## **1. It’s Not Just One Procedure**
First things first, the term ‘vaginal rejuvenation’ doesn’t refer to any one thing. “Vaginal rejuvenation is really a combination of vulva rejuvenation along with vaginal rejuvenation,” Dr. Rajagopal explains. Translation: there are options. We break them down below:

### **Surgical Solutions**
- **Labiaplasty**: “To improve the aesthetic appearance of the labia minora and labia majora, we offer a [labiaplasty](https://aedit.com/procedure/labiaplasty),” she shares. The surgical procedure can benefit those experiencing discomfort during exercise, urinary tract infections, painful intercourse, and other physical activity. The surgery can be performed alone or as part of a [mommy makeover](https://aedit.com/concern/mommy-makeover).
- **Monsplasty**: For those who have excess skin and/or fat in their upper pubic area that is impacting their quality of life, Dr. Rajagopal recommends [monsplasty](https://aedit.com/procedure/monsplasty) (a.k.a. mons pubis reduction or pubic lift). It can involve liposuction (to remove fat), surgical excision (to remove skin), or a combination of the two. 

### **Non-Surgical Solutions**
- **Emsella®**: “[Emsella®](https://bodybybtl.com/solutions/btl-emsella/) consists of high-intensity focused electromagnetic stimulation (HIFEM) that is similar to MRI magnetic technology,” Dr. Rajagopal explains. “It stimulates and enhances the function of the pelvic floor muscles.” She says the treatment is ideal for those experiencing urinary incontinence and/or an inability to achieve strong orgasms. The non-invasive treatment (you sit on a chair-like device that stimulates the muscles) calls for six 30-minute sessions in a three-week span.
- **Botox**: Yes, you read that right, Botox for vaginal health. The neurotoxin is used off-label for the treatment of [vaginismus](https://aedit.com/aedition/botox-injections-for-vaginismus), a medical condition that causes involuntary vaginal muscle spasms that can make inserting tampons and penetrative sex painful —  if not impossible. Botox is injected into the bulbospongiosus muscle (i.e. the muscle that surrounds the opening of the vagina) to relax it and relieve the spasms.
- **Infini™ RF**: If a surgical labiaplasty isn’t for you, Dr. Rajagopal says “you can also have just a labia majora reduction with [Infini™ RF](https://aedit.com/procedure/infini-rf).” When used on the vulva, the RF microneedling treatment can firm the tissue of the labia majora to enhance its appearance and even tighten the vaginal canal after natural childbirth. “We call it a vagi-facial,” she notes.
- **Laser Therapy**: Dr. Rajagopal uses [FemiLift®](https://almainc.com/products/femilift), a fractional C02 laser, to treat patients with vaginal dryness due to age and vaginal atrophy (a condition that can cause painful intercourse). The [intravaginal laser](https://aedit.com/procedure/vaginal-rejuvenation-with-laser-energy) is inserted into the vaginal canal (she assures us the treatment is much more tolerable than it sounds). Some patients enjoy the benefits after the first appointment, but Dr. Rajagopal recommends three sessions in three months for best results.

For some, the effects are immediate, while an initial series of treatments may be needed for others. Results can last three to six months.

## **2. It’s Highly Customizable**
The vulva is made up of multiple organs – including the vagina – and just about all of them can be addressed. While vaginal rejuvenation is often talked about in relation to childbirth, hormonal changes, genetics, and even stress can impact both the form and function of the genital area. “There can be functional issues that may interfere with her lifestyle and how much pleasure she gets out of sex,” Dr. Rajagopal shares. “There can also be aesthetic issues that she may want to address and that’s what we offer.” 

Many of the procedures listed above can be combined to create a treatment plan that best addresses a patient’s unique anatomy and goals. Consulting with a board certified plastic surgeon or gynecologist who specializes in vulva rejuvenation is the best way to learn more about your options.

## **3. Candidates Vary** 
As with any procedure, those interested in vaginal rejuvenation should consult with their provider to ensure their preferred procedure(s) is right for them. As we mentioned, treatments that address vaginal health and appearance aren't one-size-fits-all, nor should they be. “With each treatment, we have an examination first to see who is a viable candidate,” Dr. Rajagopal explains. Topics such as medical history and overall health and wellness should also be discussed during the examination process. Women may also want to consider whether or not they are planning to have children or want to further expand their family. While some of these solutions treat sexual health concerns, others have the ability to be impacted or undone by pregnancy or childbirth.

## **4. Sensitivity Can Be Compromised** 
Although it’s uncommon, a decrease in sensation is always possible after cosmetic procedures – and vaginal rejuvenation is no exception. There is no surefire way to prevent this, though cosmetic gynecology performed by a highly skilled provider who specializes in the practice is the best way to ensure a safe and effective outcome. Recovering from surgery is a bit more involved. Depending on the procedure, a few days to a week of downtime may be needed. Infection is a greater risk due to the nature of the area, so practicing good hygiene after surgery is key. Pregnancy causes many changes to the body, some which reverse naturally and others that require a little extra help. If you wish to achieve your pre-baby body or sculpt and contour your new physique, a [mommy makeover](https://aedit.com/concern/mommy-makeover) may be the ultimate amalgamation of procedures to do just that.

The term ‘mommy makeover’ is a catch-all for correcting any changes to a woman's body after childbirth, says [Lauren Chmielewski, MD](https://aedit.com/provider/lauren-chmielewski-md-143), a board certified plastic and reconstructive surgeon in New York City. A traditional mommy makeover typically involves a tummy tuck, liposuction, and breast surgery (breast augmentation, breast lift, or [both](https://aedit.com/aedition/combining-breast-augmentation-with-breast-lift)). Yet, today's mommy makeovers are more inclusive than ever. More women are opting to add [additional procedures](https://aedit.com/procedure/mommy-makeover-solutions) that include fat transfer to the breasts, vaginal rejuvenation, Brazilian butt lifts (as is found in the so-called ‘[hourglass mommy makeover](https://aedit.com/aedition/hourglass-mommy-makeover-with-bbl)’), and even arm and thigh lifts.

