Gynecomastia occurs when an imbalance of estrogen and testosterone cause breast tissue to enlarge in men. This condition can affect men at many different times in life, especially during times that cause hormonal shifts. Gynecomastia can be observed in male babies caused by their mother’s estrogen (which fades within several months), during puberty, and again later in life between the ages of 50-69. Certain medications and drugs that have antiandrogen effects can also cause gynecomastia, with one of the most well known being anabolic steroids. Symptoms of gynecomastia typically resolve without intervention within a few months to 2 years, however if the condition does not improve, causes pain, or greatly effects self esteem, treatment may be necessary.

For patients with true gynecomastia, glandular excision is required to reduce the size of the breast. An incision is made at the bottom border of the nipple in the shape of a crescent. The tissue will be excised and the incision closed. Depending on the severity of the gynecomastia, the nipple may be removed entirely for reposition if the patient has severely sagging skin. Incision patterns vary depending on the specific conditions and surgical preference. 

When male patients have enlarged breasts caused by fatty deposits and not breast tissue, the condition is referred to as pseudogynecomastia. A liposuction procedure can be performed to reduce the size of the breasts. A small incision will be made either at the border of the nipple or within the underarm crease. A cannula will be inserted and moved back and forth to dislodge and suction fat deposits. For some individuals liposuction and tissue removal is necessary to address enlarged male breasts or puffy nipples.

Medium depth chemical peels penetrate through the superficial epidermis of skin and reach the papillary dermis, the uppermost layer of the dermis. The most common ingredient used in medium peel is trichloroacetic acid (TCA) which can be applied in concentrations of up to 20-35%. TCA was previously used in higher concentrations but led to an unacceptable rate of scarring in patients. Now, the efficacy of a medium depth peel can be improved by combining or pretreating with other chemicals, such as a superficial peel.

Before treatment, the skin will be cleansed then prepped with a protective oil or serum around sensitive areas such as the lips or around the eyes. The customized solution will then be brushed onto the skin while a trained, skincare professional observes the chemical changes that are exfoliating the treatment areas. Once the solution has thoroughly saturated the skin, typically around 15 minutes but will be dependent on your individual needs, the solution will be neutralized to stop the chemical exfoliation process and removed. A soothing lotion or serum may be applied after treatment. Medium depth chemical peels can cause discomfort during the actual treatment itself akin to a burning sensation. It is important to choose a trained, skincare professional to administer your chemical peel to avoid adverse side effects.

The ideal chemical is chosen for the desired depth of penetration. The skin is thoroughly cleansed prior to the administration of the solution. The chemical solution is then applied to the desired areas of treatment. The candidate is closely observed for changes to the skin. Whitening of the skin is monitored to assess epidermal damage and is a sign of epidermolysis. As the peel progresses to a frosted appearance this indicates a deeper depth of penetration into the dermis.

Once the appropriate amount of time has elapsed, the solution is then neutralized with another agent such as sodium bicarbonate. Some peels do not require neutralization, as the skin naturally neutralizes them. The skin is then cleansed. Various dressings such as silicone may be applied, ointments or emollients can be utilized after the peel has completed to form a protective barrier. This is more common for deep peels and is typically not necessary for more superficial peels. Keep in mind that the depth of peel is determined not just by the chemical composition or strength of ingredients, but also by the length of time it is left on the skin.

Medium Depth Chemical Peel

The Jessner solution has been used for over 100 years as a chemically exfoliating treatment to improve acne as well as pigmentation irregularities such as hyperpigmentation, photodamage, and melasma. Jessner's solution is unique in that it combines multiple chemical exfoliators in one peel. The traditional Jessner peel combines salicylic acid, lactic acid, and resorcinol. The salicylic acid is a BHA used for its exfoliating properties, lactic acid for its moisturizing effect, and resorcinol as an antiseptic. Resorcinol has since been replaced by other AHAs such as glycolic, mandelic, azelaic, and phytic acid due to health concerns related to its toxicity and effect on the nervous system. When resorcinol is not used in the the Jessner solution, it is considered a modified Jessner peel. 

First, the skin will be evaluated. Once it is determined the patient is a good candidate for the peel and all pretreatment skincare has been completed, the peel can be performed. The skin will be cleansed to remove any dirt or oils. Next, the Jessner solution will be brushed onto the skin. Typically, 2 coats will be applied for a superficial peel however, additional layers may be applied for a deeper peel. 

No neutralizing agent needs to be applied after a Jessner peel. After application, a moisturizing cream or lotion will be applied to the skin. Patients are able to resume normal activity after the peel is applied and should wash the treatment area 4-5 hours post-procedure. Depending on the AHAs used in the peel and the concentration, some patients may be asked to leave the solution on for up to 2 days. Be sure to follow the specific aftercare instructions given by your provider.

Jessner’s Peel

Jessner's chemical peel solution is comprised of salicylic acid, lactic acid, and resorcinol to treat fine lines and wrinkles, pigmentation irregularities, and acne scarring. The modified Jessner's peel replaces resorcinol with TCA, glycolic acid, and other AHAs for an effective, medium depth exfoliation.

Phosphatidylcholine is FDA approved to break down fat found in blood that may interfere with heart health, but is not approved for any other use. Other common additives to a mesotherapy injection include vasodilators (to improve blood flow to the treatment area), anti-inflammatory medicines (to reduce any swelling), antibiotics, nutrients, and enzymes. 

