We’re all privy to the risks associated with aesthetic procedures (think: infection, poor wound healing, additional [complications](https://aedit.com/aedition/guide-to-cosmetic-surgery-complications)), but few possible adverse effects sound as terrifying as fat necrosis. It feels as though nothing is lost in translation – ‘necro’ quite literally means death – so it’s easy to let your mind go, well, there. Now, before anybody spirals, it’s important to note that fat necrosis is not a life-threatening complication or a sign that you will be unable to recuperate. It is actually fairly straightforward to diagnose and manage, though it does have some implications. A complication most commonly associated with fat grafting procedures (including [breast augmentation with fat transfer](https://aedit.com/procedure/breast-augmentation-with-fat-transfer) and [Brazilian butt lift](https://aedit.com/procedure/brazilian-butt-lift)) and [breast reconstruction](https://aedit.com/aedition/comprehensive-guide-to-breast-reconstruction-nipple-reconstruction-after-breast-cancer), fat necrosis can occur anywhere on the body. In the event that it happens to you, The AEDITION is breaking down what to look out for and treatment options.  

## **What Is Fat Necrosis?** 
To put it in layman's terms, [fat necrosis](https://pubmed.ncbi.nlm.nih.gov/25415090/) is a collection of dead or damaged fat cells that have hardened and manifested as firm bumps. When the body has restricted oxygen flow as a result of surgery, radiation, or trauma, fat cells can die and release their contents to form a small body of liquified matter that hardens over time. “Fat necrosis usually appears as hard, firm lumps or, in some circumstances, as liquified fat,” explains [Gregory A. Buford, MD](https://aedit.com/provider/gregory-buford-md-1015), a board certified plastic and reconstructive surgeon in Lone Tree, CO. 

The liquified fat (a.k.a. oil cysts) can also cause lumps. Oil cysts are fluid-filled sacs that feel smooth and squishy under the skin and can grow over time. Fat necrosis and oil cysts are different, though they are both the result of damage to the fatty tissue. As the tissue breaks down, it can harden into scar tissue or retain a more liquid composition.

Post-op, self-checks are recommended to observe any changes in the body, including lumps and bumps. Diagnosis of fat necrosis or oil cysts will require imaging technology, such as a mammogram, ultrasound, MRI, or, in some cases, a biopsy. For some, the only symptom of fat necrosis and oil cysts is the lump. Others may experience some discomfort in the area, skin that becomes thickened, bruised, or red, and/or aesthetic concerns from contour changes. 

Due to the nature of the indicators, it’s common for breast surgery patients to rush to the conclusion that this is a [sign of breast cancer](https://aedit.com/aedition/what-to-do-if-you-find-a-lump-in-your-breast-breast-cancer-awareness) – especially for those who’ve undergone [reconstruction](https://aedit.com/aedition/post-mastectomy-breast-reconstruction-process-breast-cancer) (with or without fat grafting). Despite their lumpy form, the American Cancer Society [reports](https://www.cancer.org/cancer/breast-cancer/non-cancerous-breast-conditions/fat-necrosis-and-oil-cysts-in-the-breast.html) neither fat necrosis or oil cysts are cancerous nor increase the risk of cancer anywhere on the body. In fact, they are generally considered harmless.

## **What Causes Fat Necrosis** 
The roots of post-op side effects and complications often aren’t black and white, and this is no exception. “Fat necrosis can be caused by the lack of integration of transplanted fat into a specific area when specific areas of fat do not receive an adequate amount of blood supply,” Dr. Buford explains. Additionally, a patient’s post-op care lifestyle can play a role. “Necrosis can also result when too much pressure is placed on an area,” he continues. This is most common [after a BBL](https://aedit.com/aedition/brazilian-butt-lift-procedure-safety). Fat necrosis “can be seen following a BBL if someone sits directly or applies direct pressure to the grafted fat too early after the procedure,” he shares.

Often, numerous factors need to be considered. The provider’s technique, the patient's ability to recuperate, and post-op care can all play a role in the occurrence of fat necrosis. “First, if extraordinarily large volumes of fat are grafted into any one area, this can potentially lead to less take of the fat and possible necrosis,” Dr. Buford says. “Next, how the fat is harvested and managed – both before and during transplantation – can also have a big effect on the potential for fat necrosis.” 

Whether it’s occurring on the body or the face (think: [autologous fat transfer in place of filler](https://aedit.com/aedition/comparing-dermal-fillers-vs-fat-grafting-for-facial-rejuvenation-contouring)), these components aren’t mutually exclusive, so the root can be intra- or post-operative.  

## **How To Prevent & Treat Fat Necrosis**
The primary recommendations for preventing fat necrosis are to not [smoke](https://aedit.com/aedition/impact-of-cigarette-smoking-vaping-on-skin-hair-teeth-health) or be around those who do before or after surgery, as this restricts blood flow. For those who are post-BBL, follow your surgeon’s instructions for sitting and standing to a T. While complications can occur regardless of who performs the procedure, choosing a [board certified plastic surgeon](https://aedit.com/aedition/medical-board-certification-in-plastic-surgery-cosmetic-dermatology) who specializes in the fat grafting procedure you are interested in can help mitigate risk. “I haven't seen a tremendous amount of variation in terms of potential for fat necrosis in specific areas of the body, but there is definitely a different threshold for the amount of potential for volume grafting in each area,” Dr. Buford notes. “As you can imagine, the buttocks handle far more fat than the breasts and the [face](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4271301/).”

When it comes to treatment, options range from waiting it out to (in very rare cases) surgical removal. “Firm lumps resulting from fatty necrosis often resolve on their own or can be directly massaged to break them down,” Dr. Buford explains. “If contour changes are resulting from loss of this fat, then a [second round](https://aedit.com/aedition/guide-to-revision-surgery) of localized fat grafting can potentially be performed to fill in the gaps.” In the case of oil cysts, needle aspiration is usually able to drain the lump.

## **The Takeaway** 
For those who have had autologous fat transfer or breast reconstruction, fat necrosis is a risk. But it is one that is manageable and, in some cases, preventable. Arming yourself with the right information ahead of time will go a long way towards ensuring a smooth recovery and satisfactory outcome. “Fat is Mother Nature's filler and one of the earliest substances that plastic surgeons used to either smooth out contour deformities or add the desired volume,” Dr. Buford says. “Ultimately, results from fat grafting – when performed on the right patient, by the right surgeon, in the right area, and with the right amount – can produce very nice and very natural outcomes.”

As women, there is nothing quite like the jolt of sheer panic that shoots through your body upon [finding a lump in your breast](https://aedit.com/aedition/what-to-do-if-you-find-a-lump-in-your-breast-breast-cancer-awareness). With statistical estimations of [one in eight women](https://www.breastcancer.org/symptoms/understand_bc/statistics) in the United States (that's roughly 13 percent) being diagnosed with breast cancer in their lifetimes, it presents a moment of uncertainty so anxiety-inducing that the term ‘fear by paralysis’ comes to mind. The comforting fact is that there are *many* lumpy presences in breast tissue that indicate noncancerous issues, yet the last thing you want to do is to ignore it. 

Being prepared and informed is exactly how you can get through this type of trying moment rationally. I am sharing my own experience with fibroadenoma – and some expert insight from top surgeons – in the hopes that it will help others who find themselves in my position. 

## **My Fibroadenoma Diagnosis**
I first noticed a lump in my right breast at 31. It was relatively small, felt moveable and slippery — almost like a little blueberry beneath the surface of my skin — and was not visible from the outside. I remember reaching out panickedly to two of my girlfriends in a group chat, asking, “Have either one of you ever found a lump in your breast!?” One replied that she had not, but the other shared that she had. Her experience was what helped me overcome my fear and book an appointment with my OB-GYN.

At my appointment, my doctor reassured me, stating that the type of lump I presented with was different from the types of emergent masses that typically worry healthcare providers. For her peace of mind and mine, I was referred to a diagnostic mammogram and was diagnosed with a fibroadenoma.

…Fast forward two years. I had just moved back from Paris, France, where I was supposed to be living for two years, but my stay was cut short due to the COVID-19 pandemic. My unwelcome friend was still present and, unfortunately, felt much larger. While still invisible to the naked eye, it had become easy to feel. I worried that if it grew anymore, it would start to create an unsightly bulge.

