If you spent any time on TikTok over the summer, you likely saw an endless stream of videos extolling the virtues of #TrapTox. The alliterative hashtag refers to the injection of a neurotoxin into the trapezius muscles to slim and relax the area for both aesthetic and functional benefits. 

While it seems like the ‘trap Botox’ trend came out of nowhere, it’s actually been an established – albeit under-the-radar – off-label use of botulinum toxin type-A injections for some time. So, what are the benefits of injecting Botox® and the like into the trapezius muscles and who is a candidate for treatment? Top injectors break it down.

## **Anatomy 101: The Trapezius Muscle**
Before we dive into trap Botox, let’s start with a quick anatomy lesson. The trapezius is a large, superficial back muscle that gets its name from its shape – it resembles a trapezoid. It starts at the base of the neck and extends across the shoulders and down the middle of the back. 

The main function of the trapezius (a.k.a. ‘the traps’) is stabilizing the scapula (read: shoulder blades) and controlling it during movements of the shoulders and arms (think: throwing, shrugging, raising your hand, sitting up straight, etc.). “Many of us hold tension in our shoulders,” says [Kiran Mian, DO](https://www.drkiranmian.com/), a board certified dermatologist at Hudson Dermatology & Laser Surgery in New York City. “We also hold postures in our day to day that require our shoulders to be hunched over, like working at a computer, sitting at a desk, or even on our phones.” 

The overuse of the trapezius causes hypertrophy or enlargement, Dr. Mian notes, which can lead to both functional (think: physical discomfort) and aesthetic concerns. “Most people have a bulge where the neck meets the shoulder,” says [Nigar N. Ahmedli, MD](https://www.montefiore.org/doctors/profiles/1790164341/nigar-n-ahmedli), a board certified otolaryngologist and director of facial plastic and reconstructive surgery at Montefiore Medical Center in NYC. “It can interrupt the flow of the neck, thus shortening the appearance of the neck.” This is where trap tox comes in.

## **What Is ‘Trap Tox’?**
Whether you call it ‘Trap Tox’ or ‘Barbie Botox’ (yes, the movie mania infiltrated aesthetic medicine, too), the treatment is the same. Neurotoxins like Botox®, Dysport®, Jeuveau®, and Xeomin® are injected into the trapezius muscle to address physical and/or cosmetic concerns. “Physically, Botox injections into the trapezius muscle can make the two sides more even, less bulky, and/or more proportional to the client’s overall frame,” says [Kristina Kitsos, RN](https://www.instagram.com/kristinakitsos/?hl=en), a registered aesthetic nurse in Beverly Hills. “It can also make the neck appear longer and shoulders thinner, which is why many people call it ‘Barbie Botox.’”

Much like any other botulinum toxin type-A injection, the treatment is entirely customizable. “Injections of Botox into the trapezius muscle are targeted toward specific regions within the muscle to create the desired effect – ranging from enhancing the shape of the muscle, promoting symmetry, and refining proportions or slimming the muscle,” Kitsos says. “Based on the patient’s individual goals, placement and dosage is modified.”

### **Aesthetic Benefits of Trap Tox**
Much like Botox is injected into the jaw to slim the lower face, it can be injected into the trapezius to improve the contour of upper body. “Neurotoxin injections in the trapezius can have a slimming effect on the neck,” says [Margarita Lolis, MD](https://www.schweigerderm.com/providers/margarita-lolis-m-d/), a board certified dermatologist and Mohs surgeon in New Jersey. “This is because the neurotoxin itself lowers the trapezius muscle, making the neck appear longer.” 

The injections “elongate the neck, creating a more feminine contour between the neck and shoulders,” Dr. Mian explains. The swan-like effect creates the appearance of a more delicate upper body that is likened to a Barbie doll.

### **Functional Benefits of Trap Tox**
We may associate Botox with wrinkle reduction, but BoNT-A injections are also used to treat muscle spasms associated with TMJ, lazy eye, vaginismus, hyperhidrosis (read: excessive sweating), and, yes, neck and shoulder pain. “Trap tox can relax tension or spasm in the trapezius muscle,” says[ Jennifer Levine, MD](https://www.drjenniferlevine.com/), a double board certified facial plastic and reconstructive surgeon in NYC. 

At their core, neuromodulators like Botox®, Dysport®, Jeuveau®, and Xeomin® relax muscles. In the forehead, for example, that improves the appearance of wrinkles. In the upper back, that can ease stress and discomfort. “The muscles of this region – including the paired trapezius muscles – can tighten and spasm, leading to neck and shoulder pain as well as tension headaches,” Dr. Ahmedli explains. “Injecting the trapezius muscles with neurotoxin relaxes the muscles, allowing the tightness to improve and, therefore, helping decrease pain and headaches.”

## **The Ideal Candidate for Trap Tox**
A candidate for trap tox could be someone looking to relieve tension in the neck and shoulders, someone looking to slim the area, or someone looking for a bit of both. “The ideal patient is someone with stiffness or pain in the trapezius muscles, people with overdeveloped trapezius muscles, and anyone who has neck and shoulder pain from overuse of their trapezius muscles,” Kitsos shares. “A good candidate would be anyone who would like to create a more svelte, longer-looking neck and less bulky shoulders.”

Injectors will study a prospective patient a few ways to determine candidacy. “The ideal patient for trap tox has palpable trapezius muscles – the provider can feel the muscle with their hands – with a noticeable bulge,” Dr. Ahmedli says. “The neurotoxin injection will diminish the bulge and create a sleek neckline.” Body language can also be a sign. “The ideal patient for trap tox is someone who subconsciously hunches their shoulders, causing increased tension in the muscle that builds over time as well as hypertrophy,” Dr. Mian adds. 

The treatment, however, is not for everyone. Generally speaking, neurotoxins are not recommended for those under 18, women who are pregnant or nursing, anyone with an allergy to the components of the injectables, or those with certain chronic medical conditions. When it comes to trap tox specifically, “one should avoid this treatment if they have neuromuscular conditions, nerve damage in that area, or any injury or generally weak musculature in that area,” Dr. Mian cautions. 

## **What to Expect from a Trap Tox Treatment**
If you’ve ever had neurotoxin injections on the face, Dr. Levine says you can expect pretty much the same experience when it comes to trap tox preparation, in-office experience, and recovery. “Much like facial Botox injections, trapezius muscle injections have little to no pain and take just a couple of minutes to administer,” Kitsos notes.

On the pain point, facial Botox injections rarely feel like more than a pinprick. Trap tox can have a slightly stronger – though still manageable – sensation. “A longer needle is used to get to the muscle, as the trapezius lies deeper than facial muscles,” Dr. Mian shares. “It can cause a little bit more discomfort during the procedure.” If pain is a concern, Dr. Lolis says patients may choose to numb for 15 to 30 minutes prior to treatment. 

What is different between trap tox and facial Botox is the amount of product needed. “We are talking about much higher doses of toxin for the trapezius because it is a much more dynamic and stronger muscle compared to your facial muscles,” Dr. Lolis notes. While the actual dosage will depend on the patient and product, there are some general rules of thumb. “Because the trapezius muscles are much larger than the muscles in the face, the dosage in the trapezius is often two to three times that of popular areas like crow’s feet and frown lines,” Kitsos says.

Each brand of neurotoxin requires slightly different dosing, but Dr. Ahmedli shares that if you are used to getting 20 to 35 units of Botox® in the forehead, the trapezius may need 40 to 75 “depending on the muscle size and tightness.” Dr. Levine offers a similar guesstimate. “The trapezius is a very large muscle, so it takes many units to get an effect,” she shares. “If it is 10 to 20 units for the forehead, the trapezius may need 50 or more per side.” Neurotoxins tend to be priced per unit based on the provider and location, but you can expect to pay around $1,000 for a trap tox treatment.

For the best experience, Dr. Lolis recommends avoiding exercising and lifting heavy weights on the day of the injection to ensure the traps are “as relaxed and tension free as possible.” Post-injection, avoid alcohol, strenuous activity, and massaging the treated area for the first 24 hours. “Patients mainly need to be careful not to apply any pressure on the shoulders/trapezius muscles for a few hours after injections,” Dr. Lolis adds. 

## **When to Expect Results from Trap Tox**
If you’ve ever had a neurotoxin treatment, you know the results are not instant. In the case of trap tox, the results are often staggered. “Patients will start to notice a difference a few days after injection as the muscle begins to feel less tight,” Dr. Ahmedli says. “Maximal effect is seen at the two-week mark.”