Just as important as finding a qualified plastic surgeon who can achieve the results you're after is to understand (yourself) the ins and outs of the highly involved surgery and what the recovery entails. Although most mommy makeover patients are thrilled with the outcome, getting there is not always easy. Often, the [mommy makeover recovery process](https://aedit.com/aedition/mommy-makeover-recovery-healing-process) is downplayed – especially on social media. Therefore, patients need to have realistic expectations when it comes to the steps they need to take to achieve optimal outcomes, says [Gregory Buford, MD](https://aedit.com/provider/gregory-buford-md-1015), a board certified plastic and reconstructive surgeon in Lone Tree, CO. “Proper healing requires planning and attention to detail, like attentive wound care, compression, restriction of various activities, and careful attention to what you eat,” he shares.

Generally speaking, three weeks of taking it very easy is recommended, but that’s not always feasible – particularly for moms with young children. The more time you can take to recuperate, however, the better, since bleeding, breakage of internal sutures, and the opening of wounds are possible if you try to do too much too soon, cautions [Mark G. Albert, MD](https://aedit.com/provider/mark-albert-md-77), an NYC-based board certified plastic and reconstructive surgeon.

There's plenty to know about a mommy makeover that all too often flies under the radar. Whether you are in the [research phase](https://aedit.com/aedition/best-questions-to-ask-before-a-mommy-makeover), considering booking, or getting ready for recovery, we tapped top surgeons to get their expert tips on preparing for and recovering from the surgery. Here are 10 things to know before getting a mommy makeover.

## **1. Numbness Can Last Months, If Not Longer**
It's not uncommon for the skin to feel numb for months (even upwards of a year) after the [tummy tuck](https://aedit.com/procedure/abdominoplasty) portion of a mommy makeover. It is a normal – albeit disconcerting – part of the overall [healing process](https://aedit.com/aedition/tummy-tuck-recovery-healing-process), and there's no need to worry about a lingering lack of feeling. It is related to trauma to the nerve endings and swelling. During an abdominoplasty, your surgeon lifts and removes excess skin to tighten the abdominal wall and muscles. “In doing this, we cut the small nerves to the skin, and it takes time for these nerves to grow back to the skin surface,” Dr. Albert says. As a result, there can be little or no feeling through the abdomen during the regrowth period. It's a waiting game as the nerves repair, reconnect, and regenerate independently.

Usually, this process takes a few months, and Dr. Albert says it is incredibly uncommon for someone to have permanent numbness after the procedure. “Any numbness generally subsides as the swelling goes away, which can take up to six to 12 months before completely returning to normal,” Dr. Chmielewski adds. However, some patients never regain 100 percent sensation, and some degree of numbness may always exist in the abdominal area. So, if you feel the urge to scratch an itch with little relief, that’s why. 

## **2. Walking – Even Around the House – Is Difficult at First**
Getting up and mindfully moving is essential in the first few days after a mommy makeover since gentle movement promotes circulation and proper blood flow. Dr. Chmielewski says walking helps lower the chance of developing a blood clot or deep vein thrombosis (DVT) after surgery. Many plastic surgeons advise purchasing a walker (do this [before surgery](https://aedit.com/aedition/best-plastic-surgery-recovery-nurses-centers-products) and have it ready to go when you get home), since mobility is somewhat limited and the extra support is helpful. 

If you find it difficult or painful to stand upright, it is due to the newly created tightness in the core. Any discomfort is temporary. Some surgeons instruct their patients to walk hunched over with the help of a cane to prevent [unnecessary strain](https://aedit.com/aedition/how-to-reduce-tension-scarring-after-cosmetic-surgery) on the stomach incisions, which can force them open. But not every surgeon takes this approach nor agrees with it. “I find this unnecessary and believe it creates more back pain than one would have from the procedure alone,” Dr. Albert shares. “Therefore, I encourage my patients to sit and stand upright and be up and walking the day of the procedure.”

To make sure you keep moving, you’ll want to create a simple walking schedule and have a family member, friend, or partner make sure you stick to it. “Walking for 10 minutes every two hours while awake is a great way to ensure you’re getting enough movement in,” Dr. Chmielewski shares. As for when your movement will feel less encumbered, Dr. Buford says patients usually walk fully upright by week two or three.

## **3. The Shape & Position of the Breasts Will Evolve**
If you opt for [breast implants](https://aedit.com/procedure/breast-augmentation-with-implants) to correct deflation, decreased volume, or droopiness, don’t freak out if you don’t like how your new breasts look the first time you see them. Your first impression will not be your last. The breast implants usually sit very high on the chest [following a breast augmentation](https://aedit.com/procedure/breast-augmentation-with-implants). To help the implants settle into place, Dr. Buford recommends his patients wear a breast band and perform breast implant massages. This settling process (sometimes called ‘drop and fluff’) generally resolves over the coming months. 

It's also normal for the breasts to feel tight and swollen initially. But as the tissue stretches and relaxes and the implants soften up and descend the chest over the next few months, they begin to take their final shape and become more symmetrical and natural looking. “This is a completely normal part of breast augmentation with or without a breast lift,” Dr. Albert says.

## **4. An Electric Recliner Will Make Standing Up & Sitting Down Easier**
Post-mommy makeover, patients may find it cumbersome to get in and out of bed or a chair. That's why some plastic surgeons recommend recovering in an electric recliner, which makes sitting and standing much easier on the body. [Resting in a reclining position](https://aedit.com/aedition/sleeping-better-after-cosmetic-surgery-aesthetic-procedures) (you can sleep in one at night, too) helps take the pressure and strain off the abdominal wall, which is important since unnecessary stress or a pulling feeling can cause an incision to open.

