salicylic acid which helps control the production of sebum which can make your skin and hair appear oily.

- **Give your scalp a massage:** use your fingers or a scalp massaging tool to gently massage your skin. Starting at your forehead, massage in small circular motions, and gradually move toward the crown and back of your head. If your hair isn’t too knotted, you can then firmly comb your fingers along your scalp.


- **Keep your scalp soothed & hydrated:** apply a spray or leave-in scalp serum to keep the skin on your scalp calm and happy until your next hair wash.


### **What does an ideal scalp care shelf look like?**
Now that we’ve gone over the steps you might want to include, [Dr. Garshick](https://aedit.com/provider/marisa-garshick-md-313) talks us through what a great scalp care shelf would look like for most people: 

- **Pre-wash Treatment:** Biolage Scalp Sync Purifying Concentrate - intended for use before shampooing, this scalp concentrate helps to remove the buildup of excess oil and dead skin cells using glycolic acid and fermented tea extract. 

- **Shampoo:** “When it comes to shampoo the frequency may vary based on hair and scalp type as those with oily hair may prefer to wash their hair every day or every other day while those with dry scalp or hair may prefer to wash less often.” She recommends Dove Hair Therapy Dry Scalp Care Shampoo: “this sulfate-free shampoo helps to nourish the scalp and minimizes dryness. It is formulated with vitamin B3 which helps to calm the scalp leaving the hair and scalp looking healthy and refreshed.”

- **Scalp treatment:** “For scalp treatments, some are meant to be applied before shampooing, while others may be applied to a clean scalp after shampooing and then rinsed out and others left in for a period of time.” One of her recommended treatments is Briogeo Don’t Despair, Repair Conditioner - “This nourishing conditioner, while should be focused on the hair, can help to nourish, soften and smooth the hair while also minimizing the potential for breakage,” she told us. “It incorporates conditioning ingredients such as algae extract, panthenol, and rosehip oil.”

- **Leave In:** Lastly, a leave-in product:” Dove Scalp + Hair Therapy Density Boost Serum” is recommended by [Dr. Garshick](https://aedit.com/provider/marisa-garshick-md-313). She explained: “This leave-in serum has been shown to improve overall hair density with 90 days of use while also helping to moisturize the scalp and strengthen the roots. It contains niacinamide, peptides, glycerin, and zinc gluconate which helps to prevent hair shedding and strengthen the hair.”


### **How frequently should you use a skincare routine for your scalp?**

Less is often more when it comes to scalp care, beyond cleansing daily or every other day, a more involved scalp care routine should only be practiced once or twice a week, unless your dermatologist or trichologist has advised otherwise of course. [Dr. Garshick](https://aedit.com/provider/marisa-garshick-md-313) told us “In general, exfoliation or deep conditioning treatments may only be needed 1-2 times per week or even just a few times per month depending on the specific treatment that is needed. For those with a very dry scalp, it can help to do a deep conditioning treatment once per week or once every other week, while for those with oily scalp or a lot of dandruff or buildup, using an exfoliating treatment 1-2 times per week can be helpful.”

Most people already have a scalp care routine of sorts simply by cleansing their hair multiple times a week. And while, in some cases, that may be just enough, sometimes your scalp needs a little TLC too. By adding a few steps to your routine you can keep your scalp healthy, hydrated, and flake-free to help give your hair the best chance at feeling and looking great. With TikTok creators swearing by facial massage and showing off their wrinkle-free foreheads and sculpted faces to prove it, we can’t stop wondering if it’s the long-awaited answer to our prayers. To get to the bottom line, we tapped into our expert network including Santa Monica based Board-Certified Dermatologist [Dr. Ava Shamban, MD](https://aedit.com/provider/ava-shamban-md-2325) and [Trinh Georg](https://www.instagram.com/trinhgeorgg), the founder of [Gloweva](https://www.gloweva.com/), who has amassed over 700k followers on social media with her facial exercise tutorials. ### ***What is Facial Massage?***

If you thought a ‘facial massage’ was a cross between a facial and a massage, you’re not alone. While a facial massage can be relaxing and does improve the skin, its main goals are to improve face shape and combat aging. Depending on the technique, some prefer to massage with fingers while others swear by using a handheld tool, like a gua sha. In addition to the massage, a lot of people will couple the treatment with facial exercises (aka ‘facercise’) as another organic way to help lift and sculpt the face and neck.

When it comes to deciding what type of facial massage and exercises are best for you, it depends on your goals combined with a bit of trial and error. For example, Georg says you can massage along the jawline to reduce tension and define it or you can massage around the eyes, in moderation, to reduce puffiness and dark circles. 

### **Getting Started with Facial Massage**

For those looking to get started with facial massage, Trinh recommends focusing on gentle, upward strokes using your fingertips, a gua sha, or a facial roller to release muscle tension and promote lymphatic drainage. Start from the center of your face and work outward, paying extra attention to areas of concern such as the jawline, cheeks, forehead, and neck. This will help reduce facial puffiness and promote a more sculpted appearance.

