Whether you call them cold sores or fever blisters, those who have experienced a herpes simplex virus (HSV-1) infection can feel their Spidey senses tingling before the effects even show. Oral herpes can be somewhat painful, though it’s mostly the connotation and aesthetic impact that leads many to cancel plans, feel shame, or turn to makeup as a means of covering up. 

According to the National Institutes of Health (NIH), about 90 percent of people have been [exposed](https://www.hopkinsmedicine.org/health/conditions-and-diseases/herpes-hsv1-and-hsv2/oral-herpes) to the HSV-1 virus by age 50, with 50 percent to 80 percent of adults in the United States believed to have oral herpes. All of this is to say that cold sores are quite common, but that doesn’t make them any less annoying to live with. Fortunately, they can be nipped in the bud if you strike the iron before it’s hot, and that’s what we’re exploring here today. From early treatment to medical care, here’s what the experts have to say about treating, preventing, and managing cold sores.

## **Understanding Cold Sores: Symptoms & Triggers**
Regardless of how you get the virus – be it through accidentally sharing utensils or a kiss – the small, fluid-filled clusters can happen to anyone at any age. More often than not, they are harmless. “It's important to remember that the majority of people have been exposed to herpes simplex, often in childhood,” says [Daniel Belkin, MD](https://www.nydermatologygroup.com/doctors/dr-daniel-belkin/), a board certified dermatologist at New York Dermatology Group in New York City. “So, a breakout may very well be a recurrence and unrelated to the most recent contact.” As he explains, HSV-1 is what typically infects the mouth area, while HSV-2 usually affects the genitals and is not as prevalent.

Cold sores can crop up for a variety of reasons. “Anything that impacts the immune response in the skin or creates injury in the skin can cause a breakout,” Dr. Belkin shares. While that sounds broad, those who’ve experienced a cold sore know how tricky it can be to understand the trigger. “[Sun exposure](https://aedit.com/aedition/cosmetic-procedures-treatments-to-correct-sun-damage) can reduce the immune response in the skin and cause a breakout,” he explains. “Illness can reduce the immune response in the skin and also cause a breakout, giving us the names ‘fever blister’ and ‘cold sore.’” An outbreak can also be from conditions that impair the skin barrier, such as [eczema](https://aedit.com/aedition/best-skincare-products-to-treat-eczema-flare-ups-in-the-winter) (more on that later). Lastly, Dr. Belkin notes that cosmetic facial treatments can play a role, too. “Especially around the mouth, [filler injections](https://aedit.com/procedure/dermal-filler-and-injectable-solutions), [lasers](https://aedit.com/aedition/guide-to-ablative-laser-skin-resurfacing), and [microneedling](https://aedit.com/procedure/microneedling) can cause breakouts,” he adds. 

The symptoms of cold sores evolve over their lifespan. Early on in the process, many feel “redness, pain, swelling, and blistering,” says [Amy Spizuoco, DO](https://aedit.com/provider/amy-spizuoco-19341), a board certified dermatologist and founder of True Dermatology in NYC. An HSV-1 outbreak can last anywhere from three days to a week with no indication of how big it will be. As a result, the earlier you notice the warning signs, the better. Our experts say that as soon as the swelling and tingling appear, the clock is ticking to address it.

## **How to Treat Cold Sores**
There is no cure for HSV-1, but there are some treatment options that can treat symptoms and mitigate flareups. They include: 

### **Prescription Medication**
“The best way to treat a cold sore is to have some oral medication on hand and take it as soon as you have the feeling that a cold sore is coming,” Dr. Belkin shares. “When done this way, all you need is a day of medication, and then you are done.” Dr. Spizuoco says oral acyclovir and valacyclovir are both effective. 

If you don’t have a prescription at home already, we know that squeezing in a trip to the doctor may be difficult – but it’s likely worth the effort. “Unfortunately, I don't think anything over-the-counter is very effective, and I would recommend seeing a physician to get a prescription of oral antivirals to have on hand if you are someone who gets frequent cold sores,” Dr. Belkin explains. While he notes that OTC Abreva (docosanol) “can’t hurt” and may “shorten healing time by half a day or so,” he says it is “not super effective.”

