Radiesse® by Merz Pharmaceuticals is a specially formulated injectable dermal filler used to smooth fine lines and wrinkles and also improve volume loss in the backs of the hands. Radiesse® is unique in that its primary ingredient is calcium hydroxylapatite (CAHA), which is formulated as calcium microspheres. 

CAHA is believed to both enhance skin volume and plumpness and stimulate the body’s production of natural collagen to provide natural looking, lasting results. Radiesse® is approved by the United States Food and Drug Administration (FDA) to treat moderate to severe facial wrinkles, however, it is also used off-label for volume loss in the backs of the hands. Cellulite

Cellulite affects mostly women and usually appears on the thighs, hips, and buttocks area as dimpled skin. The cottage-cheese like appearance of cellulite is caused by a number of factors: fat, skin thickness, and fibrous tissues. Connective tissues beneath the skin, called septa, pull down the skin creating dimples. These dimples are further accentuated by fat deposits and thin skin. Radiofrequency treatments can help to diminish the appearance of cellulite by targeting fat and encouraging collagen production. There are a variety of non invasive RF based cellulite products that delivery energy to the skin through a number of different hand pieces, however, their goal is all the same: to decrease cellulite. When delivered deep into the skin, RF energy helps to destroy fat cells which are then eliminated from the body naturally through the lymphatic system. Also, the heat created by RF energy encourages the body to go into a healing mode which allows for collage and elastin production, firming and volumizing the skin. 

Before a radiofrequency cellulite treatment begins, a topical numbing cream may be applied depending on the pain tolerance of the patient. A grid may be drawn on the skin as well to assist the practitioner during a session for even and targeted treatment. Next, a gel, cream, or oil will be applied to the treatment area to lubricate the skin and create an additional barrier between the skin and the hand piece. The hand piece will then be applied to the skin, working across the treatment area. When the procedure is complete, a soothing gel, lotion, or cool compress will be applied to the skin. 

A series of sessions are necessary to achieve desired results, but depending on the device used, it may require twice weekly sessions for 4-5 weeks or 1-2 weeks with an average of 8 treatments. Ongoing monthly maintenance treatments are typically necessary. Other brand name products that treat cellulite non invasively via RF energy include EndyMed 3Deep, truSculpt 3D, Vanquish, Venus Freeze, Venus Legacy, Exilis, and JuVaShape. VelaShape is another non-invasive treatment which combines RF energy, infrared light, as well as vacuum and massage to target stubborn cellulite.

RF Cellulite Treatment

Veins

Varicose veins are dilated and rope-like veins that typically appear on the lower extremities. When the valves inside of veins are unable to efficiently transport blood, the blood begins to pool and bulge. Any vein can become varicose, although the legs are more commonly affected since blood flow is constantly working against gravity to recirculate blood. Varicose veins are common and are usually just cosmetic, although sometimes they can be an indicator of a more serious health issue. Once it is determined that there is no underlying health concern, varicose veins can be easily treated with a simple, in office procedure such as radiofrequency occlusion. 

Radiofrequency occlusion is a minimally invasive treatment that helps to reduce and eliminate the appearance of unsightly varicose veins. This treatment uses RF energy to collapse and harden vein walls, causing the vein to close and blood to no longer flow through that vein. With no blood flowing through the vein, the bulging effect disappears. RF occlusion, unlike ambulatory phlebotomy and ligation and stripping methods, does not actually remove the vein from the body. However, RF occlusion can be performed prior to a vein removal surgery.

Ultrasound technology will be used to determine the location of the varicose vein and will help guide the surgeon for accurate placement of the initial needle insertion. The insertion point will be anesthetized with a local numbing agent for patient comfort and a series of guide wires and dilators will be inserted into the affected vein. The guide wires and dilators will then be removed so the radiofrequency ablation catheter fiber can be inserted. Once the fiber is in position, a tumescent fluid will be injected around the compartment that surrounds the vein. This not only provides patient comfort, but helps to protect surrounding tissue from the RF heat and improves contact between the vein wall and the catheter during treatment. 

Treatment then begins by ablating 3-7cm sections for 20-40 second intervals with RF energy as the fiber is drawn out of the leg. Once treatment is complete, the leg will be wrapped in a compressive dressing to help prevent swelling. Initial results can be seen after 1 week however final results are determined after 3-4 weeks.

Other forms of liposuction that only focus on removing fatty deposits are not able to also deliver firmer skin. Younger patients with good skin quality will notice some improvements in elasticity from traditional liposuction techniques, but degree of betterment can be unpredictable.

The Bodytite system constantly measures the temperature of the epidermis to ensure even heating and avoid injuring the patient. A rejuvenation rhinoplasty, while typically minimal in scope, provides patients maximum return for a youthful appearance. As we age, the loss of collagen and elastin as well as the forces of gravity cause the skin to droop. This droopiness, alongside the loss of volume in the lips and cheeks, can make the nose appear longer or larger. A rejuvenation rhinoplasty will commonly only address superficial aspects of the nose such as the tip or excess skin, while leaving inherent structures such as the septum intact. 

A rejuvenation rhinoplasty, much like a primary rhinoplasty, can be performed with either an open or closed approach. If an open approach is used, an incision will be made at the columella, just beneath the tip of the nose. The nose will then be retracted backwards, dissecting the nose cartilage with care being taken to not incise the medial crura, the cartilage that separates the nostrils. The flap will be raised off of the nose for maximum exposure and work can now be done to reshape the nasal tip, bridge, nostrils, or septum repair. Once completed, the flap will be redraped over the bone framework and external contouring and shaping is completed. The incision will be closed with sutures and an external splint or cast will be placed over the nose.

If a closed approach is used, incisions will be made on the inside of the nose. The nasal structure will not be exposed which means decreased visualization for the surgeon, however postoperative scarring will be hidden and discreet. The addition of an endoscope may be used during a closed rhinoplasty to increase visualization. When repair has been completed, sutures will close incisions and an external splint or cast will be placed over the nose.