Depending on the product used and the patient’s lifestyle and metabolism, the effect can last anywhere from two to six months. In some cases, more than one treatment may be needed to enjoy the full aesthetic and functional benefits. “Some patients will see results after the first treatment, while others may take more sessions,” Dr. Mian notes. 

While “the physical relief of tension will be felt after the first treatment,” she says, the aesthetic results “of elongated neck-shoulder curvature” may take a second treatment because “alleviated hypertrophy of the muscle can take some time.” In other words, “the muscle needs time to shrink now that it’s not being recruited constantly,” Dr. Mian adds.

### **A Note About Muscle Movement**
Neuromodulators temporarily paralyze the muscles they are injected into until the toxin wears off, so are you basically opting for partial neck paralysis when you get trap tox? Not exactly. “Ideally you want to obtain the effects of tension release and aesthetic results without affecting functional movement,” Dr. Mian shares. “The ideal dosing lies in that balance between relaxing the muscle enough to achieve those results, while still being able to have movement like shrugging your shoulders.”

That’s not to say you won’t notice a difference. “At first, the muscle may be a bit weaker than normal,” Kitsos says. “After the neurotoxin wears off, patients will find that their trapezius muscles will go back to normal movement and they will once again be able to easily shrug their shoulders.” In Dr. Ahmedli’s experience, it’s mostly about tension. “The main difference patients notice is a decrease in the tightness in the shoulders,” she says. “As the neurotoxin wears off, the tightness will return.”

These are all things to talk to your injector about prior to treatment. “There is definitely a potential for muscle weakness in the treated area, which can affect muscle function,” Dr. Lolis shares. “You can potentially lose muscle since it is not being worked, and the other potential downside is that the two sides might be asymmetric.”

## **The Takeaway**
Yes, trap tox can give you a longer neck and more contoured upper body. But it is really a game changer for those who experience chronic neck and shoulder pain as a result of stress or tension. It goes without saying that looking at a photograph is not the same as seeing something in real life. A two-dimensional snapshot simply cannot capture the nuance of a three-dimensional world, yet selfie and [social media culture](https://aedit.com/aedition/march-2020-study-shows-relationship-between-social-media-cosmetic-surgery) too often fails to respect that important distinction. Whether it’s through makeup, lighting, or face tuning apps, the photos that fill our feeds are not necessarily indicative of reality. But, increasingly, people are turning to cosmetic treatments to recreate a specific aesthetic that might look great in a picture but doesn’t translate to a natural-looking appearance in person because it lacks the movement and expression we expect to see. This is where an additional dimension of beauty – one that accounts for life in 2D *and* 3D – comes into play.

Recently, we hosted an Instagram Live with [Julius Few, MD](https://aedit.com/provider/julius-few-md-15580), a board certified plastic and reconstructive surgeon in Chicago and creator of [Aforé Beauty](https://www.aforebeauty.com/), to learn more about his concept of “four-dimensional beauty” and how it influences his approach to aesthetic medicine to achieve the most natural and holistic results for his patients. ## **1. Defining Four-Dimensional Beauty**
Dr. Few has been in practice for over two decades. While the original outpost of [The Few Institute](https://www.fewinstitute.com/) is in Chicago, he has a satellite office in Los Angeles where he tends to some very famous faces. As he explains, treating public figures has taught him a lot about our understanding and perception of attractiveness. “One of the things that struck me was how I could see a household-name celebrity, who, in a photograph or in a movie, looked a certain way and by all accounts was attractive,” he shares. “But you saw them in person and they went to even a more dramatic and more appealing level.” 

It’s that dichotomy between what he calls the “in-person encounter” and the “print version” that led him to formalize the idea of four-dimensional beauty. “What that represents is the concept that we deal with the three dimensions we all know – length, width, and distance – and the fourth dimension is time and moments in time,” he explains. “We're not talking about days, hours, minutes – we're talking about microseconds of movement and what is communicated both in terms of verbal and non-verbal communication.” 

While there are extreme examples of people overdoing it, even just a little too much filler or neurotoxin can look off if it impacts the natural movement of the face, he says. “That really is what four-dimensional beauty represents – that transition from moment to moment in interpersonal interaction that can either make or break a moment,” he shares. 

## **2. Photo vs. Video**
Even if your in-person schedule is not quite back to pre-pandemic strength, days spent in Zoom meetings, scrolling through TikTok, or rolling your eyes at Instagram’s ever-conspicuous push for video content are a reminder that we do, in fact, live in a three-dimensional world. The best way to look your best IRL is to consider how the result of whatever you are having done is going to impact your features and expressions. 

Dr. Few says the onus is on the provider to properly assess the patient. “Probably the most important thing that any cosmetic plastic surgeon, dermatologist, or provider of any kind really needs to hone in on is the idea that you have to look at animation during that in-office visit,” he shares. One way to educate the patient and further drive home the point is by supplementing traditional [before and after photos](https://aedit.com/aedition/importance-of-before-and-after-photos-in-plastic-surgery) with video. “I've started to incorporate more and more videography into the assessment, so I can show people,” he says. “I do believe that's going to become the norm in many ways.” 

With the ability to put a smartphone with a 4K camera into the palm of anyone’s hand, all providers need to do is standardize their lighting setup and positioning (à la traditional B&A photos). Dr. Few and his team recently [published a study](https://academic.oup.com/asjopenforum/article/doi/10.1093/asjof/ojac002/6515787?login=false) in the *Aesthetic Society Journal* based on the clinical results of using the [Aforé Beauty CR Emulsion](https://www.aforebeauty.com/products/cr-emulsion) – a serum that blends 0.2 percent retinol, [cannabidiol (CBD)](https://aedit.com/aedition/cbd-beauty-skincare-wellness-benefits), peptides, and hyaluronic acid to improve skin tone, texture, and hydration – that included a dynamic video component. “It gave us a platform to develop the videography assessment tool, which is in its infancy,” he says. “We're still working on it in terms of validating and putting it together, but we're well on our way.”

Long term, Dr. Few sees video as “a great equalizer” that will foster transparency. “There will be more and more of a demand for influencers and the like on social media to have videography versus a static picture,” he notes. “What different people got away with just because they had a great 2D picture, that's not going to be good enough.”

## **3. The Rise of Prejuvenation**
It’s usually easier to prevent something than correct it, and that is certainly true of age-related skin and aesthetic concerns. There is a lot to be said about rejuvenating procedures, but, in the last decade, the conversation has expanded to include the idea of ‘[prejuvenation](https://aedit.com/aedition/i-was-practicing-prejuvenation-before-it-was-a-trend)’ (i.e. preventative treatment). “I kind of came up with that term ‘prejuvenation’ at a multi-specialty meeting probably 10 or 11 years ago,” Dr. Few shares. “The whole concept came up during a panel because, at that time, I was just starting to really kick into taking care of a lot of models and actors, and that group really was keen on the idea of avoiding the knife.” Instead, they were interested in less invasive solutions that they could start earlier in life to potentially delay or eliminate the need for more invasive measures down the line. 

But it’s not just celebrities who want to get ahead. Dr. Few says he has long had patients who are predisposed to deeper frown lines or have a family history of jowling and want to address them as early as possible. “You can preempt that whole phenomenon just by simply targeting that area when it first starts to form, and then you hold it in suspended animation,” he explains, adding that it’s not about stopping the clock or dramatically altering someone’s appearance. “I believe if you try to take that given point where things are transitioning in maybe not the most positive way and hold it, well then that’s mission accomplished,” he says. “You've now made somebody achieve their goal of looking naturally beautiful and aging in the most graceful way, which, in my opinion, is the way to go.” 

Returning to those four dimensions of beauty, it’s all about enhancing what is already there in the most natural way. “It's not about aging ungracefully or in any artificial way or even not aging at all,” Dr. Few explains. “To me, where the artistry of cosmetic medicine and surgery comes through is in creating subtle contrast.” For example, “you have somebody who maybe has gray hair but, at the same time, has a youthful kind of very subtle cuteness that shines through,” he says. “That's their own individual expression.”