“Anyone can benefit from facial massage, but it's particularly beneficial for those looking to reduce signs of aging, improve skin tone and texture, or alleviate tension in the facial muscles,” says Trinh. 

But, remember that consistency is key. “If you stop doing facial massage, you may notice a gradual decrease in the benefits you've gained. It's best to maintain a consistent routine to sustain the results you've achieved,” Trinh advises. Luckily, she says you can still get results even if you practice just a few minutes each day. Depending on the person, you can see noticeable improvements in skin tone and texture as early as two weeks.

I have been a believer and user of gua sha for some time now and find it tremendously effective for keeping my face feeling slim. While the overall presentation of my face is dependent on multiple factors; what I’m eating that week, how much I’m working out, and how many days I’m doing the facial massage, I still see it as worthwhile and an integral part of my routine. I tend to suffer from fluctuations in cheek volume, especially when I’m not feeling the healthiest and it is a major insecurity. The most important factor to preserving a youthful-looking appearance is proper sun protection and, as Dr. Shamban says, taking steps along the way to promote collagen and elastin (aka ‘prejevnation’). These are going to be the biggest differentiators in changing the trajectory of how we age!

It is hard to overstate the positive effects semaglutides will have on the cosmetic as well as medical community with drastic improvemetns in both ones cosmetic appearance as well as cardiopulmonary health. ### **What is Semaglutide?**

While frequently used for weight loss, semaglutide is actually an antidiabetic medication commonly used to treat type 2 diabetes. The medication functions as a glucagon-like peptide-1 (GLP-1) receptor agonist. GLP-1 is a naturally occurring hormone that is responsible for several digestive functions, like triggering the release of insulin and sending satiety signals to the brain. When a GLP-1 agonist medication (either a semaglutide or tirzepatide) is taken, it mimics the hormone and triggers the same effects. So, a semaglutide will act the same way as a naturally-occurring GLP-1 hormone, signaling satiety and releasing insulin to signal the body to convert glucose (sugar) into energy. Because of that, semaglutide treatments work effectively to reduce appetite and, subsequently, food intake. 

“We are seeing a number of patients who simply can't shed that extra 10 to 15 pounds despite exercise and proper diet choices. In many cases, this program allows them to jump-start their weight loss and fine-tune the results they already have achieved,” says [Gregory Buford, MD](https://aedit.com/provider/gregory-buford-md-1015), a board-certified plastic and reconstructive surgeon in Lone Tree, CO. 

### **Risk Factors of Semaglutide**

When it comes to deciding whether or not semaglutide for weight loss is right for you, you’ll want to first look at your medical history. In addition to those with a history of heart disease, [William Bruno, MD,](https://aedit.com/provider/william-bruno-md-625) a board-certified plastic surgeon in Beverly Hill, says semaglutide treatments are not for patients with a history of pancreatitis or thyroid cancer, those who are pregnant, those who have suffered from or are currently managing an eating disorder, or those with a BMI under 27. 

That being said, [Jamie Gabel, PA-C,](https://www.myadvitam.com/provider/jamie-gabel-ms-pa-c) a certified physician assistant at New York’s Advitamin at the Shafer Clinic, says that because there are so many variables, the best way to determine if you would or wouldn't be a good candidate for GLP-1’s is through a consultation. 

“At Advitam, we always perform a comprehensive consultation and dig into our patient's health history and daily routine first. Our assessment also involves blood work, body composition analysis, and physical exam and then from there, we can better determine which medications/peptides would be most advantageous. It's not a cookie-cutter approach at Advitam, it's a customized well thought-out program,” says Jamie Gabel.

### **The Big Three: Ozempic, Wegovy, and Rybelsus**

When it comes to semaglutide treatments, there are three main players on the market. The most well-known are Ozempic and Wegovy. The drug for both is the same (semaglutide), but they are prescribed with different doses and indications. While these two brands are injectables, there is also a semaglutide treatment in tablet form under the name Rybelsus. 

In addition to these three brands, a lot of practitioners will also compound semaglutide to personalize the treatment. “Compounding,” as Dr. Buford explains, effectively means that a specific drug is "compounded" or created by a specialized compounding pharmacy to match the exact formulation of the original drug. In this case, Ozempic and other GLP-1s can be compounded. When they are compounded, then they are referred to not by their trade name (e.g. Ozempic, Wegovy, or Rybelsus) but instead by their generic name (e.g. Semaglutide). From there, Dr. Buford says other additives, like vitamins and salts, can be added to create additional desired effects. 

At Dr. Buford’s practice, he offers a compounded semaglutide called Slim Shot because he finds it to be a more comprehensive approach to weight loss. “We know that our patients are going to lose weight and we also know that there will be changes to not only their overall body appearance but also more specifically to their skin, hair, nails, etc. So we discuss these anticipated changes early on and design a plan to specifically and proactively address these potentially unwanted ancillary changes so that as the weight comes off, our patients still look great. This is a big difference in our program as compared to many others that just focus on weight loss and not the patient's overall appearance,” Dr. Buford says.