For those who can’t get ahead of the cold sore, Dr. Spizuoco has good news. “In office, the blisters can be lanced or injected with cortisone to speed up the resolution,” she says. 

### **At-Home Remedies**
Dr. Belkin explains that, when it comes to at-home cold sore solutions, “there aren't any.” Many try cold compresses to soothe pain, but you should proceed with caution. “Using irritants to try to clean or dry the area will just lead to further breakdown of the skin and prolong healing time,” he warns. “Once the virus is no longer active, keeping the eroded area moist with Vaseline or Aquaphor can help it to heal.”  

For at-home prevention, leading a healthy lifestyle may seem like a vague task, but [adequate sleep](https://aedit.com/aedition/impact-of-beauty-sleep-on-skin-health-nighttime-skincare-products), a [healthy diet](https://aedit.com/aedition/best-diet-for-healthy-beautiful-skin), and taking steps like wearing a [lip balm with sunscreen](https://aedit.com/aedition/best-lip-balms-with-spf) can help reduce the chances of a cold sore from developing in the first place. 

### **Avoid This**
Certain behaviors may be done with the best intentions but are blindly slowing the healing process, while others may be unknowingly triggering infection. Either way, here are doctors’ orders for what not to do: 

1. **Using Too Much Makeup**: “Cold sores are hard to cover up because they are textured, but I don't see a problem with dabbing on some foundation,” Dr. Belkin says. While a little can be great, layering too much (with [dirty brushes](https://aedit.com/aedition/how-to-clean-makeup-brushes-sponges)) can be detrimental. When applying, “wash your hands after, as cold sores are infectious and the virus can be spread to different parts of the face,” he cautions. To minimize its appearance or expedite the healing process, Dr. Spizuoco suggests [hydrocolloid patches](https://aedit.com/aedition/best-pimple-patches-to-treat-acne-breakouts-blemishes) to “conceal a cold sore.” Additionally, if lip gloss or lipstick is applied on the affected area, the product may be contaminated and lead to future breakouts.
2. **Eating That, Not This**: If you’re wondering if [diet](https://aedit.com/aedition/how-food-impacts-skin-aging) can impact the healing process, it absolutely can. Eat as cleanly as possible, and avoid spicy or acidic food and carbonated beverages. 
3. **Getting a Treatment**: This may seem self-evident, but it bears repeating: Aesthetic treatments, including [lip filler](https://aedit.com/procedure/temporary-lip-fillers), should be avoided until the skin is fully healed. “I would hold off on any of these if a patient had an active or healing cold sore,” Dr. Belkin advises. “Once the area is healed, we can proceed with treatment.” Even if you are not experiencing an active outbreak, share your medical history with your provider. “If a patient is prone to cold sores, I might administer some oral medication before the treatment for prophylactic purposes,” he adds.
4. **Sharing Utensils**: Sharing items that come in contact with a cold sore can lead to the other person getting the virus. For those concerned about when to share again, “cold sores are thought to be no longer infectious once the blistered or eroded skin is dried and crusted,” Dr. Belkin explains. But, to be safe, he recommends “waiting until the skin is re-epithelialized or healed over.” At that point, “the area may still be pink, but, if it feels like normal skin, it is fine to start sharing again,” he says.

## **How to Prevent Cold Sores**  
For many, prevention can mean leading a healthy lifestyle, but it's not a guarantee. As mentioned, there is no rhyme or reason as to what can cause an outbreak, but those who have the HSV-1 virus in their system will benefit from [regular exercise](https://aedit.com/aedition/best-fitness-classes-workouts-exercises-to-try-at-home), getting enough sleep, eating well, and taking care of themselves. 