## **4. Blending Surgical & Non-Surgical Solutions**
Just because Dr. Few is a plastic surgeon doesn’t mean that every patient of his is going under the knife. “I am somebody who's kind of formed an expertise in both the surgical and non-surgical space – especially for facial rejuvenation and prejuvenation,” he says. In his view, it’s not so much surgical versus non-surgical but rather a continuum. “I try to position things in a way so that if somebody is not sure which direction they want to go, it's easier to do something that's a bit more temporary – that's non-surgical – that then can lead into a surgical option if that's what they want,” he explains.

Generally speaking, prospective patients fall into three categories: 

1. I definitely want surgery.
2. I definitely don’t want surgery – regardless of what I need.
3. I’m undecided and want to know my options.

After determining which bucket someone falls into, there are a trio of factors Dr. Few considers during a consultation to formulate a treatment plan: skin quality, volume loss, and facial laxity. “When you look at those three components, then I get to go into my toolbox and decide what are the options that work for it,” he shares. “For the skin, it may be different forms of resurfacing lasers, peels, etc., and then you look at non-surgical versus surgical lifting.” Seeing as he remembers the days when [Restylane®](https://aedit.com/procedure/restylane) was only one hyaluronic acid-based filler on the market, today’s innovation – especially in the non-surgical space – means there are now “more options than you could even hope to use.” As such, “it becomes much like picking the right paint brush for the component of the canvas you're painting on,” he says.

Finding the right balance between surgical and non-surgical has actually spurred hybrid solutions over the years. Dr. Few cites the evolution of the so-called [Nefertiti neck lift](https://aedit.com/aedition/where-to-use-botox-from-head-to-toe) as one such example. While injecting neurotoxin into neck bands can have a dramatic impact on texture and contour, the effect is temporary. For a longer-lasting result, Dr. Few developed a slightly more invasive neck band procedure. “That's where I can use a special needle that's called a no cord needle to actually go in and break up the bands of the muscle – much like the [Xeomin®](https://aedit.com/procedure/xeomin), [Dysport®](https://aedit.com/procedure/dysportr), or [Botox®](https://aedit.com/procedure/botox) would do – but it does it in a more permanent way,” he says. “You can marry these concepts, which is what makes it fun.”

## **5. The Benefit of ‘Stackable’ Treatments**
Long before the pandemic made the idea of [maximizing in-office time](https://aedit.com/aedition/best-aesthetic-procedures-to-combine-to-maximize-appointments) with your provider a must, Dr. Few was honing the technique of what he calls “stackable treatments.” “When I first came up with ‘stackable treatments’ as a concept, it was the combination of [Ulthera®](https://aedit.com/procedure/ultrasound-skin-tighteningultherapy) and lasers and fillers – doing that all in one setting,” he shares. As he explains, the combination approach was, initially, “purely out of necessity.” “When I first started seeing patients in New York, I didn't have the luxury of trying to space it out over six months,” he notes. But then he realized the multimodal nature of the appointment was more than just convenient. “There's synergy in it,” he shares, that actually improves the results that would be possible with each treatment individually. 

But not all stacks need to happen at once. ## **What Is truSculpt® Flex+?** 
In simplest terms, truSculpt® Flex+ “is an electrical muscle stimulation machine,” Dr. Alvarez explains. It is cleared by the United States Food and Drug Administration (FDA) for the improvement of abdominal tone, strengthening of the abdominal muscles, and development of a firmer abdomen. It is also cleared for strengthening, toning, and firming the buttocks and thighs. At his practice, the technology is regularly employed on the arms, abs, glutes, thighs, and calves.

For two weeks in January, Dr. Alvarez became my personal trainer of sorts – shaping my buttocks while offering insight into the treatment and, at times, providing me with emotional support during the more intense parts of the glute-building experience. Unlike other muscle-building procedures on the market that have one or two paddles, truSculpt® flex+ has the ability to treat eight areas simultaneously. In my case, all eight squares were placed on my buttocks (four on each butt cheek), but they could theoretically be spread out to treat more than one area at a time. “Once a person comes in, and their medical history is assessed, we examine the areas to make sure we don’t treat over bone,” he explains. Then the gel pads are placed on the target areas, followed by the electrodes that contract the muscles. 

During the sessions, Dr. Alvarez increases the intensity of each paddle. He is a de facto personal trainer in the sense that he observes the patient's needs and pain tolerance to determine intensity. Anyone who has tried a muscle-stimulating body sculpting device knows that it creates a tingling, almost ticklish sensation during the contractions. The body spazzes and releases all on its own. In the first few minutes – the “tone” portion, as Dr. Alvarez explained – the experience was ticklish, but it soon progressed to the “sculpt” setting where a more uncomfortable feeling kicks in. 

At the end of my first session, I managed to progress to 70 percent intensity, which took a while to adjust to. By the end of my last session, I had reached the mid-nineties. If I’m being honest, it hurt. However, as the machine switched off, the pain instantly went away. The best thing I can compare it to is the pins and needles of a leg falling asleep – multiplied by a thousand. Alas, no pain, no gain.   

As far as recovery and post-treatment soreness, there was none. After each 15-minute session, I walked out of Dr. Alvarez’s office and forgot about any discomfort in a matter of minutes. Soreness was also nonexistent for me, unlike the aching inability to walk after a heavy lifting session. Consider me a convert. 

## **Who Is a Candidate for a truSculpt® Flex+ Butt Lift?** 
Apart from those with a recent surgical scar, certain injuries, or metal device in their body (think: pacemaker or internal defibrillator), most people are candidates for truSculpt® Flex+. “This machine is great for someone who has some [sort of injury](https://aedit.com/aedition/emsculpt-for-post-surgery-recovery) and isn’t ready to go back and put in the work, but they need to strengthen those muscles for their mobility,” Dr. Alvarez says. I am 30, have built a decent amount of muscle on my own, and have no conditions that would prevent me from getting the treatment. My age and relatively healthy lifestyle theoretically set me up for success and speedy results. However, truSculpt® Flex+ isn’t just for the young and fit. During a session, I met another one of Dr. Alvarez’s patients who is in her fifties. Like me, she treated her bum and saw a marked improvement in volume, shape, and even quad definition. Clearly, age isn’t a factor with the multi-directional device.  

## **When Do You See Results From a truSculpt® Flex+ Butt Lift?** 
Because I opted to go higher on the intensity level, I was able to see results quickly. Individual results vary (especially when you consider diet and lifestyle factors), but, for my behind, there was a visible improvement by the two-week mark. A perkier and more volumized posterior were the first signs. And, when it came to function, I was truly impressed.

For the duration of the treatments, Dr. Alvarez told me to lay off any glute workouts. “I usually tell patients to wait 24 to 48 hours before doing any other exercise,” he says. “It is like going to the gym – the recovery period is also important.” Having opted to dial back my workout regimen per Dr. Alvarez’s instructions, I focused the few workouts I did on the upper body during that two-week period. But, after my treatments were complete, I immediately felt the difference and it helped me surpass my plateau. While my quads and hamstrings required a jumpstart to get back on track, my glutes were able to handle more weight.

Technically, the ‘final’ results are enjoyed after three months after your final session. The ever-popular [blepharoplasty](https://aedit.com/procedure/blepharoplasty-eyelid-surgery) (a.k.a. eyelid surgery or eye lift)  procedure corrects signs of aging on the upper eyelid, lower eyelid, or, in some cases, both, including loose skin, upper lid hooding, under eye bags, and more. Younger blepharoplasty patients (think: those in their [twenties](https://aedit.com/aedition/best-cosmetic-procedures-plastic-surgery-to-consider-in-your-20s) and [thirties](https://aedit.com/aedition/best-cosmetic-procedures-plastic-surgery-to-consider-in-your-30s)) often undergo surgery to address and/or change the structure of the eyes. When asymmetries, lack of definition in the upper lid, and persistent bags are long-standing complaints, the procedure can be a long-term fix.

But not everyone is ready for, nor needs, an eye lift to improve the look of the eyes. That's why non-surgical treatment options like thread lifts, fillers, neuromodulator brow lifts, lash extensions, and even contouring makeup are considered the next best things. Here, we’re rounding up the top four ways to rejuvenate and enhance the look of the eyes – sans surgery.