### **Considerations of Semaglutide for Weight Loss**

When it comes to weighing the options (see what we did there?), there are a few things to consider. First, is budget and insurance coverage. Technically, Ozempic and [Rybelsus haven’t received FDA clearance for weight management](https://www.fda.gov/news-events/press-announcements/fda-approves-first-oral-glp-1-treatment-type-2-diabetes); the two have only been approved for people with type 2 diabetes. Wegovy, on the other hand, has been [FDA-approved](https://www.fda.gov/news-events/press-announcements/fda-approves-new-drug-treatment-chronic-weight-management-first-2014) specifically for weight management. The FDA states that Wegovy is indicated for those with a BMI of 30 or greater or a BMI of 27 or greater if there are associated health problems, like high blood pressure, type 2 diabetes, or high cholesterol.

Unless you meet the requirements set by the FDA, your insurance will most likely not cover your semaglutide prescription. When using semaglutide specifically for weight loss, a lot of programs do not go through insurance. Instead, patients will have to pay out of pocket. Dr. Buford says that since many of his patients are not trying to lose a massive amount of weight and are not diabetic, their treatment wouldn’t be covered by insurance.

Another consideration is your lifestyle and preferences for medication. Ozempic and Wegovy are at-home self-injectables that you administer once a week. Rybelsus is available as a tablet to be taken orally, but it needs to be taken daily. Each brand has a different dosage, so some might work better for others.

As with any drug, there can be side effects when taking semaglutide medication. The most common side effects are nausea, diarrhea, vomiting, constipation, abdominal pain, headache, tiredness, upset stomach, dizziness, feeling bloated, belching, gas, stomach flu, and heartburn.

### **How Long Can Semaglutide be Taken?** 

Lastly, it’s important to discuss a long-term plan for your semaglutide treatment. Currently, there are no set guidelines for how long someone should be on a semaglutide, so it really depends on the individual. Dr. Shafer, for instance, shared that he has been on it for about a year and a half and has lost about 30 pounds. And, [Wegovy’s safety and efficacy trial](https://www.fda.gov/news-events/press-announcements/fda-approves-new-drug-treatment-chronic-weight-management-first-2014) was over a 68-week period.

“From a patient perspective, I have learned new eating habits with smaller meals and avoidance of doughy foods which tend to cause some discomfort. So, no more bread, pasta, french fries, etc. Even if I miss a dose, I have learned to avoid those foods which has helped sustain my weight loss,” Dr. Shafer tells us. 

When stopping semaglutide, [research](https://pubmed.ncbi.nlm.nih.gov/35441470/) has shown that many patients gain the weight back since they often revert to their former lifestyles. So, if starting a semaglutide treatment, be aware that you’ll most likely need to have ongoing treatment for maximized results.

### **Experts Predict Semaglutide Treatments for Weight Loss are Here to Stay**

**Dr. Shafer:** Semaglutides and related treatments are definitely a game changer in the weight loss and plastic surgery world. As more understanding and protocols are developed we will see expanded uses and safety profiles. We have seen tremendous growth in our metabolic center, Advitam, at Shafer Clinic. Patients are becoming more sophisticated and understand that beauty and health start at the metabolic level so treating from the inside out is just as important as the external treatments we are performing for them. 

**Dr. Buford:** The GLP-1s are here to stay and we have only seen the tip of the iceberg in terms of their benefits. My prediction is that this sector is going to be the next BOTOX. What started as a drug for diabetics and more massive weight loss is now being used by many centers like mine for not only isolated weight loss but also before and after body contouring surgeries to effectively get at fat that surgery just can't reach. In doing so, we are seeing even better results with this combinative approach and I foresee that the entire category of GLP-1's are going to grow in popularity over time and become a more common part of our approach to total body rejuvenation. 

**Dr. Bruno:** I do believe that this class of injections for weight loss, as well as other peptides, is here to stay and more options will be available to patients over the course of the next few years. There will also be more clinical data available to support their use.

**Dr. Kennedy:** The remarkable weight loss resulting from Semaglutides use has resulted in drastic improvements in people's appearance as well as in their overall health. Exciting adoption of the drugs happens seemingly daily with most recently the Centers for Medicare approving coverage of the drug for patients with elevated cardiac risk factors. Semaglutides are the real deal and are certainly here to stay.

GLP-1’s, specifically semaglutide, have completely transformed the weight loss industry. With a daily tablet or a weekly injectable, patients have experienced life-changing results. But, there are plenty of negatives associated with being female. There’s the obvious: childbirth; the unjust: the gender pay gap; and then there’s the ugly… cellulite. And, while we hate to even mention the C word, [nearly 90%](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10324940/) of us women experience it.

