It’s a known fact that women are more inclined to dabble in aesthetic procedures than men. But thanks to ongoing education and a push in the right direction from savvy dermatologists and plastic surgeons, the male population is partaking in more preventative and rejuvenating skincare, treatments, and procedures than ever before. In fact, according to [data](https://www.plasticsurgery.org/documents/News/Statistics/2020/cosmetic-procedures-men-2020.pdf) released by the American Society of Plastic Surgeons (ASPS), minimally invasive cosmetic treatments in men totaled a whopping 11.2 million procedures in 2020. Of course, botulinum toxin (read: [Botox® and the like](https://aedit.com/aedition/the-rise-of-brotox-injections)) claims the number one spot, but could a somewhat more invasive non-surgical treatment, like the thread lift, call dibs on it soon?
 
The idea of [‘tweakments’ for men](https://aedit.com/aedition/best-non-surgical-aesthetic-procedures-for-men) has been growing for quite some time, says [Ava Shamban, MD](https://aedit.com/provider/ava-shamban-md-2325), a board certified dermatologist in Santa Monica, CA. “We are most definitely well past just ‘Brotox,’” she notes. “Men are now coming in droves for treatments and ongoing maintenance.” Procedures like [fillers and injectables](https://aedit.com/aedition/guide-to-injectable-filler-botox-for-men) have paved the way for thread lifts, which are thought of as the next ‘step up’ for lifting, contouring, and defining an aging face.
 
## **What Is a Thread Lift?**
With age, collagen and elastin levels dip, and the common signs of aging (think: fine lines, wrinkles, and sagging) become evident. Coupled with collagen degradation and volume loss, which occurs when fat begins to shift, the face can start to fall a little flat as its youthful curvatures disappear. Those changes are precisely what a [thread lift](https://aedit.com/procedure/thread-lift) targets.

Before the availability of threads, the primary tool to address [volume loss](https://aedit.com/aedition/best-procedures-to-restore-facial-volume) and [skin laxity](https://aedit.com/concern/facial-skin-laxity) was a [surgical facelift](https://aedit.com/aedition/facelifts-for-men) or the placement of a permanent suture that required more invasive techniques, says [Julius Few, MD](https://aedit.com/provider/julius-few-md-15580), a board certified plastic and reconstructive surgeon in Chicago. “Fillers can be used to restore volume but are not designed to reposition tissue, creating a void of non-invasive options to address sagging facial skin,” he explains. That’s where the thread lift comes in.
 
Today’s thread lift procedures are safer and more advanced than their predecessors. The current versions involve absorbable [PDO threads](https://aedit.com/procedure/pdo-thread-lift) bearing tiny barbs that suspend the tissue. They are inserted under the skin and then pulled and locked into place to create a tighter, lifted, more defined look. There are also threads made from poly-L-lactic acid (PLLA) and polycaprolactone (PCA). Your doctor will decide which type to use based on what the skin needs. One of the many benefits of a thread lift is the creation of new collagen. As the thread material dissolves, it prompts the body’s natural collagen production processes. “This helps prolong the benefits of the lift,” Dr. Halaas shares. While the threads last three to six months, the collagen that remains lasts indefinitely (or until it breaks down on its own). 
 
Thread lifts are a minimally invasive solution that come with no incisions or scars and hardly any downtime compared to other procedures that target similar concerns, like a facelift. The catch? The results aren't as dramatic or lasting (especially when comparted to a facelift). Even so, many doctors feel a thread lift offers a [subtle yet substantial change](https://aedit.com/aedition/pdo-thread-lift-for-facial-rejuvenation).
 
## **Who’s Getting Thread Lifts**
The average male thread lift patient is, according to Dr. Shamban, noticing skin laxity with deepening smile and marionette lines that are sometimes accompanied by neck sagging. “They are trying a range of toxins, dermal fillers, and [tissue-tightening devices](https://aedit.com/aedition/best-nonsurgical-skin-tightening-procedures),” she says, but they often need more than what non-invasive options offer and aren’t quite ready for facial surgery. “Men who are doing thread lifts are concerned with either the midface and the neck and jowl or the neck area, particularly if they have a less prominent or smaller jaw structure,” she notes. Overall, they want a sharper, smoother profile and more definition in the jawline or midface.
 
Not everyone is a candidate for a thread lift. Because the results are short-lived on more mature skin, Dr. Few suggests them for younger guys in their [mid-thirties](https://aedit.com/aedition/best-cosmetic-procedures-plastic-surgery-for-men-in-30s) and up into the [early fifties](https://aedit.com/aedition/best-cosmetic-procedures-plastic-surgery-for-men-in-50s-60s-70s). “Most patients over the age of about 55 will benefit more from facelift surgery,” he says. The exception? “Thread lifts can, however, be a facelift-alternative for older patients who are unable to have surgery for medical reasons, but the results will not be as dramatic as seen in the younger patient,” he adds. Additionally, because thread lifts are performed under [local anesthesia](https://aedit.com/aedition/everything-you-need-to-know-about-anesthesia), those with conditions that make them ineligible for surgery (like high blood pressure, type two diabetes, and cardiovascular disease) can more safely consider a thread lift. Just be sure to keep in mind that the procedure will not remove loose skin or treat significant sagging.
 
