During the consultation process, implant material and placement will be chosen based on the patient’s goals. Calf implants are generally made of either gel or solid silicone. While silicone gel implants can cause capsular contracture (although rare), solid silicon implants can be more palpable postoperatively when placed too close to the surface of the skin. Implants are available as single and symmetrical or asymmetrical pairs. Symmetrical pair implants are generally suitable for the average patient whereas asymmetrical pairs are typically reserved for bodybuilders who desire a more dramatic transformation. Implant placement will either be made just beneath the fascia or deeper within the calf muscle. While a subfascial placement allows for a speedier recovery, there is a greater likelihood the implant will shift and become palpable overtime. Submuscular placement helps to ensure the implant remains in its intended place and will be less visible, however the surgery is more involved and will result in a lengthier and more difficult recovery period.

To begin the procedure, a discreet incision will be made in the crease behind the back of the knee. The chosen implant or implants will be inserted either under the fascia or under the muscle. Incisions will be closed with sutures and a postoperative compression garment will be applied.

Calf Implants

Calf resection, although seemingly extreme, can be used to great success when contouring large or disproportionate lower legs. For patients that are unhappy with large calves and have not had success with other treatments first, such as Botox injections or selective nerve ablation, muscle resection can be used as a final option for permanent calf contouring. 

When performing a muscle resection for calf reduction, only a partial resection of the gastrocnemius will be removed. The gastrocnemius muscle makes up the bulk of the calf muscle along with the soleus muscle, which is underneath the gastrocnemius. When the gastrocnemius muscle becomes enlarged, it protrudes to both sides of the calf, widening the appearance of the lower leg. A small incision will be made behind the knee within the leg crease. Through this incision, muscle fibers will be removed, being mindful to not sever any nerves within the leg. Incisions will be closed and drains will be inserted to prevent collection of blood and fluids. The legs will then be wrapped in postoperative compressive garments and bandages. For patients who do experience complications during their surgery, leg numbness and difficulty walking will occur however will slowly resolve, with normal function returning within a year.

Calf Reduction by Muscle Resection

Calf reduction by muscle resection is a cosmetic procedure to define, contour, and balance the calf muscles through removal of muscle tissue.

A Botox injection which temporarily creates a smoother contour in the calves without any surgical procedures.

Calf reduction is typically done as a way to feminize the legs, creating a smooth contour from the knees to the ankle. When calves lack a defined curve, the lower legs can appear shorter, more masculine, and can make wearing certain clothing items (like pant and boots) difficult. For patients who are unhappy with the proportions of the lower leg due to overly developed calf muscles, there are several option to reduce the size and improve the contours of the leg. Selective nerve blocking is a minimally invasive option for patients who would like to avoid a more involved surgery such as a muscle resection. This surgery, also referred to as a calf neurectomy, severs the nerve that controls part of the lower leg muscles. The lower leg is comprised of the soleus muscle, which provides the majority of the function of the calf, and the gastrocnemius muscles. The gastrocnemius muscles, while important for providing explosive power and foot steadiness, only accounts for about 30% of the function of the lower leg. The gastrocnemius is divided into lateral and medial sections, with the lateral head of the muscle creating the outer edge of the calf and the medial head responsible for the contour of the inner calf. During this procedure, nerve blocking is focused on the medial head of the gastrocnemius muscle 

During the procedure, the patient will be put under general anesthesia. A small, discreet 1.5cm incision will be made behind the knee. The nerve branch that governs the function of the medial gastrocnemius muscle will be identified via an electric nerve stimulator. Once confirmed, the nerve will be completely severed and ligated. In doing so, the medial gastrocnemius muscle will no longer receive stimuli to flex, causing gradual muscle atrophy over the course of 1 month. Incisions will be closed and the same procedure will be repeated on the opposite leg.  In recovery it is expected for patients to walk slowly with an unsteady gait, be unable to run, and have general discomfort due to the lack of lower leg support. Normal gait generally returns 1 week postoperatively but final results can take as long as 3 months.

Calf Reduction by Selective Nerve Blocking

Calf reduction by selective nerve blocking is a cosmetic procedure to define, contour, and balance the calf muscles by altering cell signaling to the muscle.