For those with a weakened immune system or [eczema](https://aedit.com/aedition/comparing-eczema-vs-psoriasis), a diagnosis and tailored treatment will be beneficial for prevention, as this population may experience worsened symptoms. “The first breakout of cold sores is typically more severe than a recurrence,” Dr. Belkin explains. “If you think you may be having the first episode due to recent exposure, it would be a good idea to see a doctor to be evaluated and to be prescribed oral medication.” He emphasizes that “the sooner you start, the quicker the breakout will fade.” This, he says, minimizes “both embarrassment and the risk of [scarring](https://aedit.com/aedition/how-to-treat-every-type-of-scar).”

Anyone who has experienced [acne](https://aedit.com/concern/acne) knows that not all pimples are created equal, and it can be difficult to know exactly what subtype of the condition you are dealing with. For instance, is your acne bacterial or fungal in nature? Since [treatment protocols](https://aedit.com/procedure/acne-solutions) differ by type, we’re breaking down everything you need to know about the difference between the two.

## **What Is Fungal Acne?**
More often than not, conversations about acne are focused on [acne vulgaris](https://aedit.com/aedition/how-to-treat-hormonal-acne-body-acne-cystic-acne-whiteheads-blackheads), which involves the blockage and/or inflammation of the hair follicles and sebaceous glands. “Traditional acne is often associated with cutibacterium acnes and is often related to other factors as well including increased oil production, clogged pores, and inflammation,” explains [Marisa Garshick, MD](https://aedit.com/provider/marisa-garshick-md-313), a board certified dermatologist in New York City. “In some cases, it can also result from [hormonal changes](https://aedit.com/aedition/skincare-during-menstrual-cycle).”

But not all acne patients are diagnosed with acne vulgaris. Fungal acne is another subtype. Also known as pityrosporum folliculitis or malassezia folliculitis, fungal acne specifically impacts the hair follicle. “Pityrosporum folliculitis is due to the overgrowth of a normal yeast that lives on the skin, known as malassezia, that can lead to inflammation of the hair follicle,” Dr. Garshick says. 

It should be noted that the yeast responsible for fungal acne is always present on your skin, says [Snehal Amin, MD](https://aedit.com/provider/snehal-amin-md-308), a board certified dermatologist in NYC. Under normal circumstances, the body is usually able to balance the yeast, but overgrowth can occur leading to the acne-like symptoms. “These breakouts are often triggered by low immunity, [high sugar levels](https://aedit.com/aedition/relationship-between-food-and-acne-breakouts), or recent antibiotic treatment,” he explains. “Long-term steroid use can also cause fungal acne.” Warm, humid environments and trapped moisture can both disrupt the balance of bacteria and fungi on the skin.

## **How to Diagnose Fungal Acne** 
Acne vulgaris presents as non-inflammatory lesions (comedonal whiteheads and blackheads), inflammatory lesions (papules, pustules, cysts, and nodules), or a mixture of both. In the majority of cases, it is most commonly found on the face, back, and chest. While it’s easy to confuse the two, there are some key differences to keep an eye out for when it comes to identifying fungal acne:

- **Type of Breakout**: Fungal acne is generally characterized by small, pus-filled whiteheads or pimples that present in clusters on the skin. 
- **Size of Bumps**: Unlike bacterial breakouts, the bumps tend to be similarly sized.
- **Areas Affected**: Fungal acne is most commonly found on the back, arms, and chest, though it may also present on the face.
- **How It Feels**: While bacterial acne can be painful, fungal acne is often itchy. 

Dermatologists diagnose fungal acne by studying the area(s) of concern. “While the diagnosis may be made clinically during a routine dermatology examination, sometimes a scraping, biopsy, or culture may be taken to confirm the diagnosis,” Dr. Garshick says. Fungal acne does not usually respond to [traditional acne treatments](https://aedit.com/aedition/how-to-treat-prevent-body-acne), so sharing a list of products or remedies you have previously tried to treat the bumps with may help your doctor hone in on the cause. 

## **How to Treat Fungal Acne**
While your typical acne-fighting arsenal may not be as effective in clearing up fungal breakouts, there are still a wide-range of at-home and in-office solutions available. Below, Dr. Amin and Dr. Garshick share their go-to fungal acne treatments — and even how to deal with a last-minute pimple emergency!