## **1. Accentuate the Lashes**
One of the quickest and most painless ways to accentuate and enhance the appearance of the eyes is with [eyelash extensions](https://aedit.com/procedure/eyelash-extensions). By adhering the correct lash extension style to the eye, attention is drawn away from droopy or puffy eyelids for a more youthful and lifted appearance, says Clementina Richardson, a celebrity lash expert and founder of [Envious Lashes](https://www.enviouslashes.com/). An almond-shaped eye is the most requested look. “When you have almond eyes, there is more versatility in shaping,” Richardson says. However, regardless of the shape of the eyes, a custom approach is always best. The way filler works in this area is pretty simple. The addition of volume plumps up the depressions to create a more pillowy and smooth appearance. The filler also plumps up the skin so that light can better reflect off the area. “Addressing the contour deficit fills the area,” Dr. Schwarcz explains. If the texture of the under eye skin is a concern, he suggests [microneedling with radiofrequency](https://aedit.com/procedure/microneedling-with-radiofrequency), [lasers](https://aedit.com/procedure/nonablative-laser-skin-resurfacing), and [chemical peels](https://aedit.com/procedure/medium-depth-chemical-peels).

HA-based filler isn’t the only injectable that can be used under the eye. [Angela Lamb, MD](https://aedit.com/provider/angela-lamb-md-34643), a board certified dermatologist in NYC, likes to mix filler with platelet-rich fibrin (PRF) in some cases. “If the problem is true volume loss, those are my go-tos,” she says. As she explains, under eye filler works better for correcting an indent or hollowness and perhaps a mild bag, whereas surgery is best suited for more prominent bags and redundant skin.

Often, it requires taking a holistic approach and considering all aspects of the face to create a natural and harmonious appearance. With age, the cheeks can start to sag, which causes ligaments in the area to pull down on the outer corner of the eye, says [Julis Few, MD](https://aedit.com/provider/julius-few-md-15580), a board certified plastic and reconstructive surgeon in Chicago. The strategic placement of filler can address this. “Returning the [cheeks](https://aedit.com/procedure/cheek-augmentation-with-fillers) to a more elevated position also lifts the outer corner of the eye so that it appears more like a cat eye,” he explains. And don't be surprised if your doctor also injects some filler into the [temples](https://aedit.com/aedition/cosmetic-forehead-procedures), which is a prime target area for volume loss. The brow's tail gets a little boost, too, to make the temples appear less sunken and the eye area smoother overall.

## **3. Don’t Neglect the Brows**
In situations where the [eyebrows](https://aedit.com/concern/aging-and-tired-eyes) are behind the changes to the shape and appearance of the eyes, addressing the positioning of the brows can make a world of difference. Over time, the tails of the brows tend to drop and fall downward, decreasing the distance between the upper eyelid and the eyebrows. In addition, a low brow position can add weight to the upper eyelid. In severe cases, it can encroach on the eyelid space, Dr. Schwarcz shares. There are both in-office and at-home solutions to address this:

### **Botox Brow Lift**
One of the best ways to give a visible – albeit temporary – lift to the brows is with the so-called [Botox brow lift](https://aedit.com/procedure/chemical-brow-lift). It involves injecting small amounts of neuromodulators (read: Botox®, [Dysport®](https://aedit.com/procedure/dysportr), [Jeuveau®](https://aedit.com/procedure/jeuveau), or [Xeomin®](https://aedit.com/procedure/xeomin)) to improve the position of the brows for three to four months at a time. To further complement newly lifted arches, many doctors inject the crow's feet and glabellar lines that form at the outer corners and in between the eyes to smooth the skin and make everything in the area appear refreshed and youthful. Keep in mind that using neuromodulators to lift the brow won’t do anything to address loose skin. 

### **Thread Lift**
The Botox brow lift is effective for achieving some lifting and shaping but is less dramatic than other options, like a [thread lift](https://aedit.com/procedure/thread-lift), Dr. Few notes. Thread lift procedures are pretty much downtime-free – save for some minor swelling – and involve placing absorbable sutures for an instant and long-term (they stimulate collagen over the coming months) lift in those with mild brow and eyelid sagging. “The threads go underneath the skin's surface to gently lift the area via small hooks that grab and hold the skin in place,” Dr. Lamb explains. Threads can also benefit those who are beginning to notice a ‘heavy’ forehead and drooping eyes and desire a little refresher before surgery down the line. 

Precise placement of threads can help hoist the outer part of the brow, making the outer corners of the eyes appear more lifted. “I like to use threads in an upside-down ‘U’ configuration going up to the hairline to lift the outer corner of the outer third of the brow,” Dr. Few says.  As he explains, this technique helps remove unwanted hooding of the upper eyelid while gently elevating the outer corner of the eye and brow. In these cases, neurotoxin injections are necessary to relax the forehead muscles and reap the full benefits of the threads. “Otherwise, the threads will pull through the tissue and cause the brow to drop again,” he notes.

Doctors can also use threads to tweak the shape of the eyes, but it doesn't always need to result in the now-popular [fox eye look](https://aedit.com/aedition/canthoplasty-canthopexy-eyelid-surgery-for-fox-eye-trend), a term that Dr. Few coined years ago during a lecture. “The goal should not be a hyper-elevated, unnaturally appearing cat look, but rather a natural elevation that enhances the appearance of the brow and upper eyelid,” he says. “The threads should create a pleasing appearance while being more exotic in a younger person and restoring natural positioning in someone experiencing age-related changes.” This is why selecting the right provider to perform the procedure is of the utmost importance — you want their before and after results and overall aesthetic to match the look you are trying to achieve. 

Keep in mind that while threads can provide some improvement, the skin in the brow and upper eyelid area is sensitive and thin. During the first few days, it may look pleated or bunched up due to overcorrection. “Surgery is the gold standard and would be my first choice for anyone wanting a predictable, long-lasting result,” Dr. Few says. Non-surgical solutions, like the combination of threads and neuromodulators, last about six to 12 months and require maintenance to retain the results. Since the benefits are relatively temporary, it’s essential to have realistic expectations and understand what threads can and cannot fix, Dr. Lamb says.

### **Makeup**
Brow powders and pencils can be just as important as doctor-administered treatments. With age, the brow hairs tend to become patchy and thin, and their once-crisp arch melts into an unorganized silhouette. Yet, redefining the arch with expert shaping ([check out our tips for how to shape your brows at home](https://aedit.com/aedition/how-to-shape-tweeze-define-eyebrows-at-home)) and filling them in with a good brow product ([we’ve rounded up the best powders, pomades, pencils, and gels](https://aedit.com/aedition/best-eyebrow-pomades-powders-pencils-gels-tools)) can make a significant difference. “The eyebrows lend structure to the face and frame the eyes, and, when properly filled in, they can create the effect of a brow lift," says Dani Kimiko Vincent, a celebrity makeup and brow artist and founder of [KIMIKO](https://go.skimresources.com?id=170670X1638612&xs=1&url=https%3A%2F%2Fwww.google.com%2Furl%3Fq%3Dhttps%3A%2F%2Fwww.kimikobeauty.com%2F%26sa%3DD%26source%3Ddocs%26ust%3D1649198110615954%26usg%3DAOvVaw3AG3e_Yk13aNbSFMC5ngPD). 

To create the illusion of a more lifted brow, start by filling in the upper outer arch instead of the front of the brows. “This will concentrate the filling in the area that lifts the brows and draws them up and outwards for a more open eye,” Kimiko Vincent explains. Avoid drawing down at the end of the brow. “You’ll want to keep the front of the eyebrows light and always feather the product through with a spoolie brush to blend and soften,” she shares. ## **4. Use the Right Eye Makeup**
Applying the right eye makeup – using appropriate colors and proper technique – adds visible contrast and draws attention to the eyes. Everyone's eye shape is unique and, while there are specific ways to enhance different shapes, this basic principle always applies: bring light forward and recede what's dark, Kimiko Vincent says. Similarly, using liner and mascara to [lift the eyes](https://aedit.com/aedition/face-lifting-sculpting-makeup-tips) is a universal solution.

When using makeup to enhance the eyes and make them look more youthful, Kimiko Vincent says to concentrate darker shadows on the lash line and towards the outer sections of the lid. “Sometimes, I contour with shadow to help the eyes appear more doe-eyed and, when necessary, I'll use a lighter shade of shadow on the lid so that they don't appear as sunken,” she shares. One thing to be aware of is mixing crepey skin with shimmery shadows, which has the potential to accentuate laxity. If a sagging upper lid is bothersome, focus on using darker shadows and placing them on the upper, outer portions of the eyelids. “From there, use lighter colors in the inner and middle sections of the lids,” Kimiko Vincent explains. She also recommends applying eyeliner on just the top lid and keeping the liner's tail (the end) lifted.

