A good [chemical peel](https://aedit.com/aedition/guide-to-superficial-medium-deep-chemical-peels) can totally reset your skin — helping brighten pigmentation and dullness, reduce the appearance of large pores and fine lines, and improve tone and texture. They can also cause quite a bit of, well, peeling and leave skin feeling raw for some time afterwards. The good news? There are different kinds of peels that treat [different layers of the skin](https://aedit.com/aedition/ultimate-guide-to-skin-resurfacing-procedures), which means a dermatologist can tailor a treatment to directly address your skin concerns and [skin type](https://aedit.com/aedition/guide-to-fitzpatrick-skin-types). So, what are the questions to ask? Ahead, top dermatologists speak to the most important intel to have pre-peel.  

### **1. What is a chemical peel?** 
If you’ve ever used a [chemical exfoliator](https://aedit.com/aedition/difference-between-medical-grade-chemical-peels-at-home-exfoliators) (think: a glycolic acid toner or a lactic acid serum), then you’ve used ingredients similar to those in a peel. A medical-grade chemical peel, however, exposes the ingredients we use at home to be quite weak in comparison to what happens when they’re applied professionally. “Chemical peels refer to a type of topical resurfacing in which a chemical exfoliant is applied to the skin to induce controlled skin injury at a certain depth, leading to subsequent skin regrowth,” explains [Marisa Garshick, MD](https://www.mdcsnyc.com/provider/marisa-k-garshick-md), New York City-based, board certified dermatologist.

### **2. What are the different types of chemical peels?**
There are three categories of professional chemical peels: 

1. [Superficial chemical peels](https://aedit.com/procedure/superficial-chemical-peels) 
2. [Medium chemical peels](https://aedit.com/procedure/medium-depth-chemical-peels)
3. [Deep chemical peels](https://aedit.com/procedure/deep-chemical-peels) 

“These terms describe how deeply the peels penetrate into the skin,” says [Sejal Shah, MD](https://smarterskindermatology.com/about-us/sejal-shah-md/), a board certified dermatologist in NYC. Superficial peels penetrate only the epidermis (a.k.a. the outermost layer of skin); medium-depth peels affect the entire epidermis and a portion of the papillary dermis (upper to mid-dermis); and deep peels aim to penetrate to the level of the mid-reticular dermis (deeper part of the dermis). 

### **3. What ingredients are found in chemical peels?**
The depth, however, isn’t the only differentiating factor among peels. There’s also the ingredients — or cocktail of ingredients — used. As Dr. Garshick explains, superficial chemical peels often contain: 

- [Alpha hydroxy acids](https://aedit.com/aedition/guide-to-topical-alpha-hydroxy-acids-in-skincare) (like glycolic, lactic, and mandelic)
- [Trichloroacetic acid (TCA)](https://aedit.com/procedure/tca-peel) 
- Beta hydroxy acids (read: [salicylic acid](https://aedit.com/procedure/salicylic-acid-peel))
- [Retinol](https://aedit.com/aedition/guide-to-vitamin-a-retinol-retinoids-in-skincare) 

Superficial [glycolic peels](https://aedit.com/procedure/glycolic-peel) may have a concentration of anywhere from 20 to 50 percent. Lactic acid peels fall in the 20 to 30 percent range. TCA can be used to produce deeper results, but, at superficial strength, it’ll be found at 10 to 20 percent. Needless to say, those percentages increase as you wish to increase the intensity of the peel. “Medium to deep peels penetrate to the mid to deep dermis,” Dr. Garshick explains. The most common? TCA peels at 35 to 50 percent, “which may also be used in combination with other ingredients such as Jessner's solution or glycolic acid,” she adds. (Note: [Jessner’s solution](https://aedit.com/procedure/jessners-peel) includes salicylic acid, resorcinol, lactic acid, and ethanol.) 

### **4. Who is (and isn’t) a good candidate for a chemical peel?**
Superficial chemical peels are generally considered safe and effective for most skin types. Medium and deep peels require a bit more caution. If you’re dealing with photo damage, actinic keratoses (i.e. rough patches that can be considered precancerous), sun spots, fine lines, wrinkles, and discoloration, Dr. Garshick says you may be a good candidate for a medium-deep peel. Medium strength peels can also be good for patients looking to smooth [acne scars](https://aedit.com/aedition/best-professional-treatments-for-acne-and-acne-scars). 

But it’s important to consider your medical history. If you have a history of keloids or hypertrophic scars, have used certain acne treatments in the past year, have very sensitive or easily irritated skin, you may *not* be a good candidate, Dr. Shah notes. Finally, patients with [darker skin tones](https://aedit.com/aedition/the-best-skin-treatments-for-dark-complexions) must be more cautious, as deeper peels pose risks for post-inflammatory pigmentation. 

### **5. Is there downtime after a chemical peel?**
After a superficial chemical peel, you can expect to see about a week of flaking, but it shouldn’t impact your daily routine too much. When it comes to more potent peels, Dr. Shah says to expect up to a week of downtime after a medium chemical peel and up to two weeks after certain deeper peels. You’ll likely experience physical skin peeling for five to seven days and the skin may be pink or red. “While the worst of that will subside within one to two weeks, some may experience some residual erythema (or pinkness of the skin) for up to three to four weeks,” Dr. Garshick notes. “Some individuals may also experience some swelling in the first one to two days.”

### **6. What kind of results can be expected from a chemical peel?**
That said, if you can handle a week or two of downtime — the results of a medium and deep chemical peel can be impressive. Positive outcomes can include “improvements in fine lines, wrinkles, texture, hyperpigmentation, sun damage, acne, dullness, and overall skin complexion,” Dr. Shah says. Even at the more superficial end of the spectrum, you can expect to see some improvement in skin brightness, tone, and texture.

### **7. What kind of post-peel care is involved?**
In the case of superficial chemical peels, a temporary change to your skincare routine (think: laying off sensitizing products like retinol for a few days) may be suggested post-peel. Deeper peels will require more care, and your provider will likely provide thorough instructions. “Immediately after the peel, ice soaked gauze may be applied to the skin or you may be provided with a fan to help keep the skin feeling cool,” Dr. Garshick shares, adding that a thin layer of petrolatum (read: Vaseline) or wound-healing cream is often applied. 1. **Don’t Touch**: “Avoid touching, picking, rubbing, or peeling the sloughing skin,” she says.
2. **Keep It Clean**: Use a gentle cleanser to keep the skin as clean as possible.
3. **Moisturize**: You can even apply and reapply throughout the day if skin feels dry or tight. “Sometimes I will even recommend a mild hydrocortisone,” Dr. Shah adds. 
4. **Be Gentle**: Skip exfoliation (chemical and physical) while your skin heels.
5. **Stay Protected**: For good measure, we’ll remind you one more time — be extra diligent with your SPF application.

Chemical peels are making a comeback. After years of taking a backseat to high-tech, high-priced [lasers](https://aedit.com/aedition/guide-to-ablative-laser-skin-resurfacing) and the social media-fueled [microneedling](https://aedit.com/aedition/difference-between-professional-microneedling-at-home-dermarolling) craze, the humble (yet oh so powerful) peel promises a relatively quick, affordable, and versatile alternative for correcting a cadre of complexion woes.

Chemical peels have been around for millennia. An ancient Egyptian medical text known as the Ebers Papyrus (1550 BC) describes the technique, and Cleopatra herself is said to have bathed in sour donkey milk (which contains lactic acid) to smooth her skin. The Greeks and Romans, meanwhile, used corrosive limestone and grapes (tartaric acid) for the task.

From the first use of trichloroacetic acid (TCA) in the 1880s to the fine-tuning of the phenol peel in the early 2000s, peeling techniques have come a long way. ## **What Is a Chemical Peel?**
Just as it sounds, chemical peeling (a.k.a. chemexfoliation or chemical exfoliation) uses a solution (typically an acid) to strip away the skin’s outer layers in a controlled manner. This destruction to part or all of the epidermis and, sometimes, part of the dermis [stimulates the skin’s regenerative process](https://aedit.com/aedition/ultimate-guide-to-skin-resurfacing-procedures), including the production of collagen and elastin. Some peels are self-neutralizing, meaning that they lose all their potency on their own, while others require intervention to stop the action of the ingredients. Over time (usually within a week) the skin begins to peel and flake off, revealing smoother skin beneath. 

