When contemplating a cosmetic procedure, we spend so much time researching everything we can about the surgery – its benefits, [downtime](https://aedit.com/aedition/best-plastic-surgery-recovery-nurses-centers-products), side effects and [complications](https://aedit.com/aedition/guide-to-cosmetic-surgery-complications), and what we can realistically expect to achieve from it. In the case of surgical procedures, a piece of the puzzle that you may overlook is what kind of incision (or incisions) are involved. Any time there is an incision, there will be a scar. Other than genetics, one of the most important factors – especially in the first few days and weeks post-op – in determining what that scar will look like? Tension. 

While the skill of your board certified plastic surgeon will go a long way toward reducing tension and ensuring as inconspicuous a [scar](https://aedit.com/concern/scars) as possible, what you do in the aftermath of your surgery plays a large role in how incisions ultimately heal. If you’re considering a body cosmetic procedure or already have one scheduled, you’ve come to the right place. Read on to learn expert tips on what you can do post-op to ensure your incisions heal optimally and your scars are as minimal as possible. 

## **The Role of Tension in Scarring**
Late last year, we ran an article on the [kind of scarring you can expect from popular body procedures](https://aedit.com/aedition/scarring-after-body-plastic-surgery) (if you haven’t checked it out, you might want to read it first). One of the things that came up time and time again in our discussion with plastic surgeons was the idea of tension and how it impacts scarring. That gave us the idea to explore the topic further because, as it turns out, both the surgeon and the patient have control over how much ‘tension’ an incision site experiences.

In the healing sense, tension refers to any force that pulls the wound apart, says [Umbareen Mahmood, MD](https://aedit.com/provider/umbareen-mahmood-md-23), a board certified plastic and reconstructive surgeon in New York City. When there’s too much tension on an incision, “it is at risk of widening or completely separating,” warns [Lauren Chmielewski, MD](https://aedit.com/provider/lauren-chmielewski-md-143), a NYC-based board certified plastic and reconstructive surgeon. The former increases the likelihood of a wider scar, while the latter results in an open wound.

There are many ways to limit tension on post-surgical incision sites, and they start in the operating room. “As plastic surgeons, we do a thorough multi-layer closure, which means closing each layer of tissue to reduce stress on the top layer of skin (the final scar),” Dr. Mahmood explains. “We also work to leave a meticulous fine line final skin closure.”

And then there is what the patient can do at home. “The best way to care for a healing scar is to follow your surgeon’s after-care instructions and keep your post-operative visits,” Dr. Chmielewski shares. While the directions vary a bit by provider, they generally include:

- Keeping dressings in place
- Wearing protective or [compression garments](https://aedit.com/aedition/benefits-of-compression-garments-after-cosmetic-surgery)
- [Activity restrictions](https://aedit.com/aedition/when-to-start-exercising-again-after-cosmetic-surgery-procedures)
- Abstaining from [smoking](https://aedit.com/aedition/impact-of-cigarette-smoking-vaping-on-skin-hair-teeth-health)
- Limiting movement of the treatment area

“These are all directions that should be followed closely, as they can affect the ultimate outcome of the scar,” Dr. Chmielewski notes. It goes without saying, but don’t skip out on follow up appointments with your surgeon. They are a time for you to ask any questions and for your provider to monitor your recovery process. “Ultimately, having an experienced pair of eyes monitoring the healing is important, too,” she adds.

## **How to Reduce Tension & Minimize Scarring**
Now that you have a better sense of the role of tension in the scarring process, it’s time to talk about what it means for specific cosmetic surgeries. Below is a break down of expert tips for [minimizing tension and scarring](https://aedit.com/aedition/minimizing-scars-after-surgery) after some of the most popular body procedures:

### **Abdominoplasty (a.k.a Tummy Tuck)**
An [abdominoplasty](https://aedit.com/procedure/abdominoplasty), or tummy tuck, removes unwanted skin and fat and tightens the muscles of the abdomen. The scar, which is usually concealed below the bikini line, typically extends from hip to hip. “A tummy tuck, by definition, is going to be a somewhat tight closure,” Dr. Chmielewski says. To limit abdominal movement, any activities that use your core – including lifting, pushing, and pulling – should be avoided for at least two weeks. During that time period, you’ll also want to be mindful of your posture. She recommends maintaining a “beach chair” position, “as not to fully extend and stretch out the torso.”

Need a visual? When laying down, use [pillows](https://aedit.com/aedition/best-pillows-for-plastic-sugery-recovery-healing) to prop up the upper body and lower legs (similar to sitting in a beach chair), so that there is a gentle bend of 20 to 30 degrees at the hips. Sitting in a recliner has a similar effect. The goal is to take pressure off the abdomen and avoid laying flat. When standing, now is not the time to channel your inner ballerina. Hinge at your hips and refrain from standing up straight.

### **Brachioplasty (a.k.a Arm Lift)**
A brachioplasty (a.k.a. [arm lift](https://aedit.com/procedure/arm-lift)) sculpts the upper arm by removing skin and fat. The incision generally extends from the axilla (armpit) to the elbow and, depending on the surgeon, can be placed on the back of the arm or the inner arm crease. In either case, minimizing tension is tough because of how much we use our arms. “It’s obviously impossible to tell a patient not to move their arms for one month after an arm lift,” Dr. Mahmood admits. “But there are certainly movements they can be mindful of.” 

For starters, keep hair styling (think: blow drying, ponytails, etc.) and other sweeping motions to a minimum because repetitive movements that involve extending the arms can cause the scar to widen. “Even small things – like wearing a bra that closes in the front – is helpful in the first month post-op, so that a patient doesn’t have to stretch their arms behind their back,” she explains. 

Her suggestion for optimal healing: Keep the arms at your side as much as possible and do any unavoidable tasks with your forearm and hand (instead of the whole arm).

### **Breast Surgery** 
There are (obviously) big differences between [breast augmentation](https://aedit.com/procedure/breast-augmentation-solutions), [breast lift](https://aedit.com/procedure/breast-lift-solutions), and [breast reduction](https://aedit.com/procedure/breast-reduction-and-revision-solutions) surgeries when it comes to the purpose of the procedure. They also all involve unique incisions. “The biggest scar burden is with a reduction,” Dr. Chmielewski says. “A lift usually has more of a ‘lollipop’ incision, and an augmentation has the smallest scar.” Breast augmentation incisions can be in the inframammary crease (i.e. under the breast), periareolar (read: around the areola), or in the armpit. 

Even so, the way in which you go about reducing tension in the area is similar for all three. “Generally, the patient is to avoid any heavy lifting, pushing, pulling or upper body workouts for two weeks,” she explains. Refrain from raising your arms above your head during this period, too. When it comes to the breasts themselves, wearing a supportive (non-underwire) bra that provides the right level of compression is key to keeping everything in place.

To minimize tension while laying down, breast surgery patients are usually encouraged to elevate the head and upper torso. A [wedge pillow](https://aedit.com/aedition/best-pillows-for-plastic-sugery-recovery-healing) can get you into this semi-fowler position (meaning your back is at a 45 degree angle), and it also helps improve drainage and swelling. Beyond that, many doctors recommend placing a pillow or bolster under the knees to take pressure off the low back.

### **Liposuction & Fat Transfer**
[Liposuction](https://aedit.com/procedure/liposuction) is the gold standard of fat reduction, and it tends to be minimally invasive as far as incisions are concerned. As Dr. Mahmood explains, they are usually small (approximately 3 millimeters) and well-hidden. In abdominal liposuction, for example, incisions are usually concealed low in the bikini line and in the umbilical hood. “They heal well with no issue,” she notes. It should be noted that heat-based lipo modalities (like [VASER](https://aedit.com/procedure/ultrasound-assisted-liposuction) or [SmartLipo](https://aedit.com/procedure/smartlipo)) carry a higher risk of scarring.

While tension is less of a concern after liposuction due to the size of the incisions, you may be encouraged to [sleep on your back](https://aedit.com/aedition/sleeping-better-after-cosmetic-surgery-aesthetic-procedures) to maintain an even distribution of weight. Creating bumpers around yourself with pillows can help you from turning over. If you have fat removed from your arms or legs, keep the limbs elevated to ensure proper blood flow and minimize swelling.

Depending on your aesthetic goals, the fat removed via liposuction can be purified and transferred to another part of the body via injections (à la a [Brazilian butt lift](https://aedit.com/procedure/brazilian-butt-lift)). The fine needles used to inject the fat rarely result in external scars, though there might be other aspects of recovery to keep in mind.

### **Thigh Lift**
The purpose of a [thigh lift](https://aedit.com/procedure/thigh-lift) is to remove excess skin and fat and reposition underlying tissue to lessen the appearance of sagging. The incision usually runs along the inside of the leg and the groin crease, which can be problematic for healing for several reasons. “There is often a lot of tension after a thigh lift,” Dr. Chmielewski confirms. “Compression is absolutely critical to control [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery) and prevent shear forces from pulling at the incisions.” 