The classic cat eye is a favorite for creating a lifted outer corner. To do it, Kimiko Vincent recommends using liquid liner, pencil liner, or a deep eyeshadow and a smudger brush or tool. “A kitten eye starts from the center outwards and is slimmer than a traditional cat eye,” she explains. “You can subtly extend the inner corners with eyeliner to add a more feline eye shape.” Shadows are also helpful in contouring the eyes. Again, keep the darker tones towards the outer portion of the lid and concentrate mascara on the outer portion to draw the eye shape up and out.

Speaking of mascara, don't forget about the lashes. Kimiko Vincent says to curl the eyelashes first because “the eyes always benefit from a lift.” It makes them “appear instantly more youthful,” she adds. Before [swiping on mascara](https://aedit.com/aedition/mascara-tips-from-celebrity-makeup-artists), layer on a lash primer for extra volume and staying power. Then, coat the lashes with a volume and lengthening formula. Kimiko Vincent says to focus on lifting the lashes while sweeping on the mascara, paying extra attention to the outer corners of the eyes.

Patients and providers alike love minimally invasive procedures for their scarless results, in-and-out nature, and often downtime-free convenience. The flipside is the rate at which technology advances. While cosmetic surgery techniques evolve over time, it’s nowhere near as quickly as how fast non-surgical procedures wax and wane. Treatments that were once popular go-tos basically become obsolete as cutting-edge, of-the-moment tech that works faster, better, and more comfortably than before replaces those former favorites.

This can also make it tough to keep up with what’s new and next in non-surgical aesthetic medicine, which is why we’re doing the homework for you. The following up-and-coming non-invasive and minimally invasive procedures for face, skin, and body are headed for star status, and we’ve tapped top experts to learn more about why they are gaining buzz. Perhaps you'll discover a new favorite treatment that you've been waiting years to arrive.

## **1. For Next-Level Skin Tightening & Lifting**

**How It Works**: Cold temperatures have been used in body contouring for the past decade (think: [CoolSculpting®](https://aedit.com/procedure/cool-sculpting)), so the concept of harnessing the cold to improve a specific concern or condition is nothing new. However, what is innovative is the idea of cryomodulation – a proprietary cooling technology that normalizes melanin production while reducing inflammation. Glacial® RX is the first device to use the method, which temporarily freezes melanocytes to suspend melanin production and treat pigmented lesions, redness, and inflammation, says Bruce Katz, MD, a board certified dermatologist and the founder of [JUVA Skin and Laser Center](https://www.juvaskin.com/) in NYC. “It is also the first to use a cold probe to treat the skin,” he adds. 

If you are wondering how cryomodulation differs from a procedure like CoolSculpting®, they are targeting different things. CoolSculpting® freezes fat cells to permanently destroy them and reduce stubborn fatty areas like the stomach, thighs, and buttocks. Glacial® Rx, meanwhile “temporarily freezes melanocytes to suspend melanin production,” Dr. Katz shares. “When melanin production pauses, new cells form without new pigment, resulting in a [more evenly toned appearance](https://aedit.com/procedure/even-skin-tone-solutions) on the skin's surface.”

The downtime-free Glacial® Rx treatment takes 10 minutes or less and feels akin to a cooling facial, though it provides a long-term, anti-pigmentation effect that can last upwards of one year (with diligent sun protection). During treatment, a cold plate is manually applied to each area that needs to be addressed. The chill also takes down any puffiness and redness in the skin. Some minor side effects include erythema (redness), micro-crusting, and temporary darkening, all of which resolve independently. However, after two to three treatments over a few months, the full results are seen.

Speaking of results, Dr. Katz says the cooling technology immediately clears up congestion and skin dullness anywhere on the body. Created by board certified plastic and reconstructive surgeon [Gregory Mueller, MD](https://drgregmueller.com/), the hour-long procedure is different from other non-surgical under-the-chin solutions because it targets more than fat. Instead, MyEllevate® addresses platysmal banding *and* all of the fat under the chin (not a portion).

Historically, plastic surgeons have to ‘open the neck’ to address the bands surgically, says [Olivia MaDan, MD](https://aedit.com/provider/olivia-madan-md-34660), a board eligible plastic and reconstructive surgeon in Nashville. “With MyEllevate®, these bands can be released or cut without opening the neck since we suspend the skin and muscle as a composite around the mastoid fascia behind the ears,” she explains. While the treatment may sound somewhat similar to a [mini-facelift](https://aedit.com/procedure/minifacelift), it's not. A fiber optic illuminated blunt probe is used via a small entry and exit point to pass a permanent suture at the precise depth below the skin to restore a youthful jawline and lower face and improve the profile. “This technology allows for the permanent percutaneous placement of sutures using a light guide to transilluminate beneath the skin,” Dr. MaDan explains. “The color of the light-guide shows the surgeon the plane which the suture passes.” For example, red indicates if the suture is too deep, while amber specifies that the sutures are in the correct placement plane. 

[Local anesthesia](https://aedit.com/aedition/awake-surgeries-without-general-anesthesia) is administered before the treatment to make it as comfortable as possible for the patient. “Eight one-millimeter incisions are made at key points through which the suspension suture passes to address the skin and platysma muscle," Dr. MaDan shares. “However, none of the access incisions require a closure, so there is normal scarring after the procedure just like other minimally invasive procedures like liposuction or skin tightening.” Speaking of lipo, it’s used to remove unwanted fat and facilitate suture placement. “It's much like drawing blood, which requires a small poke but not an incision,” says [Philip J. Miller, MD](https://www.drphilipmiller.com/), a double board certified facial plastic and reconstructive surgeon in NYC.
 
After the procedure, there may be a feeling of tightness around the neck that settles over time. “There is also a [compressive dressing](https://aedit.com/aedition/benefits-of-compression-garments-after-cosmetic-surgery) we apply after the procedure, and we request patients to wear a soft cervical collar to dissuade them from looking down,” Dr. Miller says. “The dressing is removed on day three and the collar after 10 days.” Bruising and [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery) are also typical.

As far as maintenance goes, it's pretty much the same guidelines as most other rejuvenation procedures: take good care of the skin, stay out of the sun, maintain a healthy weight, and refrain from stretching the skin. “Supplementing the original MyEllevate® procedure with a skin-tightening procedure can also prolong the results,” Dr. Miller shares. The results from MyEllevate® should last about five years. “Of course, no procedure can stop natural aging and effects of gravity, and this procedure does not exclude someone from needing a [facelift](https://aedit.com/procedure/facelift-rhytidectomy) or [neck lift](https://aedit.com/procedure/neck-lift-lower-rhytidectomy) in the future,” Dr. MaDan says. 

## **5. For Breaking Up With Your Breakouts**

Plason by Artemis is an FDA-registered device that relies on an advanced version of [plasma technology](https://aedit.com/procedure/plasma-skin-resurfacing) to combat breakouts. 

Plason sterilizes the skin to eliminate acne-causing bacteria and germs while stimulating the skin's collagen-producing capabilities for firmer, clearer skin. It uses two different technologies – one is plasma energy (atmospheric low-temperature plasma) and the other is ultrasound technology, says Delaney Allen-Mills, director of partner training and quality at Artemis Distribution. “The real differentiation is the plasma,” she says. The device harvests plasma and then releases plasma radicals and ions into the skin, which attach to bacteria to kill it and create cleaner skin.
 
The quick five-minute, downtime-free treatment is most effective on [acne-prone skin](https://aedit.com/aedition/difference-between-oily-and-acne-prone-skin) and requires a series of treatments. It is also ideal for oily skin types who want less oil production and fewer breakouts. One to two treatments (on average) is all it takes to improve the skin.
 
The treatment also significantly increases skin absorption rate, so you'll absorb 60 to 120 times more transdermal product to the depth of the dermis than you would from topical application alone, Allen-Mills says. Plus, it's safe to use Plason in conjunction with oral medications. While micro-coring sounds like it could produce results similar to a facelift, Dr. Agha says that what you'll see afterward is much subtler. “Each session (three in total are needed) removes a small amount of skin – about eight percent,” he explains. “So, even though we are removing less skin than a facelift, that skin removal can be increased substantially without any scars.” Additionally, micro-coring doesn't address the underlying muscles or fat like a surgical facelift. 