### **1. Topical Skincare**
In place of the [retinoids](https://aedit.com/aedition/guide-to-vitamin-a-retinol-retinoids-in-skincare), [salicylic acid](https://aedit.com/aedition/guide-to-beta-hydroxy-acids-salicylic-acid-in-skincare), and [benzoyl peroxide](https://aedit.com/aedition/guide-to-benzoyl-peroxide-in-skincare) used to treat bacterial breakouts, skincare routines for fungal acne usually involve antifungal ingredients such as ketoconazole or selenium sulfide. ### **3. Cortisone Injections**
“There is no overnight cure for acne,” Dr. Amin laments. But that’s not to say your dermatologist can’t help you out in a pinch. In the same token, though, I don’t think people should feel the need to hide away simply because of clogged pores. That’s all it is. There shouldn’t be as much stigma about it, and that’s the message that I think is important to share.

One route to avoid in the quest to restore your mane? “Blindly taking supplements or self-diagnosing by Google to try to treat yourself,” cautions [Jennifer MacGregor, MD](https://www.instagram.com/nyclaserdoc/), a board certified dermatologist at [UnionDerm](https://www.unionderm.com/) in New York City. Not only could it not help your thinning hair, but it could actually hurt your cause, she adds.

So, if your daily hair shedding goes beyond the average 50 to 100 strands, it may be time to consult with an expert. If hair doesn’t respond to the OTC Nizoral shampoo, a more intense Nizoral formulation with 2 percent zinc pyrithione can be prescribed along with a selenium sulfide solution, but Dr. MacGregor warns that these options are *not* color-safe. For significant scalp inflammation, Dr. MacGregor adds pulsed topical steroid solutions into the mix, which is “a stronger topical steroid application” that is limited to once every one to two weeks. “It’s rubbed into the scalp, then you’ll sleep overnight with a shower cap, before washing it out in the morning with one of the medicated shampoos, leaving it on for five minutes before rinsing,” she explains.

### **3. Platelet-Rich Plasma (PRP) Therapy**
A favorite of Dr. Engelman, [platelet-rich plasma (PRP) treatments](https://aedit.com/procedure/plateletrich-plasma-prp-for-hair-loss) essentially use your own blood to stimulate hair growth. “PRP therapy uses injections of a concentration of a patient’s own blood platelets to restore and accelerate hair growth,” she explains. First, your doctor will draw your blood, then it’s “placed in a centrifuge to separate out the platelets, before injecting the serum back into the hair follicle,” she shares. “The plasma contains white blood cells and platelets, which are rich in growth factors and stem cells.”

By harnessing the effects of laser and light, it works to “reactivate your hair’s natural growth pattern by promoting blood circulation and collagen production,” Dr. Engelman shares. This in-office procedure works in three stages: stopping hair loss, thickening of current hair, and regrowth of new hair with a stronger, fuller hair follicle that replaces the thin, original follicle. The result? “Increased growth and thickness,” she says.

## **The Takeaway**
There is a good chance you may experience hair thinning at some point in your life. When healthy lifestyle changes and OTC remedies don’t work, it’s time to consult with a board certified dermatologist. For some, it is hard to tackle no matter what lifestyle or skincare changes are made. While many experience an accelerated amount of pimples during their hormone-induced teenage years, others face a more aggressive form of acne that can result in physical and emotional scarring.

Accutane) – can be the saving grace. However, it comes with strict user guidelines, potential risks, and side effects, not to mention a consent form. Here’s what you need to know about Accutane from both a medical professional and a patient and why it’s not as scary as the online rhetoric may have you believe. 

## **What Is Accutane?**
Technically, [Accutane](https://aedit.com/procedure/accutane) is a brand name for isotretinoin, a potent form of vitamin A that was [approved](https://www.accessdata.fda.gov/drugsatfda_docs/label/2008/018662s059lbl.pdf) by the United States Food and Drug Administration (FDA) in 1982 for the treatment of severe, resistant, nodular acne that is unresponsive to conventional therapy. Off-label (read: non-FDA approved) uses of isotretinoin include moderate acne, [rosacea](https://aedit.com/concern/rosacea), and folliculitis.