While many doctors are noticing a surge in men partaking in thread lifts, some are seeing the pendulum swing in the opposite direction. [Paul Nassif, MD](https://www.instagram.com/drpaulnassif/), a board certified plastic and reconstructive surgeon in Beverly Hills, CA, isn’t seeing as many male thread lift patients as he once did. “It was more popular in my practice pre-COVID,” he says. Nevertheless, the fellas aren't bypassing the plastic surgeon's office. Instead, Dr. Nassif notices more men coming in to treat [tech neck](https://aedit.com/aedition/how-to-treat-prevent-tech-neck). “They’re seeing extra skin on the neck, and the skin is heavier and thicker there, so it needs more aggressive work than what a thread lift offers,” he explains. “Most of the time, they opt for a [neck lift](https://aedit.com/aedition/pairing-facelift-neck-lift-procedures).”
 
## **Filler vs. Threads: Which One Is Better?**
A common misconception about fillers is that they lift. In actuality, they fill and plump. When attempting to use fillers for lifting purposes, they often end up causing puffiness and distort the face, Dr. Halaas explains. “I always educate my patients that facial rejuvenation is a cohesive approach to volume, skin laxity, and skin texture,” she says. 
 
Many doctors devise their own combinations of non-surgical solutions to create a natural-looking lift. For example, Dr. Halaas uses a two-stage approach of filler and threads to give men the maximum results possible for a lift through the midface. Similarly, Dr. Few invented what he calls the Inverso Suspension technique, which incorporates thread lifting, volume restoration of the jawline, and microdoses of Botox® (or [Xeomin®](https://aedit.com/procedure/xeomin)) in the frown muscles to create “a graceful appearance that is not ‘done’ or unnatural.” 
  
## **The Pros & Cons of Thread Lifts**
Every procedure — surgical or non-surgical — has advantages and disadvantages, and the thread lift is not without the good, the bad, and, in some cases, the ugly (more on that in a minute). These are the points of contention and praise surrounding the 30-minute procedure:
 
### **THE PROS**
- **It’s a relatively quick procedure.** From start to finish, a thread lift takes about 30 minutes. It is performed in the office under [local anesthesia](https://aedit.com/aedition/awake-surgeries-without-general-anesthesia). If additional procedures, like neuromodulators, are performed in tandem, expect to block out a little more time.
- **There’s minimal downtime.** You won’t be recovering for weeks as you do with a [facelift](https://aedit.com/aedition/facelift-recovery-healing-process). Sure, there is minor swelling and bruising during the first week after a thread lift, but that soon subsides. 
- **It’s not nearly as invasive as surgery.** While the results of a thread lift are not a mirror image of what surgery provides, that's the tradeoff for less invasivness. There is no cutting, dissection, or skin removal, making the procedure much less intrusive (it’s often categorized as ‘minimally invasive’). However, expect some minor discomfort since injecting anesthesia is necessary to place the threads properly.
- **There’s no scarring.** Because there is no cutting of the skin nor any incisions made, there are no resulting [scars](https://aedit.com/aedition/scar-minimizing-skincare-to-use-after-surgery). In addition, as Dr. Shamban explains, a thread lift does not change or move the hair-bearing regions (the beard and sideburns), which may change with a full or lower facelift.

### **THE CONS**
- **Loose skin is not addressed.** Even though the procedure can lift minimally saggy skin, it will not remove it. Therefore, the skin that remains before the lift is still intact afterward.
- **It doesn’t last that long.** On average, the lifted and improved profile lasts about nine months to two years, with the initial dissolving of the threads around the three- to four-month mark. Some patients notice the lifting effects begin to wear off after just a few months.  
- **There is the risk of puckering and pulling.** Post-treatment bunching is to be expected, which is nothing more than the sutures fully settling into place. But when the skin stays in a puckered state for weeks or even months, it’s a sign of threads gone bad. Threads (like fillers and other treatments) can cause serious issues and pain for patients when done incorrectly. “Depending on the type of threads being used, they can be nearly impossible to remove and cause permanent tethering or scar tissue,” Dr. Halaas cautions.
- **Additional procedures may be needed.** A thread lift won't stop muscle activity or the formation of lines and wrinkles , which are significant factors in aging. When hanging muscles are present in the neck, Dr. Nassif suggests Botox® to help soften it. “Non-invasive procedures, like [radiofrequency treatments](https://aedit.com/procedure/necktite) that deliver on top of the skin, can help tighten up the neck,” he adds. ## **Is a Thread Lift Right For You?** 
Thread lifts can make a world of difference in the right patient. In the wrong one, they won’t do much for the face and may even do more harm than good. While thread lifts sometimes get a bad rap because of their potential to crease and wear off quickly, they help create new collagen in the skin — something that is beneficial to anyone in their thirties and beyond.
 