How deep a peel goes depends upon several factors: 

- Acid type
- Acid concentration
- Application technique 
- Skin thickness
- Sebum production
- Skin barrier health

Generally speaking, the higher the concentration of acid or other active ingredients, the stronger the peel. Similarly, applying more solution to the skin, leaving it on longer (in some cases), and overlapping in certain areas will typically result in more layers of skin cells being removed. Professional chemical peels can be classified into one of three categories depending on their potency:

1. Superficial chemical peels
2. Medium chemical peels
3. Deep chemical peels

### **Superficial Chemical Peels**
Also known as ‘lunchtime peels’ or ‘light peels,’ [superficial chemical peels](https://aedit.com/procedure/superficial-chemical-peels) remove only the layers of the epidermis — you know, those dead skin cells that build up and can make skin look dull. Depending on their makeup, some very superficial peels only remove the stratum corneum of the epidermis, while others go deeper to reach the basal layer. These treatments can be performed by an aesthetician, board certified physician, or [even at home](https://aedit.com/aedition/what-its-like-to-try-a-medical-grade-chemical-peel-at-home) (though it’s best to consult your dermatologist beforehand when going the DIY route).

Superficial peels are used to treat a variety of complexion concerns, including:

- Fine lines
- Acne and acne scarring
- Pigmentation (both hypo and hyper)
- Uneven texture
- Dullness

There are many types of superficial peels. Depending on your [Fitzpatrick skin type](https://aedit.com/aedition/guide-to-fitzpatrick-skin-types) and the concerns you are looking to correct, you may receive one of the following: 

- **[Alpha Hydroxy Acid (AHA)](https://aedit.com/aedition/guide-to-topical-alpha-hydroxy-acids-in-skincare)**: Most commonly lactic or [glycolic acid](https://aedit.com/procedure/glycolic-peel), but also citric, malic, mandelic, and tartaric, concentrations of AHAs vary from 10 to 70 percent. Neutralization with a basic solution is required. Depending on formula and technique, may be suitable for Fitzpatrick types I to VI.
- **[Trichloroacetic Acid (TCA)](https://aedit.com/procedure/tca-peel)**: Used for over 100 years, a concentration of about 6 to 25 percent produces a superficial TCA peel that’s self-neutralizing. Lower potencies are typically suitable for all Fitzpatrick types.
- **[Salicylic Acid](https://aedit.com/procedure/salicylic-acid-peel)**: This [beta hydroxy acid (BHA)](https://aedit.com/aedition/guide-to-beta-hydroxy-acids-salicylic-acid-in-skincare) is often used to treat acne and also has anti-inflammatory properties. Concentrations usually range from 20 to 30 percent and is typically suitable for Fitzpatrick skin types I to VI.
- **[Modified Jessner’s Solution](https://aedit.com/procedure/jessners-peel)**: The first blended acid peel, it originally contained 14 percent lactic acid, 14 percent salicylic acid, and 14 percent resorcinol (later replaced by TCA). The self-neutralizing peel can address a range of concerns — from acne to aging — for Fitzpatrick types I to VI.
- **Blended Acid Peels**: These peels are popular because they combine different acids at lower concentrations to address a myriad of concerns with (potentially) fewer side effects. The makeup of blended peels varies to accomodate a patient’s specific needs and goals.
- **[Retinoids](https://aedit.com/aedition/guide-to-vitamin-a-retinol-retinoids-in-skincare)**: You might not realize it, but these vitamin A compounds — commonly known as retinol (in lower potencies) and tretinoin (prescription-strength) — are considered peeling agents because they induce cell turnover. Though suitable for Fitzpatrick skin types I to VI, higher concentrations can cause irritation and dryness.

With so many options to choose from, consulting with your provider or aesthetician will ensure you receive the best treatment for your needs.

### **Medium Chemical Peels**
[Medium chemical peels](https://aedit.com/procedure/medium-depth-chemical-peels) penetrate the entire epidermal layer to exfoliate cells within the papillary dermis. Though some states allow aestheticians to perform moderate peels, it’s important to consult with a board certified dermatologist or plastic surgeon beforehand to avoid possible complications and unwanted outcomes.

Medium peels produce more noticeable results than light peels, making them well suited for:

- Acne scars
- Fine lines and wrinkles
- Sun damage (like liver spots)
- Uneven skin tone
- Actinic keratoses (rough patches that can lead to skin cancer)

These peels remove the entire epidermis and the papillary dermis and may also penetrate down into the reticular dermis. Deep dermal peels may only be performed by a highly trained, board certified provider.

Appropriate for Fitzpatrick types I to III, deep peels address the most significant skin concerns, including:

- Acne scars
- Fine lines to deep wrinkles
- Sun damage
- Uneven skin tone
- Pre-cancerous growths
- Skin laxity

Though TCA in concentrations of 50 percent or more are capable of inducing a relatively deep peel, the most powerful chemexfoliating treatment is the Baker-Gordon Peel. Developed in the 1960s, the formula was composed of [phenol](https://aedit.com/procedure/phenol-peel) (an acid) mixed with [croton oil](https://aedit.com/procedure/croton-oil-peel) (an ancient purgative derived from the seeds of the croton tiglium plant) to resurface the skin. The results were impressive, but it sometimes resulted in unwanted hypopigmentation and a waxy, unnatural appearance to the skin.

Modern Baker-Gordon formulations adjust the amount of croton oil used to give practitioners more control over peel depth. Combined with pre-treatment regimens designed to mitigate pigmentation problems, this new generation of deep peel is experiencing a resurgence of sorts thanks to its effectiveness and relative ease (no bulky, expensive laser necessary!).

## **How to Prepare for a Chemical Peel**  
Depending on the type and degree of chemical exfoliant being applied, chemical peel prep can vary from a simple cleanse to weeks of pre-treatment prescription skincare. A good rule of thumb is that the more intensive the peel, the more involved the regimen beforehand. Here’s a general breakdown of what your provider may suggest pre-peel:

- **Skincare**: Your doctor may recommend switching to a mild cleanser and increasing your moisturizer application to keep skin calm and hydrated.
- **Retinoids**: A vitamin A exfoliant — either over-the-counter or a prescription like Retin-A — will help slough off dead cells to provide a more even peel result.
- **Bleaching Agent**: Suppressing the skin’s melanocytes with a topical bleach like hydroquinone can help mitigate post-procedure pigmentation problems.
- **Sunscreen**: Keeping your skin protected from the sun is crucial both before *and* after a chemical peel.
- **Medication**: Certain drugs (including oral contraceptives, some antibiotics, and Accutane) can make skin more prone to pigmentation problems and sun sensitivity, so you may need to temporarily stop taking them pre-and post-treatment.
- **Preventative Antivirals**: Whether you can recall having a history of cold sores or not, most of us have been exposed to the herpes simplex virus (HSV), so many docs prescribe a course of antiviral meds to ward off a possible outbreak.

## **What It’s Like to Get a Chemical Peel**
Before beginning your chemical exfoliation procedure, your practitioner will prep your skin by cleansing and degreasing it, usually with alcohol, acetone, or both. To make the procedure more comfortable, numbing cream may be applied. Some superficial peels don’t require any topical numbing at all, while deeper peels require local anesthetic, IV sedation, or [general anesthesia](https://aedit.com/aedition/everything-you-need-to-know-about-anesthesia). It all depends on the depth and area being treated. 

Once your skin is prepped, your provider will brush on the solution as evenly as possible. Self-neutralizing peels like TCA quit working in a matter of seconds, while continuous peels like glycolic acid must be timed and neutralized when necessary. When the peeling agent is applied, the skin begins to take on a white, frosted appearance. The degree of frost is an indication of the depth of the peel. Your doctor may reapply the peeling solution in certain areas (around the mouth, for example) where wrinkling is deeper. The frost makes it difficult to discern exactly how deep the peel is penetrating, so the provider’s experience and skill here is critical.

## **Recovering from a Chemical Peel**
With just about any type of chemical peel, you can expect to experience some degree of burning, itching, redness, swelling, and flaking afterwards. Recovery from a superficial peel may only last a couple of days, whereas a medium peel can take about a week. Deeper peels require about two weeks of downtime, with some swelling and redness hanging around for weeks (if not months) longer.

“You can have a peel where your skin doesn’t truly peel off but rather flakes very unnoticeably, or you could do a deep peel where your skin sheds to the point where you may have to trim some of the dead skin from your face,” explains [Julie Russak, MD](https://aedit.com/provider/julie-russak-md-4645), a board certified dermatologist and founder of Russak Dermatology in New York City. “It all depends on your personal goals, what issue we are treating, and your at-home care — we can customize all treatments for your needs.”