To date, the procedure has not been validated on darker skin types. It is currently only FDA cleared for patients who have minimal sagging and are [Fitzpatrick skin types](https://aedit.com/aedition/guide-to-fitzpatrick-skin-types) I to IV. “Ellacor™ is designed for smaller areas of skin removal,” Dr. Agha shares. “But, with more research and development, we may extend this technology to larger areas in the future.”

## **7. For a Brighter, Tighter Face & Body**

Six sessions are recommended for the body, with regular maintenance for each.

7 New Non-Surgical Treatments You Need To Know About

Aesthetics trends come and go at a rate that can be hard to keep up with (don't worry, we’ve got you covered on [what to expect in 2022](https://aedit.com/aedition/biggest-medical-aesthetics-trends-of-2022-for-face-body)). While there are no signs of interest in perennial favorites like [Botox and fillers](https://aedit.com/procedure/dermal-filler-and-injectable-solutions) slowing down now or, really, ever, there are some newer modalities – especially in the [non-surgical space](https://aedit.com/aedition/biggest-non-surgical-aesthetics-trends-of-2022) – that are rising up the ranks. So, what are these treatments and what do their emergence mean for your next trip to the dermatologist or plastic surgeon? We asked top aesthetic providers to weigh in on what procedures are worth investing in this year.

## **What’s Trending – and Why?**
We don’t need to tell you that the COVID-19 pandemic [dramatically changed](https://aedit.com/aedition/aedit-survey-reveals-how-2020-altered-aesthetic-landscape) the way we lived, worked, and socialized in 2020 and 2021. More time at home and more time dissecting our faces on Zoom, led many who had been contemplating surgery to finally opt to go under the knife (a [new report](https://aedit.com/aedition/2021-aafprs-facial-plastic-surgery-trends) shows a 40 percent increase in facial plastic surgery last year). After all, you could recover discreetly without missing a beat. And, if you did venture out, there was plenty that could be concealed behind a face covering.

But, as we increasingly return to in-person living, working, and gathering, the pendulum is swinging back the other way. “My patients are most interested in the non-invasive procedures – what they can do that is a quick treatment, with little downtime, that still delivers results,” says Bruce Katz, MD, board certified dermatologist and founder of [JUVA Skin and Laser Center](https://www.juvaskin.com/) in New York City. The same is true on the West Coast. “I’m also seeing fewer patients interested in facelifts and eyelid surgeries,” says Neda Mehr, MD, a board certified dermatologist and founder of [Pure Dermatology Cosmetic and Hair Center](https://puredermoc.com/) in Newport Beach, CA. “Instead, they’re opting for minimal-downtime procedures that can achieve desirable results without the risks of surgery.”

But it’s not just surgical versus non-surgical. Even among minimally invasive procedures, there are other factors at play. Chief among them? Pain. “Some of the older non-invasive treatments, like [Ultherapy®](https://aedit.com/procedure/ultrasound-skin-tighteningultherapy), are declining in popularity,” says [Stafford Broumand, MD](https://plasticsurgeonsnyc.com/plastic-surgeon/dr-broumand/), a board certified plastic and reconstructive surgeon and founder of 740 Park Plastic Surgery in NYC. “We have treatments that render better outcomes with less pain available now.” Dr. Mehr agrees. “I have seen a huge shift in my practice away from painful procedures like Ultherapy® to the less invasive procedures like Thermage®,” she notes. 

And then there is the very important topic of [side effects and complications](https://aedit.com/aedition/guide-to-cosmetic-surgery-complications). No matter the level of invasiveness, no procedure is without risk. Last year, Linda Evangelista opened up about her experience with [paradoxical adipose hyperplasia (PAH)](https://aedit.com/aedition/paradoxical-adipose-hyperplasia-after-cryolipolysis) after a series of cryolipolysis (read: CoolSculpting®) treatments. Since then, Dr. Broumand says the fat-freezing treatment has become a less popular request among patients. “It has also declined due to a rare side effect and negative press on the treatment,” he shares.

## **Popular Procedure Swaps**
Now that we’ve covered what’s driving patients and providers alike to opt for some of these newer, non-surgical and less invasive alternatives, it’s time to talk about exactly what some of these updated treatment protocols involve. Here are five our experts are most excited about:

### **1. New Uses For Neuromodulators**
While it’s important to talk about [neurotoxin safety](https://aedit.com/aedition/botox-neuromodulator-safety) and how necessary it is to visit a highly skilled provider, interest in the wrinkle-reducing injectables is higher than ever. “We are seeing more filler and Botox® in our practice, as patients are interested in non-invasive quick fixes to correct their facial asymmetry,” Dr. Mehr shares. For some, this may be the first time they are seeking treatment. “Since they see themselves on Zoom all day long, [they] are noticing a lot of discrepancies that may not have bothered them before,” she adds. 

One such area is the [nose](https://aedit.com/procedure/muscle-paralysis-rhinoplasty). Dr. Mehr performs what she calls a “non-surgical nose lift” using strategic placement of neuromodulators. “We use Botox® on the nasal ala (nostrils), the upper sides of the nasal wall, the nasal dorsum area (between the eyes), and the columella (base of the nose) to relax the muscle so it lifts the tip of the nose,” she explains. This can act as a minimally invasive alternative to [rejuvenation rhinoplasty](https://aedit.com/procedure/rejuvenation-rhinoplasty).

And that’s not all. Dr. Mehr and her team are also using botulinum toxin type A injections [to treat](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7424364/) patients with androgenetic alopecia or genetic forms of [hair loss](https://aedit.com/concern/hair-loss). “For patients with androgenetic alopecia, histopathology specimens show that the blood vessels are in the deeper muscular plane,” she explains. “When you soften or relax those muscles with [neuromodulators], the diameter of the blood vessels increases.” Treatment requires 150 units, and, for that reason, Dr. Mehr says she usually chooses [Dysport®](https://aedit.com/procedure/dysportr), [Jeuveau®](https://aedit.com/procedure/jeuveau), or [Xeomin®](https://aedit.com/procedure/xeomin) (rather than Botox®). “When you use beyond 100 units of Botox®, people can start to [develop antibodies to it](https://aedit.com/aedition/botox-immunity-neurotoxin-resistance), and it becomes less effective over time,” she notes. “The science is the same: If it’s a neurotoxin, it relaxes muscles.” 

When it comes to results, she says patients will start to notice an increase in the diameter of the hair follicles after about three months. By six months, there is increased hair growth. “The treatment frequency is every six months,” Dr. Mehr shears. “And ideal candidates are those with temporal thinning and vertex scalp thinning.” 

### **2. Facial Rejuvenation – Sans Surgery**
At the end of the day, nothing can fully compare to the lifting powers of a surgical facelift – but combining the forces of new skin tightening treatments with biostimulatory injectables can have a seriously rejuvenating effect. For starters, our experts all extoll the virtues of [radiofrequency (RF)](https://aedit.com/aedition/radiofrequency-skin-tighening-fat-reduction) for firming the skin. Dr. Broumand likes Morpheus8 because it marries the benefits of [RF with microneedling](https://aedit.com/procedure/microneedling-with-radiofrequency), while Dr. Mehr is a fan of Thermage®. “[It] addresses visible signs of aging in just minutes and can be used on the face or body,” she explains. “There is little to no downtime, so my patients can return to their regular activities almost immediately.” 

These energy-based [skin tightening procedures](https://aedit.com/aedition/best-nonsurgical-skin-tightening-procedures) can be used alone or coupled up as part of a larger, multimodal treatment plan. “Instead of complete facelifts, patients are opting for filler to define certain areas, like the jawline,” Dr. Mehr says. “Over time, as collagen depletes, the face begins to look more ‘boney,’ and placing filler in these areas can help patients achieve a younger-looking appearance.” She often uses [Radiesse®](https://aedit.com/procedure/radiesse) because the calcium hydroxylapatite (CAHA)-based injectable stimulates collagen production.

### **3. Lunchtime Lasers**
Lasers are pretty multifunctional in terms of the aesthetic concerns they can treat, and there are classes of them that won’t require you to hide out for an extended period of time. [RevLite®](https://www.cynosure.com/product/revlite-si/), meanwhile, is a non-ablative Q-switched laser that can be used to treat pigmentation and [uneven skin tone](https://aedit.com/concern/uneven-skin-tone). “Before RevLite® came out to treat brown spots, we used cryotherapy – or liquid nitrogen – to remove the lesions,” Dr. Katz shares. “You would apply liquid nitrogen to a swab and apply it to the brown spot, and then the liquid nitrogen would destroy the extra pigment, leaving skin more [even-toned](https://aedit.com/procedure/even-skin-tone-solutions).”