If you don't have a recliner, you may want to purchase one or borrow one from a friend or family member. If an electric recliner is out of the question, consider using a wedge pillow for additional back support when sleeping or resting and use a small pad or lumbar support under the knees for extra support. ## **5. Your Plastic Surgeon Can Remove an Existing C-Section Scar**
If you delivered your baby via c-section, it's okay to have a mommy makeover once your doctor deems you ready for surgery. However, one of the most commonly asked questions among c-section patients is if a secondary scar is necessary. In most cases, creating a tummy tuck incision below an [existing c-section scar](https://aedit.com/aedition/invasive-non-invasive-procedures-to-fix-appearance-of-c-section-scar) (they're shorter and smaller than abdominoplasty scars) removes the original one. “Many patients have existing scars from previous procedures on the abdomen,” Dr. Chmielewski says. “So, depending on the operation, it is usually possible to have a tummy tuck and use these scars.”

There are some instances where you may not be able to trade a c-section scar for the [hip-to-hip tummy tuck scar](https://aedit.com/aedition/scarring-after-body-plastic-surgery), like if the c-section scar is too low. “When this is the case, it may not be possible to have the abdominoplasty scar placed below the C-section scar,” says [Adam Schaffner, MD](https://aedit.com/provider/adam-schaffner-md-33), a double board certified facial plastic and reconstructive surgeon in NYC. 

## **6. A Compression Garment Will Be Your New Favorite Accessory**
When you wake from surgery, you'll be wearing a tight-fitting [compression garment](https://aedit.com/aedition/benefits-of-compression-garments-after-cosmetic-surgery) in at least one area. Although it is uncomfortable, it helps limit swelling in the abdominal area and promotes healthy healing. Dr. Buford calls compression “a necessary evil,” adding that “it can mean the difference between an amazing result and a ho-hum outcome.” He also says that compression enhances local lymphatic drainage and reduces the risk of fluid collections ([seromas](https://aedit.com/aedition/risk-of-seroma-after-cosmetic-surgery)) beneath the skin.

Dr. Albert says that patients know that any compression will stop as soon as they remove the garment, and they will get swollen again. “Typically, patients use the compression garment I give them in the OR for about a week and then switch to a tighter compression garment as the swelling subsides,” he notes. Every plastic surgeon has their recommendation for the length of time that patients should wear compression garments, and most stay in one for at least the first six weeks post-procedure. Throughout that process, you can size down as [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery) resolves to retain some degree of compression until you reach your final shape and size. If you do not wear a compression garment or have a manual [lymphatic drainage massage](https://aedit.com/aedition/benefits-of-lymphatic-drainage-massages-after-cosmetic-surgery) performed, it may lead to prolonged swelling.

## **7. Make Sure You Are Done Having Children**
While there is nothing stopping you from getting pregnant again after a mommy makeover, it certainly will [impact the results](https://aedit.com/aedition/how-pregnancy-affects-cosmetic-surgery-results). “The idea is to make your stomach as flat as possible and the skin as tight as possible, which can stretch again if one has another child,” Dr. Albert says. Dr. Chmielewski agrees and usually advises her patients to wait until they are finished having children to proceed with a mommy makeover. “Cosmetic surgery is an investment, and patients should be at or close to their goal weight for the results to be long-lasting and achieve the best results possible,” she says.

Moreover, getting pregnant following a mommy makeover may have implications on the way the body responds to pregnancy. “If a tummy tuck is performed in which the rectus muscles are tightened (plicated), a woman will likely feel a tighter sensation than prior pregnancies,” Dr. Schaffner says. “There is the theoretical risk of restricting the growth of the fetus.”

## **8. You’ll Need a Laxative (or two) to Get Things Moving Again**
A mommy makeover requires [anesthesia](https://aedit.com/aedition/everything-you-need-to-know-about-anesthesia), and the trade-off for comfort during surgery is constipation in the days following it. For this reason, a laxative is a must for your post-recovery shopping list. Any over-the-counter laxative will do, but, if you’re taking one that you have never used before, it’s best to test it three to five days before your surgery to make sure your body responds well. This way, you can get an idea of how fast-acting it is and any potential side effects. Another way to ensure a smooth transition following surgery is to eat plenty of fiber-rich foods while recovering, which always helps make the bowel movement process a little easier.

## **9. Pay Attention to Your Diet During Recovery**
You are what you eat, especially when in recovery mode. The food you consume while at home recouping from a mommy makeover should be fiber-rich and anti-inflammatory to promote healing. Dr. Buford says surgical nutrition is where patients and many practitioners fall short. But, if you keep to a healthy and clean diet before surgery (along with regular workouts), your recovery will likely be easier and faster — and you'll feel pretty good, too.

Protein is also crucial for recovery. A diet that lacks enough protein may lead to poor wound healing and a lack of energy, Dr. Schaffner says. Steer clear of salty foods, which can cause unnecessary water retention. The same goes for soda and processed and fried foods. They offer no nutritional value, but these inflammatory foods can lead to prolonged swelling. ## **10. You May or May Not Need Drains**
For years, drains were one of the downsides of a tummy tuck. Patients don't like emptying them, and their removal can be uncomfortable. Today, not every plastic surgeon uses drains, and it all depends on your provider’s technique and preference. As Dr. Schaffner explains, progressive tension sutures minimize the need for drains. “We also use medications before and after surgery (such as gabapentin, Celebrex, and Tylenol) and during surgery (such as long-acting anesthetics) to [minimize the need for opioids/narcotics](https://aedit.com/aedition/opioid-free-plastic-surgery-pain-management) and shorten recovery times and minimize the development of complications,” he adds. Drainless tummy tucks make the recovery process a bit easier with [better scarring](https://aedit.com/aedition/how-to-reduce-tension-scarring-after-cosmetic-surgery), too.