Even though cellulite is omnipresent, it’s been hidden and hushed about for years. But lately, celebs like [Demi Lovato, Olivia Munn, and Ashley Graham](https://people.com/health/celebrities-who-celebrate-stretch-marks-cellulite/) are showing off their natural bodies, cellulite and all. And, brands are also joining the body positivity movement to promote inclusion and acceptance of all body types and appearances. The topic has been so heavily discussed that sometimes the media makes us feel guilty if we don’t fully embrace our natural selves. But, even if we know cellulite is ubiquitous and a normal part of being a postpubertal woman, sometimes we just can’t (or don’t want to) accept it. We’ve all heard of hormones — and, most likely with a negative connotation. But, without hormones, we’d be in a lot of trouble. They’re responsible for processing many of our body’s most essential functions, like metabolism, blood pressure, sexual function, and mood. 

When our hormones aren’t optimized, we refer to it as a ‘hormonal imbalance.’ Several factors can cause this imbalance and, unfortunately, a lot of them are out of our control. For instance, there are certain situational events (e.g. puberty, pregnancy, and menopause), health-related issues (e.g. tumors, thyroid disorders, diabetes, adrenal dysfunction, etc), and genetics that can make our hormones go haywire. 

On the other hand, there are some lifestyle changes we can make to improve hormone levels. Exercising regularly, eating healthfully, managing stress, and limiting exposure to environmental toxins (e.g. smoking, alcohol, etc) have been proven to help keep hormone levels in check. 

In some instances, it becomes nearly impossible to correct hormone imbalances naturally and medical intervention is required. For patients who find themselves in this position, doctors will turn to hormone replacement therapy (HRT) to improve their patient’s quality of life. Here’s everything you need to know: 



### **What Exactly is Hormone Replacement Therapy?**

As mentioned above, hormones are responsible for regulating everything from our mood to our physical well-being. When unbalanced, they have a number of unpleasant side effects, including sluggishness, weight gain, and sleep disruption. HRT is a treatment with the goal of restoring hormonal balance in men and women to improve the overall sense of well-being.
   
HRT is available in many formats (e.g. pills, patches, gels, creams, sprays, and intrauterine devices (IUDs) and is often prescribed by an ob-gyn or primary care physician. That being said, some board-certified plastic surgeons also offer HRT in conjunction with cosmetic procedures. 

Typically, practitioners will only offer HRT for up to five years, but [Jamie Gabel, PA-C](https://www.myadvitam.com/provider/jamie-gabel-ms-pa-c), a certified physician assistant at New York’s [Advitamin](https://www.myadvitam.com/) at the [Shafer Clinic](https://www.shaferplasticsurgery.com/), says that when prescribing hormones, you have to consider each patient's individual history to best determine when to start, finish, or continue HRT.

In addition to traditional HRT, a lot of prescribers like bioidentical hormone therapy (BHRT), which many consider to be closest to natural human hormones.  [Dr. William Kennedy, MD](https://aedit.com/provider/william-kennedy-md-26292), board-certified Head & Neck Surgeon and Facial Plastic Surgeon, and Founder of [AEDIT MedSpa™](https://aedit.com/provider/aeditmed-spa-md-34673) says the hormones used in HRT are derived from the urine of pregnant horses and other organic hormones. Bioidentical Hormones used in BHRT on the other hand are derived from plants and are chemically identical to naturally occurring hormones in the body. Being chemically identical to our own hormones gives BHRT a significant advantage over traditional HRT in that you have fewer side effects and dosing can be more precisely titrated to doses that meet the patient’s exact needs.

Hormone imbalances can cause an array of health conditions and can affect the skin, reproductive organs, metabolism, and mental health. To counteract these effects, many doctors will prescribe HRT to drastically improve well-being. 

Board-certified cosmetic dermatologist in West Hollywood, [Jason Emer, MD](https://aedit.com/provider/jason-emer-md-722), says that when women experience a hormonal imbalance, they could suffer from headaches, hair loss, excessive hair growth on the face, acne, irregular periods, vaginal dryness, and discomfort during sex. Men, on the other hand, might have difficulty maintaining muscle mass, difficulty concentrating, erectile dysfunction (ED), swollen breast tissue (gynecomastia), and decreased facial and body hair growth. To determine whether or not someone would be a good candidate for HRT, most doctors will perform and assess bloodwork. 

“You can have someone who is younger with endocrine issues that needs HRT and someone who is going through decreased values with age, stress, catastrophic life events, etc. There is no specific person that is or isn't a candidate,” says Dr. Emer.

Previously, it was believed that people above the age of 60 or those with a history of blood clots or heart disease were ineligible candidates for HRT. Dr. Emer says that if someone has low levels, improving their hormone levels may help their heart disease or cholesterol. That being said, Dr. Emer adds that if someone is older with a lot of comorbidities, it may be better to hold off on treating the hormonal imbalance until the person is in better health. 