Taking care of your skin during the healing process is crucial to achieving a successful result. Follow your provider’s post-treatment instructions exactly, [keeping skin clean and hydrated](https://aedit.com/aedition/skincare-after-facial-plastic-surgery-procedures). And, however tempting, don’t poke, pick, or peel your fragile skin. For medium and deep peels, stay out of the sun until your wounds are healed. ## **Chemical Peel Risks**

Your chance of having a successful outcome depends heavily on the skill and knowledge of your provider, as well as your skin type and medical history. Discussing your complete medical history during a thorough skin exam can help decrease your chances of experiencing complications, including:

- Infection (viral, bacterial, and fungal)
- Prolonged erythema (a.k.a. redness) 
- Increased sun sensitivity (sunscreen is a must!)

As the new year approaches, what big trends in both surgical and non-surgical cosmetic procedures will it bring? In June, we declared [eyes are the new lips](https://aedit.com/aedition/popularity-of-eyelid-surgery-cosmetic-eye-procedures-during-covid-19) as a result of COVID-19 face coverings, and that trend is here to stay. But that’s not all 2021 has in store for the face.

### **1. All About Eyes**
We’re calling it now: 2021 will continue to be an eye opener. “Who knows how long masks will be part of our daily ensemble,” says [Dara Liotta, MD](https://www.facialplasticsnyc.com/), a double board certified facial plastic and reconstructive surgeon in New York City. She predicts patients will choose to combine “minor eye-optimizing surgeries” that “provide natural-looking, permanent results” (think: [upper eyelid blepharoplasty](https://aedit.com/procedure/upper-eyelid-blepharoplasty)) with non-surgical enhancements (like the [Botox® Brow Lift](https://aedit.com/procedure/chemical-brow-lift) and lateral cheekbone and [under eye filler](https://aedit.com/procedure/under-eye-filler)) to “lighten and brighten the area around the eyes.” Optimizing the eyes can do wonders for making us look better rested, less stressed, and more put together. “More and more patients will be willing to come and sit in my office chair once or twice a year for a little ‘high maintenance’ tweakment in order to be ‘low maintenance’ all year long,” she says. In her experience, it’s all about the ability to “ditch the undereye concealer and the strobing makeup that just gets all over our masks anyway.” True story. 

### **2. Couture Lips**
Just because eyes are having a moment doesn’t mean lips are a thing of the past. 2020 saw the release of [Restylane® Kysse](https://aedit.com/aedition/new-injectables-neurotoxins-fillers-for-face-body), a hyaluronic acid-based lip filler from Galderma, and Dr. Liotta expects non-surgical pout procedures to continue to be in demand. “Lips are always ‘in’ and more and more patients are willing to pay a bit more for a perfect, couture, natural-looking lip,” Dr. Liotta says. Despite the popularity, lips can be a complicated area of the face to treat. “When you really analyze the lips, they’re composed of a number of different parts,” she explains. There are the philtral columns (the lines from the peak of the cupid’s bow up toward the nose), the cupid’s bow, the vermillion border (i.e. where you’d apply lip liner), and the body of the lip itself (where you’d put lipstick). “If you feel your own lips, you’ll appreciate that each of these areas have different stiffnesses, and move differently,” Dr. Liotta says.

After analyzing the lips — both at rest and movement — she designs “a custom plan for lip augmentation” using one to three different types of fillers, plus or minus Botox®. Dubbed “Couture Lips,” the procedure rings in around $3,000 because more than one type of filler is involved. “It’s expensive, but so worth it,” she says.

If you are looking for a natural, long-lasting alternative to synthetic dermal filler, nanofat is a whole new level of [facial fat injections](https://aedit.com/aedition/comparing-dermal-fillers-vs-fat-grafting-for-facial-rejuvenation-contouring). “Because the market has now gotten really used to doing facial fillers, people are realizing how much money and what a commitment filler is,” says [Lisa Cassileth, MD](https://www.drcassileth.com/), a board certified plastic and reconstructive surgeon and founder of Cassileth Plastic Surgery in Beverly Hills. 2020 was rough, and its trials and tribulations have minted a whole new group of teeth grinders and jaw clinchers, Dr. Liotta says. “2021 is going to be about mixing wellness and beauty,” she notes, and [masseter Botox®](https://aedit.com/procedure/jawline-slimming-with-botox) is a great example of this. The masseter muscle is one of the muscles activated during chewing, and it’s located at the angle of the mandible (jaw). “In people who grind or clench their teeth, it’s extra tension in this muscle that’s the culprit,” Dr. Liotta explains. “One of the big things that's coming up is next-generation bodies,” says [Anne Chapas, MD](https://www.unionderm.com/), a board certified dermatologist and medical director of Union Square Dermatology in New York City. While most of his practice’s patients seek a lean look, new techniques allow muscle definition, too. “By using artistic skill with our technology, we can now melt the fat down to just a few cells under the skin,” Dr. Neinstein says. “For athletic patients with a great underlying muscular framework, we can now create a hill and valley concept that gives a sexy and athletic look.” 

### **9. Pectoral Muscle Repair**
[Breast implant removal](https://aedit.com/aedition/what-happens-when-you-have-breast-implants-removed) made headlines in 2020 (thanks, Chrissy Teigen!). In 2021, Dr. Cassileth believes there will be a demand to not just remove the implant but also restore beautiful breasts. “No one ever repairs the pectoralis muscle because it was never considered a critical part of the implant removal,” she laments. While muscle repair isn’t required for explanting, she considers it a necessary complement for every under-the-muscle-implant removal she performs. “The pec muscle is not only attached to your sternum and ribs, it’s also attached to your arm,” Dr. Cassileth explains, adding it’s what allows you to do a pushup. When you cut into this muscle, it can weaken by 10 to 20 percent. “Now that so many more women are doing pushups, pilates, or plank positions, they are really noticing that their pec muscle strength isn’t close to what it was prior to implant,” she shares. “If the muscle is left unrepaired after explanting, it can actually be weaker without the implant since there isn’t tension holding it tighter.” Repairing the pectoral muscle allows patients to “live a full life,” she says.

### **10. TruBody System**
Dr. Chapas says that the TruBody System from Cutera will be a big body sculpting trend for 2021. It’s a two-step system that removes fat, rebuilds muscle, and renews the skin by combining the truSculpt® iD and truSculpt® Flex technologies. Using radiofrequency technology, the devices heat up the body to 45 degrees Celsius in 15 minutes to reduce about 24 percent of the fat right below the skin. “What we're finding in some of these kinds of technologies is that heating the fat is also getting those muscles revved up to work, so they have better metabolism,” Dr. Chapas explains. You can do multiple areas at once with TruBody. “It's really popular to do the abdomen and the arms or the front of the legs or the buttocks and the back of the legs,” she says, adding a series of about four treatments are usually needed for best results.

Emsella uses electromagnetic energy to deliver thousands of supramaximal pelvic floor muscle contractions in a single session. Originally used by gynecologists and urologists to treat incontinence, the technology is now being used for sexual wellness and body strengthening. “We found that when this treatment is used in conjunction with other things, like an abdomen treatment, you're creating an enormous amount of core strength, development postural improvement, and, yes, those sexual health improvements for both men and women,” Dr. Frank explains. If you’ve considered trying a DIY dermatology treatment this year, you are not alone. Between aesthetic services being curtailed by the COVID-19 pandemic and more time at home, the temptation to attempt an at-home treatment is real. While many of the procedure hacks you see on social media are a bad and, frankly, dangerous idea ([we talked to dermatologists about DIY disasters](https://aedit.com/aedition/dangers-of-diy-at-home-dermatology-beauty-treatments)), there are some that can be safe and effective ([beauty pros shared some of their favorites with us](https://aedit.com/aedition/aestheticians-dermatologists-share-at-home-skincare-tips)).

One treatment that is rising in popularity is face shaving. If you are familiar with this process, you most likely know how much hair and dead skin a razor removes from your face. So, if you’re seeking a smoother and brighter complexion, this can seemingly be a relatively simple and cost-effective treatment to improve skin tone and texture. But what do the experts have to say? We asked dermatologists for their thoughts on shaving your face at home.

## **At-Home Face Shaving vs. Professional Dermaplaning**
If you’re looking to remove peach fuzz and give your dull complexion a boost, face shaving is an at-home option that exfoliates the skin while removing dead skin cells. If it sounds a lot like [dermaplaning](https://aedit.com/procedure/dermaplaning), that’s because it is — though the tools are a bit different. “Professional dermaplaning is usually performed by an aesthetician or dermatologist, who uses a scalpel blade instead of a razor,” says [Vivian Bucay, MD](https://www.drvivianbucay.com/), a board certified dermatologist in San Antonio, TX. “Both remove peach fuzz, but dermaplaning usually provides a little more exfoliation, which might irritate sensitive skin.” 

