Adipose-derived stem cell-conditioned medium (ADSC-CM) is rich in growth factors such as vascular endothelial growth factor, hepatocyte growth factor, platelet-derived growth factor, and insulin-like growth factor. Vascular endothelial growth factor controls hair growth and follicle size through angiogenesis. Hepatocyte growth factor is involved in the cyclic growth of hair follicles. A direct neck lift is essentially the combination of a cervicoplasty, which removes excess skin, with a platysmaplasty, which tightens the underlying muscles in the neck. A facelift will help tighten the lower half of the face but a neck lift will further define the facial contour and profile by creating an aesthetically pleasing angle from the chin to the chest.

During a direct neck lift, an incision will be placed underneath the chin where the resulting scar can be easily hidden. The skin of the neck will then be elevated to expose the underlying fat. The fat will either be removed by direct excision or with liposuction. Once the fat has been removed the underlying platysma muscles will be tightened and bound with sutures, creating structural support and the new neck angle. Any excess skin will then be removed, cutting away the appearance of a double chin or turkey wattle. The incision will then be closed with sutures. A compression garment must then be worn for several weeks after the surgery to assist in healing.

The PrecisionTX probe will be inserted just beneath the skin and inserted back and forth in a fanning motion. The first pass of the PrecisionTX laser is meant to melt unwanted fatty tissue which will then be removed with an aspirating cannula. The patient will hear loud popping noises during the procedure which indicates rupturing fat cells. 

Once the fat is melted and removed via a short liposuction procedure, the PrecisionTX laser will be reintroduced beneath the skin to tighten the tissue. The side-firing laser is directed in a more upwards position to ensure a targeted treatment. The procedure is typically completed in less than 1 hour, requiring no stitches. Patients will need to wear a compression garment around the face and neck for at least 1-2 weeks. Improvement is noticeable almost immediately after surgery.

A cervicoplasty is a common procedure performed to give patients a more defined profile in the neck area. This lidocaine in the solution numbs the area while the addition of epinephrine allows for larger doses of numbing to be administered while simultaneously elongating the effects of the anesthesia and reducing bleeding. A z-plasty cut is made on the neck, which will look like a "z" but will be closed as a seamless straight line. Excess skin and fat will be removed. No muscle tightening will be performed.

Nipple inversion severity is evaluated on a scale of 1 to 3. The level of inversion will be assessed by your physician, during an initial consultation, before deciding on the best course of action.

Level 1: sometimes referred to as ‘shy nipple,’ Level 1 is the most minor level of inversion. It is assigned when the nipple responds to stimulation — it may protrude temporarily before returning to an inverted position.

Level 2: the nipple remains inverted at all times, but can be pulled out without too much resistance.

Level 3: this is the most severe level of nipple inversion. The nipple will remain inverted despite attempts to stimulate it. This is usually a result of scarring or tissue constriction.

Patients who are having inverted nipple surgery in order to breastfeed can have a procedure designed to partially preserves the milk ducts. Inverted nipple repair with partial preservation of milk ducts usually involves a local anesthetic and a sedative before surgery. An incision is typically placed around the areola (the dark-skinned area surrounding the nipple), followed by the nipple and areola tissue being lifted from the breast and stitched into a new shape. The lifted nipple and tissue is not disconnected from the breast with this technique.

Inverted nipple surgery with milk duct detachment is typically reserved for patients who are not planning to breastfeed. The procedure can be performed using a local anesthetic in combination with a sedative. An incision is made at the base of the nipple, detach the milk ducts, and reshape the nipple for the desired results.

Both inverted nipple surgery techniques generally take only about an hour to complete on an outpatient basis.

Inverted Nipple Repair

An inverted nipple repair surgery corrects the typically benign condition of the nipples being pulled in or flattened on the breast.

Commonly referred to as the nipple, the areola is actually a separate part of the breast that consists of the pigmented area around the nipple. Areolae are typically several shades darker than the surrounding skin but can be much darker, lighter, or the same color as the breast. Size and color of the areolae generally does not indicate any health concerns, as areolae can come in all shapes, sizes, and colors. For some, the areolae can be a cosmetic area of concern. Some men and women are born with areolae that are asymmetrical or are subjectively large. Areola characteristics can also be affected in adulthood by pregnancy, breastfeeding, and weight gain. For those who desire a change in the areola, a reduction procedure can be performed for permanent results. 