### **Why Plastic Surgeons Are Excited About HRT**

It’s no secret that plastic surgeons are obsessed with outward beauty — it’s what makes us feel individualized and empowered, after all! But, as clichéd as it sounds, outer beauty starts from within. Get Ready to Glow: Doctors Unveil the Hottest Aesthetic Trends for 2024

As we anticipate the biggest aesthetic trends of 2024, one undeniable certainty emerges: a surge in the number of people opting for aesthetic procedures surpasses any previous year. In fact, the [International Society of Aesthetic Plastic Surgery (ISAPS)](https://www.isaps.org/) reported in their latest global survey an [11% increase](https://www.prnewswire.com/news-releases/the-latest-global-survey-from-isaps-reports-a-rise-in-aesthetic-surgery-worldwide-301910346.html) year over year, with almost 25 million procedures performed worldwide in 2023. 

So, what aesthetic trends should be on our radar this year? We reached out to the AEDIT provider network to get the insider scoop on what’s to come in 2024. Here, we’re sharing the latest innovations the experts are using along with the most common requests they’re getting.  

## **AEDIT’s Aesthetic Trend Predictions For 2024**

**Natural, Subtle Enhancements**

If you’ve spent any time on TikTok over the past few months, you’re probably familiar with the #cleangirlaesthetic — the latest beauty trend of looking effortlessly chic. It’s characterized by slicked back hair, a “makeup-free” face, and a simple yet polished outfit. The idea is to look naturally beautiful as if you put in zero effort. So, overly large breasts and overfilled faces that scream plastic surgery are not in keeping with the ‘clean girl aesthetic’ and are officially ‘out.’ 

Board certified plastic and reconstructive surgeon in New York City, [Dr. Mahmood, MD](https://aedit.com/provider/umbareen-mahmood-md-23), confirms this TikTok trend has already made its way to her office. Most of Dr. Mahmood’s patients are requesting to have smaller, more natural looking implants or solely using their own breast tissue to do a breast lift and skipping the implants all together. And, the same approach applies to the face. Dr. Mahmood says “patients want their face to be balanced and they want subtle adjustments that still look and feel like themselves.” 

**Smoother, Cellulite-Free Skin**	

Even though a natural aesthetic is ‘in,’ cellulite, which affects [80 to 90 percent](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10324940/) of women, is considered ‘out.’ With the introduction of [Aveli](https://myaveli.com/how-aveli-works/), a minimally-invasive device that reduces the appearance of cellulite, more and more people are investing in a perfectly smooth behind.

“It is unique among cellulite treatments in that it allows targeted incision and release of each individual septa causing a dimple,” says board certified Head & Neck Surgeon and fellowship trained Facial Plastic Surgeon, [Dr. William A. We’ve really seen artificial intelligence (AI) take off over the past year with lifelike TikTok filters and ChatGPT. Since COVID, [mental health awareness has become a hot topic](https://www.forbes.com/sites/forbesbusinesscouncil/2023/03/10/the-rise-of-mental-health-awareness-among-gen-z-what-this-means-for-brand-marketing/?sh=2ad2ee6a30e4), especially among Gen Z. As mental health is becoming more of a priority, it’s spilling over into all aspects of life, skincare included. According to Mintel, 70 percent of US consumers are willing to pay more for beauty products with mood-boosting qualities. 

“We know the external conditions of stressors on our skin but there is also a strong yet complex interplay between the dermis and our overall physiological state of well being,” [Dr. Ava Shamban, MD](https://aedit.com/provider/ava-shamban-md-2325), a board certified dermatologist in Santa Monica, CA shares with us. “We see hormone levels, sebum production and other functions of reactivity as endogenous variables so reducing stress, increasing quality sleep, relaxation and positively impacting our mental health can absolutely have a positive impact on our skin health, cellular aging and glow!”

What to Know Before Getting a Rhinoplasty

Finding and choosing the right provider is perhaps the most important decision you will make. But with social media feeds flooded with alluring before and after photos and new aesthetic practices seemingly popping up every day, it can be difficult to cut through the noise and find a highly qualified option.

## **Why Board Certification Matters** 
Titles like ‘cosmetic surgeon,’ ‘plastic surgeon,’ and ‘cosmetic dermatologist’ have seemingly become interchangeable as of late, but the qualifications of such doctors can vary greatly. 

Because there is very little regulation governing how physicians refer to themselves, understanding your provider's board certification(s) can ensure you are consulting with a doctor or surgeon who is formally trained in plastic and reconstructive surgery, otolaryngology, ophthalmology, or dermatology (i.e. the specialties that most relate to aesthetic medicine).

Unlike a medical license, board certification is not necessary to practice medicine in the United States, but it is an important tool in distinguishing whether or not a provider has specialized – and continues to specialize – in a particular area. Many medical specialty boards require recertification every decade or so, ensuring doctors keep on top of their skills.