Because a medical-grade scalpel is involved, professional dermaplaning requires a trip to the dermatologist. Oh, and there is quite a bit of skill involved. “[Dermaplaning] gets into little grooves in the skin that are difficult to do by yourself, like near your nose, eyebrows, and near your ears,” says [Morgan Rabach, MD](https://lmmedicalnyc.com/), a board certified dermatologist and co-founder of LM Medical in New York City. 

## **The Benefits of Shaving Your Face**
Whether you use a razor or the blade (far duller than a medical-grade scalpel) from a kit marketed for at-home use, shaving your face has multiple benefits. Smoother, brighter, and more even skin are the most noticeable ones. But there’s an exfoliating quality to this treatment, too. Since you are removing hair and dead skin cells with shaving, it leads to “better absorption of products with active ingredients,” Dr. Rabach explains. Another benefit? Shaving your face makes your makeup glide on better, Dr. Bucay, says. Since the skin is smooth, it glows more.

But that’s not all. From a convenience perspective, it’s important to note that you can perform the treatment when it fits in your schedule, as there is no need to schedule appointments with your dermatologist or aesthetic provider. “Shaving your face at home can be done at any time and is very inexpensive,” Dr. Bucay notes. “It’s a great option for those with busy schedules or those with tight budgets.”

If you are wondering how the hair will grow in after being shaved, know that it will not be any thicker than it was before. Because professional dermaplaning and at-home face shaving cut the hair mid-shaft, it does not impact the hair follicle. As a result, the texture will remain the same. 

## **How To Shave Your Face Properly**
Now that you know the basics, it’s time to get down to it. For starters, give yourself ample time when shaving your face. Rushing can result in damage to the skin. Before you start, make sure the face is clean and always use a fresh razor with a lubricating strip. To avoid nicks and cuts, Dr. Bucay cautions against dry-shaving. Instead, start with damp skin. “I also recommend stretching the skin to create a taut and even surface for better results,” Dr. Bucay says.

For best results, consider the following:

1. **Use New Tools**: Use a new, clean razor for each treatment. Any skincare treatment can cause sensitivity, but shaving your face at home may result in redness and swelling. Dr. Rabach says that you can experience irritation and little nicks in the skin. You’ve heard the phrase: *Just because you can, doesn’t mean you should.* But, between the YouTube tutorials and [COVID-19 quarantine closings](https://aedit.com/aedition/what-its-like-to-get-a-facial-during-covid-19), DIY beauty treatments can be so darn tempting. So, what’s safe to tackle at home and what’s better left to the pros? We asked two cosmetic dermatologists what skincare treatments we can and can’t take into our own hands.

## **The Rise of DIY Dermatology**
Our experts agree that more and more people are trying to recreate beauty treatments they’ve seen on social media — particularly Instagram and YouTube videos — from the comfort of their own homes. “I have had patients try many different cosmetic treatments during quarantine,” says [Samer Jaber, MD](https://aedit.com/provider/samer-jaber-md-3882), a board certified dermatologist and founder of Washington Square Dermatology in New York City. The promise of a quick fix combined with the chaos caused by coronavirus shutdowns has escalated the trend.

While some of these videos seem innocuous (think: the club soda face wash), others are downright scary. Interested in a full face of makeup using only nail polish? It’s got over 3 million views to date. How about DIY [microblading](https://aedit.com/aedition/microblading-treatments-for-fuller-thicker-eyebrows)? Nearly a million people (so far) have watched. A 25 percent at-home [TCA peel](https://aedit.com/procedure/tca-peel) at home? There have been over half-a-million views of that one.

It seems fairly obvious that attempting those types of procedures outside the confines of a doctor’s office, spa, or salon is a bad idea, but it’s important to understand just how dangerous a professional-strength treatment in the wrong hands can be. 

## **The Dangers of At-Home Beauty Treatments**
No matter your skin concern, pretending you're an at-home aesthetician or dermatologist can have dire consequences. “Since COVID-19, my telederm business has skyrocketed largely due to patients with concerns from unsupervised home care,” says Amy Spizuoco, DO, a board certified dermatologist and founder of [True Dermatology](https://true-dermatology.com/) in New York City. “Many patients have caused skin damage or burned their skin with what’s typically considered mild treatments, such as [vitamin C](https://aedit.com/aedition/guide-to-topical-vitamin-c-in-skincare) and even garlic.”

Both of our experts have treated patients who purchased chemical peels online (often from Amazon or foreign countries) and damaged their skin. “I have had patients try aggressive chemical peels during quarantine that they saw on YouTube or Instagram that, unfortunately, resulted in burns and a significant amount of hyperpigmentation,” Dr. Jaber cautions. While some [burns](https://aedit.com/aedition/how-to-treat-first-second-third-degree-burns), he says, are treatable with “appropriate medical prescription therapy,” not all patients are so lucky. 

Complexion tools and at-home devices can also do harm. “Many patients have bought comedone extractors and have put craters in their faces,” Dr. Spizuoco shares, adding that she’s also treated a patient who suffered broken capillaries from trying out a DIY “pore vacuum.” One of Dr. Jaber’s patients took mole removal into his own hands and swapped one concern for another. “I saw a young man that tried to remove a mole on his nose with bleach based on a video he saw on YouTube, and he traded a normal mole for a scar,” he says. 

On the mild side, the risks of trying aesthetic treatments like [microneedling](https://aedit.com/procedure/microneedling) — which creates micro-wounds to stimulate collagen production — at home include bruising and redness, though infection, scarring, and hyperpigmentation are all possible. The same holds true for eyebrow microblading, and there is the additional possibility of ending up with uneven, splotchy, or discolored brows due to poor quality pigments.

### **Don’t Be Fooled by ‘Natural’ Products**
And don’t be fooled by online promises of product hacks and ‘[all-natural](https://aedit.com/aedition/what-clean-green-natural-mean-for-beauty-makeup-skincare-products)’ beauty solutions. “Natural does not always mean safe or effective,” Dr. Spizuoco explains. “I’ve seen chemical burns with tea tree oil.” Dr. Jaber agrees. “‘Natural’ does not mean it is safer or better for your skin, and it does not mean hypoallergenic or better for sensitive skin,” he cautions. “Plant-based skincare products can irritate the skin.” After all, poison ivy is natural. 

## **Beauty Treatments That Are Safe to Try At Home**
When it comes to DIY dermatology, Dr. Spizuoco has a very short list of what she considers safe. “The only treatments I consider safe for home use are hydrating face masks, when used appropriately,” she says. “Pimple popping, comedone extractions, chemical peels, and microneedling, all should be left to the doctor’s office.”

Unfortunately, this sage advice isn’t stopping people from experimenting. But because there are so many factors that contribute to how our skin may react to a treatment (think: other skincare products, diet, sun exposure, prescription medications), it’s unwise to try anything without talking to your dermatologist first. “If you are going to do a treatment at home, please be aware of the risks,” Dr. Jaber warns.

With that in mind, there are a few professional treatments that have far less potent at-home versions that may be safe to incorporate into your routine.

### **1. Light Peels**
Dr. Jaber suggests that people who insist on trying an exfoliating chemical peel (after consulting with their doctor, of course) use something very superficial, like a [salicylic acid](https://aedit.com/aedition/guide-to-beta-hydroxy-acids-salicylic-acid-in-skincare) or [glycolic acid](https://aedit.com/aedition/guide-to-topical-alpha-hydroxy-acids-in-skincare) peel. “Make sure when you try your peel that your skin is not irritated or inflamed and always start with the lowest concentration possible,” he says. It’s important to discontinue use of any products containing salicylic acid or retinoids a few days prior to avoid unnecessary irritation and inflammation. “If you have skin of color, please be especially careful, as irritation or burning of the skin can result in discoloration,” he shares. Sure, your derm or aesthetician might make a treatment look easy, but that’s only because they have years of experience. “It's important to remember that dermatologists and plastic surgeons go to school for years so that they understand the anatomy of the face and potential complications and side effects of procedures,” Dr. Jaber notes. When in doubt, leave it to the pros.

Among the most common questions Dr. Jaber hears from patients:

- How can I prevent aging without breaking the bank? 
- What kind of sunscreen should I use?