Wound care is part of any recovery, but it’s especially important post-thigh lift due to the treatment region. “Good wound care is also important, as the incision can be in close proximity to the groin, a notoriously high-bacteria area,” she explains. “Infections prevent healing and can ultimately create a very bad scar.  

It can be nearly impossible to eliminate all tension after a thigh lift, but there are some steps you can take to relieve as much pressure as possible. “Everything that engages your legs, unfortunately, puts tension on a thigh lift scar,” Dr. Chmieleski shares. “While gentle activity – such as walking – is encouraged, there is no cardio heavy pushing, pulling, lifting for at least two weeks.” When standing, try to keep a slight bend at the hips for the first seven days or so.

## **The Takeaway**

We’re all privy to the risks associated with aesthetic procedures (think: infection, poor wound healing, additional [complications](https://aedit.com/aedition/guide-to-cosmetic-surgery-complications)), but few possible adverse effects sound as terrifying as fat necrosis. It feels as though nothing is lost in translation – ‘necro’ quite literally means death – so it’s easy to let your mind go, well, there. Now, before anybody spirals, it’s important to note that fat necrosis is not a life-threatening complication or a sign that you will be unable to recuperate. It is actually fairly straightforward to diagnose and manage, though it does have some implications. A complication most commonly associated with fat grafting procedures (including [breast augmentation with fat transfer](https://aedit.com/procedure/breast-augmentation-with-fat-transfer) and [Brazilian butt lift](https://aedit.com/procedure/brazilian-butt-lift)) and [breast reconstruction](https://aedit.com/aedition/comprehensive-guide-to-breast-reconstruction-nipple-reconstruction-after-breast-cancer), fat necrosis can occur anywhere on the body. ## **What Is Fat Necrosis?** 
To put it in layman's terms, [fat necrosis](https://pubmed.ncbi.nlm.nih.gov/25415090/) is a collection of dead or damaged fat cells that have hardened and manifested as firm bumps. When the body has restricted oxygen flow as a result of surgery, radiation, or trauma, fat cells can die and release their contents to form a small body of liquified matter that hardens over time. “Fat necrosis usually appears as hard, firm lumps or, in some circumstances, as liquified fat,” explains [Gregory A. Buford, MD](https://aedit.com/provider/gregory-buford-md-1015), a board certified plastic and reconstructive surgeon in Lone Tree, CO. 

The liquified fat (a.k.a. oil cysts) can also cause lumps. Oil cysts are fluid-filled sacs that feel smooth and squishy under the skin and can grow over time. Fat necrosis and oil cysts are different, though they are both the result of damage to the fatty tissue. As the tissue breaks down, it can harden into scar tissue or retain a more liquid composition.

Post-op, self-checks are recommended to observe any changes in the body, including lumps and bumps. Diagnosis of fat necrosis or oil cysts will require imaging technology, such as a mammogram, ultrasound, MRI, or, in some cases, a biopsy. For some, the only symptom of fat necrosis and oil cysts is the lump. Others may experience some discomfort in the area, skin that becomes thickened, bruised, or red, and/or aesthetic concerns from contour changes. 

Due to the nature of the indicators, it’s common for breast surgery patients to rush to the conclusion that this is a [sign of breast cancer](https://aedit.com/aedition/what-to-do-if-you-find-a-lump-in-your-breast-breast-cancer-awareness) – especially for those who’ve undergone [reconstruction](https://aedit.com/aedition/post-mastectomy-breast-reconstruction-process-breast-cancer) (with or without fat grafting). Despite their lumpy form, the American Cancer Society [reports](https://www.cancer.org/cancer/breast-cancer/non-cancerous-breast-conditions/fat-necrosis-and-oil-cysts-in-the-breast.html) neither fat necrosis or oil cysts are cancerous nor increase the risk of cancer anywhere on the body. In fact, they are generally considered harmless.

## **What Causes Fat Necrosis** 
The roots of post-op side effects and complications often aren’t black and white, and this is no exception. “Fat necrosis can be caused by the lack of integration of transplanted fat into a specific area when specific areas of fat do not receive an adequate amount of blood supply,” Dr. Buford explains. Additionally, a patient’s post-op care lifestyle can play a role. “Necrosis can also result when too much pressure is placed on an area,” he continues. This is most common [after a BBL](https://aedit.com/aedition/brazilian-butt-lift-procedure-safety). Fat necrosis “can be seen following a BBL if someone sits directly or applies direct pressure to the grafted fat too early after the procedure,” he shares.

Often, numerous factors need to be considered. The provider’s technique, the patient's ability to recuperate, and post-op care can all play a role in the occurrence of fat necrosis. “First, if extraordinarily large volumes of fat are grafted into any one area, this can potentially lead to less take of the fat and possible necrosis,” Dr. Buford says. “Next, how the fat is harvested and managed – both before and during transplantation – can also have a big effect on the potential for fat necrosis.” 

Whether it’s occurring on the body or the face (think: [autologous fat transfer in place of filler](https://aedit.com/aedition/comparing-dermal-fillers-vs-fat-grafting-for-facial-rejuvenation-contouring)), these components aren’t mutually exclusive, so the root can be intra- or post-operative.  

## **How To Prevent & Treat Fat Necrosis**
The primary recommendations for preventing fat necrosis are to not [smoke](https://aedit.com/aedition/impact-of-cigarette-smoking-vaping-on-skin-hair-teeth-health) or be around those who do before or after surgery, as this restricts blood flow. For those who are post-BBL, follow your surgeon’s instructions for sitting and standing to a T. While complications can occur regardless of who performs the procedure, choosing a [board certified plastic surgeon](https://aedit.com/aedition/medical-board-certification-in-plastic-surgery-cosmetic-dermatology) who specializes in the fat grafting procedure you are interested in can help mitigate risk. “I haven't seen a tremendous amount of variation in terms of potential for fat necrosis in specific areas of the body, but there is definitely a different threshold for the amount of potential for volume grafting in each area,” Dr. Buford notes. “As you can imagine, the buttocks handle far more fat than the breasts and the [face](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4271301/).”

When it comes to treatment, options range from waiting it out to (in very rare cases) surgical removal. “Firm lumps resulting from fatty necrosis often resolve on their own or can be directly massaged to break them down,” Dr. Buford explains. “If contour changes are resulting from loss of this fat, then a [second round](https://aedit.com/aedition/guide-to-revision-surgery) of localized fat grafting can potentially be performed to fill in the gaps.” In the case of oil cysts, needle aspiration is usually able to drain the lump.

## **The Takeaway** 
For those who have had autologous fat transfer or breast reconstruction, fat necrosis is a risk. But it is one that is manageable and, in some cases, preventable. Arming yourself with the right information ahead of time will go a long way towards ensuring a smooth recovery and satisfactory outcome. “Fat is Mother Nature's filler and one of the earliest substances that plastic surgeons used to either smooth out contour deformities or add the desired volume,” Dr. Buford says. “Ultimately, results from fat grafting – when performed on the right patient, by the right surgeon, in the right area, and with the right amount – can produce very nice and very natural outcomes.” 

The complication of fat grafting and breast reconstruction is not nearly as alarming as its name would lead you to believe.

If you’re considering plastic surgery or a cosmetic procedure, there are many things that no doubt factor into your decision. [Cost](https://aedit.com/procedure-costs-directory), [recovery](https://aedit.com/procedure-recovery-directory), and efficacy are always top of mind, as are the quality and safety of the results. Even in the most skilled hands, the risk of complication or an unsatisfying outcome still exists. While revision statistics can be hard to come by, the United States Food and Drug Administration (FDA) [estimates](https://www.fda.gov/files/medical%20devices/published/Update-on-the-Safety-of-Silicone-Gel-Filled-Breast-Implants-%282011%29.pdf) 20 to 40 percent of breast augmentation patients and 40 to 70 percent of breast reconstruction patients undergo reoperations in the first eight to 10 years of having implants placed. Meanwhile, [studies show](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6191217/) between five to 15 percent of rhinoplasty surgeries are revised.

When a procedure requires revision because of complication or undesirable results, what are your options? Should you stick with the same surgeon or look for another practice? How do you decide on next steps? Whether you're considering a procedure and want to know the good, the bad, and the in-between or you’ve already had surgery that didn’t go according to plan, we spoke with top plastic surgeons to learn more about the options that may be available to you.

## **How to Tell If a Procedure Has ‘Failed’**
Let’s get this out of the way up front: This topic is quite nuanced. Exactly what constitutes a ‘failed’ procedure or surgery often depends on who you ask. For some, it’s when complications arise (learn more in our [comprehensive overview of cosmetic surgery complications](https://aedit.com/aedition/guide-to-cosmetic-surgery-complications)). For others, it’s when they aren’t aesthetically pleased with the outcome. Or maybe it's a combination of the two.