Back to the doctor I went. I was again referred for a diagnostic mammogram and biopsy. If you ever find yourself in this position and experience fear that you deem to be irrational — in my case, it was because I had already received a benign diagnosis in the past — you are not alone. This is an objectively nerve-wracking process. During the mammogram, your breast is positioned on the machine and photographed. The most uncomfortable part is the squeezing. You are numbed for the needle biopsy, but the noisy (it sounds like a staple gun), handheld machine does leave an extremely small scar. I nervously got through the procedure thanks to an incredibly supportive staff. “Ninety-nine percent of the time, these are absolutely nothing,” the nurse told me comfortingly. “But we just want to make sure, because sometimes — rarely — they surprise us.”

Even though my fibroadenoma is benign, it's large in size. And, because it grew significantly over the course of two years, it was time to get it removed. Before scheduling my surgery, I met and consulted with both of the doctors below — a breast health expert and surgeon and a board certified plastic surgeon — in order to discover my best bet for removal. As a beauty editor, I was also curious as to whether the process would impact the integrity of my breast shape and size post-surgery or if the diagnosis could interfere with getting breast implants. Here is everything I've discovered throughout the process, from my diagnosis to my non-surgical removal.

## **What Is a Fibroadenoma?**
“Fibroadenomas are growths within the breast tissue made up of healthy tissue elements,” says [Heather Richardson, MD](https://www.bedfordbreastcenter.com/mastectomy-surgeon-heather-richardson-md), a general surgeon and breast health expert at the Bedford Breast Center in Beverly Hills. “They are not associated with breast cancer and don't turn into breast cancers.” As she explains, their presentation is typically described as “slippery” and having “an almost bubbly sensation” when a breast exam is performed. They are round with smooth edges and feel as if they are moving beneath the tissue should you try to secure it. “This is in sharp contrast to most breast cancers, which feel very hard and stuck in place,” Dr. Richardson shares. 

Present in approximately 20 to 25 percent of patients (with only little familial significance), those who have been diagnosed with a fibroadenoma are 20 to 25 percent more likely to experience another in the future. Fibroadenomas can appear at any age, though they most often occur in women between the ages of 15 and 35. They are usually diagnosed with ultrasounds. “Mammography is also used to gain more information about the nature of these growths, but ultrasound is the study of choice,” Dr. Richardson notes. Fibroadenomas are identified as smooth round or oval masses of tissue (in my opinion, it looked like a potato on the screen) and tend to be monitored over a period of time. Some growth is normal, and the protocol for monitoring and treatment changes as you age. 

In your twenties, the fibroadenoma will typically be monitored by your doctor in six month intervals. But when you get to your forties and fifties (or, in my case, thirties), the likelihood of biopsying the fibroadenoma becomes more likely. Dr. Richardson explains that this is simply to ensure that it is not something malignant presenting atypically. “We should also consider a biopsy of a suspected fibroadenoma if it is enlarging over time or has a change in shape or consistency,” she continues. Furthermore, if there is ever a questionable finding or a family history of cancer (or even a high level of patient anxiety regarding cancer), there should be a conversation about the risks and benefits of proceeding with a biopsy.

Another thing to keep in mind: “Very rarely, if they are very large or have been present for a long time, fibroadenomas can start to grow very quickly and can be reclassified as a type of tumor growth called a phyllodes tumor,” Dr. Richardson says. Again, this is *not* related to cancer. “They are typically treated with removal and do not require additional treatments,” she adds.

## **Removing a Fibroadenoma**
The removal of your fibroadenoma is largely dependent upon its position. Mine was located close to the surface of the breast. Sometimes doctors will recommend their removal, and sometimes it is patient-initiated because monitoring them with in-office visits can be a nuisance (not to mention, feeling a lump can – as a rule – be unsettling). “Patients may want to remove biopsy-proven fibroadenomas if they are noted on imaging to be changing or growing significantly,” Dr. Richardson says. Additionally, “if the patient feels that they are symptomatic, causing pain, discomfort, a cosmetic change in the breast, or even anxiety,” removal is an option, she notes. 

In cases where the patient has multiple fibroadenomas, the doctor will often only remove the more concerning ones and leave the rest in order to preserve the aesthetic appearance of the breast. There are both non-surgical and surgical removal methods, which we outline below:

### **Non-Surgical Treatments**
In the past, Dr. Richardson frequently performed cryoablation, wherein a probe is used to freeze the unwanted, internal breast tissue, destroying the area with the fibroadenoma. Over the course of a few months, the dead or dying tissue temporarily hardens, but then will slowly be flushed away by the body — somewhat akin to [CoolSculpting®](https://aedit.com/procedure/cool-sculpting) for the breast. “The downside is that we typically do still want to monitor the areas that have undergone cryoablation in the past, so those patients would not be simplifying their follow-up as they might hope,” she says.

More recently, Dr. Richardson prefers using the BARD vacuum-assisted biopsy tool, which presents patients with a minimally invasive (and more affordable) alternative to surgery. “It offers an excellent alternative for patients who don't want to have surgery or have multiple areas that would create a lot of surgical trauma in the healthy tissue if they were all removed at once,” she says. She recommends the process for small-to-medium fibroadenomas – roughly 2 centimeters or less – that are found deep in the breast. 

The treatment is performed awake with numbing agents. You will have the option for a prescribed sedative, like Xanax, but, in that case, you will need someone to drive you to and from the appointment. The entire fibroadenoma removal can be completed in an hour, with the bulk of which is taken up by the numbing. She says to think of the gradual removal process like an electronic Pac-Man that is taking bites from the fibroadenoma. The first couple of days might consist of some bruising and soreness, which can be managed by over-the-counter pain relievers.

Some doctors also use [radiofrequency (RF)](https://aedit.com/aedition/radiofrequency-skin-tighening-fat-reduction) and [ultrasound](https://aedit.com/aedition/sofwave-for-skin-tightening-collagen-production) energy to remove fibroadenomas, however, the critique in the aesthetics industry with these types of devices is that they can — in the hands of the wrong provider — result in the unintended loss of fat. Needless to say, many women would not welcome this side effect. 

### **Surgical Treatments**
The surgical excision of a fibroadenoma is, obviously, the more invasive option. It is most appropriate for larger masses. Post-op, the area will no longer need to be monitored, and, in certain cases, surrounding tissue can be sent out for analysis for greater reassurance. The procedure is typically handled by a general surgeon specializing in breast health, though, in instances where the patient is already considering a cosmetic procedure, a plastic surgeon may be involved. 

The lump may be able to be removed as part of a larger surgery. “It is rare that I would see a patient simply for a fibroadenoma, however, I will sometimes remove them in conjunction with breast surgery like breast reduction, breast lift, and breast augmentation,” says [Robert Cohen, MD](http://www.drrobertcohen.com), a board certified plastic and reconstructive surgeon in Beverly Hills. “There is obviously a scar after this surgery to remove the fibroadenoma, but often the scar can be hidden along the edge of the areola or in the breast crease.”

### **The Cost of Removing a Fibroadenoma**
Of course, cost is a significant factor to consider when choosing a treatment option. Because the surgery would have been an out-of-network procedure and my [insurance](https://aedit.com/aedition/insurance-coverage-for-plastic-surgery-cosmetic-procedures) deductible has not yet been reached, surgery would have been much more expensive for me (think: over $6,000). That being said, if you are in-network, it might be the better option for you. 

For reference, the cost of the BARD procedure was $2,000 out-of-pocket. Insurance has not yet caught onto the ease with which the tool can be used to excise fibroadenomas; thus, they only allow providers to bill for a biopsy. “The reimbursement is the same whether you take a pinch from a core needle that costs the center $20 or if you spend 20 to 60 minutes with a much more expensive device,” Dr. Richardson explains. Thus, not many centers offer the procedure for fibroadenoma removal. If, like in my case, the out-of-pocket cost is still cheaper than the price of what surgery would be, however, it provides a pretty compelling alternative.

### **Will Fibroadenoma Removal Leave An Indentation?**
Depending on the placement and size of the fibroadenoma, its removal may result in an indentation or even a change in the size of the breast. Our experts point out that this is most often the case when there are several fibroadenomas removed or a larger mass. “Quite often, the body will contract the tissue together as it heals and these depressions become less noticeable with time,” Dr. Richardson shares.

But this is not always the case, which is when an aesthetic procedure, like fat grafting, may become appealing. Following your fibroadenoma removal, Dr. Cohen advises waiting to see how the body heals itself before coming in to be assessed for a corrective treatment. “If there is a permanent or noticeable depression, the injection of [harvested fat] in tiny, thread-like injections through fat grafting is an excellent way of building back volume in an uneven area of the breast,” Dr. Richardson confirms. “This is considered safe and performed frequently for patients who have been treated for cancer and have uneven areas of the breast as a result.”