## **What Your Dermatologist Wishes You Would Ask**
Whether it’s during an annual skin exam or their [first cosmetic consultation](https://aedit.com/aedition/questions-to-ask-your-doctor-during-plastic-surgery-consultation), questions naturally arise. But that doesn’t mean patients are checking all the boxes. Here are three questions patients aren’t asking that their dermatologists wish they would: 

### **1. Should I Really Get This Done?**
For Dr. Emer, pre-preventative care questions are vital. “When patients come to me after having had a bad prior experience or even a botched procedure, I definitely wish they would’ve asked more questions before jumping the gun,” he explains. “It’s so important to ask about all options so you can weigh out the risks and benefits. For example, ‘Would I benefit from [threads](https://aedit.com/procedure/pdo-thread-lift) or [filler](https://aedit.com/aedition/facelifts-vs-injectable-dermal-fillers-which-one-is-right-for-me)?’ The answer is easily ‘both’ but it is up to the patient to ask the deeper questions like, ‘Which product would complement my best features for a natural result and why?’ No two faces are the same, so it’s crucial to make a very clear blueprint of what you envision for your most desired outcome.” 

### **2. What Is in My Skincare Products?** 
“I wish people would ask more about ingredients in skincare and really be open to thinking about what they are using and how it affects their skin,” Dr. Rabach says. Whether you simply wash your face with soap and water or enjoy a multi-step routine, your regimen should, above all, be a well-researched one. 

To start, you must figure out what is best for your skin — not what is most buzzed about or what the current ‘problem-solving’ miracle product is. “Many cosmetic products claim certain things that aren’t necessarily scientifically proven or tested. Sometimes people ‘like’ a cream and aren’t open to using something that has more science behind it for the health and look of their skin,” Dr. Rabach explains. “If people enjoy or ‘relax’ by using certain products, that’s also important for self care. But certain active ingredients most definitely work better than others.”

Dr. Emer shares a similar sentiment, noting the importance of sourcing your products from reputable providers. “If you are in search of products that will be effective in treating the four main components of aging skin – texture/tone, laxity, movement lines, and loss of volume – you must choose top-of-the-line brands,” he says.  

### **3. How Can I Better Protect My Skin?** 
“We wish our patients asked more questions about skin cancer protection and the risks of sun damage,” Dr. Jaber says. Preventative care with the use of sunblock is paramount for your overall health and appearance. “The number one cause of aging is [sun damage](https://aedit.com/aedition/cosmetic-procedures-treatments-to-correct-sun-damage),” he explains. “It is important to remind our patients that sunscreen, while helpful and necessary, is not equivalent to actually blocking the sun. Even with sunscreen, spending significant time in the sun will still cause sun damage.” And the dangers of prolonged sun exposure go far beyond the cosmetic. “In addition to the negative cosmetic effects of aging, significant sun exposure also significantly increases their risk of skin cancers,” he warns.

### **4. Can You Please Explain This A Bit More?**
“I think the amount of information out there naturally overwhelms people, and by the time they end up at the dermatologist’s office, the degree of confusion is so vast that people aren’t sure how to vocalize their questions,” explains board certified dermatologist [Nikhil Dhingra, MD](https://springstderm.com/physicians/nikhil-dhingra/), of Spring Street Dermatology in NYC. Everyone is at a different point in their skincare journey, so confusion is natural. Dr. Dhingra recommends patients focus on what concerns pertain to them and to go from there. “Coming up with a list of your top three skincare concerns based on your own assessment and then asking your dermatologist what’s right for each concern is a good starting point,” he adds. 

## **Addressing Your Preventative Skincare Misconceptions** 
When it comes to cultivating a skincare routine, most people have the best intentions. But that’s not to say patients aren’t getting a few things wrong. From skimping on sunscreen to not scheduling annual skin checks, skin health is about more than the latest lotions and potions or cosmetic procedures. 

Speaking of skincare, Dr. Emer shares that what you see from Hollywood shouldn’t replace the insight of a dermatologist. “It’s bothersome when I read articles about celebrities using drugstore products – indicating that all anti-aging products work the same. They absolutely do not,” he explains. While sticker shock is real, there is usually a reason why the prices surge for certain types of products. “They have a high price tag for a reason. The ingredients are usually proprietary, rare, and sourced from exclusive regions of the world,” Dr. Jaber adds. “A drugstore retinol cannot be compared to a medical-grade product.”

Additionally, sourcing skincare advice online can create a skewed image of what a routine needs to look like for each individual. “Nowadays, with blogs and social media, there is this need to do too much to the skin and if we’re not, it means we’re not taking good care of ourselves,” adds Dr. Dhingra. “If you’re feeling overwhelmed or struggling to generate a routine for yourself, rather than diving through the bowels of the internet for information from random resources, seeking help from a dermatologist can be the most productive thing to do for your skin,” he explains.

## **How Often Should You Visit A Dermatologist, Anyway?** 
Generally speaking, all patients should visit the dermatologist at least once a year for a regular skin check. If you are also visiting your provider for cosmetic treatments and procedures, Dr. Rabach recommends scheduling an appointment two to four times a year for “maintenance of complexion, such as routine skin checks, chemical peels, Botox®, and more.”

Dr. Jaber warns that family history needs to be taken into account as well. “If you have a family or personal history of skin cancers, like basal cell carcinoma, squamous cell carcinoma, melanoma, you should come in at least every six months,” he says. It is important to remember that your skin is your body’s largest organ, and you need to pay attention to skin tone, dark spots, blemishes, sun spots, and any other areas of concern that might come up over a period of time. “You should also see a dermatologist if a certain mole worries you,” he shares. “For example, if a new mole appears or if a current mole because darker, develops an irregular border, or has any signs of itching, burning, bleeding, you should have it evaluated by your dermatologist.”

And those who have yet to schedule their first appointment or have skipped their annual check ups because they’ve sought out help online, Dr. Dhingra suggests coming in sooner rather than later. “There are so many arbitrary pseudo-scientific ‘rules' consumers feel they need to follow or they feel they’re doing their routines inadequately,” he warns. “By the time they come to our office, they’ve been through the wringer and they’re frustrated and need someone to guide them through the mess.”

2020 has arrived! As we enter this new decade, what big trends in both surgical and non-surgical cosmetic procedures will the new ‘roaring ‘20s’ bring? To find out, we tapped some of the biggest names in dermatology and plastic surgery to get their takes on what hair, face, and body trends we can expect to see in medical aesthetics in 2020.

## **2020 Hair Trends**
*No longer just about treating hair loss once it happens, hair restoration procedures can have a preventative effect as well.*

### **1. Platelet-Rich Plasma (PRP) to Prevent Hair Loss** 
PRP is now considered standard protocol for hair growth, says board certified dermatologist [Rita Linkner, MD](https://aedit.com/provider/rita-linkner-md-8907), of Spring Street Dermatology in NYC. The treatment, which works for both men and women ([Kourtney Kardashian documented her experience](https://aedit.com/aedition/kourtney-kardashian-prp-injections-to-treat-hair-loss) with the procedure on *Keeping Up With the Kardashians*), involves injecting platelet-rich plasma from the patient’s own body into the scalp to repair damaged hair follicles and encourage fuller, thicker growth. 

The treatment’s mechanism of action and injection protocols are being demystified daily, as the topic continues to come up in cosmetic journals. “PRP’s use as adjuvant treatment to lasers and as a stand-alone injectable has been gaining traction and will only continue to rise come 2020,” Dr. Linkner says. 

## **2020 Face Trends**

### **2. PRP Skin Resurfacing** 
Surprise! [Platelet rich-plasma](https://aedit.com/procedure/microneedling-with-prp) is also used on the skin. “The use of PRP with small amounts of additives, like neuromodulators and hyaluronic acid, delivered with handheld microneedle chambers to provide [mesotherapy](https://aedit.com/procedure/mesotherapy) to the superficial structures of the skin can provide complexion improvements that were only previously seen with laser resurfacing,” Dr. Linkner says of the technique that is often colloquially referred to as the ‘vampire facial.’  

### **3. New Filler Products**
New York City-based board certified plastic surgeon [Umabreen Mahmood, MD,](https://aedit.com/provider/umbareen-mahmood-md-23) says the U.S. Food and Drug Administration (FDA) will likely approve several new soft tissue fillers in 2020 that are already used abroad in Europe, “which will be an exciting addition to the current portfolio of fillers.” 

### **4. No-Downtime Laser Rejuvenation**
New year, new lasers! Board certified dermatologist [Claire Chang, MD](https://www.unionderm.com/about/dr-claire-chang/), of Union Square Laser Dermatology says that 2020 is all about [Picosecond lasers](https://aedit.com/procedure/nonablative-fractional-lasers). One of the newest lasers on the market, it can be used for everything from overall skin rejuvenation and skin brightening to the targeted treatment of fine lines, brown spots, melasma, acne scars, and unwanted tattoos. It has minimal downtime and is very well tolerated, she says. 

