Chemical peels use a variety of ingredients to exfoliate the skin to reveal a more rejuvenated complexion. TCA peels are popular for both at-home and professional use. DIY versions are sold at lower concentration ranging from 5-15% while higher concentrations of 20-35% should be used only by licensed providers. TCA peels have been performed at even higher concentrations but were associated with a greater risk of scarring. Some plastic surgeons and dermatologists recommend that regardless of concentration, chemical peels should be done in an office setting to ensure proper application and depth of penetration. Otherwise, patients may accidentally cause severe chemical burns to the treated area. 

Once a patient is determined to be a good candidate for a TCA chemical peel, they will need to complete any pretreatment skincare regimens, such as a daily tretinoin cream to reduce healing time or a series of lighter chemical peels to prepare the skin for a deeper exfoliation. On the day of treatment, the patient may require general, local, or no anesthesia depending on the depth and concentration of the chemical peel. The customized solution will be applied to the face, with multiple layers applied to areas of concern such as deep wrinkling and uneven pigmentation. The solution will be left on the skin until a frosting affect occurs. The provider will determine the depth of exfoliation visually, by observing the progression of skin frosting. Once the desired depth of exfoliation has occurred, the chemical solution will be neutralized and removed from the skin. A soothing lotion or serum may be applied after treatment.

TCA Peel

A skin resurfacing technique that causes the skin to exfoliate and peel to enhance its appearance.

The ideal chemical is chosen for the desired depth of penetration. The skin is thoroughly cleansed prior to the administration of the solution. The chemical solution is then applied to the desired areas of treatment. The candidate is closely observed for changes to the skin. Whitening of the skin is monitored to assess epidermal damage and is a sign of epidermolysis. As the peel progresses to a frosted appearance this indicates a deeper depth of penetration into the dermis.

Once the appropriate amount of time has elapsed, the solution is then neutralized with another agent such as sodium bicarbonate. Some peels do not require neutralization, as the skin naturally neutralizes them. The skin is then cleansed. Various dressings such as silicone may be applied, ointments or emollients can be utilized after the peel has completed to form a protective barrier. This is more common for deep peels and is typically not necessary for more superficial peels. Keep in mind that the depth of peel is determined not just by the chemical composition or strength of ingredients, but also by the length of time it is left on the skin.

Overcrowded teeth can cause one's smile to look cramped, while gaps can make the teeth appear small. Both conditions can be embarrassing and make an individual self conscious about their smile. Teens and adults alike may postpone fixing their teeth especially if they have already experienced traditional metal braces that are easily noticed and hard to conceal. However, modern orthodontic systems such as lingual braces and clear aligners have made adult braces a more practical solution to crooked smiles. 

Invisalign is one of the most well-known clear aligner programs that uses a progressive system of trays to gently reposition the teeth into a straighter, more aesthetically pleasing smile. A visit to an orthodontist will be necessary to determine if your dental concerns can be corrected by Invisalign. Patients with more severe issues who also require jaw realignment may better benefit from a more aggressive orthodontic treatment. 

A 3D scan will be taken of the teeth in order to create a treatment plan. Additional x-rays and images may be taken during your appointment as well. It will take 3-4 weeks to create the first tray. Once the tray arrives, patients will return to the orthodontist's office to ensure proper fit. Trays will be worn for a period of 2 weeks and should only be removed for eating and brushing the teeth. For optimal results, trays should be worn for 20-22 hours daily. Certain Invisalign systems will have a blue indicator dot on the upper back molars that will gradually disappear with wear, easily notifying your provider if you are wearing your Invisalign tray for the required time. After 2 weeks, a new tray will be worn. Trays will be replaced every 2 weeks for the duration of treatment which typically lasts 10-15 months but can be as short as 3 months or as long as 2 years.

Much like other orthodontic systems, a visit to the orthodontist is first needed to determine if your individual case can be corrected with the ClearCorrect system or if a more aggressive treatment is needed to adjust the position of the jaw. Many adults who wore braces as children still suffer from crooked teeth, especially when proper aftercare was not stringently adhered to. A threadlift is a minimally invasive procedure that gives patients a moderate lift and volumizing effect to the face, cheeks, neck, and jawline. The Silhouette Soft threadlift is a type of threadlift that has additional cones along the thread to further lift the facial tissue for more noticeable improvement. Silhouette Soft is manufactured by the same company that makes Silhouette Instalift. The difference between the two products are the materials used to create the thread and cone structures. The Silhouette Soft thread is made from Polylactic Acid (PLLA) while the cones are made of poly (L-lactide-co-glycolide) (PLGA). Silhouette Instalift sutures and cones are both made from PLGA. 