For the uninitiated, fat grafting requires fat be harvested from a donor site (typically from areas like the outer thighs or flanks) via liposuction, purified, and then injected into another area of the body (i.e. the breasts). Naturally slim or athletic patients run the risk of not having enough fat to harvest, while the best case scenario could actually yield enough fat to [augment the breasts](https://aedit.com/procedure/breast-augmentation-with-fat-transfer). “The more experienced the surgeon with aesthetic surgery of the breasts, the better the result will generally be,” Dr. Cohen says.

Though extremely rare, permanent deformity or an alteration to the size of your breast might lead you to consider [breast implants](https://aedit.com/procedure/breast-augmentation-with-implants). Even so, as one of the top five [most popular](https://www.plasticsurgery.org/documents/News/Statistics/2020/plastic-surgery-statistics-full-report-2020.pdf) cosmetic surgeries in the U.S., you might be considering breast augmentation anyway. Dr. Richardson assures us that the presence of a fibroadenoma need not deter you from undergoing any cosmetic breast surgery. “The two decisions should not have major impacts on one another,” Dr. Cohen adds.

But there are some things to consider if you have been diagnosed with a fibroadenoma. For example, if the fibroadenoma is very superficial, the addition of the implant could make it more visible. “In these cases, I would generally do both an augmentation and fibroadenoma removal with possible fat grafting, if needed,” Dr. Cohen explains. He has also had cases where the fibroadenoma was in deeper tissue, and he was able to remove it while placing implants. As an added precaution, Dr. Richardson recommends having your plastic surgeon work with a breast health specialist to ensure that the tissue is removed cleanly, expertly, and aesthetically. 

## **My Decision**
After consulting with both doctors, I decided to go through with the removal of my fibroadenoma. Dr. Cohen helped me decide to wait approximately three months to see if it leaves any sort of indentation, at which point I could consider fat grafting. As I am quite petite, there will only be enough fat to fill any hollowness that is, perhaps, present. In other words, any increase in volume would have to come via implants (and I'm not ready for those just yet).

For my removal itself, I opted for minimally invasive removal using the BARD device with Dr. Richardson. She is one of the few doctors in the entire country that removes fibroadenomas this way, and her practice graciously fit me into an appointment the very next day after my consultation — minimizing the timeframe I would otherwise experience anticipatory anxiety.

The entire appointment lasted a little bit longer than an hour. The bulk of the time was spent numbing my right breast via injections. Because the numbing contains epinephrine (which helps to constrict the blood vessels and minimize bleeding), it actually made me feel physically nervous. I felt my shoulders hunch, my heart pounded, and my voice was shaky. On the inside, though, I felt calmer than I appeared. I opted not to take a prescribed sedative so that I could drive home and keep working, as I'm a Virgo with workaholic tendencies!

As mentioned, the BARD device is used like a little mechanical Pac-Man. Dr. Richardson expertly used an ultrasound to locate and watch the fibroadenoma during the procedure, which is how she found a second — previously unknown — mass that is most likely another fibroadenoma. The larger of the two took 15 minutes to remove, a time frame she assures me is *way* longer than the norm because of its size. The other literally took five minutes! Both will be sent off to pathology to confirm the diagnosis because it is possible that the larger of the two is a low-grade phyllodes tumor.

Following the procedure, they applied a tan bandage that will remain on for five days over the tiny incision in my areola (it looks like a large pin prick!) and covered the area with gauze. Then, they wrapped me in a bandage that felt reminiscent of a corset. My breast remained numb until just before I went to bed, at which point I took two Tylenol. My outpatient paperwork indicates that I can expect soreness for a few days, but, to be frank, I cooked myself dinner the same evening as my removal and have been easily lifting my small dogs without any trouble since. I will definitely take a day off from any exercise, but am considering a ‘non-vigorous’ elliptical session tomorrow.

As for the breast itself, the appearance has not been altered a single bit. Upon touch, it feels eerily hollow for now — though it's still much better than feeling the mass — and Dr. Richardson assures me that my breast's natural tissue should fill in the space over the next three months. At that point, if I have any hesitations at all, I will be heading straight back to Dr. Cohen's office for fat grafting. And who knows? Maybe by then I'll feel [ready for implants](https://aedit.com/aedition/how-to-choose-the-right-breast-implants), as both surgeons assured me that, if and when I make that decision, my diagnosis of fibroadenomas should absolutely not be a deterrent.

No matter their origins, [scars](https://aedit.com/concern/scars) are often fraught with meaning. For some, they’re a sign of victory: the mark of survivorhood. For others, they’re a reminder of a chapter they’d prefer to close. Regardless, there remains the fact that, for a multitude of reasons, some scars heal better than others. 

When it comes to [mastectomy](https://aedit.com/aedition/what-to-expect-before-during-after-mastectomy-breast-cancer) and [breast reconstruction](https://aedit.com/aedition/comprehensive-guide-to-breast-reconstruction-nipple-reconstruction-after-breast-cancer), for example, how well your scar blends into your surrounding skin over time depends on your surgeon’s skill and your unique physiology. Proper aftercare, along with [topical scar therapies](https://aedit.com/aedition/scar-minimizing-skincare-to-use-after-surgery) (think: silicone gel or strips) and [cosmetic procedures](https://aedit.com/aedition/how-to-treat-every-type-of-scar) (like microneedling and lasers) can make a noticable difference.

In recent years, a newer scar-minimizing treatment has been gaining popularity. It’s called scar camouflage, and it uses techniques similar to [permanent makeup](https://aedit.com/aedition/permanent-makeup-after-chemotherapy-cancer) (a.k.a. cosmetic tattooing) to, well, camouflage the appearance of the mark. Here’s what you need to know about the trend.

## **What Is Scar Camouflage?**
Also known as skin color tattooing or camouflage tattooing, scar camouflage is a needle and pigment technique that blends scars into the surrounding natural skin using permanent makeup pigments. Typically performed by a medical or paramedical tattoo technician/artist, skin repigmentation has become increasingly popular for its ability to improve the appearance of scars, [stretch marks](https://aedit.com/concern/stretch-marks), and other areas affected by hypopigmentation (read: missing color).

Just like other forms of paramedical tattooing, scar camouflage isn’t the same as a typical decorative tattoo. Let’s review what makes cosmetic tattooing different from traditional tattoo techniques:

### **Traditional tattoos**
- Considered a ‘cosmetic’ by the FDA (meaning they’re not regulated)
- Ink (black and bright colors) derived from a variety of chemicals, including metals
- Injected deeper into the dermis
- Cannot be broken down by the body (i.e. they’re permanent)
- May fade slightly and/or become fuzzy over time

### **Cosmetic tattoos**
- Considered a ‘cosmetic’ by the FDA (meaning they’re not regulated)
- Micropigments (neutral skin tones adjusted with white) made primarily from titanium dioxide, as well as organic and inorganic substances
- Injected more superficially into the dermis
- Partially broken down and faded by the body (i.e. they’re semi-permanent)
- Likely to fade completely over time

## **What Type of Scars Can Be Camouflaged?**
When evaluating a scar for corrective micropigmentation, the source of the scar is not as important as its look and feel. Once fully healed, any type of pigment loss is generally amenable to micropigment color correction, including:

- Scars (from surgery or injury)
- Stretch marks
- Areas lacking pigmentation (from skin conditions like [vitiligo](https://aedit.com/aedition/guide-to-vitiligo-causes-treatments))

“Any type of surgery scar can be camouflaged, what matters most is how the scar healed,” says [Nicole Johnston](https://www.instagram.com/nicolejohnstontattoo/?hl=en), a cosmetic and restorative tattoo artist at [Studio Sashiko](https://studiosashiko.com/) in British Columbia, Canada. Typically, technicians recommend waiting until the scar is, at a minimum, one year old before opting for scar camouflage. “Scars cannot be worked on shortly after surgery,” Johnston explains. “A scar will go through many stages of healing and will change in appearance over time,” she notes. As such, Johnston typically asks clients to allow their scar to heal for at least two years before visiting her. 

Whether or not a particular scar can be repigmented depends on both the artist’s skill and the nature of the scar. The most ideal scars and stretch marks are those that are flat (or nearly flat) and paler than the surrounding tissue. “Most scars can be camouflaged,” Johnston shares. “But there are times when we won’t see ideal results.” She lists several characteristics that will likely impede a technician’s ability to successfully camouflage a scar:

- Keloid or highly textured
- Hyperpigmentation 
- Thinned, translucent skin

## **How to Prepare for a Scar Camouflage Treatment**
If you think skin color correction may be the solution for your scar, there are a few things you need to know beforehand:

### **1. Do Your Homework**
When considering corrective pigment camouflage, finding a skilled and qualified medical or paramedical tattoo artist is extremely important. Although some states are stricter than others, the United States Food and Drug Administration (FDA) considers scar camouflage to be a subset of permanent cosmetics and does not impose much regulation.