Although mesotherapy has been practiced for decades in Europe, there have been no thorough scientific studies in regards to the effectiveness of the treatment. The American Society for Plastic Surgeons warns that patients should be wary of the treatment until further data is collected about the safety and efficacy of the procedure. Studies are being conducted to better understand and regulate the treatment. 

Before injection, the practitioner will evaluate the patient in order to create a customized cocktail for injection which can include a number of ingredients. The treatment area will be cleansed and the injection will be placed in the subcutaneous fat layer. Several session are needed depending on the severity of the issue being treated. There are a variety of minimally invasive treatments used to improve the appearance of the skin from facials to microdermabrasion, chemical peels to laser resurfacing. Radiofrequency energy has been used for decades to rejuvenate and tighten the skin. Heat created by radiofrequency waves causes the body to produce collagen and elastin, the main supportive proteins in the skin. Microneedling elicits a similar response from the body by creating tiny injuries within a treatment area that activate the wound healing cascade. This process also encourages collagen and elastin production. The Infini RF device combines these two processes for an effective skin rejuvenating treatment that smooths fine lines and produces younger looking skin over a series of treatments. 

Prior to treatment the skin will be cleansed and a topical anesthetic will be applied to numb the skin. Once the skin is anesthetized, the Infini RF device will be gently pressed against the skin, creating 40 tiny micro-injuries. The microneedles will simultaneously deliver radiofrequency energy into the deeper layers of the skin. The microneedling device will be stamped across the treatment area, customizing the depth of treatment for individualized results. Multiple treatments are typically necessary for desired results.

Infini™ RF

The Infini RF is a microneedling device that also utilizes radiofrequency energy to treat fine lines and wrinkles. After a series of treatments, skin will appear smoother, firmer, and more rejuvenated.

On the day of treatment, the skin will be cleansed and a topical numbing agent will be applied to the treatment area. While the skin is being numbed, a small amount of blood will be drawn and processed to isolate the PRP, or platelet rich plasma. Once the treatment area is thoroughly number, the microneedling procedure is performed. The microneedling device will create tiny holes in the skin as it is gently passed over the skin. After the microneedling is complete, the extracted PRP will be immediately applied to the treated skin. Next, the treated skin will be enriched with the application of stem cell growth factor serum. Both the PRP and growth factor serum are clear on the skin and should be allowed to stay on the skin for the remainder of the day. 

This variation of microneedling takes advantage of the microchannels created by the microneedling device. Without the addition of the stem cell growth factor serum, this procedure would be known as a vampire facial. Other popular combinations of microneedling include the addition of radiofrequency energy to further enhance the collagen-inducing properties of the micro-injuries created during treatment.

Microneedling with PRP and Stem Cells

After cleansing the skin surface and applying a topical anesthetic, the microneedling device is gently pressed against the treatment area to create numerous, tiny, micro-channels. Microneedling with radiofrequency uses the added technology of flow needles to deliver RF energy into the dermis. The RF energy heats the dermis which not only encourages collagen growth but tissue tightening as well. 

The penetration of the microneedles into the skin leads to a release of growth factors and triggers the wound healing cascade, encouraging collagen production which leads to younger looking skin. The needles also help to mechanically break down the scar tissue. Since the epidermis is not destroyed, as is the case with all microneedling treatments, recovery time is much shorter when compared to more aggressive laser resurfacing treatments or deep chemical peels.

Trifecta Therapy is a combination of treatments each designed to rejuvenate the skin, but done together to enhance results. Microneedling, platelet rich plasma, and stem cell serum are used during the procedure to smooth fine lines and plump the skin. The microneedling procedure is completed first to create hundreds of tiny microchannels within the treatment area. The platelet rich plasma, or PRP, and growth factor serum are applied immediately after to deeply penetrate the tissue for optimal results. 

Prior to treatment the skin will be cleansed to removed any makeup, oils, or lotions. A topical numbing cream will then be applied for optimal patient comfort. While the skin is being numbed, a small amount of blood will be collected from the patient and placed in a centrifuge to extract the PRP. Once the skin is completely numbed, the microneedling procedure will begin. Immediately following the microneedling, the PRP will be applied to the skin as well as the stem cell serum. The PRP and stem cell serum, which will appear clear on the skin, should be left on for the remainder of the day. Normal skin care routines can be resumed the day after treatment.

The process for microneedling with PRP begins with the collection of blood to make the PRP. First, blood will be drawn from a vein in the arm, typically 5-15mL. The blood sample is then placed in a centrifuge which isolates the platelets, forming the platelet-rich plasma (PRP) concentration. This step in the process typically takes 15 minutes.

While the centrifuge is distilling the PRP, the patient’s skin will be cleansed in preparation for the treatment. A light numbing agent will be applied to the treatment area to help prevent discomfort during the procedure. The practitioner will gently glide the microneedling device over the area to be treated, creating tiny holes in the skin penetrating approximately .5 to 2mm in depth. The microneedling device is a hand held machine that will have between 12-80 sterile, stainless needles that penetrate the skin. The depth and number of needles on a device will depend on the brand used by the practitioner.

Once the area has been thoroughly treated, the practitioner will then apply the now developed PRP concentration. Skin will begin to feel taut shortly after the session. Following treatment it is advised to not wash off any residual residue so as to allow the PRP to continue to penetrate the skin. Skin will progressively improve in the weeks to come in tone and texture.