### **5. The “LitLift”**
Manhattan-based board certified facial plastic surgeon [Dara Liotta, MD](http://facialplasticsnyc.com), says that one of the biggest non-surgical requests she receives at her practice is for [general facial optimization with fillers and neurotoxins](https://aedit.com/aedition/how-to-reshape-your-face-with-filler-botox-injectables). Dubbed “the LitLift,” Dr. Liotta says “millennials, in particular, don’t want to look like someone else.” Instead, “they just want to ‘dust’ a bit of filler and Botox® around the face to make them look like their best selves without makeup,” she says. #NoFilterJustFiller is one of her favorite hashtags to see patients use after an appointment with her. “I like to say that the new trend for 2020 is to be a bit ‘high maintenance’ in my office chair once or twice a year in order to be ‘low maintenance’ all year long,” she explains. 

### **6. Minimally Invasive Skin Tightening** 
Dr. Chang believes minimally invasive skin tightening procedures like [Ultherapy®](https://aedit.com/procedure/ultrasound-skin-tighteningultherapy) will continue to be on the rise in 2020. “As the global life expectancy increases, many patients are searching for non-surgical, preventative treatments to start in their twenties, thirties, and forties to avoid or delay the need for more aggressive surgical procedures in the future,” she says. 

Ultherapy® uses microfocused ultrasound to stimulate collagen deep in the skin, causing a skin-tightening effect to prevent and treat sagging, loose skin. “With the advent of treatments like Ultherapy, which have been shown to be safe and effective in skin tightening and lifting, patients are reaching for less invasive rejuvenation options,” Dr. Chang says. 

### **7. Preventative Injectables** 
Board certified dermatologist and founder of Spring Street Dermatology [Sapna Palep, MD](https://aedit.com/provider/sapna-palep-md-8908), says the light use of neuromodulators ([think: Botox®, Dysport®, Jeuveau®, and Xeomin®](https://aedit.com/aedition/comparing-neurotoxin-botulinum-type-a-injections-botox-dysport-jeuveau-xeomin)) and fillers early in life has been clinically shown to have anti-aging benefits. “We see those results commonly in patients proactively using injectables in their twenties and thirties to resolve mild aesthetic issues, rather than investing in plastic surgery later in life as a full overhaul,” Dr. Palep explains. With the technical advancements of neurotoxins and fillers lasting longer and having additional flexibility within the skin, it’s possible to achieve natural results without a lot of product. It is important to note that less is always more and the earlier you begin small aesthetic procedures, the less maintenance will be required over time, she adds.

### **8. Bespoke Lip Augmentation** 
At her Manhattan practice, Dr. Liotta offers a custom, non-surgical lip treatment she calls “couture lips” that she expects will continue to gain popularity in 2020. “Lips are always ‘in,’” she shares. “More and more patients are willing to pay a bit more for a perfect, couture, natural-looking lip.”

The anatomy of the lip and mouth area is actually quite complicated area. The lips are composed of a number of different parts, including the philtral columns (i.e. the vertical lines that run from the cupid’s bow to the nose), the vermillion border (where you’d put lip liner),  and the body of the lip itself (where you put lipstick). “If you feel your own lips, you’ll appreciate that each of these areas have a different stiffness, and move differently,” Dr. Liotta explains. After analyzing the lips at rest and in movement, Dr. Liotta creates a custom lip augmentation plan using anywhere from one to three types of filler — plus or minus Botox® — to help create the perfect pout. 

### **9. Microneedling with Radiofrequency**
Whether you look at is as radiofrequency reaping the benefits of microneedling or vice versa, Dr. Palep says the combination of the two treatments is seriously amplifying results. “A wound-healing process ensues,” she says of the technique. “Results peak through within two weeks.” The procedures are blood-free due to the coagulation effects of the needles, which means patients rebound quickly post procedure. [RF microneedling](https://aedit.com/procedure/microneedling-with-radiofrequency) treatments like Vivace® further up the ante with the addition of red and blue LED light, which further speeds up the anti-acne and brightening effects. Another add on? PRP. “The complement of PRP to this laser treatment also minimizes downtime while maximizing results,” Dr. Palep says. “That increased efficacy is well-received by millennials.”

### **10. Masseter Botox® for TMJD and Jaw Slimming** 
“2020 is all about mixing wellness and beauty, and masseter Botox® is a great example of this,” Dr. Liotta says of the off-label use of the neurotoxin that can [treat temporomandibular joint disorder (a.k.a. TMJD or TMD) and slim the jaw](https://aedit.com/aedition/botox-to-treat-tmj-disorders-and-jaw-slimming). The masseter muscle sits at the back of the mouth at the angle of the mandible (i.e. angle of the jaw) and is one of the muscles activated during chewing. “In people who grind or clench their teeth, it’s extra tension in this muscle that’s the culprit,” Dr. Liotta points out. 

She says that Botox® can be used both functionally to temporarily weaken the masseter muscle (the main teeth-grinding muscle) and improve the pain of TMJD. “When Botox® is used to relieve the pain of TMJ and tooth grinding, patients can expect some pain relief in as little as one to two weeks,” she says. Cosmetically, Botox® can be used to thin the masseter muscle and take away a square look to the angle of the jaw, softening the look of the lower face. In that case, the maximum effect is not seen for six to eight weeks, Dr. Liotta notes. 

## **2020 Body Trends**
*The body contouring trend is here to stay, and 2020 will see the evolution of both surgical and non-surgical techniques.*

### **11. Non-Surgical Body Contouring**
“For 2020, trends in plastic surgery are towards a more sophisticated body experience where skill, technology, and human touch blend,” says New York City-based board certified plastic surgeon [Ryan Neinstein, MD](https://neinsteinplasticsurgery.com/), who has seen an uptick in body treatment requests. 

In his practice, non-surgical body sculpting options include [EmSculpt®](https://aedit.com/aedition/why-everyone-is-talking-about-emsculpt) (to help define abs and buttocks) combined with skin tightening and lymphatic drainage massages. “Here people get the combination of modern technology with the traditional warmth and professional feel of human touch,” he says. To complement those services, patients enter into a weekly body treatment program at the practice and undergo [light therapy](https://aedit.com/aedition/everything-you-need-to-know-about-the-benefits-of-light-therapy) (LED or infrared) to maintain the results. “These treatments help detoxify, recover, and firm the body from the wear and tear we all go through,” he says. 

Dr. Mahmood, meanwhile, notices that patients are increasingly bothered by skin laxity of the face, abdomen, arms (to name a few), but may not be ready to undergo a facelift or abdominoplasty. As a result, non-surgical skin tightening options using radiofrequency and heat are becoming increasingly attractive options. 

### **12. Reshaped Rears**
For over a decade now, a plump, shapely rear has been the aesthetic in the U.S. But Dr. Neinstein says he has started surgically reshaping overly filled buttocks from the [Brazilian Butt Lift](https://aedit.com/procedure/butt-lift) craze. “We are now reducing and sculpting these buttocks to give a more athletic and feminine look,” he says.

### **13. Body Shaping with Botulinum Toxin**
Many beauty trends travel to the U.S. from overseas, and South Korea is one of the pioneering countries when it comes to aesthetics, Dr. Chang says. “Dermatologists in South Korea were using platelet-rich plasma (PRP), oral tranexamic acid, [threadlifts](https://aedit.com/procedure/thread-lift), and botulinum toxin for jawline slimming for years before we were in the United States,” she notes. 

Next up? Botulinum toxin treatments for other body parts. “In my practice, I place botulinum toxin injections to slim the muscles of the neck, shoulders, calves, and thigh bulge for body contouring,” says Dr. Chang. The effects can be seen in as little as  four to six weeks and last an average of six months. “An increasing number of patients have started asking for this treatment recently,” she says. “And it will likely rise in 2020.”   

### **14. VelaShape III for Cellulite** 
[VelaShape III](https://aedit.com/procedure/velashape) is a procedure used to reduce the appearance of cellulite and provide circumference reduction of the thighs by using a combination of technologies that deep-heat fat cells and surrounding tissue. The technology has improved in regard to the delivery of the energy to provide more wattage, which speeds up the heating of the tissue more comfortably for patients, says board certified dermatologist [Jennifer Chwalek, MD](https://www.unionderm.com/about/dr-jennifer-chwalek/), of Union Square Laser Dermatology. Each thigh requires just 15 minutes of treatment, though a series of one treatment a week for three or four weeks is recommended. Patients will see results after the third treatment, and Dr. Chwalek notes it’s particularly popular in the summer or before winter beach vacations. 