Here’s what you need to look for when searching for a provider:

- **Training & Certification**: A one-day training session isn’t going to make a technician an expert. Look into your local municipality’s laws around credentials and be sure to research how long the provider has been practicing and what certifications they’ve acquired.
- **Experience & Knowledge**: As mentioned above, a certain level of expertise can only come from years on the job, and you'll also want to be treated by someone who has experience working with clients with similar features or concerns to your own.
- **Facility**: Check to make sure the salon or office the technician works at is clean and appears to be following local standards.

- **Happy Clients**: A skilled and reputable artist will never shy away from allowing past clients to share their experience with you, so don’t be afraid to ask.

According to Johnston, “experience and honesty” are two of the most important qualities to look for in a skin camouflage professional. “Scars are complicated and every situation is different,” she explains. “If an artist gives you an immediate ‘yes’ without indications of what to expect with your results, you may want to keep researching.” As it relates to honesty: “I regularly turn away clients if I know I cannot make worthwhile improvements,” she adds.

### **2. Expect a Consultation**
During your consultation, your tattoo artist will evaluate your particular scar and ask many questions — particularly about your medical history — in order to gain a better understanding of your body chemistry. This includes questions about:

- Medications (including vitamins and supplements)
- Any traditional, medical, or cosmetic tattoo history
- Skin type
- Allergies
- Autoimmune conditions
- Scar tendencies
- Ability to schedule follow-up treatments

### **3. Be Prepared to Prep**
Based on the appearance of your scar, your medical tattoo artist may suggest any number of pre-treatments. “Often, I will recommend a client get laser or microneedling done on a scar prior to coming to me so that we can see the best results,” Johnston says. “[Laser](https://aedit.com/procedure/nonablative-fractional-lasers) and [microneedling](https://aedit.com/procedure/microneedling) will help flatten the scar and improve its texture.” Another benefit of lasers, she adds, is that they can also lighten [hyperpigmentation](https://aedit.com/concern/uneven-skin-tone). “By doing this first, the client can see a much better result than if I were to only work on the color of the scar,” Johnston shares.

### **4. Consider Cost**
The price of a skin repigmentation varies by geographic location and the experience level of your tattoo professional. Expect to pay anywhere from several hundred dollars for work on a very small scar to several thousand for larger, more complex scar work performed by an in-demand artist. Typically, scar camouflage is not [covered by insurance](https://aedit.com/aedition/insurance-coverage-for-plastic-surgery-cosmetic-procedures) — unlike other types of medical tattooing, like [reconstructive nipple and areola](https://aedit.com/aedition/reconstructive-nipple-tattoos-for-breast-cancer-survivors-dominique-bossavy), for which some insurance companies do reimburse patients (at least partially).

## **What to Expect After a Scar Camouflage Treatment**
“I always tell my clients that they shouldn’t expect to see immediate results,” Johnston says. The reason for this is because the tattoo needs to continue to be perfected. “In order for the results to look the most natural, I am conservative with the amount of color I add in one session to the scar,” she shares. “My focus is for the result to look like skin and not like I’ve applied makeup to the area.” As a result, “clients will see full results after their touch up session,” Johnston notes — and she offers additional touch ups as needed. 

As with any healing process, it’s important to remember that the recovery from scar camouflage is an evolution. “A phrase I use with my clients is, ‘everything looks worse before it looks better,’” she says. “Scars and stretch marks are more fragile than the surrounding skin, therefore they tend to respond with a bit more redness or swelling when being worked over.” These side effects usually subside within a couple days, Johnston explains, but, for some individuals, the residual redness can last a few weeks.

### **What Are the Risks of Scar Camouflage Tattooing?**
Just like a conventional tattoo, corrective cosmetic tattooing carries certain risks. Choosing a well-qualified provider will minimize your chances of complications, which include:

- Infection
- Allergic reaction
- Granulomas 
- Poor color match
- Fading
- Discoloration

Sticking to the recommended after-care regimen is also extremely important, as your tattoo is essentially an open, healing wound for two to four weeks

## **Is Scar Camouflage Reversible?**
[Laser tattoo removal](https://aedit.com/procedure/laser-tattoo-removal) has come a long way in recent years. When it comes to skin repigmentation tattoos, however, trying to reverse the process isn’t as straightforward as it seems. Lasers target the color molecules and break them apart to be excreted by the body, but they can’t be used on titanium dioxide (a white pigment very often used to match lighter skin tones). Lasers turn this white metal gray, leaving an even more conspicuous mark.

Although there are alternative options for removing permanent makeup pigments, scar tissue doesn’t behave the same way healthy skin does. All of these factors make it much more difficult to alter a cosmetic tattoo than a decorative one or even [microblading](https://aedit.com/aedition/microblading-treatments-for-fuller-thicker-eyebrows).

## **The Takeaway**
A well-executed scar camouflage tattoo can be life-changing for clients — improving self-esteem and helping them move on. “This procedure can have incredible effects on a patient’s quality of life,” Johnston notes. As she explains, scars are often associated with difficult memories. “For our breast cancer survivors, it can help them feel like themselves again and recognize what they see in the mirror,” she shares. “We also help clients cover the scars from their abusers or from self-harm done in the past.” In many cases, scar camouflage represents a fresh start. “It’s a way for them to move on,” Johnston says, “and not see the reminder every day.”

Treatments like chemotherapy and radiation are a necessity for many cancer patients. According to [Elizabeth Comen, MD](https://www.mskcc.org/cancer-care/doctors/elizabeth-comen), a medical oncologist at Memorial Sloan Kettering Cancer Center in New York City, the exact effects differ from patient to patient and tend to correspond to the course of treatment they are undergoing. Even so, there are some commonalities. “Broadly, chemotherapy can make the skin drier and itchier,” she says. “The skin can also become more sensitive to the sun and [hyperpigmentation](https://aedit.com/aedition/what-its-like-to-treat-hyperpigmentation-with-skincare-cosmetic-treatments).” 

The good news, she notes, is that, “once a specific treatment ends, skin changes usually resolve within a few weeks.” But that doesn’t mean there aren’t steps you can take to mitigate some of the side effects in the meantime. Rogers, MD, a Seattle-based board certified dermatologist and founder of [Doctor Rogers Restore](https://go.skimresources.com?id=170670X1638612&xs=1&url=https%3A%2F%2Fwww.doctorrogers.com%2F). “Instead, the chemotherapy kills anything that is growing quickly.” This, she explains, is why people lose their hair, too. “It is a fantastic, life-saving treatment, but your whole body suffers through chemotherapy — the good cells and the bad cells,” she notes

Radiation is also tough on the skin. “The targeted radiation has to get to the organ being treated through the skin, making the skin right in its blast path,” Dr. Rogers shares. As she explains, when people go through radiation, their skin becomes red and then begins to lose its integrity. It first presents “as [eczema-like](https://aedit.com/aedition/best-skincare-products-to-treat-eczema-flare-ups-in-the-winter) changes” that “can progress to open sores,” she adds. Cynthia Besteman, a breast cancer survivor who created [Violets Are Blue](https://go.skimresources.com?id=170670X1638612&xs=1&url=https%3A%2F%2Fvioletsareblueskincare.com%2F) skincare as a result of her own experience, agrees. Besteman notes that both chemo and radiation “suck up” all of the skin’s moisture.

Because of these effects, Dr. Comen says that there are dermatologists who work hand-in-hand with patients as they undergo cancer treatments. In her opinion, a dermatologist should be part of the medical team for both preventative and corrective purposes. “It is important that patients not only see their dermatologist for [routine skin checks](https://aedit.com/aedition/dermatologist-skin-cancer-screenings-mole-checks), but also check in with their dermatologist should they be undergoing treatment for any type of cancer,” she says. 

## **How to Deal with Skin Changes During Cancer Treatment**
The body’s main job during cancer treatment is an obvious one: to fight the cancer. A skincare routine during this time should be one that is nourishing, restorative, and, most importantly, feels good. During this time, “everything you use on your skin will be more absorbed, so you need to be selective to prevent unwanted reactions,” Dr. Rogers cautions. As such, you should avoid exfoliation and active ingredients like hydroxy acids ([alpha](https://aedit.com/aedition/guide-to-topical-alpha-hydroxy-acids-in-skincare), [beta](https://aedit.com/aedition/guide-to-beta-hydroxy-acids-salicylic-acid-in-skincare), or [poly](https://aedit.com/aedition/guide-to-topical-polyhydroxy-acids-in-skincare)), [retinols](https://aedit.com/aedition/guide-to-vitamin-a-retinol-retinoids-in-skincare), and even [vitamin C](https://aedit.com/aedition/guide-to-topical-vitamin-c-in-skincare).  