If you have been diagnosed with polycystic ovary syndrome (PCOS), we don’t need to tell you how difficult it can be to keep up with and treat the diverse array of symptoms. For example, the hormonal imbalances associated with PCOS can lead to skin and hair concerns ranging from [breakouts](https://aedit.com/aedition/how-to-treat-hormonal-acne-body-acne-cystic-acne-whiteheads-blackheads) and pigmentation or unwanted hair growth and loss. 

We know that the lack of information out there on the topic is, at best, frustrating, which is why we tapped our expert network to break down the complexities of PCOS diagnosis and treatment. From what PCOS is (and what kind of provider you should go to if you think you are experiencing symptoms) to how to manage it, here’s what you need to know about polycystic ovary syndrome.

## **An Intro to Polycystic Ovary Syndrome (PCOS)**
So, what is PCOS exactly? Polycystic ovary syndrome, better known as PCOS, is a hormonal disorder in which the ovaries produce an abnormal amount of androgens (read: male sex hormones). The name describes the small cysts that form in the ovaries, though not everyone with PCOS experiences cysts. It should also be noted that not all females with ovarian cysts have polycystic ovary syndrome.

### **Diagnosing PCOS**
While PCOS is increasingly common among females of childbearing age, it is not a straightforward diagnosis. “[It] can be a difficult condition to diagnose due to its varied presentation,” says [Iyanna Liles, MD](https://www.womenshealthct.com/practices/comprehensive-gynecology-of-ct-formerly-fine-and-gillette/providers/iyanna-liles-md/), Connecticut-based OB/GYN and women’s health specialist. “In general, most patients who are evaluated for PCOS by an OB/GYN present with symptoms of irregular periods.” 

But that’s not all. “They can report other symptoms such as increased [acne](https://aedit.com/concern/acne), abnormal body hair growth (requiring extensive hair removal techniques), infertility, or metabolic syndromes including diabetes or high cholesterol,” Dr. Liles explains. Weight gain, anxiety and depression, [hair loss](https://aedit.com/concern/hair-loss), and sleep apnea are also possible side effects.

When it comes to diagnosing PCOS, providers often start with a discussion of medical history — including, skin, hair, and weight changes — and a physical exam. From there, a pelvic exam, blood tests, and an ultrasound may be in order.

### **Causes of PCOS**
The symptoms are well-documented, yet there is less known about what causes PCOS. “While we do not completely understand PCOS, we do know that this condition involves increased available circulating androgens due to multiple possible causes,” Dr. Liles says. Factors that are thought to play a role include insulin resistance and genetics. Generally speaking, PCOS signs and symptoms tend to be more severe in patients who are obese.

### **Treating PCOS**
Just as there is no singular presentation of PCOS, there is no one-size-fits-all treatment plan. There are, however, a number of ways the condition can be managed depending on a patient’s symptoms. “In general, we start treatment for PCOS with lifestyle changes such as diet and exercise,” Dr. Liles says. “These interventions decrease the amount of circulating androgens and their effect on the body.” It’s also important for your provider(s) to know if and when you hope to become pregnant, as “some medications will either prevent pregnancy or should not be used in pregnancy,” she cautions.

Many PCOS diagnoses begin at the gynecologist because patients are concerned about irregular periods or difficulty conceiving. But long-term care plans often include additional specialties and experts. “If a patient primarily has skin and/or hair issues without menstrual issues, a dermatologist or skincare specialist may take on treatment,” Dr. Liles explains. “It can be confusing to know who to go to first, but both types of providers can refer to the appropriate specialist based on the patient's concerns.”

[Elizabeth Trattner, AP, DOM](https://www.instagram.com/dreliztratts/?hl=en), a board certified acupuncturist and doctor of Chinese and integrative medicine in North Miami Beach, FL, works with clients with PCOS and advocates for a comprehensive approach. “I believe it takes a team of professionals to create a cohesive treatment plan,” she shares. “I always triage patients to gynecologists and dermatologists [because they] like a teamwork approach.” This is especially important when prescription medications are involved. “There are many times that patients are under other physician’s care and are on medication,” she adds. “It is important to work in tandem with other doctors to monitor blood levels, track progress, and lower medication as the patient improves.”

At [Dr. Trattner’s practice](https://elizabethtrattner.com/), PCOS is treated using a combination of Traditional Chinese Medicine (TCM) and integrative medicine techniques. “Chinese Medicine, including acupuncture and herbal medicine, are a comprehensive and effective system for treating PCOS,” she explains. “It alleviates symptoms along with regulating the endocrine and reproductive system.” In fact, there is a good deal of [research](https://pubmed.ncbi.nlm.nih.gov/20230329/) into the many benefits of acupuncture for patients with PCOS. “[It] can [regulate](https://pubmed.ncbi.nlm.nih.gov/23482444/) ovarian and adrenal sex hormone levels,” she notes.

As it relates to integrative medicine, she says dietary changes are an important component of treatment. “This multi-pronged approach helps address both root issues and symptoms,” Dr. Trattner says.

## **The Impact of PCOS on Skin & Hair**
As we mentioned, skin and hair concerns are common among patients with PCOS. Acne is a common side effect, as is both excessive hair growth and [hair loss](https://aedit.com/aedition/common-causes-of-hair-loss-thinning-shedding-in-women). Below, we break down what you need to know about treating and managing skin- and hair-related PCOS symptoms.

