The epidermis at these sites is coagulated and destroyed, but the areas between these MTZs remains intact, leading to much faster healing. Each MTZ is one-tenth the size of a hair follicle. The Fraxis Duo machine is equipped with two separate hand pieces: one for microneedling, one for laser resurfacing. First, the skin will be cleansed and anesthetized with a topical numbing cream for up to an hour. Some patients may also receive local anesthetic shots or an oral sedative for increased comfort. When the patient is thoroughly anesthetized, the topical cream will be removed and the microneedling treatment will begin. The hand piece will be guided across the face, creating controlled micro-injuries within the skin while simultaneously delivering RF energy. 

Once the microneedling treatment is complete, the laser resurfacing will begin. Atop the skin treated with microneedling, a laser hand piece will deliver targeted CO2 laser energy, creating additional, although not as deep, micro-injuries. SmartSkin®

SmartSkin CO2 skin resurfacing is a type of ablative fractional laser which diminishes fine lines, wrinkles, acne scarring, hyperpigmentation, and other sign of photo-damage. Depending on the level of intensity of treatment, patients can see dramatic improvement in as little as 1-3 sessions.

An alternative to dermabrasion or chemical peels which uses an aggressive CO2 laser to treat scars, wrinkles, and more.

Similar to CO2 laser, the erbium:yttrium-aluminum-garnet laser targets water in the skin tissue leading to controlled vaporization of superficial cells and thermal injury in the dermis that results in collagen remodeling and healing. Also known as Er:YAG, or Erbium laser for short, this technique is performed in a similar manner to other laser treatments. The wavelength of the laser is 2,940nm and is absorbed much more efficiently by water in tissues than the CO2 laser. This results in less associated thermal damage to surrounding tissues to achieve tissue ablation, which then results in faster re-epithelialization and faster recovery times. These properties also made the hemostasis achieved with this laser less than with CO2 laser, and newer techniques have modulated the properties of the Erbium laser, or combined its use with CO2 laser, to improved hemostasis while maintaining the benefits associated with the Erbium laser.

The depth of penetration is to the reticular dermis at most. The Erbium laser is a more superficial ablation compared to traditional CO2 laser. Also, the changes in skin color utilized during CO2 laser ablation that correspond with tissue depth are not present with the Erbium laser due to the fact that it does not cause the same coagulative effect. Instead, pinpoint bleeding it utilized to monitor for proper depth of treatment.

Ablative Erbium Laser

There are 4 categories of lasers: ablative or non-ablative, and fractional or non-fractional. Ablative lasers are wounding lasers that create tissue damage to encourage collagen generation for firmer, plumper skin. Non-ablative lasers create heat beneath the skin to gradually encourage skin regeneration. Fractional lasers direct laser energy into precise columns for targeted treatment as opposed to non-fractional lasers which treat an overall area. 

The Pixel CO2 laser is an ablative, fractional laser. Unlike other lasers within the same category, the Pixel CO2 laser does not require patients to undergo a lengthy recovery period. Most ablative skin resurfacing treatments require up to 3 weeks of recovery time, whereas with a Pixel treatment, patients can return to most normal activity within 1 week. While many patients will see improvement in their skin concerns after 1 treatment, a series of up to 4 treatments may be needed for optimal results. 

Before treatment the skin will cleansed and anesthetized. Only a topical numbing cream is needed although a local anesthetic may be used. Once the patient is thoroughly numbed, the cream will be removed and the treatment will begin. Depending on the patient's concern, either a 7x7 or 9x9 grid pattern will be used to deliver the laser energy to an 11x11mm patch of skin. The 7x7 grid is better suited for more aggressive treatment of scarring whereas the 9x9 grid is indicated for skin peeling and tightening. Additional hand pieces will be used as indicated. The laser hand piece may be pulsed several times in the same area for deeper heat penetration. Typically, full face treatment can be completed in less than 15 minutes. Patients will likely experience a sunburned sensation as well as redness in the treated area. Results can be seen after 1 treatment but may require an additional 1-3 sessions.

Pixel CO2

The Pixel CO2 laser is an ablative, fractional skin resurfacing device that improves the appearance of scars, skin discolorations, fine lines, and other imperfections. This system comes with a variety of hand pieces to customize treatment for a variety of patient concerns.

The basic idea behind any ablative laser skin procedure is the vaporization of more superficial layers of the skin, with some minor injury to the deeper layers which stimulates the bodyÕs natural healing properties to induce collagen formation to lead to clinical results.

A carbon dioxide (CO2) laser with a wavelength of 10,600 nm is utilized for laser skin resurfacing. This particular wavelength is well absorbed by water. The skin contains water which is targeted and leads to a precise ablation with consistent results. Hemostasis is excellent with this technique. 

The depth of ablation is assess by characteristic color change. The maximal depth is the reticular dermis, and the superficial aspect of this layer is reached when a gray color. Less aggressive ablation may be performed and as the skin turns pink this is confirmation that the papillary dermis has been reached. Given the precision associated with lasers, particular scars, deep lines, or lesions may be specifically targeted for more extensive treatment.

CO2 lasers can cause debris or char as the skin is ablated, and this must be wiped away to proceed. Take care to blend at the periphery of the treated region to prevent any sharp demarcation lines. Often CO2 laser is performed on the full face to avoid this issue, but care still must be taken at the margins of treatment at 
the hairline or the neck.

Always take laser precautions including eye protection when using the laser. Remember that the CO2 laser itself is not in the visible spectrum, a red aiming beam may be utilized to focus on target tissue, but this is not the CO2 laser itself.

Prior to treatment, the skin will be evaluated, cleansed, and a topical numbing cream will be applied. When the patient is thoroughly numbed, the cream will be removed and the laser resurfacing treatment will begin. Additional local anesthetic shots may be administered for optimal patient comfort. The CO2RE settings will be customized to meet the patient's individual needs, adjusting laser strength, pulse duration and penetration depth, density of treatment, and treatment pattern. The provider will closely monitor the skin to ensure an effective but comfortable session. Once the skin has been thoroughly treated, a soothing serum or ointment will be applied and a bandage may be applied to the skin. The treated area will be red and slightly swollen. Cool compresses and OTC pain medications may be used to alleviate any discomfort.

CO2RE®

CO2RE is a type of ablative fractional laser resurfacing treatment. This procedure helps to reduce wrinkles, smooth uneven skin texture, and improve overall skin tone in as little as one treatment session.
