Thread lifts have become a minimally invasive alternative to the facelift for patients who are not ready to undergo more aggressive treatment options. Although the effects from a thread lift will be temporary and not as dramatic as a facelift, improvement in the appearance of fine lines and wrinkles can be seen gradually as the procedure stimulates natural collagen production. 

PDO sutures are a type of material used for thread lifts. PDO, which stands for polydioxanone, is an absorbable polymer that is flexible and durable for minimally invasive thread lifts. PDO thread lifts, sometimes referred to as PDS thread lifts, come in a variety of textures. There are three main textures of PDO sutures: mono, cog, and screw threads. Mono threads are singular, smooth sutures that are anchored to the face that provide a small lift to the face. Cog threads have a barbed texture that provide better lift and increased collagen production. Screw threads have 1-2 intertwined threads which provide volume to the treated area. For patients who only desire skin tightening and rejuvenation, mono threads may be sufficient. Patients who desire more lifting and volume will better benefit from textured cog or screw threads.

Before treatment, the patient's skin will be marked for insertion and direction of the inserted threads. Once marked, the insertion points will be throughly anesthetized. Multiple threads can be inserted through the same point in the skin. PDO threads are attached to a long cannula. The cannula will be pushed through the tissue beneath the skin. When the thread has reached its predetermined end point, the thread will be released and the cannula removed. This process will be repeated until the desired lift has been achieved, with as many 1-15 threads inserted depending on the treatment area and concern being addressed. Any extra thread will be trimmed. The threads will be massaged to ensure the tissue is smooth and no dimpling is present. 

Recovery is minimal after a PDO thread lift procedure. Most patients are able to resume normal activity within 1 week postoperatively.

The Pearl Fractional Laser is a type of ablative skin resurfacing treatment that improves the appearance of fine lines, wrinkles, and other skin blemishes. Before treatment begins, the skin will evaluated, cleansed, and numbed with a topical anesthetic cream. When the skin is completely numbed, the cream will be removed and treatment will begin. The Pearl hand piece will be applied to the skin where a scanner controls the placement of 300 microchannels. Multiple passes may be made over concern areas. Treatment of the entire face is completed in as little as 20 minutes. Three to four days after the procedure, the skin will begin to peel and shed. Much like breast implants, pectoral implants augment and define the contours of the chest. For men who are unsatisfied with the size of their chest or have a chest deformity, implants can be used to improve the appearance of the pectoral muscles. Chest implants are typically made of a solid silicone gel material that is relatively soft and flexible to imitate the look and feel of muscle. Incisions are well hidden either within the hair-bearing region of the underarm or at the border of the areola. Implants are placed beneath the pectorals major muscle for the most natural result. 

Before surgery, the chest area will be cleansed and anesthetized. Either general or local anesthesia with sedation is used to ensure patient comfort throughout the entire procedure. An incision will be made either within the underarm or at the border of the areola. A pocket will be made for the implant, ensuring proper placement and stability. Once the implant is in place the incision will be closed with sutures. Some patients will also have fat grafting done at the same time as a pectoral implant for a more natural appearance to the chest. Drainage tubes may be inserted to make sure fluid does not build up within the chest. Patients will be required to wear a compression vest for several weeks to reduce swelling. Results are noticeable immediately after treatment and final results are determined after a few months postoperatively.

The most invasive solution to ED that provides permanent results, uses a custom-fit device placed into the penis.

For men who are unable to have an erection, it can be both physically and mentally devastating. Erectile dysfunction can negatively impact relationships, self-worth, and overall confidence. When medications and other therapies are unable to treat ED, a penile implant can allow for reliable and natural feeling erections for both the individual and their partner. There are 2 types of implants: semi-rigid rods and inflatable implants. The semi-rigid rod is used for patients with ED as well as during gender confirmation surgeries. The semi-rigid rod has the advantage of being simple to use and less likely to malfunction than its inflatable counterparts. Semi-rigid rods however, will result in a phallus that will always be slightly rigid, and can be difficult to conceal at times. 

During surgery, the patient will be put under general anesthesia. The groin area will be cleansed and prepped for surgery. Depending on the patient, an incision will be made either within the lower abdomen, at the base of the penis, or at the head of the penis. The surgeon will then choose the correct implant size by stretching the spongy tissue, the corpora cavernosa, that expands with blood during an erection. Once a size has been chosen, they will be implanted inside the penis and incisions will be closed.  This surgery typically takes between 45 minutes to 1 hour to complete. After surgery when the patient would like to use the device, the malleable rods simply need to be positioned outwards to be erect, and downwards when complete.