### **How PCOS Can Affect the Skin**
There are a number of ways PCOS can affect the skin. Many patients experience face and body [acne](https://aedit.com/aedition/how-to-treat-prevent-body-acne). Some also develop [melasma](https://aedit.com/concern/melasma) and (in rarer cases) [skin tags](https://aedit.com/concern/birthmarks-and-moles). Not everyone deals with any or all of these symptoms, but, for those that do, they are often treated with a combination of professional and at-home remedies.

For starters, it’s important to consider what your plans are for the months ahead. “If a patient is actively pursuing [pregnancy](https://aedit.com/aedition/most-common-skin-hair-conditions-during-pregnancy) in the next year, I would recommend lifestyle changes and discuss various skincare techniques,” Dr. Liles says of treating patients with PCOS-associated skin concerns. “If a patient is not pursuing pregnancy, I recommend lifestyle changes, [improved skincare](https://aedit.com/aedition/best-skincare-ingredients-to-treat-acne-blackheads-whiteheads), and combined oral contraceptives (birth control pills) for about three to six months.”

As she explains, birth control pills “decrease ovarian androgen secretion and increase a protein that binds circulating androgens with the intent to decrease circulating androgen levels.” She specifically prescribes pills with a progestin component that has low androgen potential. If patients do not see their desired effect during the trial period with oral contraceptives alone, Dr. Liles usually refers them to a dermatologist for further evaluation.

Additionally, Dr. Trattner says that Chinese Medicine and acupuncture can be employed to “regulate the endocrine system and lower the number of androgens in the body.” To complement acupuncture sessions, she also integrates Western supplements and Chinese herbs and recommends [blue light therapy](https://aedit.com/aedition/difference-between-professional-at-home-led-therapy) for skin concerns. “Using these modalities lower testosterone and insulin levels,” she explains. “It takes at least three months to rebalance hormones, but, within that time, excessive hair growth and skin issues — including acne and melasma — begin to wane.”

### **How PCOS Can Affect Hair**
Like the skin, the hair is also affected by PCOS in many ways. Because PCOS increases the amount of male sex hormones in the body, a female with PCOS could find [unwanted hair growth](https://aedit.com/concern/unwanted-hair) on the thighs, chest, stomach, and around the nipples. The hormonal imbalance can also lead to [male pattern hair loss](https://aedit.com/aedition/common-causes-of-hair-loss-balding-thinning-in-men). “The increased circulating androgens act on the pilosebaceous units (hair follicles) in the skin leading to hair disorders such as hirsutism (abnormal sexual hair growth in male pattern) or hair loss,” Dr. Liles explains.

As with the skin, treatment protocols will depend on whether or not a patient is looking to get pregnant. “In the treatment of PCOS associated with unwanted hair growth, if a patient is actively pursuing [pregnancy](https://aedit.com/aedition/managing-treating-postpartum-hair-loss-thinning) in the next year, I would recommend lifestyle changes and mechanical hair removal techniques,” Dr. Liles says. For patients not pursuing pregnancy, she recommends birth control pills for about three to six months. If combined oral contraceptives do not yield an adequate result, Dr. Liles says she may suggest adding anti-androgen medications such as [spironolactone](https://aedit.com/procedure/spironolactone-for-unwanted-hair), [finasteride](https://aedit.com/procedure/finasteride-propecia), or flutamide. “You should always make sure patients have reliable contraception with anti-androgen medications, as they can negatively affect a developing fetus with male organs during pregnancy,” she cautions. If unwanted facial hair growth is a concern, eflornithine cream (a.k.a. [Vaniqa](https://aedit.com/procedure/vaniqa)) may be an option. At the end of the day, “the choice of medication is individualized to the patient,” Dr. Liles says.

## **The Takeaway**
When beginning treatment for PCOS, it’s important to remember that patience will be needed. “When I begin with a new patient for PCOS, I create a comprehensive treatment plan,” Dr. Trattner says. “Once symptoms subside — after approximately three months — I will reevaluate the patient’s case on a regular basis and then transfer them to maintenance.” That maintenance will look different for everyone, but consistency is key. “It can vary from every week to twice a month,” she notes. “Hormones always need to be regulated with PCOS.” 

Additionally, regular care will allow your healthcare team to keep an eye out for more serious complications. “Other possible conditions to consider in patients with PCOS include diabetes, high cholesterol, infertility, and endometrial cancer,” Dr. Liles says. “If someone thinks they have PCOS or has been diagnosed with PCOS, they should make sure they have a clinician closely monitoring them for these conditions and assisting them in preventing the negative side effects seen with PCOS.”

Let’s be honest, the thought of an injection in the vulva is somewhat terrifying. Add in the fact that the shot involves [platelet-rich plasma (PRP)](https://aedit.com/aedition/prp-and-stem-cells-are-changing-the-face-of-the-cosmetics-industry), and you may be even more ready to jump to another article. But women have found the rejuvenating results of such injections to be worth any momentary discomfort. Intrigued? ## **What Is O-Shot?**
O-Shot refers to an “injection of platelets into numbed areas of the vagina, clitoris, and labia,” says [Jason Emer, MD](https://aedit.com/provider/jason-emer-md-722), a board certified cosmetic dermatologist in West Hollywood. The purpose is to heighten orgasms and improve sexual dysfunction. “This process uses your own body’s growth factors to stimulate healing and rejuvenation,” he explains.

The most well known version of the treatment is the Orgasm Shot®, which was coined by Charles Runels, MD — the same doctor that popularized the [vampire facial](https://aedit.com/procedure/vampire-facelift) with Kim Kardashian and the [P-Shot](https://aedit.com/aedition/p-shot-prp-penis-injections-erectile-dysfunction) (i.e. PRP injections into the penis). Although it’s gaining attention as a treatment option, there remains a lack of research into the use of PRP for [women’s sexual health](https://aedit.com/concern/female-sexual-wellness). In a [2014 pilot study](https://www.longdom.org/open-access/a-pilot-study-of-the-effect-of-localized-injections-of-autologous-platelet-rich-plasma-prp-for-the-treatment-of-female-sexual-dysfunction-2167-0420.1000169.pdf) that did not appear in a peer-reviewed publication, improvements in patient satisfaction and pain were noted but not considered statistically significant.