Of course, that doesn’t mean a patient needs to forgo their skincare routine all together — especially if it maintains a sense of normalcy (and maybe even pampering) during an otherwise challenging time. Karen Ballou, a cancer survivor and founder of [Immunocologie](https://go.skimresources.com?id=170670X1638612&xs=1&url=https%3A%2F%2Fimmunocologie.com%2F), recommends looking for ingredients that are nourishing, hydrating, and anti-inflammatory. *Since 1985, October has served as Breast Cancer Awareness (BCA) Month — a period devoted to educating the public on the disease that, [according to the Centers for Disease Control and Prevention (CDC)](https://www.cdc.gov/cancer/breast/statistics/index.htm), is the second most common form of cancer in women, regardless of race or ethnicity. Men can also be diagnosed with breast cancer (albeit at a much lower rate), and The AEDITION is devoting much of its coverage this month to [BCA](https://aedit.com/aedition/category/breast-cancer-awareness), from expert guides to mastectomies and reconstructive breast surgery to powerful patient perspectives and roundups of products that give back.*

The physical and emotional toll a breast cancer diagnosis takes is considerable. In addition to the fear and confusion often associated with various treatment options, breast cancer patients  must also consider the [breast reconstruction process](https://aedit.com/aedition/post-mastectomy-breast-reconstruction-process-breast-cancer) and how that might impact their overall health and sense of identity. Fortunately, breast reconstruction techniques have come a long way in recent years, and one such example is the rising popularity of one-stage breast reconstruction. Here, we’re breaking down everything you need to know about the singular surgery — from who’s a candidate to what’s involved — with one its pioneers. 

## **What Is One-Stage Breast Reconstruction?**
Also known as direct-to-implant (DTI) breast reconstruction, [one-stage breast reconstruction](https://aedit.com/aedition/comprehensive-guide-to-breast-reconstruction-nipple-reconstruction-after-breast-cancer) refers to when a plastic surgeon places implants at the time of a mastectomy. “This is in contrast to tissue expander reconstruction, which is more commonly performed, where a special expansion device is put in the breast at the time of mastectomy,” explains  Lisa Cassileth, MD, a board certified plastic and reconstructive surgeon and founder of [Cassileth Plastic Surgery](https://www.drcassileth.com/surgeons/dr-lisa-cassileth.html) in Beverly Hills.

For women who are interested in breast implants [after a mastectomy](https://aedit.com/aedition/mastectomy-breast-cancer-experience-from-patient-perspective), the standard has long been to schedule a delayed breast reconstruction. Instead of putting the implants in during the mastectomy surgery, the patient would have tissue expanders sutured into the breast pocket. In the weeks and months that follow, the expanders are gradually inflated in a plastic surgeon’s office using air or saline. Once they reach the appropriate size, the expanders are swapped out for implants. 

The expansion process can be painful, but some surgeons believe it is a necessary evil. Dr. Cassileth and her team, however, have been refining what they consider to be a more desirable alternative. Back in 2012, Dr. Cassileth’s [original research](https://pubmed.ncbi.nlm.nih.gov/21734545/) on one-stage reconstructive surgery was published in the *Annals of Plastic Surgery*. The study found the procedure to have lower risks compared to traditional two-stage tissue expander reconstruction and highly satisfactory outcomes. The technique has only improved from there.

## **Ideal Candidates for One-Stage Breast Reconstruction** 
For many years, it was thought that only women with smaller, non-droopy breasts could be candidates for DTI, but that is no longer the case. “DTI is good for women who qualify for [nipple-sparing mastectomy](https://aedit.com/aedition/what-to-expect-before-during-after-mastectomy-breast-cancer), since no skin will be cut away during the mastectomy and the lost volume of the breast can immediately be replaced without worrying about skin tension,” Dr. Cassileth says. With that said, patients with cancers that grow into the skin and must have skin removed may not qualify.

## **How One-Stage Breast Reconstruction Works** 
At Cassileth Plastic Surgery, DTI is performed over the muscle for the most natural-looking results. “This is in contrast to the under-the-muscle procedure, which was pretty standard years ago and now is more rarely performed because it cuts the pectoralis muscle,” Dr. Cassileth notes. When the breast implant is placed above the pectoralis major muscle, you avoid the so-called ‘pec flex’ deformity some sub-pectoral implant patients see and also experience a less painful recovery because the muscle remains intact.

Rather than simply wrap the implant in an acellular dermal matrix and place it in the mastectomy flap, Dr. Cassileth creates an internal bra structure to support it. By using a dermal matrix to bridge the gap between the implant and the chest wall, she prevents drooping at the bottom of the breast pocket, softens the contour of the implant, and decreases the need for [fat grafting](https://aedit.com/procedure/breast-augmentation-with-fat-transfer). Additionally, Dr. Cassileth performs a nerve graft at the same time as the reconstruction to restore sensation to the breast. Sensation improves as you heal.

It is important to note that the success and safety of a DTI breast reconstruction is not just in the hands of the plastic surgeon. One-stage reconstruction should only be performed when a mastectomy surgeon has a low rate of flap necrosis. Dr. Cassileth says that the rate should be less than one percent or else it is not safe to go direct-to-implant because the breast skin may not survive.

Following a mastectomy and one-stage breast reconstruction, patients stay in aftercare for two to three days. Drains are typically removed one to two weeks after surgery. “They can have normal day-to-day activities, like driving, after two weeks, and are allowed to lift heavy objects and [exercise](https://aedit.com/aedition/when-to-start-exercising-again-after-cosmetic-surgery-procedures) after one month,” Dr. Cassileth says. 

## **A Non-Implant Alternative...**
For women who do not wish to have implants or are not a candidate for them, there is another innovative option to consider. Developed by Dr. Cassileth and mastectomy surgeon [Heather Richardson, MD](https://www.bedfordbreastcenter.com/mastectomy-surgeon-heather-richardson-md/), of the Bedford Breast Center in Beverly Hills, [SWIM breast reconstruction](https://aedit.com/aedition/swim-breast-reconstruction-for-breast-cancer-mastectomy-patients) combines a nipple-sparing mastectomy with a breast reduction (SWIM stands for skin-sparing wise-pattern internal mammary perforator). 

In this procedure, the mammary tissue is removed during the mastectomy portion of the surgery, but the skin and subcutaneous fat are preserved. During reconstruction, the extra fat is folded and placed to create the breast shape. Unlike traditional TRAM and DIEP flap surgeries, no muscle is sacrificed and no abdominal, back, or buttocks tissue is transferred. This provides an alternative for women who want to avoid the extensive surgery, incisions, and long recovery period involved with other flap procedures.

Breast reconstruction techniques have come a long way in recent years, and one such example is the rising popularity of direct-to-implant surgery. Here’s what you need to know.

Women and men coveting the treatments’ ‘I woke up like this’ promise have propelled the industry into big business. But for many clients, permanent and semi-permanent makeup services go far beyond convenience. In the case of cancer survivors and chemotherapy patients, ‘beauty’ treatments like microblading, permanent eyeliner, lash enhancement, and scar camouflage can be a major step toward regaining self-confidence and a sense of normalcy. Here, we break down everything you need to know with the help of our medical and aesthetic experts.

## **How Semi-Permanent Makeup Helps Cancer Patients**
Cancer treatments (like surgery, chemotherapy, and radiation) can often have an impact on how patients look and feel. Hair loss related to cancer treatments is known as alopecia. It varies from patient to patient and is dependent upon the chemotherapy drugs prescribed and how they react with each person's physiology. “Undergoing chemotherapy, the chances of hair loss are very real and usually rapid for both men and women,” says [Craig Ziering, DO](https://zieringmedical.com/), a board certified dermatologist and hair restoration specialist in West Hollywood. “As the chemotherapy drugs rapidly attack growing cancer cells they also attack other rapidly growing cells in your body — including those in your hair roots.”