## **How Doctors Become Board Certified**
A licensed doctor is not the same as a board certified one. A medical license is required for all doctors who wish to legally practice medicine in the U.S. Medical licensure is handled at the state level, and physicians must receive individual licenses in each state they wish to practice in. Board certification, meanwhile, signifies that the doctor has successfully completed a residency in their chosen specialty. 

In order to become board certified, a physician must have completed his or her undergraduate education, medical school, one-year internship, residency in their chosen specialty, and passed the residency board exam. 

## **The ‘Core Four’ Board Certifications at AEDIT**
The AEDIT Provider Network is comprised of medical doctors (MDs) and doctors of osteopathic medicine (DOs) who are board certified by specific member boards of the American Board of Medical Specialties (ABMS) and American Osteopathic Association (AOA) or a very select group of unaffiliated boards for specialties like facial plastic and reconstructive surgery and oculoplastic surgery.

Here is closer look of the specialties that make up the AEDIT Provider Network:

### **Plastic and Reconstructive Surgery**
General plastic and reconstructive surgeons complete three to five years of general surgery training followed by a two to three year cosmetic and reconstructive plastic surgery residency. Plastic and reconstructive surgeons listed with AEDIT can pursue board certification via the American Board of Plastic Surgery or American Osteopathic Board of Surgery - Plastic and Reconstructive Surgery. In the case of the former, they may be dual-certified by the American Board of Surgery. While these physicians are board certified to perform cosmetic and reconstructive surgeries from head to toe, their work may focus on the breasts and body. 

### **Facial Plastic and Reconstructive Surgery**
After completing a five-year residency in otolaryngology (a.k.a. ears, nose, and throat), facial plastic and reconstructive surgeons pursue year-long fellowships in cosmetic and reconstructive procedures specifically related to the face and neck (think: rhinoplasty, facelift, chin and jaw surgeries, etc.). 

There is no ABMS Member Board specifically for facial plastic and reconstructive surgery. Instead, facial plastic surgeons in the AEDIT Provider Network are board certified by the American Board of Otolaryngology – Head and Neck Surgery or the Otolaryngology/Facial Plastic Surgery certification within the American Osteopathic Board of Ophthalmology and Otolaryngology - Head and Neck Surgery. The American Board of Facial Plastic and Reconstructive Surgery is also an accreditation that many facial plastic surgeons seek.

### **Dermatology**
Medical dermatologists diagnose, treat, and prevent conditions affecting the skin, hair, and nails. All dermatologists affiliated with AEDIT are board certified by the American Board of Dermatology or American Osteopathic Board of Dermatology, which means hey have undergone non-surgical, cosmetic dermatology training during their residency program. Dermatologic surgeons refer to physicians who are qualified to perform surgical procedures like liposuction, fat transfer, and Mohs surgery, in addition to non-invasive treatments. They are often fellows of the American Society for Dermatologic Surgery.

### **Oculoplastic Surgery**
While a facial plastic surgeon is able to perform eyelid surgeries like blepharoplasty, oculoplastic or oculofacial surgeons are experts in procedures that reconstruct the eyelids, tear ducts, and orbital area. At AEDIT, they are board certified by the American Board of Ophthalmology or have an Ophthalmology certification within the American Osteopathic Board of Ophthalmology and Otolaryngology - Head and Neck Surgery. Many also have additional fellowship training with the American Society of Ophthalmic Plastic and Reconstructive Surgery.

## **Fellowships & Societies**
Doctors who wish to further specialize in a particular area of medicine may choose to continue their training and education via a fellowship. Fellowship programs are often highly competitive and last one to three years each. Due to the lack of ABMS Member Boards in certain subspecialties, fellowships may be used to denote expertise. An ophthalmologist who wishes to subspecialize in oculoplastic surgery, for instance, would complete a two-year fellowship through the aforementioned ASOPRS. 

Society memberships, meanwhile, offer another way that is separate from licensing, board certification, and fellowships for providers to continue their education, in addition to offering mentoring and networking opportunities. Below are some of the academies and societies related to aesthetic medicine, cosmetic surgery, and dermatology:

Breaking Down the ‘Core Four’: What Board Certification Means at AEDIT

If you spent any time on TikTok over the summer, you likely saw an endless stream of videos extolling the virtues of #TrapTox. The alliterative hashtag refers to the injection of a neurotoxin into the trapezius muscles to slim and relax the area for both aesthetic and functional benefits. 

While it seems like the ‘trap Botox’ trend came out of nowhere, it’s actually been an established – albeit under-the-radar – off-label use of botulinum toxin type-A injections for some time. So, what are the benefits of injecting Botox® and the like into the trapezius muscles and who is a candidate for treatment? Top injectors break it down.

## **Anatomy 101: The Trapezius Muscle**
Before we dive into trap Botox, let’s start with a quick anatomy lesson. The trapezius is a large, superficial back muscle that gets its name from its shape – it resembles a trapezoid. It starts at the base of the neck and extends across the shoulders and down the middle of the back. 