During the procedure the patient will be placed under local anesthesia, typically with a sedative as well. Patients who are combining an areola reduction with another procedure such as a breast augmentation, breast lift, or breast reduction, general anesthesia will likely be used. The patient will be marked preoperatively with 2 concentric circles around the areola. The inner circle will denote the new size of the areola. The outer circle will be made at the edges of the areola, with the area between the inner and outer circle representing the tissue that will be excised. If the patient would like the areola to sit at a higher position on the breast, more skin will be removed at the upper portion of the areola instead of removing an even amount of tissue around the nipple. 

The skin around the areola will then be infiltrated with a local anesthetic. Incisions will be made as per the preoperative markings and the excess tissue will be undermined and removed. Next, the wound will be closed, typically with a dissolvable circular stitch that closes with a “purse string” effect. It is common for additional permanent stitches to be placed to reduce tension and future stretching of the areola. The nipple will then be covered with a simple dressing. Most patients return to work within 1-2 days of the procedure and should avoid strenuous or sexual activities for 7-10 days.

The size and shape of the nipple, while not typically an indicator of a health concern, can cause aesthetic distress for some individuals. The nipple is cylindrical in shape with a curved top, however nipple hypertrophy occurs when the projection or diameter of the nipple is overly tall or wide. Nipple hypertrophy can be present during puberty but can also be caused by pregnancy, breastfeeding, or trauma to the tissue. 

When performing a nipple reduction, the focus is more commonly on the projection, or height, of the nipple. The diameter, or width, of the nipple can also be corrected during the procedure. First, preoperative marks will be drawn on the nipple. Depending on the preference of the patient, a circular mark will be made at the base of the nipple as well as between 6-8mm from the base of the nipple, delineating the new nipple height. If the diameter of the nipple is being reduced, additional rectangular or wedge markings will be made on the top and bottom of the nipple, equaling the amount of tissue to be removed. 

Only local anesthesia is necessary during a nipple reduction procedure. The base and trunk of the nipple will be infiltrated with lidocaine. Incisions will be made following the preoperative markings. Extra skin will be dissected and removed. Sutures will be used to attach the nipple to the base, reducing the height. If the width of the nipple is being reduced, the skin on the circumference of the nipple will be sutured back together as well. A bandage will be placed over the nipple and areola. Most patients are recovered 2 weeks after the procedure.

Non-ablative fractional laser skin resurfacing is a technique that incorporates a non-wounding laser to remove layers of skin as well as minor skin blemishes such as dark spots and fine lines. This treatment targets skin imperfections over the course of several treatments with shorter recovery times and has long lasting results.

In non-ablative resurfacing the endpoint is coagulation of the tissue, whereas in ablative resurfacing the endpoint is vaporization of the tissue. Either can be performed in a fractional technique, which delivers energy to the dermis to stimulate collagen remodeling through many narrow columns called microthermal treatment zones (MTZs). The epidermis at these sites is coagulated and destroyed, but the areas between these MTZs remains intact, leading to much faster healing. Each MTZ is one-tenth the size of a hair follicle. The energy deposited in the dermis then acts to tighten skin and stimulate collagen remodeling beneath a mostly intact epidermal layer. Remember that this energy is less than a full ablative technique and therefore multiple treatments are likely to be required.

Depending on the laser, a topical numbing cream may be used for the patient's comfort however some laser devices will have a cooling element built directly into the handpiece. The laser will then be expertly applied to the treatment area, targeting uneven skin texture, irregular pigmentation, fine lines, and other skin imperfections. The provider will closely monitor the skin to ensure an effective but comfortable session. Once the skin has been thoroughly treated, a soothing serum or ointment will be applied and a bandage may be put on the skin.

For men who are unable to have an erection, it can be both physically and mentally devastating. Erectile dysfunction can negatively impact relationships, self-worth, and overall confidence. When medications and other therapies are unable to treat ED, a penile implant can allow for reliable and natural feeling erections for both the individual and their partner. There are 2 types of implants: semi-rigid rods and inflatable implants. The semi-rigid rod is used for patients with ED as well as during gender confirmation surgeries. The semi-rigid rod has the advantage of being simple to use and less likely to malfunction than its inflatable counterparts. Semi-rigid rods however, will result in a phallus that will always be slightly rigid, and can be difficult to conceal at times. 

During surgery, the patient will be put under general anesthesia. The groin area will be cleansed and prepped for surgery. Depending on the patient, an incision will be made either within the lower abdomen, at the base of the penis, or at the head of the penis. The surgeon will then choose the correct implant size by stretching the spongy tissue, the corpora cavernosa, that expands with blood during an erection. Once a size has been chosen, they will be implanted inside the penis and incisions will be closed.  This surgery typically takes between 45 minutes to 1 hour to complete. After surgery when the patient would like to use the device, the malleable rods simply need to be positioned outwards to be erect, and downwards when complete.