As it relates to the former, your plastic surgeon should educate you on what to expect post-op and the things you should keep an eye on. If you notice anything that doesn’t look or feel quite right, it’s a good idea to check in with your provider. “Complications like hematoma (blood buildup) or seroma (fluid buildup) are not ideal, but they can usually be resolved with a return trip to the operating room,” says [Olivia MaDan, MD](https://aedit.com/provider/olivia-madan-md-34660), a board eligible plastic and reconstructive surgeon in Nashville. Others, like [capsular contracture after breast implants](https://aedit.com/aedition/capsular-contracture-breast-implants), can take months or years to become evident and may require revision.

The more ambiguous type of procedure ‘failure’ is the kind that involves a patient [not being satisfied](https://aedit.com/aedition/what-to-do-if-you-are-unhappy-with-your-plastic-surgery-results) with their results. For starters, it’s important to remember that even though you may be cleared to resume regular activities at two- or three-month post-op mark, it can take a year (or more) for the body to fully heal after cosmetic surgery. The final result is not truly visible until the healing process is complete, but that’s not to say you can’t raise concerns along the way. If you are concerned about how you look after a procedure, tell your surgeon why you’re unhappy. Any reputable provider will talk through your concerns, advise on recovery, and discuss next steps – which leads us to…

## **How to Proceed After a ‘Failed’ Procedure**
We said it before, but it can’t be emphasized enough: “The first step is to allow the body to heal from the first surgery,” explains [Jules Walters III, MD](https://aedit.com/provider/jules-walters-md-34662), a board certified plastic and reconstructive surgeon in Metairie, LA. “Most revisions can be considered around six months after the original surgery.” There may, however, be exceptions to this rule, which is why it is important to keep in close contact with your surgeon. “When a procedure fails and complications arise, it will be up to your medical provider to help guide you through your recovery process,” says [Martin Benjamin, MD](https://aedit.com/provider/martin-benjamin-md-34666), a board certified plastic and reconstructive surgeon in Scottsdale, AZ. 

It’s necessary to figure out the root cause of the failure. “The most important issue is to determine why the procedure failed,” Dr. MaDan notes. Medical imaging or other tests may be ordered to precisely establish what has gone wrong and why it happened. Depending on the cause of the post-op complications or cosmetic concerns, there may be different courses of action. “It will be important to discuss what revisions or alternative procedures are available to you in an effort to still achieve your desired results,” Dr. Martin shares. “Make sure that you discuss all personal questions and concerns in efforts to best formulate a plan moving forward.” 

## **What Revision Entails** 
How you treat or correct a poor procedural outcome depends on your initial surgery. “Discussing the best treatment plan moving forward with the surgeon that performed the procedure is the first option that should be explored,” says [Michael Somenek, MD](https://aedit.com/provider/michael-somenek-md-30787), a double board certified facial plastic and reconstructive surgeon in Washington, DC. “With this, the patient will be maximally informed about their options both surgically and, perhaps, non-surgically.” 

Revision procedures are generally considered more complicated than primary procedures because of how the anatomy has been altered, how surgical techniques have evolved over time, how the body healed itself, and more. “Revision surgery is a wholly different field from primary surgery because a surgeon must contend with the patient's distorted anatomy, as well as scar tissue and the other surgeon's subpar result,” says [Barry Weintraub, MD](https://aedit.com/provider/barry-weintraub-md-385), a board certified plastic and reconstructive surgeon in New York City. 

Just as no two surgeries are the same, every revision is unique. “All invasive and non-invasive modalities must be explored to produce the superior result the patient truly deserves,” Dr. Weintraub shares. In some cases – like [revision rhinoplasty](https://aedit.com/aedition/what-you-need-to-know-about-revision-rhinoplasty) or [replacing breast implants](https://aedit.com/aedition/guide-to-replacing-breast-implants) – the secondary surgery may bear some resemblance to the original. “Depending on the initial surgery, revision may involve a part of the original treatment,” Dr. MaDan explains. “Alternatives after surgical failure are on a case-by-case basis.” 

While it is often recommended to first return to the provider who performed your procedure, you are free (and encouraged) to seek a second opinion. “You must always choose a [board certified plastic surgeon](https://aedit.com/aedition/medical-board-certification-in-plastic-surgery-cosmetic-dermatology), which is the minimum criteria one should look for," Dr. Walters cautions. “Once a patient confirms that the surgeon is board certified, then a patient must make sure the surgeon performs the chosen procedure regularly and can show multiple before and after photos.” That includes doing your due diligence to ensure the provider has extensive experience with revision, Dr. Weintraub adds.

### **Revising Non-Surgical Procedures**
While we’re mostly discussing surgical procedures in this article, it’s important to note that non-surgical procedures can also fail. Last year, supermodel Linda Evangelista opened up about the disfiguring effects she experienced due to [paradoxical adipose hyperplasia (PAH)](https://aedit.com/aedition/paradoxical-adipose-hyperplasia-after-cryolipolysis) after a series of [CoolSculpting®](https://aedit.com/procedure/cool-sculpting) treatments. Correction requires liposuction to remove the hardened fat deposits. Not happy with your hyaluronic acid (HA)-based filler? ## **What Are Opioids?**
Opioids refer to a class of drugs found naturally in the opium poppy plant or synthetic compounds. They are most often used in medicine to relieve pain (including via anesthesia) by interacting with opioid receptors in your brain. This is why they commonly are prescribed to treat discomfort after surgery. 

When used safely and in moderation, opioid medications can be a helpful resource to maintain comfort levels after surgery. However, at higher doses, they can cause drowsiness and even slow down breathing or heart rate. Some people feel a sense of relief or even joy after taking opioids, which makes the drug susceptible to becoming addictive.

For those who have overcome opioid addiction or who are allergic to the category, finding alternative pain relief is vital. One of the ways surgical teams can operate without using products that contain opioids is by using a method known as [total intravenous anesthesia (TIVA)](https://pubmed.ncbi.nlm.nih.gov/23335648/). TIVA is an anesthetic that generally allows people to go home on the same day as their surgery with minimal discomfort. Unlike more common forms of anesthesia that can combine the use of gaseous anesthetics delivered via vaporizers and intravenous drugs, TIVA only uses intravenous drugs. When you are sedated for the procedure, your anesthesiologist will place an IV, usually in your arm, and you will start to feel sleepy shortly thereafter.

## **How Can You Manage Post-Op Pain Without Opioids?** 
Postoperatively, there are several [opioid-free pain relief methods](https://pubmed.ncbi.nlm.nih.gov/31568318/) that can help you remain comfortable during the first few days after surgery when pain is most common. One of the most widely used products is a long-acting local anesthetic called [Exparel®](https://www.exparel.com/). It acts over several days to numb the area operated on to help lessen your need for further pain relief medication. Exparel®, or bupivacaine, isn’t a narcotic or an opioid, so it is generally seen as a good option for patients with specific medication-related concerns.

Regardless of how you will be managing your pain post-op, your surgical team will provide you with a plan to stick to when you return home. It will include details of medications you can safely take, the timing, and the dosage. When considering having an aesthetic procedure – be it surgical or non-surgical – risks and complications can feel like a daunting topic to explore. Whether you’re an avid researcher or processing the information your provider shares with you, calculating whether the reward is greater than the risk can be difficult to compute. While the treatments offered by reputable, board certified dermatologists and plastic surgeons are largely considered safe and effective, no procedure is without risk and it’s important that patients understand that before booking an appointment.

We certainly appreciate that people have become more transparent about their nips and tucks (here’s looking at you, [Marc Jacobs](https://aedit.com/aedition/marc-jacobs-facelift)), but the pitfall of normalization is that conversations around safety, efficacy, cost, and recovery are often lost in the shuffle. “Plastic surgery carries similar complications as other surgical specialties,” says [Jules Walters III, MD](https://aedit.com/provider/jules-walters-md-34662), a board certified plastic and reconstructive surgeon in Metairie, LA. “Patients must realize this and not just be caught up on the glamorous side of plastic surgery shown on [social media](https://aedit.com/aedition/finding-a-plastic-surgeon-on-social-media).” 

This article is not to dissuade you from getting a procedure but rather to ensure you make the best decision for your health and aesthetic goals. With the help of top plastic surgeons, we’re exploring the most common cosmetic surgery complications, how severe they can be, and what steps patients can take to minimize the risk.