### **15. Body Injectables**
“Dimples, cellulite, stretch marks, and lax skin can all be targeted with Sculptra®, which is in the process of running FDA trials to approve these off-label uses,” says Dr. Linkner. Because there is no laser that can currently address these issues, [customizable injections](https://aedit.com/aedition/everything-you-need-to-know-about-dermal-fillers) are needed to fill this niche, she adds. Because of the semi-permanent nature of polylactic acid, or PLLA, Sculptra® results last anywhere from two to five years, which she believes is the strongest draw for clients.

### **16. Muscle Contouring**
The philosophy surrounding body contouring is shifting, and the focus is now on refining muscles, says Dr. Palep. “With technologies like FlexSculpt that work to exercise key core muscles groups, we sculpt patients in a way that can’t be achieved with a trainer in the gym,” she explains, adding that refinement means even those with BMIs under 25 who might not be ideal candidates for fat-contouring machines can now non-invasively treat something for which liposuction was once the standard. “No downtime and quick treatments make this lunchtime procedure one of the most popular offered at Spring Street Dermatology,” she says.

According to the American Society of Plastic Surgeons’ [2018 Plastic Surgery Statistics Report](https://www.plasticsurgery.org/documents/News/Statistics/2018/cosmetic-procedure-trends-2018.pdf), the market grew 163 percent between 2000 and 2018, and the trend is expected to continue tracking upward. 

Among the go-to treatments? Injectables (think: [Botox®](https://aedit.com/procedure/botox-facial) and filler) and non-invasive fat reduction, which just so happen to be among the best ways to non-surgically lift, firm, plump, and contour the face and neck. Whether you are looking to combat visual signs of aging like volume loss, fine lines and wrinkles, and sagging or are simply looking to further define your facial features, there is an array of non-invasive cosmetic procedures that can reshape the face — sans surgery.

## __Dermal Filler__
Fillers aren’t just for plumping your pout. [Soft tissue fillers](https://aedit.com/aedition/everything-you-need-to-know-about-dermal-fillers) like Juvéderm®, Radiesse®, Restylane®, and Sculptra® can be used to re-shape the entire face. Made of hyaluronic acid, collagen, calcium hydroxylapatite, and poly-L-lactic acid (PLLA) to name a few, filler can be injected into the skin to restore volume, smooth out fine lines and wrinkles, and enhance facial contour.

While it probably comes as no surprise that dermal filler is often employed under the eyes to eliminate bags, in the mid-face region to sculpt cheekbones or add volume to sagging nasolabial folds, and on the lower face to define the jawline, a board certified dermatologist or plastic surgeon can recommend less obvious uses as well.

“Dermal fillers can be placed on the bone and within the muscle of the chin to augment its shape and projection,” explains board certified dermatologist [Rita Linkner, MD](https://aedit.com/provider/rita-linkner-md-8907), of New York’s Spring Street Dermatology. “The chin is oftentimes forgotten but adds so much to facial structure. A syringe of filler in this delicate area goes such a long way.”

Using filler to augment the chin is a non-surgical solution to refine a facial feature that previously would have required a more invasive cosmetic procedure — like a [facial implant](https://aedit.com/procedure/alloplastic-chin-implant) — to correct. So-called [liquid rhinoplasties](https://aedit.com/procedure/liquid-rhinoplasty) and [chemical brow lifts](https://aedit.com/procedure/chemical-brow-lift) are other common examples of filler being used to mimic the effects of traditional plastic surgery.

One key difference, however, between non-surgical cosmetic procedures and their surgical counterparts is the level of permanence. The results of dermal fillers, while immediate, last an average of six months to two years (depending on the region being treated) compared to the much longer lifespan of surgical procedures. But the downtime associated with filler is minimal, with only about a week (or less) of minor redness, swelling, or bruising.

## __Botulinum Toxin Type A Injections__ 
Easily confused with dermal filler, botulinum toxin type A (BoNT-A) injections (read: Botox® and its competitors) work in an entirely different way. While dermal filler quite literally fills in the hollows of the face to restore volume and fullness, [BoNT-A injectables](https://aedit.com/aedition/comparing-neurotoxin-botulinum-type-a-injections-botox-dysport-jeuveau-xeomin) like Botox®, Dysport®, Jeuveau®, and Xeomin® are FDA approved to reduce the appearance of fine lines and wrinkles caused by repeated facial expressions (think: frowning, squinting, smiling, and the like) by injecting it into the muscles underlying the affected area. But their usefulness goes far beyond smoothing wrinkles.  

BoNT-A injections can be used to complement cosmetic surgery procedures like [blepharoplasty](https://aedit.com/procedure/upper-eyelid-blepharoplasty) (a.k.a. eyelid surgery) or [brow lifts](https://aedit.com/procedure/traditional-midbrow-lift) and can be strategically injected to refine the face shape or define facial features. Botox® and the like can be used to slim out a round face, lift the corners of the mouth, and even soften the jawline (ideal for teeth grinders or those lacking definition).

“I inject a neurotoxin into the masseter muscle to slim the face very commonly,” says Dr. Linkner. “It turns a square-shaped face into a more aesthetically pleasing oval. Studies have been done that show the jawbone itself slims on CT scans with toxin use in the masseters. As the muscles exert less pressure on the bone, the osteoclasts start to reshape the bone itself.”

Like dermal fillers, BoNT-A injections are temporary. Once injected, the results can take up to a week to kick in (patients may experience minor bruising and pain at the injection site) and typically last around three to six months.

## __Kybella®__
Another player in the minimally invasive injectable game? [Kybella®](https://aedit.com/procedure/kybella).

A form of deoxycholic acid (i.e. the naturally occurring molecule that aids in the breakdown and absorption of dietary fat in the body), Kybella® is FDA approved to reduce fat under the chin and thus improve the contour of the neck. Off-label uses include breaking down stubborn fat deposits on the stomach and back and above the knees and elbows.

When deoxycholic acid is injected directly into the submental fat under the chin, the fat-storing adipose tissue is destroyed. Over time, the fat cells are removed from the treated area via the body’s natural metabolic processes.

The best thing about Kybella®? The results are permanent. Dr. Linkner says that the body metabolizes the destroyed adipocytes (fat cells) over the proceeding six to eight weeks. Unlike [liposuction](https://aedit.com/procedure/power-assisted-liposuction), the fat will not redistribute, but most patients will need multiple sessions (up to six), spaced at least a month apart to see the most optimal results. 

Additionally, there is a bit more downtime associated with Kybella® than, say, Botox® or dermal filler. “Expect a 36-hour period of swelling that truly peaks 12 hours post-procedure,” Dr. Linkner warns. “Bruising is common, so I give all my patients arnica post-Kybella.”

## __PDO Thread Lift__
While nothing can replace the long term lifting and firming benefits of a surgical [facelift](https://aedit.com/procedure/smas-facelift), thread lifts are a worthy non-surgical alternative. Ideal for patients looking for facial rejuvenation (especially as it relates to the lower face and jowls), the minimally invasive cosmetic procedure involves the subcutaneous placement of polydioxanone (PDO) sutures to lift sagging tissue.

The winter months are perfect for hibernation, but once the warm weather hits, our social calendars start picking up again. Summer is the peak season for social engagements like weekend BBQs and weddings. While there are a number of cosmetic procedures that are not recommended during the summertime (mostly because they can cause sensitivity to sun exposure), there are five treatments we highly recommend to put your best foot forward.

## **Body Sculpting**

Body sculpting, also known as body contouring and body shaping, is a treatment that removes fat deposits under the skin. There are many types of treatments that fall under the body sculpting umbrella, but some are more invasive than others. For instance, liposuction is a surgical procedure that permanently removes fat cells and has a longer downtime. Because of its invasive nature, [liposuction](https://aedit.com/procedure/liposculpture) often requires patients to wear a compression bandage for several weeks to aid the healing process. Fortunately, there are other body sculpting alternatives that will allow you to bounce back quicker and squeeze in more beach time.

Non-surgical body sculpting treatments include technologies like [CoolSculpting®](https://aedit.com/procedure/cool-sculpting), [SculpSure®](https://aedit.com/procedure/sculpsure), and UltraShape®. Instead of surgically removing the fat, they work by freezing fat cells or administering intense heat by means of ultrasound or radiofrequency. Like liposuction, they can be used to reshape a variety of areas, like the stomach, thighs, love handles, butt, or upper arms. There is a minimal recovery period though many patients experience some redness and swelling afterward. 