If you notice sagging in the neck and a lack of definition in the jawline, you may want to consider a thread lift assuming:

- **It fits in your budget.** Generally, thread lifts are less expensive than surgery. But, since most providers charge per thread (it can start at $400 per thread and go up from there), it can get pretty costly to treat larger areas.
- **Downtime associated with surgery is not an option.** You may be a little sore or swollen for the first few days or week after a thread lift, but you won’t have to take time off work to recover as you do with a facelift. Remember, a thread lift does not remove excess fat or skin, so there needs to be small amounts of sagging to be effective.
- **You’ll be happy with an understated result.** Compared to a facelift, a thread lift result is less dramatic and won’t last as long. That’s because there is no skin excision or muscle, fat, or tissue repositioning. Dr. Nassif finds that his patients tend to be initially pleased with a thread lift, “and then, once the results start to go back, they’re not so happy with it.”
- **Your goal is to avoid surgery.** As with all non-invasive and minimally invasive procedures, a desirable outcome that eliminates the need for surgery is a win-win for the patient. Dr. Halaas adds that the convenience of threads, the quick recovery, and the result make it very appealing.

The late 2000s and early 2010s saw a decline in men wearing makeup (at least in an overt way), but social media, vlogs, and the rise of men as beauty and grooming gurus have ushered in a resurgence that’s hard to ignore. “I think Gen Z and social media influencers have made it more acceptable,” says [Charlie Riddle](https://www.instagram.com/charlieriddle/?hl=en), a celebrity makeup artist and global beauty director of [Stila](https://go.skimresources.com?id=170670X1638612&xs=1&url=https%3A%2F%2Fwww.stilacosmetics.com%2F). “Men are seeing themselves back for the first time and seeing more tutorials or applications on Instagram and TikTok, and it’s become more normalized.”

In fact, the United States has some catching up to do. “In Korea, they have been using male models to promote [BB creams](https://aedit.com/aedition/semi-permanent-foundation-bb-glow-safety) and makeup for quite some time, and we are starting to see that in beauty campaigns for U.S. brands now as well,” he adds. ## **What Causes Hyperpigmentation?** 
As it relates to hyperpigmentation, one thing that differs between men and women is the cause. Generally speaking, dark spots crop up as a result of sun damage, certain types of hormone imbalances, and post-inflammatory pigmentation (PIH). “Men get a lot of hyperpigmentation from their beard as a result of shaving and ingrown hairs,” says [Paul Jarrod Frank, MD](https://pfrankmd.com/), a board certified celebrity dermatologist in New York City. “This is the type of post-inflammatory pigmentation that is very significant and common in men, probably the most common.” 
 
In the case of ingrowns, there are two issues to address. “We not only have to treat the problem with the ingrown hair but the side effect of the pigmentation,” Dr. Frank adds. The most effective treatment plan usually involves a combination of in-office treatments and at-home topicals. No matter the cause, this multi-modality approach is often the key to success. 
 
While there are plenty of products in the skincare aisle that promise to brighten the skin, hyperpigmentation can be difficult to treat purely with at-home and over-the-counter remedies, says [Blair Murphy-Rose, MD](https://www.laserskinsurgery.com/meet-the-doctors/dr-blair-murphy-rose/), a board certified dermatologist in NYC. This is especially true for patients with stubborn and extensive cases. Professional treatments, meanwhile, pack a more serious punch, but even those tend to need reinforcement. “Hyperpigmentation can be an ongoing condition that may not be resolved with one treatment and at-home care is often necessary as well,” she adds.  

## **How Does Hyperpigmentation Differ for Men?** 
“Hyperpigmentation is fundamentally the same in both men and women,” Dr. Murphy-Rose shares. The exception is [melasma](https://aedit.com/concern/melasma), a common hyperpigmentation disorder that is believed to be caused by some combination of sun exposure, hormonal changes, and genetic predisposition. It is seen more frequently in women (especially after pregnancy), but, even so, it can affect men as well.

Rather than be differentiated by sex, hyperpigmentation is moreso differentiated by where it exists in the skin. “The biology of the skin is not that much different,” Dr. Frank says of the comparison between men and women. “It's less of a gender issue and more of a [skin type](https://aedit.com/aedition/guide-to-fitzpatrick-skin-types) issue.” For example, darker skin tones are more likely to get deeper, darker pigmentation, he notes. As such, patients with melanin-rich skin tend to have more of an issue with hyperpigmentation. 
 