## **Side Effects vs. Complications**
Before we dive into complications, it’s important to understand how they differ from side effects. Side effects refer to undesired or negative consequences that are expected when a drug or treatment is administered. For example, nausea is a common side effect of [general anesthesia](https://aedit.com/aedition/everything-you-need-to-know-about-anesthesia), while redness, [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery), and tenderness are all side effects of injectables. Complications, meanwhile, are uncommon consequences that, however rare, can occur. While side effects tend to resolve on their own, complications usually require medical intervention and possible revision. In this article, we are focusing on complications. 

Both the side effects and potential complications of a procedure should be discussed during the consultation process and should certainly be considered before deciding on a procedure. The two vary in severity, but complications are the more serious – and rarer – of the bunch. “In general, the most common complications in plastic surgery are bleeding (hematoma), bruising (ecchymosis), fluid collection (seroma), infection, or [poor scarring](https://aedit.com/aedition/minimizing-scars-after-surgery),” says [Olivia MaDan, MD](https://aedit.com/provider/olivia-madan-md-34660), a board eligible plastic and reconstructive surgeon in Nashville. In the case of the latter, “poor” can include widened, atrophic, hypertrophic, keloid scars, and more. “Some procedures can also lead to sensory changes, contour deformities, delayed wound healing, and need for revision,” she continues. “Less common complications include blood clot, fat embolism, and cardiac arrhythmia.” 

The likelihood of these complications depends on a range of factors inclusive of both the patient and provider. The type of surgery you are undergoing, your medical history, and the surgeon’s level of expertise can all play a role in how smooth your [recovery from surgery](https://aedit.com/aedition/best-plastic-surgery-recovery-nurses-centers-products) will be.

### **Procedural Risk**
As it relates to the relationship between procedure type and complications, the [Brazilian butt lift (BBL)](https://aedit.com/procedure/brazilian-butt-lift) is perhaps the most infamous. For the uninitiated, the procedure involves harvesting fat from a donor site via liposuction, purifying that fat, and then injecting into the buttocks. A [2017 survey](https://academic.oup.com/asj/article/40/8/864/5822125?login=true) from the Aesthetic Surgery Education and Research Foundation (ASERF) determined the BBL mortality rate to be one in 3,448. It decreased to one in 14,952 in 2019. The 2019 figure indicates mortality has reached an ‘acceptable’ level that is similar to that of abdominoplasty (one in 13,193). This is thanks in large part to [changes in technique](https://aedit.com/aedition/brazilian-butt-lift-procedure-safety) that have led to a safer result. 

Despite the progress, many surgeons do not perform the procedure. [Barry Weintraub, MD](https://aedit.com/provider/barry-weintraub-md-385), a board certified plastic and reconstructive surgeon in New York City, says he doesn’t offer BBL at his practice because of the risks associated with it. And Dr. MaDan says it’s not just the butt enhancer. “Fat transfer procedures – BBL, [facial fat grafting](https://aedit.com/procedure/facial-fat-grafting) – are more likely to carry risks of contour deformity and fat embolism,” she notes. 

As a reminder, trendy surgeries aren’t the only ones that carry risk. “Tummy tuck ([abdominoplasty](https://aedit.com/procedure/abdominoplasty)) procedures are more likely to have complications like hematoma and seroma,” Dr. MaDan shares. Breast surgeries, including [breast lifts](https://aedit.com/procedure/breast-lift) and [breast implants](https://aedit.com/procedure/breast-augmentation-with-implants), meanwhile, “are more likely to have complications like sensory changes, implant malposition, and poor scarring,” she adds.

## **How to Mitigate the Risk of Complication**
In some ways, plastic surgery complications are a numbers game. Even in the best hands, there is always a possibility that something can happen. With that said, there is much that can be done pre- and post-op to mitigate risk.

### **1. Prioritize Your Health**
The best way to prepare for surgery? Seeing to it that you’re in good overall health, says [Martin Benjamin, MD](https://aedit.com/provider/martin-benjamin-md-34666), a board certified plastic and reconstructive surgeon in Scottsdale, AZ. “Maintaining a healthy weight, exercising, and having [good nutrition](https://aedit.com/aedition/meal-plans-diet-before-after-cosmetic-surgery) will help to optimize your recovery,” he says. In some cases, a surgeon may even recommend committing to certain healthy lifestyle changes before undergoing treatment. [Body mass index (BMI)](https://aedit.com/aedition/why-you-should-lose-weight-before-plastic-surgery), for example, plays a role in whether or not someone is a candidate for surgery because of its impact on recovery and wound healing, while [smoking](https://aedit.com/aedition/impact-of-cigarette-smoking-vaping-on-skin-hair-teeth-health) can adversely affect results, too.  

Additionally, it’s imperative to share any information you think may be significant with your surgeon or anesthesiologist during the consultation process, so they can adapt your care accordingly. “Understanding the patient’s medical history, as well as any previous issues with healing and surgery-related issues, can help to prevent future complications,” says [Michael Somenek, MD](https://aedit.com/provider/michael-somenek-md-30787), a double board certified facial plastic and reconstructive surgeon in Washington, DC. Dr. Benjamin agrees. “It will be important to make sure to discuss any of your past medical history and obtain the appropriate surgical clearance and preoperative labs, as well as avoid any tobacco or drug use before your operation,” he shares. 

### **2. Choose a Skilled Surgeon**
One of the most crucial steps to prevent possible complications is [choosing the right provider](https://aedit.com/provider/michael-somenek-md-30787). “When it comes to complications, the best prevention is to go with a trained surgeon, who is board certified in plastic surgery,” says [Lauren Chmielewski, MD](https://aedit.com/provider/lauren-chmielewski-md-143), a NYC-based board certified in plastic and reconstructive surgeon. “While even under the best of circumstances a complication can occur, you can improve your odds by researching the surgeon’s credentials.” This expertise can actually become even more important in case of an emergency, when the extensive training and experience kicks in.

In addition to vetting the credentials of the surgeon, also do your due diligence in scouting their team, facility, and body of work (think: patient testimonials, [before and after photos](https://aedit.com/aedition/importance-of-before-and-after-photos-in-plastic-surgery)). Dr. Chmielweski recommends asking your surgeon if the surgery center where your procedure will be performed is accredited. You can also inquire about who else will be on the surgical team – including the qualifications of the [anesthesiologist](https://aedit.com/aedition/how-much-cosmetic-surgery-costs).

The consultation process is a good way to get to know potential providers (in addition to learning more about treatment options), which is why we always recommend consulting with at least two surgeons. “Patients should always [go with their gut](https://aedit.com/aedition/how-i-found-my-plastic-surgeon-trust-your-gut),” Dr. Weintraub says. “In my revision cases where I’m correcting another surgeon’s subpar result, patients always say that they didn’t feel right about going ahead with their prior surgeon but did so anyway.” 

We know that there can be a lot of ground to cover during the consultation, but there are some factors not related to treatment that you should pay attention to. “A surgeon should not be in a rush and should answer all questions during a consultation,” Dr. Weintraub shares. “[It] should be up to 60 minutes with thorough explanations of the procedure requested, a tour of the surgical facility, and an in-depth meeting with the practice manager.” His key takeaway: “Surgery is not sales – there should be no pressure and no rush.”

### **3. Ask Questions** 
As we’ve started to cover, the most crucial step before surgery is the consultation. It’s your primary opportunity to explore possible treatments, ask questions, learn more about the surgical team, and get a sense of the surgeon’s bedside manner. “Having a thorough discussion with your surgeon regarding your surgical procedure’s risks, benefits, and alternatives are of utmost importance during your consultation,” Dr. Benjamin emphasizes. “It will help guide your decision-making and best prepare you should there be any unanticipated surgical complications.” 

A few questions you could ask regarding complications include:

- Am I a suitable candidate for this procedure?
- What are the known side effects and complications of this procedure?
- Are there any lower-risk alternatives I should consider?
- Have any of your patients experienced complications following this procedure?
- What will recovery look like? Are there any signs of complications I should look out for?

- **Move Mindfully**: Depending on the procedure, it could be weeks or months before you are cleared to [exercise](https://aedit.com/aedition/when-to-start-exercising-again-after-cosmetic-surgery-procedures) and get your blood pressure up again. While short walks and gentle movement are necessary to prevent clotting and such, slow and steady wins the race.
- **Wear Compression**: If your recovery calls for [compression garments](https://aedit.com/aedition/benefits-of-compression-garments-after-cosmetic-surgery), wear them according to your surgeon’s recommendations. They help minimize bruising and fluid retention.

However, post-BBL, it appears that all areas are, shall we say, sensitive. And comfort and some semblance of normality are days, if not weeks or months, away.