The main function of the trapezius (a.k.a. ‘the traps’) is stabilizing the scapula (read: shoulder blades) and controlling it during movements of the shoulders and arms (think: throwing, shrugging, raising your hand, sitting up straight, etc.). “Many of us hold tension in our shoulders,” says [Kiran Mian, DO](https://www.drkiranmian.com/), a board certified dermatologist at Hudson Dermatology & Laser Surgery in New York City. “We also hold postures in our day to day that require our shoulders to be hunched over, like working at a computer, sitting at a desk, or even on our phones.” 

The overuse of the trapezius causes hypertrophy or enlargement, Dr. Mian notes, which can lead to both functional (think: physical discomfort) and aesthetic concerns. “Most people have a bulge where the neck meets the shoulder,” says [Nigar N. Ahmedli, MD](https://www.montefiore.org/doctors/profiles/1790164341/nigar-n-ahmedli), a board certified otolaryngologist and director of facial plastic and reconstructive surgery at Montefiore Medical Center in NYC. “It can interrupt the flow of the neck, thus shortening the appearance of the neck.” This is where trap tox comes in.

## **What Is ‘Trap Tox’?**
Whether you call it ‘Trap Tox’ or ‘Barbie Botox’ (yes, the movie mania infiltrated aesthetic medicine, too), the treatment is the same. Neurotoxins like Botox®, Dysport®, Jeuveau®, and Xeomin® are injected into the trapezius muscle to address physical and/or cosmetic concerns. “Physically, Botox injections into the trapezius muscle can make the two sides more even, less bulky, and/or more proportional to the client’s overall frame,” says [Kristina Kitsos, RN](https://www.instagram.com/kristinakitsos/?hl=en), a registered aesthetic nurse in Beverly Hills. “It can also make the neck appear longer and shoulders thinner, which is why many people call it ‘Barbie Botox.’”

Much like any other botulinum toxin type-A injection, the treatment is entirely customizable. “Injections of Botox into the trapezius muscle are targeted toward specific regions within the muscle to create the desired effect – ranging from enhancing the shape of the muscle, promoting symmetry, and refining proportions or slimming the muscle,” Kitsos says. “Based on the patient’s individual goals, placement and dosage is modified.”

### **Aesthetic Benefits of Trap Tox**
Much like Botox is injected into the jaw to slim the lower face, it can be injected into the trapezius to improve the contour of upper body. “Neurotoxin injections in the trapezius can have a slimming effect on the neck,” says [Margarita Lolis, MD](https://www.schweigerderm.com/providers/margarita-lolis-m-d/), a board certified dermatologist and Mohs surgeon in New Jersey. “This is because the neurotoxin itself lowers the trapezius muscle, making the neck appear longer.” 

The injections “elongate the neck, creating a more feminine contour between the neck and shoulders,” Dr. Mian explains. The swan-like effect creates the appearance of a more delicate upper body that is likened to a Barbie doll.

### **Functional Benefits of Trap Tox**
We may associate Botox with wrinkle reduction, but BoNT-A injections are also used to treat muscle spasms associated with TMJ, lazy eye, vaginismus, hyperhidrosis (read: excessive sweating), and, yes, neck and shoulder pain. “Trap tox can relax tension or spasm in the trapezius muscle,” says[ Jennifer Levine, MD](https://www.drjenniferlevine.com/), a double board certified facial plastic and reconstructive surgeon in NYC. 

At their core, neuromodulators like Botox®, Dysport®, Jeuveau®, and Xeomin® relax muscles. In the forehead, for example, that improves the appearance of wrinkles. In the upper back, that can ease stress and discomfort. “The muscles of this region – including the paired trapezius muscles – can tighten and spasm, leading to neck and shoulder pain as well as tension headaches,” Dr. Ahmedli explains. “Injecting the trapezius muscles with neurotoxin relaxes the muscles, allowing the tightness to improve and, therefore, helping decrease pain and headaches.”

## **The Ideal Candidate for Trap Tox**
A candidate for trap tox could be someone looking to relieve tension in the neck and shoulders, someone looking to slim the area, or someone looking for a bit of both. “The ideal patient is someone with stiffness or pain in the trapezius muscles, people with overdeveloped trapezius muscles, and anyone who has neck and shoulder pain from overuse of their trapezius muscles,” Kitsos shares. “A good candidate would be anyone who would like to create a more svelte, longer-looking neck and less bulky shoulders.”

Injectors will study a prospective patient a few ways to determine candidacy. “The ideal patient for trap tox has palpable trapezius muscles – the provider can feel the muscle with their hands – with a noticeable bulge,” Dr. Ahmedli says. “The neurotoxin injection will diminish the bulge and create a sleek neckline.” Body language can also be a sign. “The ideal patient for trap tox is someone who subconsciously hunches their shoulders, causing increased tension in the muscle that builds over time as well as hypertrophy,” Dr. Mian adds. 