Some patients will have patchy hair loss or thinning, while others may lose more — including eyebrows, eyelashes, and armpits. Radiation therapy can also impact the hair, Dr. Ziering says, though usually only in the treatment area. Depending on their needs, patients can choose from both permanent makeup options (with traditional tattoo ink) and the more modern semi-permanent ones (using fruit- and vegetable-based pigments) to restore the appearance of lost hair. These treatments include:

- [Eyebrow microblading](https://aedit.com/aedition/microblading-treatments-for-fuller-thicker-eyebrows)
- Eyeliner and lash enhancement

Permanent makeup expert [Dominique Bossavy](https://www.dominiquebossavy.com/) says that about 20 percent of her clientele is [breast cancer patients](https://aedit.com/aedition/category/breast-cancer-awareness). “They come to me first for eyebrows and lashes in anticipation of losing them to chemo or once they are done with chemotherapy,” she explains. “Usually eyebrows will not regrow or not regrow properly and they may have bald spots. Lashes typically thin out dramatically following chemotherapy. And then there is the need for replacement in trompe l'oeil for the areola.”

But it’s not just for women. Dr. Ziering specializes in hair restoration for men — including men who have lost hair to cancer treatment. He favors a method called scalp micropigmentation (SMP). “SMP is a type of tattooing technique in which microparticles of metabolically inert pigment granules are implanted below the epidermis with a specific, artistic technique of strokes to appear similar to a closely shaved scalp,” he explains. “Over time with SMP, the pigment will fade but the color will not change, so it may give a cancer patient confidence, support, a positive outlook and other positive psychological effects while their hair is growing back.”

## **Permanent Makeup Before & After Chemotherapy**
Timing is important for cancer patients considering permanent and semi-permanent makeup procedures. Because these techniques cause trauma to the skin, they should not be performed on those with compromised immune systems. The general recommendation is to wait six weeks before or after any type of cancer treatment, though it’s absolutely necessary to consult with your oncology team before undergoing any elective or cosmetic procedure.

Both the technique and type of ink or pigment used should play a role in the timing, says permanent makeup expert Yelena Nesterenko. As she explains it, chemotherapy can alter the appearance of the organic, semi-permanent pigments that she prefers to work with — turning browns to blue, black, and even purple. While that’s a big drawback for permanent eyeliner, it’s not as big of a deal for microblading to replace lost eyebrow hair, since it tends to fade more quickly.

While many patients seek treatment (especially for eyebrows) prior to losing their hair, some experts advise a “wait and see” approach. “Methods used to help eyebrow loss may not be necessary in everyone, since not all patients lose brows,” says [Evangelia Kirimis, MD](https://www.uclahealth.org/evangelia-kirimis), a board certified hematologist and medical oncologist at UCLA. “So, I would recommend reserving until after chemo to see what level of help is needed.”

Nesterenko prefers to work with post-treatment patients, though she makes sure all of her clients are prepared for every possible outcome, including additional rounds of treatment. “Your technician should explain everything with details about the color and about the quality,” she says. “Nobody knows how many times the cancer patient will need to undergo chemo.”

If you're concerned about replicating the color and shape of your natural brows exactly, it’s a good idea to meet with your permanent makeup artist for a pre-chemo consultation. If you do end up losing your brows and lashes, photos, sketches, and notes can later serve as a benchmark for your desired results.

## **Permanent Makeup Safety for Cancer Patients and Survivors**
At this point, you’re likely wondering about the safety profile of such treatments. Most experts agree it’s relatively safe, as long as a highly trained technician using quality pigments performs the procedure. Because the body is so sensitive during cancer treatment, any form of tattooing —  which introduces foreign substances into the body through trauma — should be vetted by your cancer care team. The type of pigments and/or ink used, the application technique, and the timing should all be carefully considered.

Many breast cancer patients undergo some form of hormone therapy, since certain types of breast cancer are stimulated by estrogen and/or progesterone. “We know that even low dose birth control pills can affect the pigment,” Nesterenko says. “So, women have to be especially careful about any kind of chemo that affects the hormones.”

Our permanent makeup experts always make sure they have a thorough understanding of their clients’ medical histories and treatment plans. Nesterenko typically works with cancer patients who are referred by doctors she regularly partners with. Bossavy says that many physicians refer patients to her, too, and she requires that clients undergoing treatment receive clearance from their doctor first.

### **Potential Risks and Complications** 
Cancer patients face many of the same risks and complications from permanent makeup as the general population. These risks include:

- Premature/unsightly pigment fade
- Infection
- Allergic reaction
- Scarring
- Granulomas

As mentioned, infection risk may be higher in chemotherapy patients. According to Nesterenko, the effects of chemotherapy on the body often alters the way the skin interacts with both ink and organic pigments, which is why she prefers to work with clients who have finished treatment.

Could tattoo inks and permanent makeup pigments possibly pose an additional cancer risk? Currently, there’s no evidence to suggest it. But Bossavy tells us that, when it comes to the long term consequences of permanent makeup, there’s just not that much scientific evidence out there. “More research is needed to further understand the true implications of these findings and to develop guidelines for the safest tattoo procedures,” she says.

## **How to Find a Permanent Makeup Artist Post-Chemotherapy**
If you are curious about how to find a permanent or semi-permanent makeup artist post-chemo, it’s important to devote the same time and care into researching the technician’s credentials as you would any other provider. Below are five tips for finding the best person for your needs:

1. **Talk to Your Doctor**: Asking your physician(s) for a recommendation is always the best place to start, Nesterenko says.
2. **Consult with Support Groups**: Referrals from other patients can help tremendously when you’re already overwhelmed with treatment-related research.
3. **Ask About Credentials**: It’s important to seek out someone who has worked extensively with cancer patients, not just for cosmetic clients.
4. **Interview Previous Clients**: Ask prospective technicians if you can talk to their other post-cancer clients to learn about their experience and how their results fared over time.
5. **Find Out Inks/Pigments**: “Ink is more stable,” Nesterenko says. “It doesn’t change over time like pigment does, but it’s not safe to use those chemicals on the face.”

Bossavy also tells us that building a rapport is equally critical. “First and foremost is a trust connection. This is a very important part of the process,” she shares. “Many emotions will surface, so I would say [to find] someone that has compassion and someone who loves what they do.” As she explains, it's not uncommon that a cancer survivor will be overwhelmed and moved to tears during the session, which makes finding an artist who is warm, patient, and understanding (in addition to well-qualified, of course!) is all the more important. 

##**The Takeaway**
In the end, the decision to seek permanent or semi-permanent makeup before or after cancer treatment is a very personal one. “Many survivors refer to it as life-changing,” Bossavy says. “It brings back a sense of normalcy and makes them forget the constant reminder of their traumatic and arduous journey of survival during the battle they just had to fight.”

For some, it doesn’t happen often, if at all. No, this isn’t an excerpt from a *Sex And The City* episode — it’s a concern women *and* men face. It is estimated that 10 to 20 percent of the female population experiences inverted nipples, and there are many ways to address it. Whether the desire for a more projected look is for aesthetic or functional (think: breastfeeding) reasons, we’re breaking down the treatment options for inverted nipples. 

## **What Are Inverted Nipples?**
Before we get started, it’s important to note that inverted nipples are a benign condition if you’ve always had them. If you experience a sudden nipple inversion, however, it is important to consult with a medical professional, as it could be a symptom of [breast cancer](https://aedit.com/aedition/category/breast-cancer-awareness) or another medical condition. 

Anatomically speaking, the nipple is the part of the areola (the circular ring of pigmented skin around the nipple) that typically protrudes through stimulation or a decrease in temperature. If that doesn’t happen, it may be because the nipple is inverted. “Inverted nipples may be congenital (many patients are born with them) or can be caused by breastfeeding (due to milk duct scarring), trauma, cancer, surgery, or infection,” says [Umbareen Mahmood, MD](https://aedit.com/provider/umbareen-mahmood-md-23), a New York-based board certified plastic and reconstructive surgeon.

To better understand the difference between permanently inverted nipples and those that lay flat on occasion, the technical categorization is as follows: 

- **Grade 1**: “The nipple is either flat or mildly inverted,” Dr. Mahmood explains. “It is easily manually everted, can become everted by touch or temperature, and it maintains its projection.”
- **Grade 2**: “The nipple is manually retractable, but with difficulty, and the nipple tends to retract again immediately,” she says. 
- **Grade 3**: The most severe form, Dr. Mahmood shares that this level of inversion does not allow for manual retraction and “breastfeeding is unlikely.” 

No matter where someone falls on the scale, there are options for those who want a more everted appearance that can be done at home, in a piercing studio, or with the help of a medical professional. 

## **Non-Surgical Solutions for Inverted Nipples**

Suction products, massage options, and even piercings claim to help the nipple protrude. 