## **How Does O-Shot Work?**
While it may sound hard to believe, O-Shot is one of those things where the knowledge of what’s happening is actually more painful than the experience itself. “With the topical numbing cream, they should not experience anything more than a little discomfort, if any at all,” Dr. Emer says. 

Like a PRP treatment used for other aesthetic and therapeutic purposes, the procedure involves drawing a blood sample (usually the arm or some other non-invasive locale) and putting it into a centrifuge to isolate the platelet-rich plasma, which is packed with healing growth factors. The PRP solution is then injected into different parts of the vulva with a fine-gauge needle.

Depending on the patient’s preferences, the PRP can be injected into three major areas: 

- The labia
- The internal vaginal canal
- The clitoral hood 

When injected into the labia majora, it helps maintain volume and improve overall appearance. Injecting into the vaginal canal and clitoral hood, meanwhile, is thought to provide the functional and sensorial benefits. O-Shot boosts blood flow to the treatment area, which may heighten sensitivity to stimulation and increase libido. Some patients also report an increase in vaginal lubrication, as well as improvement in urinary function and incontinence.

The entire procedure takes 15 to 30 minutes, though it’s not a one-and-done thing. “O-Shot requires a series of treatments first, typically monthly for three treatments,” Dr. Emer says. After the initial three sessions, the patient can come in every three to six months to “maintain results and prevent the symptoms of aging in this area,” he adds. 

As with any injection, “sensitivity and irritation are not uncommon or abnormal” after treatment, Dr. Emer says. Patients may feel a bit of pressure during treatment as the solution is injected, and there may be a mild stinging sensation that resolves within 15 minutes or so.

## **Best Candidates for O-Shot**
The ideal candidate for O-Shot is a healthy patient concerned about sexual dysfunction or hoping to increase arousal, lubrication, sexual desire, and/or orgasm. Women with urinary leakage, chronic pain from childbirth, and those who experience painful intercourse may also benefit from O-Shot.

## **O-Shot Recovery & Results** 
After an O-Shot treatment, patients can engage in sexual activity the very same day, though Dr. Emer notes that “some sensitivity in the area should be expected for a day or two.” As a result of the needle, there may be mild bleeding or spotting, but it typically resolves in less than 24 hours.

In terms of when you can expect to feel the results, everyone is different. “Some of my patients see results almost immediately, while others may have to wait three to four weeks to see any sort of difference,” Dr. Emer explains. So, how long do results last? While it depends on the patient, Dr. Emer says it’s usually between three and six months, at which point, you can come in for another treatment.

From acne and hyperpigmentation to stretch marks and laxity, pregnancy can have a pretty significant impact on the skin. On one end of the spectrum, women tend to enjoy thicker fuller hair during gestation, while increased blood flow helps promote that flushed, pregnancy ‘glow.’ But not all changes are quite so positive. “There are massive changes in the skin in the first trimester and there may be hypersensitivities in the skin,” says skin and wellness expert Marie Reynolds of [Marie Reynolds London](https://www.mariereynoldslondon.com/). Here, we asked the experts what exactly happens to the skin and hair during pregnancy and how to address those shifts. 

## **1. Acne**
Not just a teenage concern, [acne](https://aedit.com/aedition/how-to-treat-hormonal-acne-body-acne-cystic-acne-whiteheads-blackheads) is common in pregnancy. “Many women who are on oral contraceptive pills have clear skin because the pill regulates the hormones and oil production,” [Joshua Zeichner, MD](http://www.zeichnerdermatology.com/), a board certified dermatologist in New York City. “Going off of the oral contraceptive pill means there is no longer that control.” Additionally, the normal hormonal shifts that women experience during pregnancy stimulate oil glands, pushing them to overdrive, and, often, causing acne, he adds. 

But that’s not all. “Pregnancy may mean [cravings for food](https://aedit.com/aedition/relationship-between-food-and-acne-breakouts) you don’t normally eat and sugary and starchy foods,” Dr. Zeichner says. This increases inflammation in the skin and activates oil production. Reynolds agrees that certain cravings can impact the health of the skin. Sugar cravings, for instance, can create imbalances in the skin and affect the microbiome. “The growing fetus will put pressure on the digestive system, as well,” Reynolds adds. “Be well hydrated and make sure the baby isn’t dictating what you crave.” She recommends eating flaxseeds and chia seeds during pregnancy. 

Connecticut-based board certified dermatologist [Mona Gohara, MD](https://dermatologyofct.com/about/our-providers/mona-gohara-m-d/), says that acne is a wildcard in pregnancy. If you have it, it can get worse or better. If you never had it, you can get it. The tricky part is treatment. As we’ve outlined in our [guide to skincare during pregnancy](https://aedit.com/aedition/skincare-ingredients-to-avoid-during-pregnancy-breastfeeding), many common ingredients are off limits for pregnant women. “Even over-the-counter products like benzoyl peroxide shouldn’t technically be used in pregnancy,” Dr. Gohara says.