For those unfamiliar with the ingredient, vitamin A derivatives are a dermatologist-favorite for their ability to address a myriad of skin concerns — ranging from wrinkles to acne. Unlike its topical counterparts (including [retinols and retinoids](https://aedit.com/aedition/guide-to-vitamin-a-retinol-retinoids-in-skincare)) that can be used regularly for prolonged periods of time, isotretinoin is an orally-prescribed medication that is used on a short-term basis and has stronger side effects (we’ll get into them later) than the so-called retinization process of adjusting to topicals. Then again, it is meant to be a treatment option for those who haven’t found a solution for their acne concerns through other lifestyle modifications or skincare-altering means.  

## **How Accutane Works**
So, how does Accutane work? “It treats acne by reducing the size of oil-producing glands and oil production, which, in turn, may lead to decreased acne-causing bacteria,” explains [Michelle E. Park, MD](http://www.washingtonsqderm.com/about/#park), a board certified dermatologist at [Washington Square Dermatology](http://www.washingtonsqderm.com/) in New York City. “It also inhibits keratinization and reduces inflammation.”

Because it works from the inside out, it can address the pores, cell turnover cycle, and oil production in a way topicals alone cannot. However, due to its exfoliating nature, other forms of exfoliation will have to be stripped from the skincare routine while isotretinoin is prescribed. And that’s not the only adjustment your skincare regimen will need. Similar to topical vitamin A, Accutane may cause sensitivity to sunlight and will require adequate [sun protection](https://aedit.com/aedition/best-sunscreens-of-2021). 

## **Side Effects of Accutane** 
“The main concern everyone has regarding Accutane are the adverse effects,” Dr. Park confirms. “Every systemic medication comes with risks, and this medication can certainly be high-risk if it is used incorrectly or without proper guidance.” If you’ve ever talked to someone on Accutane or someone who knows someone who has been on Accutane, you are likely familiar with some of the side effects. “In reality, every person on Accutane will get dryness of the skin, particularly on the lips,” she says. “Muscle aches or joint pains are also possible.” From there, Dr. Park says all of the other known side effects “are rare.” 

It’s important to note that, alongside the physical side effects of dry skin, muscle aches, and upset stomach (if the instructions aren’t followed), Dr. Park acknowledges being on isotretinoin can have an impact on mental health. Some patients recount thoughts of suicide and depression while taking the medication. But, while there is a risk, Dr. Park says it is not common. “The black box warning is there as a reminder that one may experience this while on Accutane, but it is not common,” she says. “The overwhelming majority of my patients do great while on this medication.” 

## **The Right Candidate for Accutane**
“People with severe nodulocystic acne should take Accutane,” Dr. Park explains. For patients who have tried other remedies without success, isotretinoin may prove to be the solution. “Anybody who has exhausted the use of topical prescription acne medications and failed antibiotics or anybody who experiences [significant scarring](https://aedit.com/aedition/how-to-treat-acne-scars) due to uncontrolled acne may take Accutane,” she adds. 

To find out if you’re eligible for the prescription, you’ll need to visit a dermatologist and undergo a thorough check. Those who are using retinoids are advised to stop prior to beginning Accutane. Additionally, it will not be prescribed to those who are pregnant, are planning to become pregnant, or have issues with their liver.

## **How to Use Accutane**
As you may have guessed, going on Accutane requires adjusting to a scrupulous regimen. “This medication is mandated to come with strict guidance and monitoring to be prescribed,” Dr. Park cautions. “It is administered with close monitoring by a dermatologist, with safeguards to prevent pregnancy, and clear communication of medication risks.” When these protocols are followed,  she says it is “highly effective” and “relatively safe.” 