Radiofrequency nerve and muscle ablation involves burning part of the gastrocnemius muscle away to reduce its size. There are a number of noninvasive fat reduction methods available for patients who have stubborn fatty deposits that are resistant to diet and exercise. Reducing fat stores can also make cellulite less apparent on the thighs and buttocks. BodyFX, like VelaSmooth and VelaShape, use radiofrequency energy to heat deep tissues and reduce fat cells. 

The BodyFX hand piece will be placed on the treatment area. The skin will be gently vacuumed into the hand piece for even heating of the tissue for several seconds and then alternate between adjacent treatment areas. The hand piece will gradually heat the tissue to 40-44° centigrade. Treatment sessions can last between 30-60 minutes depending on the size of the area. A series of sessions will be needed to see results, typically once a week for 6-12 weeks with upkeep treatments every 3-4 months to maintain results. Results tend to improve several months after the final treatment as it takes time for collagen production to complete as well as the lymphatic system to naturally eliminate destroyed fat cells.

Cellulite affects mostly women and usually appears on the thighs, hips, and buttocks area as dimpled skin. The cottage-cheese like appearance of cellulite is caused by a number of factors: fat, skin thickness, and fibrous tissues. Connective tissues beneath the skin, called septa, pull down the skin creating dimples. These dimples are further accentuated by fat deposits and thin skin. Radiofrequency treatments can help to diminish the appearance of cellulite by targeting fat and encouraging collagen production. There are a variety of non invasive RF based cellulite products that delivery energy to the skin through a number of different hand pieces, however, their goal is all the same: to decrease cellulite. When delivered deep into the skin, RF energy helps to destroy fat cells which are then eliminated from the body naturally through the lymphatic system. Also, the heat created by RF energy encourages the body to go into a healing mode which allows for collage and elastin production, firming and volumizing the skin. 

Before a radiofrequency cellulite treatment begins, a topical numbing cream may be applied depending on the pain tolerance of the patient. A grid may be drawn on the skin as well to assist the practitioner during a session for even and targeted treatment. Next, a gel, cream, or oil will be applied to the treatment area to lubricate the skin and create an additional barrier between the skin and the hand piece. The hand piece will then be applied to the skin, working across the treatment area. When the procedure is complete, a soothing gel, lotion, or cool compress will be applied to the skin. 

A series of sessions are necessary to achieve desired results, but depending on the device used, it may require twice weekly sessions for 4-5 weeks or 1-2 weeks with an average of 8 treatments. Ongoing monthly maintenance treatments are typically necessary. Other brand name products that treat cellulite non invasively via RF energy include EndyMed 3Deep, truSculpt 3D, Vanquish, Venus Freeze, Venus Legacy, Exilis, and JuVaShape. VelaShape is another non-invasive treatment which combines RF energy, infrared light, as well as vacuum and massage to target stubborn cellulite.

Cellulite affects the majority of women, with estimates as high as 80-90%. Some women find the skin dimpling caused by cellulite to be unsightly and see little improvement with diet and exercise. The massage and vacuum rollers of the hand piece help to distribute heat and manually soften fibrotic strands that pull down the skin. Treatment sessions typically last 30-45 minutes.

VelaSmooth™

VelaSmooth™ is an FDA-approved cellulite reduction treatment combining infrared light, radiofrequency energy, and tissue mobilization.

A cellulite reduction method that goes deep into tissue to melt fat, firm skin, and lessen the appearance of dimples.

Cellulite is caused by rigid, fibrotic strands, also called septae, that pull down the skin and accentuate pockets of fat, creating a visible dimpling effect. Cellulite mostly affects women and has no relation to the fitness level of the affected individual. While a healthy diet and regular exercise can lessen the appearance of cellulite, it does not have an affect on the septae that pulls down the skin. VelaShape, an improved upon version of VelaSmooth, allows  for fat reduction, collagen production, and a softening of the septae for improved body contours. 

During treatment the patient will lie comfortably on a table. The VelaShape hand piece will be guided along the treatment area, deliver radiofrequency and infrared energy while simultaneously vacuuming and massaging the skin. The heat energy generated helps to break down and melt fat cells while the mechanical massage encourages blood flow, stimulating the lymphatic system to take away cellular debris. Results are seen after 4-6 treatments with continued improvement as collagen production increases and smoothens skin texture. Maintenance treatments are typically needed every 3 months.