## **2. Pigmentation**
Pigmentation is another common skin condition during pregnancy, says New York City-based board certified dermatologist [Rosemarie Ingleton, MD](https://ingletonmd.com/). Known as the “mask of pregnancy,” Ames Melasma affects many women. Dr. Zeichner notes that the reason for [unwanted pigmentation](https://aedit.com/aedition/what-its-like-to-treat-hyperpigmentation-with-skincare-cosmetic-treatments) is unclear, but melanocytes (i.e. pigment-producing cells) are sensitive to hormones like estrogen. “During high-estrogen states, pigment-producing cells are pushed to produce more melanin, our skin’s natural pigment,” he explains. To treat it or, at the very least, prevent it from getting worse, Dr. Ingleton suggests using [mineral sunscreen](https://aedit.com/aedition/difference-between-chemical-sunscreen-physical-mineral-sunscreen) and skin brighteners, like her [ROSE MD Complexion Brightening Booster Serum](https://www.rosemdskin.com/products/rose-complexion-booster). ## **3. Stretch Marks**
[Stretch marks](https://aedit.com/aedition/how-to-treat-stretch-marks) are essentially scars that develop when the skin is stretched faster than it can adjust. During pregnancy, the growth of the child can exceed the ability of the dermis to compensate, explains [Christopher Funderburk, MD](https://neinsteinplasticsurgery.com/), an associate plastic surgeon at Neinstein Plastic Surgery. The growth is so rapid, in fact, that some areas of the skin are not able to receive the proper amount of nutrient-rich blood, and the collagen and elastin fibers physically break from the strain. This leads to areas of ‘dead’ skin that is devoid of its natural, adaptive properties. “This skin no longer has elastic properties, and the skin has a change in coloration,” he says. 

Once the child is born and the skin deflates, you see those areas as stretch marks. They most often show up in the lower abdomen, below the navel, but they can impact the breasts, thighs, and arms as well. “Many patients tell us that they have spent a small fortune on various treatments and have seen little to no effect,” says Dr. Funderburk says. “And, since stretch marks are ‘dead’ skin, no amount of cream or lotion will bring these areas back to life.” Instead, in-office treatments are required.

“During our modern [mommy makeover](https://aedit.com/aedition/the-mommy-makeover-how-to-bounce-back-after-a-baby), we are able to remove this area with a simple skin incision,” Dr. Funderburk says. “The incision is often the size of a C-section scar, and we can often place this in an even lower position. Since the incision mimics that of a C-section scar, no one needs to know that the mom ever required a [tummy tuck](https://aedit.com/procedure/abdominoplasty).” Non-surgical treatments for stretch marks include [microneedling](https://aedit.com/procedure/microneedling) and [Fraxel®](https://aedit.com/procedure/nonablative-fractional-lasers) laser treatments. It should be noted that white stretch marks are often the most difficult to treat, but Dr. Ingleton says they can be improved a bit by employing one of these options. 

## **4. Abdominal Skin Laxity**
Needless to say, the same growth that leads to stretch marks can cause abdominal skin laxity. Dr. Funderburk says that with new, energy-based technologies, skin contraction is possible. “This allows us to combat skin laxity and thereby minimize the length of the incision needed to recreate a taut abdomen,” he notes. Previously, women looking to recreate their pre-pregnancy contour required a lengthy incision. “This is no longer the case, as we employ [energy-assisted liposuction](https://aedit.com/aedition/high-definition-liposuction-techniques-for-body-contouring) devices and treatments such as J-Plasma to contract the collagen fibers and physically shrink and tighten the skin,” Dr. Funderburk shares. 

## **5. Facial Changes** 
During pregnancy, women commonly gain weight and retain fluid, and this may mean puffiness of the face. In some cases, hormonal changes may cause women to develop dark hair on the chin or sideburns, where they previously did not have it. After delivery, with relatively rapid weight loss after relatively rapid weight gain, the face may appear sunken or hollow. “In some cases this will improve on its own, while, in others, cosmetic procedures like injectables might be needed to restore lost volume,” Dr. Zeichner says. 

## **6. Veins**
“When the baby grows, it creates pressure on your venous system and, poof, varicose veins,” Dr. Gohara says. She generally advises leg elevation and, if needed, mild compression stockings. “If they persist, wait until you are done having kids to fix them cosmetically,” she shares. There is a common misconception that hair loss more commonly affects men. On the contrary, the [American Hair Loss Association reports](https://www.americanhairloss.org/women_hair_loss/introduction.html) that 40 percent of Americans dealing with hair loss are women. Impacting women at all stages of life for a variety of reasons, hair loss and thinning may be temporary or permanent depending on the situation. In honor of National Hair Loss Awareness Month this August, here are the eight most common causes of hair loss in women and how to treat and, in some cases, prevent them. 

### **1. Genetics**
While the myth that your hair is determined by your mother’s father isn’t entirely true, it’s also not completely false. “You cannot look to just one relative to predict hair growth,” says [Benjamin Paul, MD](https://aedit.com/provider/benjamin-paul-md-392), a New York City-based double board certified facial plastic surgeon. “Though your mom’s dad (maternal grandfather) is an important player, he is not the only player impacting the growth and longevity of your hair.”

Even so, your hairline is very much influenced by family history. “If many of your relatives have shown a similar pattern of hair loss to your pattern of loss, you know who to thank for the genetics involved,” he adds.

While male pattern baldness is more common, women can experience female pattern hair loss (especially after menopause), which is hereditary. In both cases, the condition is known as androgenetic alopecia. The difference is how the baldness presents — men are more likely to experience receding hairlines and balding at the top of the head, whereas women typically see thinning around the crown. 

“Female pattern hair loss is best characterized using the Ludwig classification,” explains [Gary Linkov, MD](https://aedit.com/provider/gary-linkov-md-6976), a double board certified facial plastic surgeon in New York City. “The scale ranges from stages I to III. Stage I begins with thinning on the top of the head. In stage II the scalp starts to show. All of the hair at the crown of the head may be lost when the hair loss progresses to stage III.” Treatment options range from oral and topical medication (think: [Propecia](https://aedit.com/procedure/finasteride-propecia) and [Rogaine](https://aedit.com/procedure/minoxidil-rogaine)) to wigs and [hair transplants](https://aedit.com/procedure/hair-transplant), depending on the severity of the condition.