Non-Inflatable Penile Implant

A non-inflatable penile implant is a penile prosthesis plastic surgery solution for a more natural erection feel and appearance.

Devices that can be surgically inserted into the penis to improve sexual health for men who suffer from erectile issues.

For men who suffer from ED that has been unresponsive to medication and other therapies, penile implants may be a good option to help correct the condition. Individuals who are undergoing gender confirmation surgeries also benefit from the results achieved with penile implants. Several options exist for penile implants, with the most popular either being semi-rigid rods or inflatable implants. The 2-piece inflatable implant is a simple device that offers patients reliable results with the advantage of being easily concealed when not in use. Non-inflatable implants, while being the easiest to use, are not as discreet and will always retain a feeling rigidity. 

2- and 3-piece inflatable implants consist of expanding rods and a pump, so that an erection can be easily achieved whenever the patient desires. The difference between 2- and 3-piece implants is where the saline solution that is used to inflate the rods is kept. 2-piece implants contain fluid within the implanted rods, while 3-piece implants retain the fluid in a separate reservoir that is placed within the abdominal cavity. Since the fluid is kept within the rods implanted directly in the penis, patients with a 2-piece implant will have a slightly more noticeable device when the penis is flaccid as opposed to a 3-piece implant. While each implant has both its advantages and disadvantages, it is best to have a consultation with a licensed urologist and plastic surgeon to determine the best treatment plan for your individual needs. 

During surgery, the patient will be placed under general anesthesia. An incision is typically made at the base of the penis or in the lower abdomen for implant insertion. The surgeon will determine the best implant size for the patient, customize the tubing for the device, and implant the cylinders inside the penis shaft, within each corpora cavernous. The pump for the implant will be placed within the scrotum. No tissue is removed during the procedure. Once the device is secure within the penis, incisions will be closed. This procedure typically takes between 45 minutes to 1 hour to complete.

2-Piece Inflatable Penile Implant

A 2-piece inflatable penile implant is a penile prosthesis plastic surgery solution for a more natural erection feel and appearance.

The most sophisticated and natural appearing devices for patients that give a naturally appearing and feeling erection.

Penile implants are typically the last course of action available for patients who have exhausted all other options to treat erectile dysfunction (ED). ED not only affects the physical and sexual health of a patient, but the mental well-being too. Addressing complications associated with ED can greatly improve the quality of life for patients. Individuals who are undergoing gender confirmation surgeries can also benefit from the realistic results achieved by a penile implant. 

There are 2 different kinds of implant devices: semi-rigid and inflatable. Semi-rigid implants have the advantage of being simple to use and is a fairly simple procedure. Inflatable implants, while more surgically involved, offer patients an easier to conceal implant and more natural results than their non-inflatable counterpart. Inflatable implant devices are comprised or either 2 or 3 pieces. 2-piece implants insert rods within the penis shaft and a pump inside of the scrotum. Saline fluid is contained within the rods and becomes stiff when the pump is squeezed. 3-piece implants contain saline fluid within a separate reservoir that is implanted within the abdominal cavity. When the pump is squeezed, fluid infiltrates the rods and allows for an erection. While a 2-piece device is a less complicated surgery, a 3-piece device allows for the best concealment of the implant when flaccid. Each implant has its advantages and disadvantages. Speak to a licensed urologist and plastic surgeon to determine the best treatment for your individual needs. 

During surgery, the patient will be placed under general anesthesia. An incision will typically be made either at the base of the penis of the lower abdomen. Implant size will be determined by stretching the spongy tissue within the penis. The implant rods are customized for each patient to ensure best results. The inflatable rod implants are inserted within the corpora cavernosa inside of the penis shaft. A pump will be placed within the scrotum as well as fluid reservoir beneath the abdominal wall. Once securely in place, incisions will be closed. Penile implant surgery usually takes 45 minutes to an hour.

3-Piece Inflatable Penile Implant

A 3-piece inflatable penile implant is a penile prosthesis plastic surgery solution for a more natural erection feel and appearance.

FTM Gender Confirmation

A Penile Implants procedure is a plastic surgery procedure to assist with dysfunction of natural erections. These surgical procedures are a last line therapy for erectile dysfunction or Peyronie’s disease, and are only utilized when all other treatment options have failed. A Penile Implant does not heighten sexual experience or increase libido; it is only used to mechanically create an erection. 