As he explains, the RevLite® procedure is “much more efficient” and allows for “more precise” results. “You can do both cheeks with RevLite® in less than five minutes and see great results, whereas you’d only be finished with a portion of a cheek using cryotherapy in the same time,” he says. After treatment, expect your dark spots to turn from brown to white and exfoliate away over the course of a few days.

### **4. A Neck Lift Alternative**

Even so, many are not interested in or ready to go under the knife. “A lot of patients don’t want surgery, so they either walked away without treatment or eventually came back for surgery,” Dr. Katz explains. “Now, we can offer those patients something easy and accessible to address their neck concerns.”

What is that “easy and accessible” option you may ask? Enter [MyEllevate®](https://myellevate.com/). The pulses alter the cells in the treatment zone, resulting in a gradual clearing of the cells that caused the lesion without damaging healthy collagen or nearby tissue. “CellFX® is more efficient and less invasive,” Dr. Katz shares. “It not only targets the lesion on the skin’s surface, but treats the cause of the lesion at the cellular level, while leaving the surrounding skin tissue unharmed.”

Regardless of what the trend reports may say, the right treatment for you is the one that delivers the best results for your aesthetic concern(s) in the most safe and effective way. With that said, it can be helpful to know how technology and techniques have evolved because, in some cases, it means that quicker, easier, and less invasive alternatives exist. Much like the fashion and beauty industries, each year brings a new wave of trends in the aesthetics space. In [2020](https://aedit.com/aedition/aedit-survey-reveals-how-2020-altered-aesthetic-landscape) and [2021](https://aedit.com/aedition/biggest-medical-aesthetics-trends-of-2021-for-face-body]), the COVID-19 pandemic saw patients and providers focus on more invasive procedures for the eyes, nose, and the body due to face coverings and more discreet downtime at home to recover. With that said, non-surgical treatments never go out of style. We’re collectively hoping that 2022 will usher in a return to in-person gatherings, travel, and (pretty please!) a little less Zoom, which means non-invasive and minimally invasive treatments that boast little to no downtime are just what the doctor ordered. And, fortunately, there is plenty of innovation for both face and body in the category.

### **1. New Categories of Injectables**
When it comes to cosmetic injectables, neurotoxins (hi, [Botox®](https://aedit.com/procedure/botox), [Dysport®](https://aedit.com/procedure/dysportr), [Jeuveau®](https://aedit.com/procedure/jeuveau), and [Xeomin®](https://aedit.com/procedure/xeomin)) and hyaluronic acid-based fillers (think: [Juvéderm®](https://aedit.com/procedure/juvederm) and [Restylane®](https://aedit.com/procedure/restylane)) have been household names for a while now. The former can minimize the appearance of fine lines and wrinkles, slim the jaw, and even treat medical conditions like migraines or [excessive sweating](https://aedit.com/procedure/botox-for-hyperhidrosis), while the latter can plump and contour the lips, cheeks, under eyes, and more. Together, they account for the most popular cosmetic procedures of any kind with nearly eight million injections [performed in 2020](https://www.plasticsurgery.org/documents/News/Statistics/2020/plastic-surgery-statistics-full-report-2020.pdf) alone.

So, what’s coming next in the category? HA fillers already made a splash with the [FDA approval](https://aedit.com/aedition/fda-approves-juvederm-volbella-xc-under-eye-filler) of Juvéderm® Volbella XC for improvement of infraorbital hollows (a.k.a under eyes) in adults over the age of 21. On the neuromodulator front, it’s expected that new longer and shorter acting products will continue to make their way through the approval process.

And then there are the [biostimulatory injectables](https://aedit.com/aedition/hyaluronic-acid-filler-vs-biostimulatory-injectables) to consider. Dr. Shamban is intrigued by updated uses for calcium hydroxylapatite (CAHA) fillers like [Radiesse®](https://aedit.com/procedure/radiesse). By hyper-diluting the product with lidocaine and saline, you can treat areas that previously required other modalities to address. “You inject it in areas where there's crepiness or deep lines or wrinkles that you would typically retreat with laser,” she explains. “You could do it in the neck, the decolletage, the arms, and it’s also a way to treat cellulite.” For best results, she recommends one treatment every three weeks for a total of three.

### **2. Next-Generation Threads**
A [thread lift](https://aedit.com/aedition/pdo-thread-lift-for-facial-rejuvenation) involves subcutaneous placement of dissolvable ‘threads’ (i.e. sutures) along a planned trajectory in the face or body. They are pulled to achieve a subtle skin lift, while also encouraging collagen production for more volume in the treated areas. It’s performed in-office under local anesthesia, and there is no scarring and little downtime. 

Thread lifts have had a major upgrade. While they’ve been in use for more than 20 years, the threads of today are lighter, more durable, and more comfortable. “The old threads were non-absorbable and made of nylon,” Dr. Shamban explains. “The newer generation threads are biostimulators, so they stimulate your own production of collagen and, over time, you get a nice lift from the thread.” Increasingly, [men](https://aedit.com/aedition/thread-lifts-for-men) are turning to thread lifts to correct sagging along the neck area and jowls.

In the future, Dr. Shamban says to keep an eye out for even more advanced threads that introduce [hyaluronic acid](https://aptos.global/post/116/brand-new-aptos-thread-hyaluronic-acid) (yes, the same ingredient that’s used in your favorite filler and skincare products) to the equation for additional benefits.

### **3. A Multi-Modal Approach**
Combination therapy — also referred to as a multi-modal approach — has gained steam in the aesthetics space due to the fact that many procedures complement each other nicely and, let’s face it, the last couple years have forced us to [maximize](https://aedit.com/aedition/best-aesthetic-procedures-to-combine-to-maximize-appointments) our in-office time. Just as you might consider combining a [neck lift with a facelift](https://aedit.com/aedition/pairing-facelift-neck-lift-procedures) or a [breast lift and augmentation](https://aedit.com/aedition/combining-breast-augmentation-with-breast-lift), you can think about pairing non-surgical treatments. 

“The whole is greater than the sum of the parts,” Dr. Shamban shares. “So, that's why we like to design a treatment plan for people where we figure out what we're doing when.” No two patients are the same, and there are endless options as far as combos are concerned. “For people who have more laxity – for the older patient population – we often will combine radiofrequency microneedling with an ultrasound tightening,” she explains. That might mean alternating [Morpheus8](https://aedit.com/procedure/microneedling-with-radiofrequency) and  [Sofwave™](https://aedit.com/aedition/sofwave-for-skin-tightening-collagen-production) sessions. “Depending on where you are in your life, we may just do one and then we may do the other later on,” Dr. Shamban notes. “And then, if you're a little bit more mature, we may do both.”

### **4. Lip Architecture** 
It would seem as though the hype around the oversized and over-plumped [lip filler](https://aedit.com/procedure/temporary-lip-fillers) of years past is fianlly fading in favor of a more customizable focus on [lip shape](https://aedit.com/concern/lip-shape) (this is a trend you can also see in the beauty sphere). “You can't really change the shape of your lip, but you can enhance it,” Dr. Shamban explains.

She says the so-called ‘Russian lip’ technique is intriguing because it involves “injecting in a different way that can really emphasize the shape” while honoring the natural anatomy. “We all have a unique shape,” Dr. Shamban reiterates. “So, you started emphasizing the shape by injecting different patterns going across [the lip].” It’s a more complicated and involved injection than just adding volume, but, in the hands of a skilled injector, the results are a plush and proportional pout.

### **5. Topical Carboxy Treatments** 
Are you familiar with [carboxy therapy](https://aedit.com/procedure/carboxy-therapy)? If you’re not, you’re not alone. But you heard it here first: Topical versions of the carbon dioxide-based treatment are about to have a moment thanks to their ability to stimulate blood flow, boost elasticity, and improve the overall appearance of the skin. While carboxy treatments historically involved injections and downtime, new topical iterations are proving to be just as effective at eliminating toxins, improving nutrient delivery, and boosting wound healing. This makes carboxy masks a good option for patients healing from certain laser or resurfacing treatments. Dr. Shamban says to expect both professional and at-home options to continue to enter the market.