Each body sculpting procedure does something different. For instance, cryolipolysis devices, such as CoolSculpting®, destroy excess fatty tissue but do not do much for skin tightening. Radiofrequency devices like Ultherapy® and Exilis® are skin tightening procedures that do not remove much fatty tissue. 

“Many patients do a combination of both the fat reducing and skin tightening modalities to hit both issues to improve body sculpting for the summer,” board certified plastic surgeon [Roger Tsai, MD](https://aedit.com/provider/roger-tsai-md-556), shares. “As far as which is better than the other, they are all fairly comparable and will depend on each person's individual goals.” 

It is important to keep in mind that the treatments can take 12 to 16 weeks to see full results and that results will be significant but not as strong as a surgical procedure like liposuction. So make sure to plan ahead – you won't see overnight results.

For non-invasive body sculpting treatments, it can take at least two to three treatments to get the desired results. “Although there is significantly less downtime and patients can go back to normal activities right away, it is not quite as powerful and sometimes takes longer to get the desired results due to the number of treatments it takes to get there,” Dr. Tsai explains. “If the surgical route is performed, this will provide the most powerful sculpting and is done in one shot. However, depending on what is actually performed, it could take one to three months for the body to heal.”

## **Dermal Fillers**

Unlike a lot of other facial treatments, like [chemical peels](https://aedit.com/procedure/medium-depth-chemical-peels), injectables do not have negative interactions with the harsh summer sun, making it a perfect treatment for summertime. [Injectable dermal fillers](https://aedit.com/procedure/cheek-augmentation-with-fillers) are soft tissue fillers that restore volume and smooth out fine lines. They work quickly to reduce signs of aging and have minimal downtime, and last anywhere from six months to two years. Which means you can get back to your summertime activities in no time. 

Board certified dermatologist [Cheryl Karcher, MD](https://aedit.com/provider/cheryl-karcher-md-4323), recommends dermal filler during the summer because it does not matter if you are exposed to the sun or not. “Filling up a little hollowing [under the eyes](https://aedit.com/procedure/under-eye-filler) can make one look very refreshed for the summer,” Dr. Karcher says. “Putting a little [filler](https://aedit.com/procedure/temporary-lip-fillers) where the lips turn down can make one look refreshed and happy.”  

There are many types of dermal fillers, but the most common include hyaluronic acid (e.g. Juvéderm®, Belotero® or Restylane®), collagen, calcium hydroxylapatite (e.g. Radiesse®), and PPLA (e.g. Sculptra®). Dr. Karcher recommends any of them for use during the summer. 

## **Botox®**

Similar to [dermal fillers](https://aedit.com/procedure/liquid-facelift), [Botox®](https://aedit.com/procedure/botox-facial) is another non-invasive way to smooth out unwanted facial lines and wrinkles. Botox® is a brand name for the generic drug botulinum toxin, which works by temporarily paralyzing facial muscles. 

In addition to reducing signs of aging, it can also slim out the face, making it the perfect alternative to more invasive facial slimming procedures. Botox® gets injected into the muscle responsible for chewing, called the masseter muscle. It relaxes that muscle, rounding out the jawline and slimming down the cheeks. Dr. Karcher recommends Botox® in the crows feet to help keep from squinting or in the frown lines keep from looking upset. It can also be used in the armpits to help curb excess sweating.

There is no downtime with Botox®, and sun exposure does not affect it. “There’s no special skincare needed after Botox®,” Dr. Karcher says. “You can continue whatever skincare process works for you.” 

It can take around two weeks to see noticeable results and its effects will last around three to four months, giving you a slimmer looking face all summer long.

## **Ultherapy®**

If you are looking for something a little more impactful than Botox® or fillers, [Ultherapy®](https://aedit.com/procedure/ultrasound-skin-tighteningultherapy) might be a good option. The procedure is different than traditional [laser treatments](https://aedit.com/procedure/pulsed-dye-laser) because it uses ultrasound energy which stimulated collagen production. As a result, it tightens and lifts the skin, giving its patients a more youthful appearance. Similar to a [facelift](https://aedit.com/procedure/deep-plane-facelift), it treats loose skin around the neck and chin and smooths out unwanted lines and wrinkles around the chest.

Ultherapy® is a non-invasive treatment so its results are not as strong as a traditional facelift, but it is a great alternative for those who are not ready to commit to a surgery or who want a speedier recovery.

Summer is great for a lot of things, but not always for cosmetic treatments. Here are the best options that won't cut into your beach time.

Skin treatments are more than just surface level. Results depend on a variety of factors that extend far beyond skin type and one of those factors we don't often think about is the skin's color. During my first laser [hair removal](https://aedit.com/procedure/laser-hair-removal) consultation, I was told I would require additional treatments over a longer period of time due to my darker complexion. I was new to the world of cosmetic procedures and this was the first time I'd heard that my skin color could affect the outcome of my treatment. It turns out that our complexions play a significant role in the outcome of certain procedures, especially lasers. 

## **Why Different Complexions Need Specific Treatments**

The issue with lasers and some other technologies when treating different skin colors, and the reason behind adverse outcomes on darker complexions, is the amount of energy absorbed by the skin. Darker skin types absorb more energy and are thus prone to developing hyperpigmentation, irregular pigmentation, and hypopigmentation. Lighter skin types, on the other hand, don’t absorb energy and respond directly to the treatment.

For a long time the only options were a range of diode and alexandrite lasers, which still carry a risk of pigmentation changes. It’s only within the last 10 years that traceless cosmetic treatments have become available for darker complexions. We now have the choice of lasers and microneedling radiofrequency devices that can be used on darker skin types, with little to no side effects.

But even with the increasing amount of available treatments, people with darker complexions still need to search out doctors who know how to accurately treat darker skin tones. Board certified dermatologist [Jason Emer, MD](https://aedit.com/provider/jason-emer-md-722), acknowledges that some doctors don’t know how to successfully treat darker skin and aren't informed of the side effects certain cosmetic procedures have on dark skin types.

“There are a lot of doctors claiming that [Erbium lasers](https://aedit.com/procedure/ablative-erbium-laser), or what are called ‘cool lasers’, and even [CO2 lasers](https://aedit.com/procedure/ablative-co2-laser), are safe for darker skin types," he says. "These lasers are ablative and burn the skin – they’re very high risk. All ablative lasers, anything that’s delivering a lot of inflammation into the skin, should be avoided.”

Some doctors also use [IPL hair removal](https://aedit.com/procedure/ipl-for-unwanted-hair), which should be avoided when treating darker skin types, since it has a chance of developing both hypo and hyper pigmentation. Another laser that is damaging, and used regularly, is [Fraxel®](https://aedit.com/procedure/fraxel-repair). If used incorrectly, Fraxel can be extremely dangerous and cause rebound hyperpigmentation, burns, and damage to the overall skin texture. 

When it comes to skin rejuvenation, Dr. Emer suggests people with dark skin avoid energy-based treatments and focus on peels, skincare, and at-home dermal rolling. Here are some of his favorites.

### **Chemical Peel**

A signature treatment of Dr. Emer's, Enlighten is a face peel designed to treat darker skin types for pigmentation, dark spots, and pores. It contains retinol, hydroquinone, kojic acid, vitamin C, and licorice root for anti-aging, skin brightening, and lightening. The peel can be done every month or two to brighten the skin tone without risk of damaging skin pigment, unlike other [chemical peels](https://aedit.com/aedition/chemical-peel-vs-laser-treatment-what-is-best-for-your-skin).

### **Microneedling With Radio Frequency**

This three-step treatment exfoliates the outer layers of skin, deposits customized serums into the skin, and uses LED therapy to decrease inflammation. It’s an all-in-one treatment that can be done every month to maintain youthful, rejuvenated skin.

### **YAG Laser**

While laser hair removal seems straightforward, a darker complexion can confuse the laser into treating the skin, rather than the hair, because it targets dark pigment. The YAG laser treatment is a good alternative to traditional hair removal lasers, as it flashes so quickly that the upper surface of the skin does not get affected and the laser only works on the hair follicles. The YAG laser is the ideal hair removal option for dark complexions, such as African or South Asian skin tones.

Regardless of the procedure, to achieve the best results you’ll need to combine your treatment with vitamin C and glycolic creams to reduce inflammation in the skin and promote exfoliation, further reducing any risk of pigmentation.