## **The First Few Days After a BBL**
In some ways, the beginning stages of the BBL recovery mirrors other cosmetic surgeries. “Like any surgery, the first week usually entails the most intense pain, [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery), decreased mobility, and some degree of bruising,” explains [Ryan Stanton, MD](https://aedit.com/provider/ryan-stanton-md-644), a board certified plastic and reconstructive surgeon in Beverly Hills. Additionally, he notes that, despite minimal incisions (fat is harvested via liposuction from a donor site and then injected into the buttocks), recovery entails a bit of cleanup. “Often, there is also fluid leakage from the areas of the very small liposuction cannula incisions,” he shares. “This is very normal for the first 12 to 24 hours.” 

What makes BBL recovery unique is not just the lack of mobility or the discomfort, but rather the cocktail of considerations. “Patients are expected to change dressings as needed, take prescription pain medication as ordered, and wear provided compression garments,” says [Kurtis Martin, MD](https://aedit.com/provider/kurtis-martin-md-34658), a board certified plastic and reconstructive surgeon in Cincinnati, Ohio. Your surgeon and/or surgical team will likely check in before then, but “patients should have their first follow-up visit one week following their procedure,” he adds. 

While the exact recovery protocol will depend on your surgeon, here are a few things to keep in mind about the healing process:

### **1. Sitting & Standing**
You probably already know this, but “sitting or laying on the buttock is prohibited for anywhere from two to 12 weeks,” Dr. Stanton shares. Most commonly, the restrictions will last three to six weeks, but clearance will vary by surgeon and patient. 

You may not be able to sit on your butt for any real length of time, but that doesn’t mean you can or should be totally comatose. In fact, Dr. Stanton recommends mindful movement. “Standing and walking within the home is actually encouraged to help fluid and blood circulation and thus minimize the risk of blood clots,” he explains. Dr. Martin also suggests light physical activity – such as standing up or taking short walks within a secure space. 

What can’t be on the agenda? Squatting, [exercise](https://aedit.com/aedition/when-to-start-exercising-again-after-cosmetic-surgery-procedures), sex, and strenuous activities that raise blood pressure. Sitting will also be monitored and limited to under 10 minutes in the first few weeks. In the month following, you can usually bump up to 15-minute intervals. Alternate sitting, standing, and walking with laying face down to prevent blood clots and maintain strength.

### **2. Pain Management**
For pain management, providers usually prescribe specific medication, however, that is not the only factor to consider as it relates to physical discomfort. “I think most surgeons would agree that a caregiver is necessary for the first 48 hours postoperatively or, at the very least, the first night while the patient is still recovering from anesthesia and may be unstable on their feet/ambulating,” Dr. Stanton says. 

We often note that a caregiver is necessary for leaving the OR after anesthesia, but, once you’ve made it to the comfort of your bed, [an aide](https://aedit.com/aedition/plastic-surgery-recovery-nursing-concierge) will prove vital. From providing the medication to helping with bandages, a companion (be it a partner, friend, or professional) can ease stress and give the patient that much less to worry about. 

### **3. Hygiene**
While bandaged up, limited in movement, and feeling sore from head to toe, showers and bathroom visits will be difficult. To prepare for this, Dr. Martin recommends over-the-counter stool softeners, staying hydrated, and eating a [balanced diet](https://aedit.com/aedition/meal-plans-diet-before-after-cosmetic-surgery) with fiber and protein to keep you regular and avoid strain on your digestive system. When you can safely bathe again will be indicated by your surgeon.

### **4. Compression Garments** 
[Compression garments](https://aedit.com/aedition/benefits-of-compression-garments-after-cosmetic-surgery) are common in body contouring procedures, and BBL is no exception. “Our patients are provided with a special garment that provides compression in the liposuction areas while allowing for the buttock to be open without pressure,” Dr. Martin says. The goal is to improve swelling, reduce movement of the skin in the area, and promote better healing. “This garment is to be worn for several weeks following the surgery,” he adds. For the first 14 to 21 days, you’ll be wearing it most of the day. After three weeks, wear-time is often reduced to 12 hours per day. In terms of fit, the garment should be snug but not so tight that it feels obstructive. 

When it comes to clothing over compression garments, Dr. Martin says sartorial choices should revolve around comfort. “Loose-fitting, zip-up clothes” and “lots of extra gauze and chux pads to sop up the liposuction fluid drainage” are doctor's orders. 

### **5. Sleeping** 
Experts can’t stress enough the importance of adequate rest as you recover. While you will no doubt be tired, [falling and staying asleep](https://aedit.com/aedition/sleeping-better-after-cosmetic-surgery-aesthetic-procedures) may prove difficult with the physical limitations. Sitting or sleeping on the gluteal area can create asymmetrical results, which is why pressure should be avoided at all costs. Therefore, sleeping on the stomach and using [specialty pillows](https://aedit.com/aedition/best-pillows-for-plastic-sugery-recovery-healing) to keep yourself in place is recommended for best results. Speaking of pillows, you’ll also want to invest in a custom cushion for any sanctioned sitting.

## **The First Few Months After a BBL**
It should come as no surprise that, according to Dr. Martin, “the most challenging aspect for most patients post-op BBL is that you cannot put any pressure on the buttocks for three weeks.” This will impact every aspect of life and should be properly prepared for (more on that below). As we mentioned, sitting is permitted in short intervals. Each provider will have specific guidelines to follow regarding the time you’re allotted. As you heal, you will be allowed to sit for longer periods, but Dr. Martin recommends “laying on your stomach in the back seat of the car, avoiding sitting, or using a special pillow to sit.”

The caveat: “It is not recommended until weeks after the procedure,” he cautions.

For many, resuming normal activities is possible within a month or two of the surgery – with proper precautions. Dr. Martin suggests patients “take it slow and listen to their body.” And please don’t think you know better than your surgeon. “Restrictions are in place for proper healing,” he says. “Follow provided instructions, and do not hesitate to reach out to your provider with any questions.”

So, what does that actually mean in terms of what you can expect in the days, weeks, and months after treatment?

## **The First 48 Hours After Surgery**
The first few days after virtually any surgery are the most crucial from a recovery standpoint, and the same is true for a neck lift. Depending on the preference of the surgeon and patient, the procedure can be performed under [general anesthesia](https://aedit.com/aedition/everything-you-need-to-know-about-anesthesia) or ‘awake.’ During an [awake neck lift](https://aedit.com/procedure/awake-neck-lift), a so-called conscious sedation or twilight anesthesia with regional or local anesthetics are utilized. 

The type of anesthesia used will impact how you feel immediately after surgery. “If the surgery was fairly lengthy and required general anesthesia, patients may require a day or two to feel recovered from the anesthesia,” says [Thuy-Van Tina Ho, MD](https://aedit.com/provider/thuyvan-ho-md-25248), a double board certified facial plastic and reconstructive surgeon and founder of [Tina Ho MD Facial Plastic Surgery](https://www.drtinaho.com/) in Philadelphia. Those lingering effects include nausea, drowsiness, and general malaise.

Because neck lifts are outpatient procedures, “patients go home the same day,” Dr. Ho shares. She typically places drains at the conclusion of the surgery and has her patients wear a head and neck [compression dressing](https://aedit.com/aedition/benefits-of-compression-garments-after-cosmetic-surgery) that is freshened up the next day. “I see patients the following day for drain check and dressing change,” she notes.

A neck lift can be performed to reduce skin laxity, tighten the underlying musculature, remove excess fat, or a combination of the three. Needless to say, some patients require more correction than others, and this plays a role in the healing process. The depth “is determined by how much excess skin and fat the patient has,” she adds. Additionally, you may also have a headache that feels similar to a sinus headache. 

Yes, you are going to want to take it very easy for a few days, but surgeons don’t recommend full-on bed rest. “Getting up to go to the bathroom is ok,” says [Smita Ramanadham, MD](https://www.instagram.com/thedrsmita/?hl=en), a board certified plastic and reconstructive surgeon in New Jersey. When you are in bed, avoid laying fully horizontally. “We ask that patients sleep on [two or three pillows](https://aedit.com/aedition/best-pillows-for-plastic-sugery-recovery-healing) or in a recliner, so the neck is above the heart level,” she says. 

Oh, and if you’re curious about how getting a neck lift in tandem with a facelift might affect the recovery process, Dr. Ho says there is usually “not a significant difference” aside from “more swelling and bruising.”

## **Pain Management After Neck Lift Surgery**
“Patients may require [pain medication](https://aedit.com/aedition/opioid-crisis-plastic-surgery-recovery-pain-management) to help control their pain or discomfort,” Dr. Ho says, and your surgeon will prescribe accordingly. In her practice, Dr. Ramanadham typically offers about three days of prescription medication before her patients switch over to over-the-counter options like Tylenol.

Pain is obviously subjective, but it may encourage you to hear that, according to Dr. Koo, “there isn’t that much” of it after a neck lift. It’s common to feel pressure or a dull ache around the head and neck, and you may get hit with twinges of pain when trying to sleep on your side. But, overall, she says the level of discomfort is manageable for most people.