There are three types of implants, and all are placed within the corpus cavernosum. The corpus cavernosum is two separate columns of spongy erectile tissue that form the bulk of the penile shaft. These areas fill with blood to achieve erection in men with normal sexual function. 

Penile implants do not make the penis longer, and they also do not extend into the penile head, which typically leads to a shortened penis length when erect. Most men report high satisfaction rates following penile implant surgery.

Penile implants, or penile prostheses, are a plastic surgery procedure used as a last resort option in men with erectile dysfunction who have failed alternative therapies.

For men who feel inadequate because of the size of their penis, there are several procedures available to increase girth and length. Fat grafting is a promising, minimally invasive procedure that transfers purified fat from the patient’s body to the penile shaft. A type of stem cell has  been found in adipose tissue which could also help stimulate blood flow and natural growth, although changes would be subtle and have not been clinically studied. Fat grafting, unlike dermal fillers, also has the advantage of being derived from the patient so there is no chance of substance rejection. 

Before a fat grafting procedure, adipose tissue must be collected and purified. Liposuction will be performed, typically on the stomach or thighs, although fat can be collected from most anywhere on the body with unwanted fatty deposits. Once enough fat is collected the fat will be purified, removing cell debris, blood, and infiltration fluid. A small incision will be made at either the base of the penis or within a previous circumcision scar so that the cannula can be inserted. Fat will then be distributed around the shaft of the penis, between 30-65mL, although amount may be more or less depending on the patient. Once complete, the fat may be manually manipulated to ensure even dispersal of the fat. No stitches are typically necessary unless combined with a more invasive implant procedure. Patient will have to wear loose fitting clothes for 1-2 weeks, can resume penetrative sex after 1-2 weeks, and should refrain from strenuous exercise for 1 month.

Penis Enlargement: Fat Transfer

A penis enlargement with fat transfer plastic surgery procedure can be used to increase the length of the penis and girth of the penis shaft.

Injections are used to increase the girth of the penis while also increasing the length of the flaccid penis.

The size and length of the penis can greatly affect the confidence and self esteem of a man. For those who desire a minimally invasive method for temporarily increasing the size of the penis, dermal fillers can be injected around the penis shaft to augment girth and length. While there is no method of penis enlargement that is universally recognized as safe or effective, hyaluronic acid fillers can achieve an increase in size while offering temporary and reversible effects if results are unsatisfactory or complications arise. If a more permanent result is desired, a penile implant can be surgically placed to increase size and also help those who suffer from erectile dysfunction. 

Before injection, the groin area will be cleansed and the penis anesthetized with a topical numbing agent. Next, filler will be injected around the penile shaft with either a hyaluronic acid or collagen based filler just beneath the skin. The procedure will take approximately 30 minutes to complete. Most patients will have swelling and bruising in the groin area and may experience bumps, lumps, or skin irregularities where the filler was placed.

Penis Enlargement: Dermal Fillers

A penis enlargement by dermal fillers plastic surgery procedure is used to increase the length of the penis and girth of the penis shaft.

Increases the flaccid length of the penis without having any effect on the length of an erect penis.

For men who are self conscious about the size of their flaccid penis, ligament transection can increase the length of the penis when it is not erect. This surgery releases the suspensory ligament which anchors the penis to the pubic symphysis. The pubic symphysis is a cartilaginous joint that connects part of the left and right sides of the pubic bone. This ligament between the penis and the pubic symphysis helps to keep a portion of the penis safely within the body while also providing support for the penis when it is erect. It also creates the appearance of an arched angle between the penile base and pubic area. By cutting this ligament, the flaccid penis is effectively lengthened since there is no support to keep the penis inside of the body. The penis will also lay lower against the body at an obtuse angle, giving the perception of lengthening. 

During surgery, the patient will be placed under general anesthesia. The ligament will be accessed through a V shaped incision on the pubic area. Through this incision, the suspensory ligament will be severed and a buffer will be placed between the penis and the pubic symphysis to help prevent reattachment. Buffer materials may include tissue flaps or silicone implants. Complete release from the pubic bone is possible for further lengthening however this is accompanied by more complications including denervation and devascularization of the penis. When the procedure is complete, the V shaped incision will be closed in the shape of a Y for further lengthening on the penis.