- **At-Home Exercises**: For those with grade 1 or grade 2 levels, Dr. Mahmood recommends “manual at-home exercises that place pressure at the base of the nipple or suction cup devices that are placed over the nipple and gently stretch the ducts over time.”  
- **Nipple Piercings**: Nipple piercings with barbells are another non-surgical treatment to keep nipples everted for grades 1 and 2, Dr. Mahmood notes. The piercing is not nearly as painful as it sounds and should heal in a few months if proper protocol (wearing a compression garment like a sports bra for the first few days and cleaning regularly) is followed. Another benefit? Many cite an increase in sensitivity to the area after piercing. 

If the at-home methods aren’t working and you aren’t interested in a piercing, surgery may be the best solution. 

## **Surgical Solutions for Inverted Nipples** 
[Inverted nipple repair](https://aedit.com/procedure/inverted-nipples) is a relatively simple surgery performed under local anesthesia that offers permanent results. A small incision is made at the base of the nipple so that “the constricted and tethered ducts [can be] released,” Dr. Mahmood explains. A suture is then placed at the base to support the released nipple. “Pain is very minimal, but there may be some soreness,” she says. Work can be resumed within a few days but patients are “advised to abstain from vigorous exercise for two weeks.” 

Some patients may experience a loss of sensation after surgery, though it is rare. “Nipple sensation is rarely affected after the surgery,” Dr. Mahmood says. Breastfeeding is another consideration. “Nipple stretching techniques for grade 1 and grade 2 do not affect breastfeeding, as milk ducts are left intact,” she explains. “For grade 3, the constricted ducts need to be divided, which will impact breastfeeding.” While diminished milk supply is a potential side effect, “things like pumping as well as medications that stimulate milk production can help women breastfeed,” she adds. 

At the end of the day, women who wish to [breastfeed](https://aedit.com/aedition/how-pregnancy-and-breastfeeding-impact-breast-implants) need to weigh their options. “It depends on how critical breastfeeding is to them,” Dr. Mahmood says. “If they are a grade 3, they may have zero chance at breastfeeding whereas, with the surgery, they may be able to.” 

## **The Takeaway** 
Inverted nipples are often a genetic condition that is not a cause for concern — unless it happens suddenly. While there are non-surgical solutions for those with grade 1 and grade 2 inversion, women who are grade 3 and want a chance to breastfeed may consider an inverted nipple repair surgery.

For women affected by breast cancer or those with the BRCA gene mutation who are proactive, post-mastectomy life used to be either an implant or no-breast kind of existence. There is, however, a promising new reconstructive procedure offering [mastectomy patients](https://aedit.com/aedition/what-to-expect-before-during-after-mastectomy-breast-cancer) a third option. It’s called SWIM Breast Reconstruction, and we spoke to the woman who is pioneering the technique, Beverly Hills-based board certified plastic and reconstructive surgeon [Lisa Cassileth, MD](https://www.drcassileth.com/surgeons/dr-lisa-cassileth.html), to learn more about the method.

## **History of SWIM Breast Reconstruction** 
First and foremost, you may be wondering what ‘SWIM’ means. SWIM stands for skin-sparing wise-pattern internal mammary perforator. The procedure, which Dr. Cassileth says “spares the skin you need and spares the mastectomy appearance,” combines a nipple-sparing mastectomy with a breast reduction. Blood flow to the nipple is preserved, all breast mammary tissue is removed, and the leftover skin and fat from the original breast and surrounding area is folded to create a new breast.

The idea for this procedure was born when Dr. Cassileth and mastectomy surgeon [Heather Richardson, MD](https://www.bedfordbreastcenter.com/mastectomy-surgeon-heather-richardson-md/), of the Bedford Breast Center in Beverly Hills, were talking about the limited options patients are offered. The fact of the matter is, all too often, the [plastic surgeon isn’t in the room](https://aedit.com/aedition/post-mastectomy-breast-reconstruction-process-breast-cancer) for reconstruction conversations before or during a mastectomy procedure. Getting rid of the cancer is, understandably, the only goal. As a result, many patients simply leave with a scar. 

When the two surgeons finally worked on the same patient, it wasn’t an ideal situation. There was a radiated breast on one side and new cancer on the other. The patient — Dr. Cassileth’s coordinator’s mother in law — needed a bilateral mastectomy, and the doctors wanted to do the best thing they could for her. The duo created the idea of the SWIM reconstruction procedure, performed it together, and were pleased with the results. 

Within weeks, they had a similar patient. “We developed this procedure over time for these needy patients with nowhere to go and no options for them,” she explains. Previously, many women who had total mastectomies end up wearing a bra with a ‘chicken cutlet’ insert or a padded bra to match the other breast. But, when they raise their arms, the bra can “stick” in their mastectomy divot, Dr. Cassileth says. This is a common experience for thousands of women. As time went on, another segment of the population also embraced the SWIM technique — a younger demographic who wanted to avoid implants.

## **How SWIM Breast Reconstruction Works**
Before this procedure, Dr. Cassileth says her practice encountered women who simply didn’t want implants or aren’t a good candidate for them. The way the SWIM procedure works is that, after a mastectomy, the skin and fat surrounding the breast are meticulously folded and reshaped to create a new breast, while preserving the existing nipple. The breast reconstruction takes place during a one-stage surgery and adds a couple hours to the mastectomy.

First, the mammary tissue is removed during the mastectomy portion of the procedure, while the skin and subcutaneous fat are preserved. During the reconstruction portion of the surgery, the extra fat is folded and placed to create the breast shape. Unlike traditional flap surgeries, no muscle is sacrificed and no abdominal, back, or buttocks tissue is transferred. This provides an alternative for women who want to avoid the extensive surgery, incisions, and long recovery period involved with a [TRAM and DIEP flap procedures](https://aedit.com/aedition/comprehensive-guide-to-breast-reconstruction-nipple-reconstruction-after-breast-cancer). 

There are no long-term negatives or additional surgeries with the SWIM breast reconstruction, Dr. Cassileth says. But there are some finer details to understand. Below, she answers some common questions about the procedure:

### **1. Who Is A Good Candidate for SWIM Breast Reconstruction?** 
Women with breasts that are a B cup or larger are good candidates for this procedure. “Any configuration of a female bigger than a B, you’ve definitely got extra skin,” she explains. You take the breast tissue out of the breast, and you still have all the skin. “You use the extra and fold it up to make this cute breast ... and there’s no maintenance on it,” she says.

Proactive patients, such as those with the BRCA gene mutation, have been opting for the SWIM procedure because they often have a bit more time to consider their options. “The more time people have to research, the more likely they are to find this surgery,” Dr. Cassileth says. These women represent about 50 percent of Dr. Cassileth’s patients for this procedure. “Many women don’t want implants because the implant is foreign body,” she explains. “Plus, they don’t want surgery every 15 years to redo it.”

### **2. Who Isn’t A Candidate For SWIM Breast Reconstruction?** 
If someone is an A cup or smaller, they’re not a good candidate. “They need leftover skin to make a breast out of,” Dr. Cassileth explains. 

### **3. Is there more risk of cancer returning with SWIM reconstruction?**
No, the mammary tissue is completely removed with the SWIM flap procedure, Dr. Cassileth says. “It follows a complete, total, skin-sparing mastectomy — the same mastectomy as is done with a typical reconstruction,” she adds. It’s important to note, however, that no mastectomy has zero recurrence risk. Recurrence is one to three percent for every mastectomy, regardless of the reconstruction technique. 

### **4. Can the new breast(s) be customized?**
“The hard thing to counsel on is gauging how big people will be, and they do ask,” Dr. Cassileth says. It’s a guess, as she must see what’s left over from the skin and subcutaneous fat after the mastectomy. “You could have very heavy breasts but a small amount of subcutaneous fat,” she explains. You can also have little breasts with a lot of subcutaneous fat. Dr. Cassileth can estimate from there and use whatever her patients “give her.” “I’ll keep whatever I can,” she says. 

While a patient typically won’t go from a D cup to an A, sometimes people say they want to be at least a C. In this case, patients can return for an additional procedure, like [fat grafting](https://aedit.com/aedition/why-everyone-is-talking-about-fat-grafting), down the line. “That is an additional procedure and is all natural, all you,” Dr. Cassileth explains. “But you have to build the foundation and go from there.”

### **5. What Is SWIM Breast Reconstruction Recovery Like?** 
Dr. Cassileth says it’s similar to mastectomy reconstruction and takes about a month. You have a week of being sore and a second week of being functional, but you can’t jog or lift anything heavy. “I won’t let someone do super heavy weights until the fourth week,” she says, but, by the third week, you’re pretty much back to your regular routine. This recovery is two weeks shorter than direct-to-implant reconstruction, which is a procedure she still performs regularly. 