## **The Takeaway**
Here’s the spark notes: In 2022, you can expect to see injectables being used in new ways, as well as a combination approach to maximize your results. Thread lifts will be more high tech than ever, lip shape begins to take precedence over lip size, and topical carboxy masks will become a popular post-procedure add-on to boost results or a stand-alone option to improve your complexion. 5 Non-Surgical Aesthetics Trends To Watch For In 2022

I am a soon-to-be-31-year-old beauty editor, and, for as long as I can remember, I have considered my laugh lines to be no laughing matter. Many of us have a facial feature we fixate on. In my case, that feature has always been my [nasolabial folds](https://aedit.com/aedition/nasolabial-fold-treatments). Those parentheses-like creases look different on everyone. For the women in my family, they tend to run deep — and get deeper with age. 

I mention the fact that I am a beauty editor because it means that I have long been spoiled by the opportunity to try great skincare products. I have a [pretty solid routine](https://aedit.com/aedition/best-skincare-routine-products-ingredients-for-30s) that includes [exfoliating acids](https://aedit.com/aedition/guide-to-topical-alpha-hydroxy-acids-in-skincare) to improve tone and texture, [antioxidants](https://aedit.com/aedition/facts-about-vitamin-c-in-skincare) to brighten and repair, and, of course, [daily sunscreen](https://aedit.com/aedition/best-sunscreens-that-act-as-makeup) to protect. But even the best lotions and potions cannot do much (if anything) to soften said folds. So, what’s a girl to do? 

As I shared a couple years ago, [I started getting Botox® injections](https://aedit.com/aedition/everything-you-need-to-know-before-getting-botox-for-the-first-time) just shy of turning 29 in an effort to get ahead of the increasingly omnipresent nature of my forehead lines. Last month, I headed to see [Lesley Rabach, MD](https://www.instagram.com/lmmedical.nyc/), a double board certified facial plastic and reconstructive surgeon, co-founder of [LM Medical](https://lmmedicalnyc.com/) in New York City, and my go-to aesthetic provider, for a neurotoxin touch-up with nary a thought of doing anything else. During our quarterly catch-up session (she fills me in on all the interesting things she’s noticing in her practice and patiently answers my questions about some of the trends we’re considering covering here at The AEDITION), however, something compelled me to broach the subject of my stubborn smile lines.

As Dr. Rabach emphasized, some amount of creasing is necessary in the region — after all, babies are born with nasolabial folds — but there are both surgical (hi, [deep plane facelift](https://aedit.com/procedure/deep-plane-facelift)) and non-surgical (think: filler) solutions that can help soften their appearance. In my case, half a syringe of [hyaluronic acid (HA)-based filler](https://aedit.com/aedition/everything-you-need-to-know-about-dermal-fillers) was all I needed to subtly yet substantially rejuvenate my face (keep reading to see before and after photos) for up to a year. Intrigued? Here’s what you need to know.

## **Returning to the Fold**
Also called laugh lines and smile lines, the nasolabial folds run from the corners of the nose to the corners of the mouth. Look back at your baby photos and you’ll see that those creases have always been there. “Even children have smile lines,” Dr. Rabach shares. As such, “it is imperative to maintain a fold because without it, we look very unnatural,” she adds. 

Depending on your anatomy, genetics, and lifestyle, however, the current state of your nasolabial folds may be quite different from their OG appearance. “As we get older, the cheeks [lose volume](https://aedit.com/aedition/best-procedures-to-restore-facial-volume) and, together with years of smiling, the nasolabial folds deepen,” Dr. Rabach explains. Yes, smiling is one of the causes of deepening folds, which means those lines are a sign of a life well lived and laughed. While I can’t help but smile at that, it doesn’t totally reconcile my relationship with said creases. 

From my teenage years through my mid-twenties, I always had a fuller midface. My nasolabial folds were fairly prominent then, too, but something about the overall proportion of my face seemed to camouflage them. By my late-twenties, my countenance bid adieu to any lingering baby fat, which added a bit more emphasis to those pesky parentheses.

It’s been in the last year or so, though, that I’ve noticed the most visible change. Doubling down on my [fitness regimen](https://aedit.com/aedition/what-its-like-to-get-emsculpt-neo), the stressors of everything going on in the world, and the anatomical realities of being a 30-something seemed to converge in a way that left my mid- and lower-face looking… blah. As someone who has always been conscious of my penchant for RBF (IYKYK), combining that with a leaner face and deepening folds made me look tired, stern, and just not like the best version of myself.

## **Fixing the Folds**
Sitting in Dr. Rabach’s Greenwich Village office on an unusually mild December afternoon, I explained to her the evolution of my smile lines and, based on how my mom, aunt, and grandma have aged, forecasted the future of my folds. While she was quick to quip that my nasolabial folds were no deeper than those of her four year old, she did admit that a little HA filler could likely go a long way.

This is, she says, “a common area to be treated with dermal fillers,” but the placement of the product can be customized based on the patient and their aesthetic goals. “Nasolabial folds can be treated either by directly filling into the fold or by [filling the cheek](https://aedit.com/procedure/cheek-augmentation-with-fillers) to lift the fold,” Dr. Rabach shares. “It depends on the individual anatomy which will be more natural and effective.” 

I was more familiar with the latter option ([augmenting the cheek](https://aedit.com/aedition/cosmetic-procedures-to-define-cheekbones) for a more lifted appearance), but, as it turns out, I am not an ideal candidate at this point in my life. “Younger patients, where the folds aren't so deep, often benefit from a very small amount of filler placed just in the deepest area to soften that,” she explains. In my case, the deepest area was by the nose. Put another way: Because my cheeks are (relatively) full, it's better to inject into the fold. “If your cheeks were flat, then injecting into the cheek is better,” Dr. Rabach notes. Either way, the goal is the same. “The goal is always maintaining a natural amount of depth,” she reiterates. 

Sure, [cheek filler](https://aedit.com/aedition/restylane-contour-cheek-filler) could help rejuvenate my folds, but it would also address a concern ([cheek volume](https://aedit.com/concern/cheek-volume)) that didn’t, well, concern me. As Dr. Rabach explained, the amount of filler I would need to see results, coupled with the fact that my goal was not cheek enhancement, made it inefficient from both a cost and benefit perspective. Instead, she said splitting a half-syringe of Versa™ by Revanesse® between my two folds would give me the subtly satisfying look I was after.

## **Fill ‘Em Up**
Until now, my experience with cosmetic injectables has been limited to [Botox®](https://aedit.com/aedition/where-to-use-botox-from-head-to-toe), which requires a bit of patience. Anyone who has ever gotten neuromodulators knows that it takes a minimum of a few days (and up to two weeks) to fully enjoy the effects of the wrinkle relaxers. Hyaluronic acid-based fillers, on the other hand, offer instant gratification.

While some areas of the face require [numbing cream](https://aedit.com/aedition/numbing-cream-breakouts) before filler injections, the nasolabial folds are not one of them. Dr. Rabach gave me a stress ball to squeeze, but, truth is, it hardly felt like anything more than a pinprick (there is lidocaine within the filler to further ease any potential pain). After two pricks into the upper part of the fold on my right side, she handed me a mirror to inspect the first half of the results. Much to my surprise — and delight — the crease was softer. She repeated the same thing on the other side and, in all of five minutes, I looked totally rejuvenated in the most understated way.

Dr. Rabach used Versa™ on me because, she says, “it is very versatile for the folds, moves with the person, and integrates well into the tissues.” Depending on the patient, however, she also likes [Restylane®](https://aedit.com/procedure/restylane), Teoxane Teosyal RHA® 3, and [Juvéderm® Ultra](https://aedit.com/procedure/juvederm) for this area. For most, HA filler results last nine to 12 months in the nasolabial fold region.

## **Recovery & Results**
Regardless of the product, the most common side effects of HA fillers are temporary swelling and mild discomfort for a few days. As with any injectable, [bruising](https://aedit.com/aedition/how-to-treat-prevent-bruising-after-lasers-filler-botox) is possible (though relatively rare in this part of the face). As you can see in my ‘after’ photos, I experienced a bit of redness at the injection sites for about 24 hours. After that, I found the area to be slightly tender to the touch (most noticeable when I washed my face) for about five days. 

A month later, I am pleased to report that the results have exceeded my expectations. Not a single person — including my ever-observant mother, with whom I FaceTimed when I got home from my appointment — has been able to pinpoint what I had done. Yet, when I show them my B&A, the results are abundantly clear.

For some, the subtlety may be a letdown. For me, it’s the draw.
