Deep chemical peels penetrate the skin to reach the reticular dermis. Skin is comprised of 3 main layers: epidermis, dermis, and hypodermis. The dermis is divided into two additional depths, the papillary and reticular. The reticular layer contains hair roots, sebaceous glands, sweat glands, and blood vessels as well as collagen and elastin. By reaching the reticular layer, deep chemical peels have dramatic effects on more severe wrinkles and skin discolorations however the recovery time is upwards of 3 months. Due to its aggressive resurfacing, your provider may advise sedation during treatment.

The most common ingredient used in a deep peel is phenol with full strength phenol reaching 88%. The phenol will be diluted with other ingredients to customize the solution however as the concentration decreases, the depth of the peel will actually increase. Another popular ingredient used in deep peels is croton oil which is commonly mixed with phenol. 

Like medium peels, deep chemical peels will begin with the skin being pretreated and cleansed. It is common for a deep chemical peel to be preceded by a superficial chemical peel to prep the skin and allow for deeper, more even penetration. Once the skin is thoroughly prepped and cleansed, the solution will be methodically brushed onto the skin and observed for rate of change. The solution will begin to turn white as it chemically exfoliates away skin impurities. When treatment is complete, the solution will be neutralized and removed from the skin. Following treatment a soothing balm or ointment will be applied and the face may be wrapped in a garment.

Deep Chemical Peels

A skin resurfacing technique that causes the skin to exfoliate and peel to enhance its appearance.

The ideal chemical is chosen for the desired depth of penetration. The skin is thoroughly cleansed prior to the administration of the solution. The chemical solution is then applied to the desired areas of treatment. The candidate is closely observed for changes to the skin. Whitening of the skin is monitored to assess epidermal damage and is a sign of epidermolysis. As the peel progresses to a frosted appearance this indicates a deeper depth of penetration into the dermis.

Once the appropriate amount of time has elapsed, the solution is then neutralized with another agent such as sodium bicarbonate. Some peels do not require neutralization, as the skin naturally neutralizes them. The skin is then cleansed. Various dressings such as silicone may be applied, ointments or emollients can be utilized after the peel has completed to form a protective barrier. This is more common for deep peels and is typically not necessary for more superficial peels. Keep in mind that the depth of peel is determined not just by the chemical composition or strength of ingredients, but also by the length of time it is left on the skin.

Chemical Peels

A chemical peel is a skin resurfacing technique to improve the appearance of the skin. A solution is applied to the skin which causes it to exfoliate and "peel off". There are many chemical peels available that range from superficial to deep. The choice depends on the desired depth of penetration, treatment effect, and recovery time. Typically, the deeper the peel, the greater the results, but they are also associated with greater recovery period and adverse effects.

However, Smile Direct Club ships patients every tray to be used throughout treatment at once, eliminating the need to visit the orthodontist every few weeks for replacements. Some orthodontists advise patients to be wary of the Smile Direct program, as it is important to have a licensed and experienced dental professional overseeing any orthodontic treatment.

In order to create the series of custom trays, patients will first need to send an impression of their teeth to the Smile Direct Club company. Impressions can be done using an at-home kit that is then mailed to Smile Direct Club, or can go to a Smile Direct Club SmileShop for an in-person digital scan of the teeth. Impressions will be reviewed by a dentist or orthodontist to determine candidacy for the clear aligner program. While the custom trays are manufactured, approved patients will be sent a 3D model of what their results will look like when treatment is complete. Patients will receive the entire treatment series by mail, and will replace each tray every 3-4 weeks to progressively straighten their smile. Once the program is complete, patients are advised to visit an orthodontist to review their treatment. Patients should then wear a retainer nightly to ensure that the newly aligned teeth do not migrate to their former positions.

The process for placing dental veneers begins with the dentist taking X-rays and photos of your teeth. These will be used in the dental lab to manufacture customized veneers unique to each patient. Numbing agents are used for keeping patients pain free. Some gum swelling and discomfort may be experienced for a few days after placement. The average cost for a single dental veneer is $1,000-$3,500. With proper oral care, a veneer will typically last 7-12 years.

Lingual Braces

Traditional metal braces are more effective for patients that have overcrowded mouths than some “invisible” devices like clear aligners The main downside of the metal braces is that they are highly visible to other people. Lingual braces placed on the backsides of the teeth next to the tongue produce similar corrective results and are much less obvious to others.