Prior to treatment, the patient's face and neck area will be marked to indicate insertion and exist point for the threads. Each thread will require one central insertion point and two exit points. All marks will be infiltrated with a local anesthetic. Once thoroughly anesthetized, a small incision will be made in the center of each central insertion point so that the needle can easily be guided beneath the skin.

Each Silhouette Soft thread is inserted in two parts. The needle will be inserted into the center insertion mark and guided linearly to the previously marked exit point. The needle will be forced through the skin and pulled outwards, pulling half of the thread through the face. The second half of the thread will be inserted through the same center point and again, threaded linearly though the skin and out the pre-determined exit point. Excess thread will be trimmed. This process will be repeated for every thread. Once all threads have been placed, the provider will massage the face so that the cones are able to properly attach and lift the tissue. 

Results are noticeable immediately after treatment, however, final results will be more apparent after several months. Stimulated collagen production will gradually improve facial volume, firming the skin, and smoothing out fine lines and wrinkles. Inserted threads will eventually dissolve with effects lasting for as long as 18-24 months depending on lifestyle.

Thread lifts have become a minimally invasive alternative to the facelift for patients who are not ready to undergo more aggressive treatment options. Although the effects from a thread lift will be temporary and not as dramatic as a facelift, improvement in the appearance of fine lines and wrinkles can be seen gradually as the procedure stimulates natural collagen production. 

PDO sutures are a type of material used for thread lifts. PDO, which stands for polydioxanone, is an absorbable polymer that is flexible and durable for minimally invasive thread lifts. PDO thread lifts, sometimes referred to as PDS thread lifts, come in a variety of textures. There are three main textures of PDO sutures: mono, cog, and screw threads. Mono threads are singular, smooth sutures that are anchored to the face that provide a small lift to the face. Cog threads have a barbed texture that provide better lift and increased collagen production. Screw threads have 1-2 intertwined threads which provide volume to the treated area. For patients who only desire skin tightening and rejuvenation, mono threads may be sufficient. Patients who desire more lifting and volume will better benefit from textured cog or screw threads.

Before treatment, the patient's skin will be marked for insertion and direction of the inserted threads. Once marked, the insertion points will be throughly anesthetized. Multiple threads can be inserted through the same point in the skin. PDO threads are attached to a long cannula. The cannula will be pushed through the tissue beneath the skin. When the thread has reached its predetermined end point, the thread will be released and the cannula removed. This process will be repeated until the desired lift has been achieved, with as many 1-15 threads inserted depending on the treatment area and concern being addressed. Any extra thread will be trimmed. The threads will be massaged to ensure the tissue is smooth and no dimpling is present. 

Recovery is minimal after a PDO thread lift procedure. Most patients are able to resume normal activity within 1 week postoperatively.

PDO Thread Lift

A PDO thread lift is designed to subtly lift the facial features to reduce fine lines and wrinkles while encouraging collagen production for more volume in the treated areas. PDO threads come in a variety of textures for customized treatments.

For those who are not ready to pursue a more aggressive or surgical procedure, a thread lift is a minimally invasive, outpatient treatment that offers immediate improvement. The Silhouette Instalift is a type of thread lift that has additional cone structures on each thread. These cones allow for easier tissue fixation to the thread, providing more noticeable results. It is common for a thread lift to be combined with various other facial rejuvenation treatments such as laser resurfacing or dermal fillers to further enhance skin volume and texture.

Tofacitinib is a Janus kinase (JAK) inhibitor. JAKs are enzymes that work within the immune system and other cells to transmit signals that influence basic cell functions. By inhibiting JAKs in immune cells, Xeljanz can interfere with the immune processes which attack hair follicles causing hair loss. This pill seeks to reverse hair loss caused by alopecia areata which is a common autoimmune skin disease, causing hair loss on the scalp, face and sometimes on other areas of the body.

Tofacitinib and similar medications like dutasteride are not primarily used for treatment of hair loss but have shown to decrease hair minimization. Tofacitinib is available in 5 mg tablets and the recommended dose is 5 mg twice daily. Take the medication as directed by the provider and do not increase intake if a dose is missed.