## **The Takeaway**
Earlier this year, Dr. Cassileth and Dr. Richardson had an [article published](https://www.journalacs.org/article/S1072-75152030076-4/fulltext) in the *Journal of the American College of Surgeons* about the procedure, and the pair have been training fellow surgeons in the technique. The hope is that, long term, SWIM breast reconstruction will catch on and become a safe, effective, and common breast reconstruction option for women around the world.

Because breast cancer awareness has become so mainstream in recent years with everyone from celebrities to the NFL dedicating time and resources to supporting the cause, men and women alike are increasingly aware of the warning signs. Women especially are encouraged to regularly conduct their own self-breast exams and may even ask their partners to let them know if they notice any changes, too. 

But what happens if/when you find a lump? Because breast cancer has such a high profile and statistics like *one in eight women will be diagnosed in their lifetime* are well known, people are often fearful to seek medical advice after noticing a change in their breast tissue because they assume the worst. 

While it is absolutely essential to get any changes checked out by a medical professional, it is also important to remember that 80 to 85 percent of lumps found in women under the age 40 are benign and caused by fibrocystic changes, cysts, fibroadenomas, or fat necrosis to name a few.

With this in mind, it is important to understand the function and importance of breast exams. ## __Breast Check Basics__
For women with no family history of breast cancer, [the American College of Obstetricians and Gynecologists advises](https://www.acog.org/patient-resources/faqs/gynecologic-problems/mammography-and-other-screening-tests-for-breast-problems) people in their twenties and thirties have a breast examination carried out by a healthcare provider every one or three years. The American Cancer Society, meanwhile, [recommends annual mammograms](https://www.cancer.org/healthy/find-cancer-early/cancer-screening-guidelines/american-cancer-society-guidelines-for-the-early-detection-of-cancer.html) for women between the ages of 40 and 55. Women over 55 can switch to mammograms every two years or continue with yearly screenings. But that doesn’t mean you should just sit around for your next trip to the gynecologist. 

Women are encouraged to conduct a self-examination about once a month. Because benign lumps are known to appear over the course of a woman’s menstrual cycle, it is best to perform the exam at the same time every month — ideally a few days after your period ends.

During the exam, it is important to be on the lookout for any changes in the appearance of both the exterior breast (skin, areola, and nipple) *and* the interior tissue. Things to feel and look for include:

- A visible change in the shape or size of the breast or nipple
- A change in how the breast skin looks or feels (think: dimpling or puckering) 
- Soreness, redness, or rashes on the breast or underarm area
- Any areas that are visibly different compared to the rest of your breast tissue
- A lump (can be a small as the size of a pea) that persists in the breast or underarm area

If you notice any of these symptoms or something just doesn’t feel like your version of ‘normal,’ it is time to consult a medical professional for a more thorough check. Chances are, it is simple to treat. But if it is breast cancer, early detection is key.

## __Patient Perspective__ 
There are no two ways about it: finding a lump in your breast or noticing some other change to the chest can be alarming. But that is not an excuse for inaction. Here, The AEDITION speaks to three women who lived through a breast cancer scare about their experiences and why they encourage everyone to consult a doctor as soon as they notice something isn’t quite right.

    What caused you to become concerned about your breasts?

    I was at college and aware that I needed to check myself every so often. I didn’t check as often as I now know I should, but one day I was in the bathroom and found a lump. It was probably around the size of an olive. I panicked and decided I wouldn’t tell anyone. My theory was that if I ignored it, it would go away. I would prove to myself that it wasn’t anything serious. But after a while, it was still there. A family friend had been diagnosed with cancer recently, so I guess it was on my mind. I made an appointment to see my doctor. At that point, I was convinced the only thing it could be was cancer.

    What happened during your doctor’s appointment?

  <Response party={Anna}>
    I explained to the physician that I found this lump and that I thought I might have breast cancer. I was so anxious, but the doctor took the time to listen to me while I gave my garbled version of events. She then checked the lump herself, which was uncomfortable, but it didn’t take too long. She then asked me whether I had any pain, whether it changed during my cycle, and whether or not I’d noticed any other symptoms. I was referred for an ultrasound — my doctor explained it would give her a clearer idea of what was going on — but she also took some time to reassure me that it could very easily be something simple to treat and not cancer at all.

    What did the next steps look like for you?

    First of all, I told a friend, which was probably the best thing I did throughout the process. She was able to reassure me and she also came with me to my other appointments. I had the ultrasound quite soon after the first appointment. Again, it was uncomfortable in that I’m not keen on being naked in front of random people, but, other than that, it wasn’t painful or anything. About a week after that I went back to the primary care doctor, who explained to me that it was a cyst. Because it was filled with liquid and not solid, I didn’t even need to have a biopsy. She told me to keep an eye on it, and if it became painful, they could offer me some other treatment options. That was about four years ago now, and I haven’t had any problems since. I do check my breasts regularly though, and I’m such an advocate for people getting any concerns checked out quickly.

    What advice do you have for someone who finds themselves in a similar situation?

    I would say do the brave, grown-up thing and get it checked. Don’t bury your head in the sand because, if it is cancer, that’s literally the worst thing you could do. I think the awareness we have of breast cancer is amazing now, but it can make finding an issue so scary because the first thing that comes to mind is cancer. I also think people should share their concerns. Chances are a friend has been through a very similar thing — especially by the time you reach your late twenties. I know so many people who have been through the same panic. It’s best to share with both friends and doctors.

    What led you to believe you might have breast cancer?

    I was checking my breast, which I do regularly now that I’m older. I felt something a bit different on my right side — almost in my underarm area. I had a sinking feeling when I first felt it and managed to calm myself down enough to have a Google, which, in hindsight, was not my best idea. I was pretty sure what I found could be a sign of breast cancer, and, honestly, I was scared. It took me a few days to gather together the courage to get a consult, but I didn’t want to leave it because I know how important it can be to get a diagnosis as soon as possible.

    What pushed you to visit a doctor?
  </Question>
  <Response party={Stephanie}>
    I think breast cancer awareness has reached this amazing level where most of us know to check ourselves and not to mess around with it if we do find something a bit suspicious. I gave myself a couple of days to accept the potential reality of the situation and went to see my doctor. I explained the situation, and he took a look. Fortunately, the office also has an ultrasound room and I was able to sit and wait for it to come available there and then. I was told I had a liquid-filled cyst, and I was booked in for a biopsy. A couple of days after the biopsy, I received a call from my doctor, who explained what it was. He told me that I had an oil cyst, which can happen when fat is damaged. It wasn’t cancer at all. He praised me for being so reactive when I found it and told me to go and get on with my life — but to keep on checking in the future.

    Did you know about fat necrosis when you initially felt the scar tissue in your breast?

    I honestly thought I was well informed about all things breasts, but apparently I was not. I hadn’t ever heard of it. I think it’s really important that, as much as we now all learn about checking for cancer, we also get told about other, far less life-altering issues we could develop in that area. I think it can be reassuring — especially for younger people — to know there are other conditions out there. Finding out quickly can save a lot of stress, but it is also important if it is cancer.

    Could you give us an idea of the symptoms that led to your concern?

    It happened not long before I stopped breastfeeding my daughter, so I was kind of acutely aware of what was going on with my breasts. One of them started to get a little painful and, over time, got somewhat swollen and warm. My main concern was getting it seen quickly. Not only was I in pain, but I was scared that if I left it, I could jeopardize my future with my daughter.

    What was your experience like with your doctor?

    I went to see my daughter’s pediatrician for an appointment that had been booked for weeks. While I was there, I broke down in tears and explained what was going on. The doctor was so lovely. She told me it sounded like an infection called mastitis, which is super common for new moms. She explained to me that I just needed some antibiotics and to keep an eye on how it progressed. She was so sweet and completely understood why I was so worried about the situation. Since then, I’ve done a fair bit of research just out of curiosity, and it turns out there are so many breast conditions I had no idea even existed. I think it’s so amazing that cancer awareness pushes people to check themselves and to consult quickly. I’m certain it’s helping to save hundreds of lives every year.

    What advice would you give to someone who is feeling worried about consulting a doctor about a concern they have with their breasts?

    I think the concern stems more from the fear of it possibly being something ‘big’ as opposed to the fear of consulting in itself. And I do think that people knowing there are other things the symptoms could point to — aside from cancer — is reassuring on that front. That being said, I think the urgency that has been created from awareness is crucial when it actually is cancer. I think, if you’re concerned, ask for help as soon as possible, but hold on to the fact that 85 percent of lumps and bumps that people consult about are not cancer at all.