Believe it or not, she finds that patients often feel uncomfortable as a result of the bandages, not the surgery itself. “You can expect to have some type of compression wrap around your full head and underneath your chin,” Dr. Koo explains. “For people who might be a little claustrophobic, that might be a little intimidating.” The wrap will stay on – though your doctor may remove some padding – for the following four to five days, and you should still wear it at home for at least another two weeks. You’ll receive specific instructions from your provider on the timeline. 

## **Resuming Normal Activities After a Neck Lift**
As you’re feeling stronger and ready to resume non-strenuous activities, Dr. Koo says patients should feel comfortable doing so. “My patients will put a headwrap and scarf on and go out in two days,” she shares. “They get their Audrey Hepburn on.” That could mean a walk around the block, tidying up around the house, or any other gentle movement. Not yet on the agenda? Yoga, cardio, or anything that raises blood pressure.

Even if your recovery is relatively pain-free, you still need to be mindful of the healing process and wait for clearance before resuming certain activities. At four days, Dr. Koo tells her patients they can begin a “mall-pace” walk, and a brisk walk is usually safe at the two-week post-op mark. Dr. Ho takes a similar approach. “Two weeks for heavy lifting and strenuous activity,” she says, adding that you may return to work sooner if you’re not self-conscious about swelling and bruising or stitches. She likes to see patients one week, two weeks, and four weeks out to monitor their progress.

For more intense [exercise](https://aedit.com/aedition/when-to-start-exercising-again-after-cosmetic-surgery-procedures) — bootcamp, pilates, hot yoga — that results in heavy sweating or can raise blood pressure, Dr. Koo recommends waiting four to six weeks. As she explains, non-compliance can “stir up more swelling” and lead to the possibility of “more scar tissue.” Her advice: “It’s just best to wait.”

## **Final Neck Lift Result**
When it comes to recovering from a neck lift, Dr. Ho says you can think about the healing process in phases. The first two to four weeks are the “immediate recovery” period, yet it takes six to 12 months to enjoy the final results. “It takes time for the skin to relax, for all of the swelling to be gone, and for the [scars to mature and fade](https://aedit.com/aedition/scarring-after-facial-plastic-surgery),” Dr. Ramanadham notes. 

Still, you will be able to see progress early on. “Even after four days when you take the wrap off, the neck looks amazing,” Dr. Koo shares. It will, however, be another four to six weeks until you are, as she describes it, “photo-ready.” Depending on their healing progress, Dr. Ho says likes to see her patients every one to three months for the first year. After six months, Dr. Ramanadham tells her patients that their results are “80 to 85 percent” finalized. After a year, the final result is in.

## **Types of Non-Surgical Body Contouring**
Non-surgical body contouring procedures generally fall into one of the following categories:

### **2. Skip the Salt**
In the same vein, keep your sodium intake in check. “The body has been placed in a short term inflammatory condition, so we encourage limiting salt intake to essentially zero and refrain from alcohol use for 48 hours,” Dr. Neinstein notes. “Both will contribute to swelling and bloating.” [Paul Jarrod Frank, MD](https://pfrankmd.com/), a NYC-based board certified dermatologist, also emphasizes the importance of avoiding salt, especially in between Emsculpt Neo® treatments.  
 
While you’re at it, lay off sugar and simple carbohydrates that can cause inflammation in favor of a [well-balanced, healing diet](https://aedit.com/aedition/meal-plans-diet-before-after-cosmetic-surgery) that consists of lean protein, plant-based fats, unprocessed carbohydrates, and antioxidant-rich fruits and vegetables (think: vitamins A, C, E).
 
### **3. Avoid the Gym**

### **4. Make Sure to Massage**

After a skin treatment or facial procedure, your skin may be in a compromised state. As a result, it requires specific care and attention that is different from what you use daily. This is true of both skincare and makeup. Due to the vulnerability of the skin — plus redness, inflammation, [scarring](https://aedit.com/aedition/scarring-after-facial-plastic-surgery), and more — it's best to switch over to [post-treatment-approved products](https://aedit.com/aedition/skincare-after-facial-plastic-surgery-procedures) for the initial healing process. Read on to better understand what your skin needs post-op and the best products to bolster your recovery process.

## **Take a Gentle Approach**
After a cosmetic treatment, procedure, or facial plastic surgery, there is always a specialized skincare regimen (depending on the chosen procedure) that your provider will advise you to follow. Often, that protocol will focus on reinforcing the natural protective barrier of skin by using a nourishing moisturizer and a gentle cleanser, says [Michele Green, MD](https://aedit.com/provider/michele-green-md-34642), a board certified dermatologist in New York City. “This is particularly important because many cosmetic procedures involve inducing collagen production in the skin through targeted and controlled trauma,” she says. Whether the modality is a [chemical peel](https://aedit.com/aedition/guide-to-superficial-medium-deep-chemical-peels) or a [skin-resurfacing laser](https://aedit.com/aedition/guide-to-ablative-laser-skin-resurfacing), Dr. Green explains that it’s normal for the skin to become sensitive and more delicate after treatment.

For this reason alone, it's imperative to restore the skin barrier and ‘nurse’ it back to health. Encouraging restorative healing on just-treated skin will allow it to rebound quicker and for the results to be that much better. However, after a heavy-duty treatment or undergoing plastic surgery of any type is not the time to start experimenting with new products. Plus, there are plenty of ingredients that you'll want to steer clear of until your doctor gives you the green light (usually one week or so depending on the procedure and how the skin is healing) to resume using them. Those actives include:

- **Retinol & Retinoids**: For as much good as [retinol and retinoids](https://aedit.com/aedition/guide-to-vitamin-a-retinol-retinoids-in-skincare) do, the one time you want to stay away from the vitamin A derivatives at all costs is after anything invasive. Some providers advocate for using retinol to help speed up the exfoliation process post-treatment, but this needs to be done carefully and under strict supervision.
- **Anti-Acne Ingredients**: Those who regularly use anti-acne ingredients like [salicylic acid](https://aedit.com/aedition/guide-to-beta-hydroxy-acids-salicylic-acid-in-skincare) and [benzoyl peroxide](https://aedit.com/aedition/guide-to-benzoyl-peroxide-in-skincare) should not use them (or any other blemish fighters) until the skin fully heals because acids and other potent actives can further compromise and dry out the skin.
- **Vitamin C**: Some skin types are inherently sensitized by [vitamin C](https://aedit.com/aedition/guide-to-topical-vitamin-c-in-skincare), depending on the concentration and source, which is why many experts suggest laying off of it in the days following a procedure. 

That same approach to mindful TLC goes for cleansing, too. Even after a treatment, you’ll still want to partake in regular cleansing to help lift away dead skin. Post-procedure cleansing should always consist of calming ingredients and be [free of fragrance](https://aedit.com/aedition/fragrance-free-beauty-skincare-for-sensitive-skin). Also, stay away from acids, anti-acne ingredients, and grainy exfoliators.

## **Build Back Up the Barrier**
Perhaps the most essential element of ensuring the skin heals efficiently is to protect and restore the skin barrier (i.e. the delicate layer of defensive skin). “This will help best protect skin from bacteria and other environmental irritants that may interfere with the healing process,” Dr. Green explains.

Adhering to a relatively strict post-treatment protocol for more invasive treatments like [ablative lasers](https://aedit.com/procedure/ablative-nonfractional-lasers), [deep chemical peels](https://aedit.com/procedure/deep-chemical-peels), and even surgery is crucial for healthy healing, too. Dermatologists and plastic surgeons often recommend [thick occlusive ointments](https://aedit.com/aedition/difference-between-creams-lotions-moisturizers) to help restore the barrier and mitigate irritation and inflammation, which are known to cause hyperpigmentation and other reactions. Then, once your doctor sees improvement, it's usually time to switch to a more traditional moisturizer with [hyaluronic acid](https://aedit.com/aedition/guide-to-hyaluronic-acid-in-skincare-dermal-filler) and/or [ceramides](https://aedit.com/aedition/guide-to-topical-ceramides-in-skincare).

Depending on the treatment and the patient, there may be some slight modifications to this protocol. For example, [Jason Emer, MD](https://aedit.com/provider/jason-emer-md-722), a board certified dermatologist in Los Angeles, says that petrolatum-based ointments aren’t always great to use post-laser because they keep heat in the skin and retain redness. “So, now, instead, we’re using a lot of growth factors in my practice to repair the barrier along with hyaluronic acid and even vitamin C,” he shares. 

Remember, the point of a non-ablative resurfacing treatment, like a [light laser](https://aedit.com/procedure/nonablative-fractional-lasers) or [microneedling](https://aedit.com/procedure/microneedling), is to access deeper layers of skin to deliver supercharged active ingredients. “Flooding those channels with occlusives kind of defeats the purpose, which is another reason why I don’t like occlusives so much,” he explains.