### **2. Hormones** 
Hormonal changes and imbalances can lead to a variety of conditions, hair loss among them. “The most common type of hair loss in women is female pattern hair loss, which is analogous to male pattern hair loss and involves testosterone,” Dr. Linkov explains. For men and women alike, the hormone dihydrotestosterone (DHT) is responsible for hair loss. “Testosterone gets converted to DHT, which is largely responsible for hair miniaturization,” he says. “The pattern of hair loss, however, is different between men and women.”

Androgenic alopecia is considered the most serious form of hair loss because it is usually permanent (due to the hair follicles being miniaturized). The severity of the impact can be mitigated, however, with treatment. Women who start to notice thinning along the scalp should consult with a board certified provider. 

Hormonal changes due to pregnancy, birth control, stress, and more can also lead to temporary hair loss, which we explore below.

### **3. Hair Styling** 
‘Beauty is pain’ has its limits. Unfortunately, tying your ponytail or bun too tight, heat styling, and using harsh chemicals can take its toll on your hair. Tight hairstyles, especially cornrows and braids, can actually cause traction alopecia, which can result in a thinning hairline and gradual hair loss. Taut ponytails and braids can have a similar effect, as chronicled in a season 17 *Keeping Up With The Kardashians* episode, in which Kourtney’s sky-high ponytail led to patchiness that was [treated with PRP injections](https://aedit.com/aedition/kourtney-kardashian-prp-injections-to-treat-hair-loss). While the occasional braid, bun, or pony shouldn’t cause much harm, regular styling could lead to more permanent hair loss. 

Similarly, excessive heat styling wreaks havoc on the hair. A side effect of thermal damage is brittle, weak hair shafts, which can result in breakage. Giving your hair a break from your hair straightener, curler, and dryer will benefit your strands’ long term health. If heat styling is a must, be sure to use a heat-protectant product to mitigate damage.

Hair treatments, like perms, color, and chemical straightening, can also [weaken the hair](https://aedit.com/aedition/tips-for-styling-fine-thinning-hair-from-celebrity-hairstylists) when performed regularly. They have the potential to dry out both the hair and scalp, which may lead to hair loss.

### **4. Pregnancy & Childbirth** 
Many women report enjoying luscious locks during pregnancy, but hormonal changes mean that hair is often shed in the weeks and months after giving birth. Postpartum hair loss is common for new moms (read our guide to managing it [HERE](https://aedit.com/aedition/managing-treating-postpartum-hair-loss-thinning)), but it is rarely long-lasting. Most women see total hair restoration within a year and find that shedding is at its worst around four months postpartum. As a result, many doctors do not recommend treatment. 

### **5. Diet**
Nutritional deficiencies are also another common contributor to hair loss among women. Meeting the required dietary allowances (RDA) for vitamins and minerals — particularly iron, zinc, vitamin B, and protein — are critical for hair and overall health. 

Unhealthy eating habits, eating disorders, or certain diets can all play a part in nutritional deficiencies. Those on a vegan diet, for example, may find it difficult to consume adequate protein and vitamin B. If you think you may have hair loss related to diet, work with a board certified doctor or nutritionist to ensure proper diagnosis and treatment. Vitamins and supplements may help provide balance.

### **6. Medication**
As we’ve all heard in the laundry list of side effects at the end of pharmaceutical commercials, many medications can cause hair shedding. According to Dr. Paul, birth control is a common culprit. “A bad reaction to medication may induce telogen effluvium or shock loss,” he shares. “There is an extensive list of potential medicines that may cause this and not all medicines on the list will cause hair loss.”

If you experience hair loss after starting a new medication, consult with your provider on other potential treatment options. “It is best to review all your medicines — current and new — with your doctor at each visit,” Dr. Paul says. “If you are on a medicine that may cause shedding, you can discuss changing medicines or treatment strategies with your doctor.”

### **7. Stress**
Stress, be it psychological (e.g. the loss of a loved one or a challenging work situation) or physical (e.g. weight loss, surgery, etc.), can cause hair loss and thinning ranging from acute to severe. This kind of hair loss is known as acute telogen effluvium. “The first step is to try and eliminate or lessen the stressor,” Dr. Paul says of treating the condition. While undoing the effects of stress takes time, most patients do see a return to normal growth. “The next step is to try and stop shedding and support regrowth,” he explains. “For many, the cycle of shedding, resting, and regrowth can take as long as a year. Many, but not all, see a near full return of the hair that was lost.”  

Dr. Linkov notes that recent studies in mice have shown signaling from the sympathetic nervous system, when subjected to stress, led to the depletion of a stem cell population in the hair follicle. “This discovery sheds light on why stress turns hair prematurely grey and contributes to hair loss,” he says. In the future, such research could improve treatment and prevention options.

### **8. Medical Conditions** 
There are a number of medical conditions and treatments (think: chemotherapy) that are known to cause hair loss. Autoimmune disorders like hyperthyroidism, lupus, rheumatoid arthritis, and sickle cell anemia, may cause shedding.

Alopecia is another example of an autoimmune disease that results in hair loss. In this case, the immune system misidentifies the hair follicles and attacks them. Depending on the person, hair loss from alopecia can be temporary or permanent.  

Scalp psoriasis, a variation of the common skin condition, causes red, scaly patches to form around the scalp. The condition itself doesn’t cause hair loss, associated itching and scratching can. Assuming it is controlled quickly, hair usually grows back. 

## **The Takeaway**
Many women experience hair loss at some point in their life, but, for many, the condition is temporary. Consulting with a board certified provider will help to get to the root of why the shedding or thinning is occurring, so proper treatment options can be recommended.