Here’s what to expect from Accutane:

1. **Sign a Consent Form**: Yes, really. Before a treatment plan can begin, a consent form is required. At Washington Square Dermatology, Dr. Park explains that “everyone must register on [iPledge](https://www.ipledgeprogram.com/iPledgeUI/home.u) to demonstrate that they understand this medication is extremely dangerous to take while pregnant and they cannot become pregnant while taking the medication.” Alongside the consent form, women are required to take two pregnancy tests (spaced 30 days apart) before starting the medication. 
2. **Skip the Drink**: Since isotretinoin can impact liver function, being on the medication will require abstinence from alcohol (which may not be such a bad thing for [overall skin health](https://aedit.com/aedition/the-impact-of-candy-sugar-sweets-on-teeth-and-skin)). “Abstain from alcohol or any other medications and ingested substances that can cause liver damage while on this medication,” Dr. Park says. 
3. **Adjust Your Skincare**: Because the medication works to accelerate cell renewal and to help pores decrease oil production, Dr. Park advises skipping exfoliants such as [alpha hydroxy](https://aedit.com/aedition/guide-to-topical-alpha-hydroxy-acids-in-skincare) and [beta hydroxy acids](https://aedit.com/aedition/guide-to-beta-hydroxy-acids-salicylic-acid-in-skincare), scrubs, and other forms of topical vitamin A. ## **What Causes Pimples?**

If you’ve ever looked in the mirror to discover tiny, whitehead-esque bumps under your eyes, then you’ve probably had milia. But, while these yellow and white dots may look like [acne](https://aedit.com/concern/acne), they are actually small cysts that require different care and attention than the pustules, papules, and, yes, even whiteheads you may also find on your face and body. Here’s what you need to know about milia. 

## **What Are Milia?**
Milia (the plural of milium) are “small, firm whitish cysts that occur primarily on the face,” says [Samer Jaber, MD](https://aedit.com/provider/samer-jaber-md-3882), a board certified dermatologist and founder of Washington Square Dermatology in New York City. They are most common in newborns (some 50 percent of infants develop milia, he notes), but they can affect anyone, regardless of age or ethnicity.

These cysts are usually found in groups and are especially common around the eyes — though they can also be found on the lips, cheeks, and even the torso and genitals. Generally speaking, milia are not itchy or painful and don’t necessarily need to be treated. Depending on where they are on the body, however, they can become red and irritated due to friction.

There are two types of milia: primary and secondary. Primary milia occur spontaneously — most often on the eyelids, forehead, cheeks, or genitals — and usually clear up on their own. They pop up when “keratin or dead skin cells get trapped under the skin, essentially clogging it,” explains [Lily Talakoub, MD](https://www.mcleanskin.com/lily-talakoub-md/), a board certified dermatologist in McLean, VA. 

Secondary milia, meanwhile, are the result of skin damage (think: injury, irritation, rash, burn, abrasion, etc.) and may be permanent. For example, it is possible to experience milia after [skin resurfacing procedures](https://aedit.com/procedure/skin-resurfacing-solutions), such as [dermabrasion](https://aedit.com/procedure/dermabrasion) and [laser treatments](https://aedit.com/aedition/guide-to-ablative-laser-skin-resurfacing). In these cases, the milia occur in the location where the skin is compromised. Milia can also develop if the skin loses its natural ability to exfoliate, which is common of the aging process. 

## **How to Treat Milia** 
As we mentioned, milia doesn’t do any harm and, in time, can often resolve on their own. With that said, they may pose an aesthetic concern — especially on the face. Due to their resemblance to whiteheads, it can be tempting to pick at or even try to pop milia, but dermatologists advise you to avoid this at all costs or else you risk damaging your skin.

While we don’t recommend [DIY extractions](https://aedit.com/aedition/dangers-of-diy-at-home-dermatology-beauty-treatments) of any kind, removing a milium is nothing like popping a pimple. Unlike a pustule, a milium does not feature a soft core of dead skin cells, sebum, and pus that flow out of a pore. Instead, it’s composed of hardened dead skin cells that have become trapped just below the surface of the skin. As such, squeezing a milium is not going to get you very far.

To avoid scarring and infection, you should visit a board certified dermatologist to diagnose and remove the milium. The extraction process involves “sterilizing the area, puncturing it with a sterile needle first, and then applying pressure to the area,” Dr. Jaber explains.