The treatment, however, is not for everyone. Generally speaking, neurotoxins are not recommended for those under 18, women who are pregnant or nursing, anyone with an allergy to the components of the injectables, or those with certain chronic medical conditions. When it comes to trap tox specifically, “one should avoid this treatment if they have neuromuscular conditions, nerve damage in that area, or any injury or generally weak musculature in that area,” Dr. Mian cautions. 

## **What to Expect from a Trap Tox Treatment**
If you’ve ever had neurotoxin injections on the face, Dr. Levine says you can expect pretty much the same experience when it comes to trap tox preparation, in-office experience, and recovery. “Much like facial Botox injections, trapezius muscle injections have little to no pain and take just a couple of minutes to administer,” Kitsos notes.

On the pain point, facial Botox injections rarely feel like more than a pinprick. Trap tox can have a slightly stronger – though still manageable – sensation. “A longer needle is used to get to the muscle, as the trapezius lies deeper than facial muscles,” Dr. Mian shares. “It can cause a little bit more discomfort during the procedure.” If pain is a concern, Dr. Lolis says patients may choose to numb for 15 to 30 minutes prior to treatment. 

What is different between trap tox and facial Botox is the amount of product needed. “We are talking about much higher doses of toxin for the trapezius because it is a much more dynamic and stronger muscle compared to your facial muscles,” Dr. Lolis notes. While the actual dosage will depend on the patient and product, there are some general rules of thumb. “Because the trapezius muscles are much larger than the muscles in the face, the dosage in the trapezius is often two to three times that of popular areas like crow’s feet and frown lines,” Kitsos says.

Each brand of neurotoxin requires slightly different dosing, but Dr. Ahmedli shares that if you are used to getting 20 to 35 units of Botox® in the forehead, the trapezius may need 40 to 75 “depending on the muscle size and tightness.” Dr. Levine offers a similar guesstimate. “The trapezius is a very large muscle, so it takes many units to get an effect,” she shares. “If it is 10 to 20 units for the forehead, the trapezius may need 50 or more per side.” Neurotoxins tend to be priced per unit based on the provider and location, but you can expect to pay around $1,000 for a trap tox treatment.

For the best experience, Dr. Lolis recommends avoiding exercising and lifting heavy weights on the day of the injection to ensure the traps are “as relaxed and tension free as possible.” Post-injection, avoid alcohol, strenuous activity, and massaging the treated area for the first 24 hours. “Patients mainly need to be careful not to apply any pressure on the shoulders/trapezius muscles for a few hours after injections,” Dr. Lolis adds. 

## **When to Expect Results from Trap Tox**
If you’ve ever had a neurotoxin treatment, you know the results are not instant. In the case of trap tox, the results are often staggered. “Patients will start to notice a difference a few days after injection as the muscle begins to feel less tight,” Dr. Ahmedli says. “Maximal effect is seen at the two-week mark.”

Depending on the product used and the patient’s lifestyle and metabolism, the effect can last anywhere from two to six months. In some cases, more than one treatment may be needed to enjoy the full aesthetic and functional benefits. “Some patients will see results after the first treatment, while others may take more sessions,” Dr. Mian notes. 

While “the physical relief of tension will be felt after the first treatment,” she says, the aesthetic results “of elongated neck-shoulder curvature” may take a second treatment because “alleviated hypertrophy of the muscle can take some time.” In other words, “the muscle needs time to shrink now that it’s not being recruited constantly,” Dr. Mian adds.

### **A Note About Muscle Movement**
Neuromodulators temporarily paralyze the muscles they are injected into until the toxin wears off, so are you basically opting for partial neck paralysis when you get trap tox? Not exactly. “Ideally you want to obtain the effects of tension release and aesthetic results without affecting functional movement,” Dr. Mian shares. “The ideal dosing lies in that balance between relaxing the muscle enough to achieve those results, while still being able to have movement like shrugging your shoulders.”

That’s not to say you won’t notice a difference. “At first, the muscle may be a bit weaker than normal,” Kitsos says. “After the neurotoxin wears off, patients will find that their trapezius muscles will go back to normal movement and they will once again be able to easily shrug their shoulders.” In Dr. Ahmedli’s experience, it’s mostly about tension. “The main difference patients notice is a decrease in the tightness in the shoulders,” she says. “As the neurotoxin wears off, the tightness will return.”

These are all things to talk to your injector about prior to treatment. “There is definitely a potential for muscle weakness in the treated area, which can affect muscle function,” Dr. Lolis shares. “You can potentially lose muscle since it is not being worked, and the other potential downside is that the two sides might be asymmetric.”

## **The Takeaway**
Yes, trap tox can give you a longer neck and more contoured upper body. But it is really a game changer for those who experience chronic neck and shoulder pain as a result of stress or tension.