As an alternative to thick occlusive agents, hyaluronic acid may be a good stand-in. Since it’s a humectant, it has the ability to draw moisture into the skin and restore hydration levels. “This is beneficial for skin health and helps aid in the healing process after having a cosmetic treatment," Dr. Green says. Another important component? It is non-comedogenic. “It won't clog or congest the pores as some moisturizers may do to oily skin types, which is why it works wonders for [oily skin types](https://aedit.com/aedition/difference-between-oily-and-acne-prone-skin) — and all skin types — post-treatment," she adds.

## **Bland Is Best**
After any cosmetic or facial surgery, there is often a significant amount of swelling and inflammation in and around the area, which, San Diego-based double board certified facial plastic and reconstructive surgeon [Amir Karam, MD](https://www.drkaram.com/), says causes a lot of patients to complain of their skin being unusually dry. “Simple, bland moisturizers help moisturize the skin,” he says. After using an ointment in the first few days post-op, most patients will transition to a no-frills moisturizer packed with nourishing ingredients.

When in doubt, skincare products with calming agents are the way to go. And, as far as makeup goes, mineral-based formulas, which won’t [clog pores](https://aedit.com/aedition/at-home-treatments-for-clogged-pores), are best. Dr. Emer also likes ultra-gentle enzyme cleansers for post-laser and microneedled skin to “help break up the dead skin cells, scabs, crusting, and even the blood that remains. “These types of products break all of that apart enzymatically,” he adds, which means no manual exfoliation.

[Sunscreen](https://aedit.com/aedition/dermatologist-approved-sunscreen-for-face-body) is the backbone of any good skincare routine, and it holds just as much weight — if not more — on just-treated skin. After any procedure, it's best to avoid direct sun exposure (post-laser skin, for example, can become damaged and irritated when exposed to the sun during healing) and slather up with a [physical-blocking](https://aedit.com/aedition/difference-between-chemical-sunscreen-physical-mineral-sunscreen) sunscreen. “Sunscreen should be immediately used — just not on the first day if you had something more aggressive done — and tinted versions are helpful to conceal any redness,” Dr. Emer suggests. 

Spending time and money on something like a laser or deep peel to undo [sun damage](https://aedit.com/aedition/cosmetic-procedures-treatments-to-correct-sun-damage) and then not protecting that investment with a good sunscreen that costs $40 or less is just silly, so always apply (and reapply throughout the day) sunscreen with an SPF of at least 30 or higher.

## **Color Safe**
The procedure or treatment you just opted for pretty much dictates how soon you can swipe back on your favorite eyeshadow or concealer. Dr. Green usually instructs patients to stay away from makeup after a procedure since it can interfere with skin healing and lead to excessive irritation.

After chemical peels and lasers that break or resurface the skin, the epidermis should heal (this can take up to a few weeks) before applying most traditional makeup, Dr. Karam says. When it comes to procedures with specific incision sites, the rules are usually a little different. “With surgeries like eyelid surgery, facelifts, fat transfers, and such, there isn't a limitation to being able to use skincare products or makeup shortly afterward," he adds. 

There is, however, one serious caveat: Never apply anything directly on an incision unless it's a specific type of makeup and your surgeon says it's okay. He recommends his patients wear mineral-based cosmetics instead of chemical-based formulas because they tend to be gentler and less sensitizing.

There are two main factors that determine how surgical incisions heal. One you can control, and the other you can't. “The most important factor in scarring is genetics,” says [Jacob Unger, MD](https://aedit.com/provider/jacob-unger-md-2649), a board certified plastic and reconstructive surgeon in Nashville, TN. “This, unfortunately, is not something any doctor or patient has control over, but it is the most prominent deciding issue when identifying how well someone scars.”

This is true regardless of the procedure, and your medical history should shed some light on how your plastic surgery recovery will go. “For all surgeries — abdominoplasty, brachioplasty, breast surgery, etc. — the primary factor that impacts how a scar heals is the patient’s own wound healing,” says [Umbareen Mahmood, MD](https://aedit.com/provider/umbareen-mahmood-md-23), a board certified plastic and reconstructive surgeon in New York City. “Some patients form [keloids or hypertrophic scars](https://aedit.com/aedition/how-to-treat-every-type-of-scar) with any trauma to the skin, whereas other patients heal with imperceptible scars.”

As Dr. Mahmood explains, arm lift scars “tend to be one of the more difficult scars for some patients to accept because they can widen or become hyperpigmented.” This is because the arms are such an integral part of just about every motion we make. “Activities as simple as putting hair up in a ponytail or putting on a bra can cause tension on the scar,” she notes.

With that said, the incision generally extends from the axilla (armpit) to the elbow. “Some surgeons choose to place the incision at the back of the arm,” she says, noting that she likes to put it in the inner arm crease. “In this location, it is not visible when the patient's arms are at their side or in a relaxed communication position,” she shares.

## **Breast Surgery**
While augmentation mammaplasty (read: [breast augmentation](https://aedit.com/procedure/breast-augmentation-solutions)) remains one of the most popular cosmetic procedures in the United States, it’s not the only type of breast surgery. Below is what you can expect from the most common breast procedures:

### **Breast Augmentation**
[Breast augmentation with implants](https://aedit.com/procedure/breast-augmentation-with-implants) is a procedure that allows for quite a bit of customization. [Patients choose](https://aedit.com/aedition/how-to-choose-the-right-breast-implants) the type and general size of the implant they are interested in, and the surgeon will insert the implants using their preferred technique. “This surgery typically has a very small scar — one to two inches — along the undersurface of the breast along the fold or possibly in the areola or under the arm,” Dr. Unger explains. He personally prefers the “under breast approach” because, with “proper positioning, delicate tissue handling, and meticulous closure technique,” the scars “can commonly be made to be nearly invisible once healed.”

### **Breast Lift**
Unlike a breast augmentation, a mastopexy (a.k.a. [breast lift](https://aedit.com/procedure/breast-lift)) requires a more prominent incision. There are an [array of techniques](https://aedit.com/aedition/mastopexy-breast-lift-after-pregnancy-breastfeeding) depending on the degree of ptosis (read: drooping), but it can often involve a vertical incision from the nipple to the lower border of the breast fold. In Dr. Unger’s estimation, this scar tends to be “concerning” for patients because “it is visible along the lower pole of the breast.”

With the right care, however, it can be greatly minimized. “At least in my experience, this scar almost always heals incredibly well, and is usually imperceptible for most patients after a healing period,” Dr. Unger says. “For some this means it's almost invisible by three months; for some it's more like one to two years.” Regardless of which camp you fall into, “it almost always fades away to a level that is difficult to identify it at all,” he adds.

### **Breast Reduction**
For Dr. Frederick, the pivotal component that makes scars better in a reduction mammoplasty (a.k.a. [breast reduction](https://aedit.com/procedure/breast-reduction)) is the technique. As he explains, in a “classic, old-school” inferior pedicle breast reduction, flaps are raised and closed “under tension over the top of the pedicle.” With a superior or superior-medial pedicle, meanwhile, “no flaps are raised” and “the pillars are closed together so there’s minimal tension on the skin,” he shares.

### **Gynecomastia Surgery**
[Male breast reduction](https://aedit.com/procedure/male-breast-reduction), or gynecomastia surgery, is one of the most popular cosmetic procedures for men in the U.S. Like a female breast reduction, patients may need fat removal, skin excision, or a combination of the two depending on the grade of gynecomastia. Unlike female breast reduction, the majority of patients do not need significant incisions. “Most gynecomastia is done with liposuction through a small opening,” Dr. Frederick says. “This external scarring is typically not an issue.”

## **Liposuction & Fat Transfer**
Speaking of liposuction, the gold standard of fat reduction tends to be minimally invasive as far as incisions are concerned. “Liposuction scars are small — approximately 3 millimeters — and placed in well-hidden areas of the body,” Dr. Mahmood explains. For example, in abdominal liposuction, incisions are usually made low in the bikini line and in the umbilical hood for concealment purposes. “Incisions are minimal and tend to heal well,” she says. It should be noted, however, that if you opt for a type of lipo that is heat-based (like [VASER](https://aedit.com/procedure/ultrasound-assisted-liposuction) or [Smartlipo](https://aedit.com/procedure/smartlipo)), “there is a higher risk of scarring at the liposuction cannula site,” Dr. Mahmood cautions.

Depending on your aesthetic goals, the fat removed via liposuction may be purified and injected into another part of the face or body. As for injecting the fat from the donor site, Dr. Frederick uses an 18 gauge needle and “small pokes” so there are no external scars. “The important thing is to fan out the fat evenly and quickly because it swells almost instantaneously and that will distort the [treatment area] and prevent you from seeing where you injected the fat if you do it too slowly,” he shares.