## **How to Prevent Milia** 
The best way to address milia at home and to potentially prevent them from developing in the first place is to use chemical exfoliants. ### **1. Use SPF 30 (or Higher!)**
If you take one thing from this article, let it be this: The American Academy of Dermatology (AAD) [recommends](https://www.aad.org/public/everyday-care/sun-protection/sunscreen-patients/sunscreen-faqs) using sunscreens that have a sun protection factor (SPF) of 30 or higher. It should be broad spectrum, which means it protects against both UVA and UVB rays. Side effects of hormone therapy often show up on the skin in the form of [acne](https://aedit.com/concern/acne), [uneven texture](https://aedit.com/concern/uneven-skin-texture), [hyperpigmentation](https://aedit.com/concern/uneven-skin-tone), and [unwanted hair](https://aedit.com/concern/unwanted-hair) (to name a few). To better understand the skin concerns and treatment options available, The AEDITION asks the experts. 

## **How Hormone Therapy Affects the Skin** 
Hormone therapy is often one of the first medical treatments that both [male to female (MTF)](https://aedit.com/concern/mtf-gender-confirmation) and [female to male (FTM)](https://aedit.com/concern/ftm-gender-confirmation) patients go through. There are different groups of hormones used, and each of them has specific effects on the body that can also impact the patient’s skin. 

### **Male to Female Hormone Therapy**
Transgender women are often prescribed estrogen, which helps feminize features and slows the growth of facial and body hair. “In those transitioning from male to female, estrogen or anti-androgens, like spironolactone, are often used,” explains [Samer Jaber, MD](https://aedit.com/provider/samer-jaber-md-3882), a board certified dermatologist and founder of Washington Square Dermatology in New York City. “In the skin, estrogen decreases sebum production, decreases body and facial hair, and can change or darken moles.” 

Reduced sebum production can make [skin feel drier](https://aedit.com/aedition/how-to-treat-prevent-dry-skin-face-body-hands), which may necessitate a more hydrating skincare routine, but patients may also seek treatment for another side effect of estrogen: increased pigmentation. “[Melasma](https://aedit.com/concern/melasma) or [hyperpigmentation](https://aedit.com/aedition/what-its-like-to-treat-hyperpigmentation-with-skincare-cosmetic-treatments) on sun-exposed areas of the face can worsen in patients on estrogen therapy,” Dr. Jaber notes.

### **Female to Male Hormone Therapy**
On the other hand, transgender men are generally prescribed testosterone. This hormone is responsible for bodily changes ranging from the thickening of vocal cords to redistribution of [body fat](https://aedit.com/concern/excess-fat). It affects the skin by increasing sebum production and can also make skin feel a little thicker to the touch. “Transgender men who take androgen treatment see increased oiliness to the skin and increased facial and body hair,” explains [Michael Somenek, MD](https://aedit.com/provider/michael-somenek-md-30787), a double board certified facial plastic and reconstructive surgeon in Washington D.C. and member of the [AEDIT Adviosry Board](https://aedit.com/beauty-board). 

Needless to say, more sebum (read: oil) means more of a chance for breakouts. “The increased oil production tends to produce [acne](https://aedit.com/aedition/how-to-treat-hormonal-acne-body-acne-cystic-acne-whiteheads-blackheads) and can be a significant undesired effect of testosterone therapy,” he shares, adding that it “usually peaks by six months.”

## **How to Adapt Your Skincare During Hormone Therapy** 
Based on what we outlined above, it should come as no surprise that transgender women will generally want to curate a skincare routine that focuses on moisturizing the skin and reducing any hyperpigmentation that arises as a result of hormone therapy. During the healing process, it’s important to keep the incision site out of the sun to prevent discoloration. In such cases, Dr. Kaskas suggests the so-called “sandwich” method. “Your nighttime moisturizer can be used to help make medicated topicals more tolerable or less drying and irritating,” she explains. This is often recommended to patients who are acclimating to a retinoid routine. “For example, the ‘sandwich’ method can be used when adjusting to topical retinoids,” she says. “A layer of moisturizer can precede and follow the retinoid to help make it less irritating.”