## **Thigh Lift**
The purpose of a [thigh lift surgery](https://aedit.com/procedure/thigh-lift) is to remove excess skin and fat and reposition underlying tissue to lessen the appearance of sagging. According to Dr. Frederick, the incision “almost always” runs along the inside of the leg and the groin crease, which can be problematic for healing. “The problem with the groin crease scar is that it’s fighting gravity, as the weight of the leg pulls down on it,” he explains. There are steps a surgeon can take while operating to lessen the burden. Ultimately, “it’s important to anchor the thigh to the more sturdy aspects of the pelvis so that it doesn’t drop back down with gravity,” Dr. Frederick shares.

## **How to Care for Scars After Plastic Surgery**
As we’ve alluded throughout the article, [well-healed scars](https://aedit.com/aedition/minimizing-scars-after-surgery) start in the operating room. “As plastic surgeons, we are very familiar with scarring and various techniques to help minimize the visibility and size of scars,” Dr. Unger shares. “There are methods and materials we can use to help improve scar tone and texture and quality.” This includes specific closure techniques and sutures that yield a ‘tension free’ result. “We use multiple layers of stitches commonly and try to make sure there is no ‘pulling’ on the final incision line,” he explains. “Tension or stress across an incision is a major determinant in how wide or thick a scar may end up.”

Once you’re home, you too can work to keep tension to a minimum. “Significant tension on the scar either at the time of surgery or by the patient’s increased activity early in the post-op period, affects how the scar heals,” Dr. Mahmood cautions. [Compression garments](https://aedit.com/aedition/benefits-of-compression-garments-after-cosmetic-surgery) (worn in accordance with your surgeon’s post-op instructions) are one way to decrease [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery) and, in turn, minimize undo tension, as is being [mindful of your movement](https://aedit.com/aedition/when-to-start-exercising-again-after-cosmetic-surgery-procedures). “For example, stay hunched over after a tummy tuck,” Dr. Frederick notes. 

Topical treatments, like [silicone gels and strips](https://aedit.com/aedition/scar-minimizing-skincare-to-use-after-surgery), can be a helpful part of the recovery process — but consistency is key. “Silicone sheeting can help, but it’s like putting a tomato in a greenhouse,” Dr. Frederick says. “It needs to be there everyday for six months to get a 10 to 15 percent better yield.” 

It’s important to remember that scars can take a year or more to fully heal and fade. If you are unhappy about the appearance of your surgical scars, talk to your surgeon. As Dr. Unger shares, there are “a range of minimally invasive skin-remodeling techniques such as [dermabrasion](https://aedit.com/aedition/dermabrasion-might-be-the-scar-treatment-youve-been-looking-for) and lasers that can help place the body into optimal position for the best possible healing.”

In a perfect world, cosmetic surgery would be incision-free. We’re not quite there yet, but, in the hands of a skilled surgeon, scarring after body procedures can be minimal.

Prospective patients, be aware: rhinoplasty — the [cosmetic or reconstructive surgery](https://aedit.com/aedition/difference-between-cosmetic-rhinoplasty-vs-functional-rhinoplasty) that addresses [nose shape](https://aedit.com/concern/nose-shape), [nose size](https://aedit.com/concern/nose-size), and/or [breathing concerns](https://aedit.com/concern/breathing-issues) — functions on its own timeline. While the healing process is relatively straightforward, it can take 12 months or more for the true result of the surgery to emerge. Needless to say, patience is required.

If you’re looking for instant (or instant-ish) gratification, non-surgical [liquid rhinoplasty](https://aedit.com/procedure/liquid-rhinoplasty), which involves injecting filler to sculpt the nose, may be an option. [David Shafer, MD](https://aedit.com/provider/david-shafer-md-153), a board certified plastic and reconstructive surgeon in New York City, actually prefers the non-surgical alternative for many patients, though he concedes that “there are some things surgical rhinoplasty can do that liquid can’t.”

## **The First Few Days After Rhinoplasty**
Plan to go home the same day as your [rhinoplasty](https://aedit.com/procedure/rhinoplasty-nose-job) surgery (with the help of a friend or loved one) and to spend the first two to four days post-op taking it as easy as possible. “You don’t have to be bedridden, but you should definitely be resting,” says [Samuel Hahn, MD](https://aedit.com/provider/samuel-hahn-md-20403), a double board certified facial plastic and reconstructive surgeon in Cockeysville, Maryland. “Even though rhinoplasty is not a big area of surgery, your body still needs proper rest to recover.” 

He directs patients to limit exertion to “non-strenuous daily activities.” You may be able to return to at-home tasks, like working on a computer or talking on the phone, quite quickly, but there is to be no heading to the gym just yet. “The first couple days, you’re going to be constantly aware that you just had surgery,” Dr. Shafer says. “You’re going to have swelling, you’re going to have some discomfort, you might be taking some pain medication.” 

Due to [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery) in the nasal passages, you’ll feel congested and will likely have to breathe through the mouth. For those early days (and weeks) to come, you’ll have to [sleep on your back](https://aedit.com/aedition/sleeping-better-after-cosmetic-surgery-aesthetic-procedures) to avoid pain and help decrease swelling. Swelling will start about 24 to 48 hours after surgery. As the swelling goes down, [bruising](https://aedit.com/aedition/how-to-treat-prevent-bruising-after-lasers-filler-botox) around the nose and under the eyes may appear — though it’s important to remember that every patient is different. “Some might have no bruising, another might look like they took a punch to the face,” Dr. Shafer says. Most patients fall somewhere in-between those two extremes.

It’s important to understand that swelling and bruising subside over the course of several months, and your new nose won’t be visible in these early stages. The good news? The most discomfort is during this initial, brief recovery period. “After several days, the worst of it is over,” Dr. Shafer confirms. 

### **Pain Management After Rhinoplasty**
Plastic surgeons’ approach to pain management is informed in part by the [addictive nature of opioids](https://aedit.com/aedition/opioid-crisis-plastic-surgery-recovery-pain-management). You’ll likely receive a few days of prescription pain medication to take as needed during that initial recovery period only. After that, over-the-counter pain meds should suffice. Following the first 72 to 96 hours, sensations of pain will start giving way to feelings of pressure. “Most people will start to feel pressure between their eyes, forehead, and cheeks,” Dr. Hahn explains. “Then the pain starts lessening.” 

## **Resuming Normal Activities After Rhinoplasty**
After the initial downtime, rhinoplasty patients can resume non-strenuous tasks as their body feels able. In those first few weeks, Dr. Shafer suggests avoiding any activities that raise the heart rate or blood pressure, which can increase swelling.

After about two to three weeks (and, importantly, a follow-up visit where your surgeon will give you clearance) expect to be able to [return to the gym](https://aedit.com/aedition/when-to-start-exercising-again-after-cosmetic-surgery-procedures), starting slowly with light cardio. After about four weeks, Dr. Hahn’s patients often resume strength training, yoga, or any other activity that puts stress on the face. As with any recovery period, listen to your body (and your surgeon) and take your time before returning to vigorous activity. 

## **Final Rhinoplasty Results**
Your plastic surgeon should take care to explain exactly what to expect from surgery, recovery, and results — and, when it comes to results, that means setting a [realistic timeline](https://aedit.com/aedition/how-long-does-it-takes-to-recover-from-cosmetic-surgery-procedures). “I go over expectations with patients multiple times, so they have a good understanding of what to expect,” Dr. Hahn shares. “If you give a false presentation of what their results may be, you may have an unhappy patient even if it’s a good surgery.” 

Because the nose is a particularly vascular part of the body, with a lot of blood flowing to and from the it, postoperative swelling can linger. Dr. Hahn estimates that most patients will have visible swelling for three or four weeks after surgery. After that, it’s usually a few more months of subtle swelling that, he says, “they themselves alone will notice.” 

It can take up to a year for the final results to be fully visible and settled. For a patient who requested subtle adjustments to the nose, those changes often take the longest to become noticeable. “The tip of the nose tends to remain swollen the longest,” Dr. Hahn notes. “Especially for someone who wants tip work done, I advise them that they won’t see the final results for quite a while.” 

As the nose is settling, Dr. Shafer says other procedures like [neurotoxins](https://aedit.com/aedition/where-to-use-botox-from-head-to-toe) and [fillers](https://aedit.com/aedition/how-to-reshape-your-face-with-filler-botox-injectables) are still available to you. Keep in mind that, should you want to continue to tweak the appearance of the nose with a liquid rhinoplasty, your injector should be aware of and experienced in treating surgical rhinoplasty patients. “Once you’ve had surgery, it changes the anatomy of the nose, and you need an experienced doctor to avoid complications,” Dr. Shafter cautions. Of course, once the swelling goes down and the bruises have faded, you may not ever need anyone to touch your new nose again.