Another factor to consider is the layer of the skin the pigment is in. As Dr. Frank explains, most [sunspots](https://aedit.com/aedition/whats-the-difference-between-freckles-and-sun-spots) are “very superficial,” as the pigment is located at the base of the epidermis. Age spots and even melasma, meanwhile, may sit deeper in the dermis and require different treatment.
 
## **In-Office Hyperpigmentation Treatments**
We’ll give it to you straight: Treating hyperpigmentation takes patience, but there are a host of in-office treatments that, over time, can even out the tone and texture of the skin. “Which is best for you will depend on the type of hyperpigmentation you have, the depth of that pigment in the skin, as well as your skin tone,” Dr. Murphy-Rose explains. “Often, a combination of multiple modalities may be most effective.” Below, a few of the most effective:
 
### **1. Chemical Peels**
[Chemical peels](https://aedit.com/aedition/guide-to-superficial-medium-deep-chemical-peels) exfoliate outer skin layers, helping to lighten darkened areas. They come in different potencies — [superficial](https://aedit.com/procedure/superficial-chemical-peels), [medium](https://aedit.com/procedure/medium-depth-chemical-peels), and [deep](https://aedit.com/procedure/deep-chemical-peels) — based on the concentration of the solution (usually a combination of acids). In any case, the solution is applied to clean skin and denatures the outer layers of skin. As a result, the skin peels or sheds off, Dr. Murphy-Rose says. 

The depth of the peel will determine how much peeling there is. A light [glycolic acid peel](https://aedit.com/procedure/glycolic-peel) or [salicylic acid peel](https://aedit.com/procedure/salicylic-acid-peel), for example, may only have a bit of mild flaking for a few days, while a deep [phenol peel](https://aedit.com/procedure/phenol-peel) or [croton oil peel](https://aedit.com/procedure/croton-oil-peel) can have weeks of downtime, with some swelling and redness hanging around even longer. Once the dead skin is sloughed off, brighter, smoother skin is revealed. Depending on the type of peel, an initial series of treatments may be recommended to achieve results, followed by less frequent maintenance appointments. 
 
### **2. Lasers**
[Laser skin resurfacing](https://aedit.com/aedition/guide-to-ablative-laser-skin-resurfacing) is another tried-and-true treatment for hyperpigmentation. By the time we reach middle age, the skin condition is more present in women (12 percent) than men (3 percent). Acne grows rarer as we get older, but the emotional toll of breaking out may not. 

Though many facets of [acne treatment](https://aedit.com/procedure/acne-solutions) are the same regardless of sex, the causes and some of the solutions can differ. We spoke to board certified dermatologists to learn more about the specifics of adult acne in men.

## **What Causes Acne in Men?**

Simple personal care changes, like always showering after exercise or changing the pillowcases more often, can help prevent breakouts. Because [beards and mustaches](https://aedit.com/aedition/facial-hair-treating-razor-burn-ingrown-hairs-beard-dandruff) can house acne-causing bacteria and trap oil, good grooming habits are especially important for men with facial hair. Let’s face it, everyone wants to look like the best and most rejuvenated version of themselves, and Dr. Friedman says that men and women alike are concerned about how they look as they age. “Our patients mostly all want to look very natural after any surgical intervention,” he notes. 

Generally speaking, women seek a facelift to correct an aging face *and* neck, while men primarily focus on the neck. They are also often concerned with the possibility of resulting scars, says [David Abramson, MD](https://aedit.com/provider/david-abramson-md-137), a board certified plastic and reconstructive surgeon in New York City. Men who ‘need’ a facelift have jowling and heaviness in the lower face and excess skin on the neck. The kicker? They need to have enough hair to hide the ensuing scars. “This is especially the case if the patient wears their hair short,” he shares. “However, if he is completely bald, it’s challenging to hide those scars.”

Those patients are “not an ideal candidate for the procedure,” Dr. Abramson notes, adding that it’s not standard practice to place scars on or inside the ears. For this reason alone, facelifts on men can be more challenging to perform than women. “Of course, there are trade-offs to a facelift, and there are other procedures and treatments we can do, like strictly neck procedures,” he adds. But a [facelift without a neck lift](https://aedit.com/aedition/pairing-facelift-neck-lift-procedures) is like eating a hamburger without a bun — it’s doable but rarely as good as the combination.

For example, the goal of a facelift is to address signs of aging on the lower face and neck, but the procedure will not get rid of skin-related concerns that affect men, like [sun damage](https://aedit.com/aedition/cosmetic-procedures-treatments-to-correct-sun-damage) and [acne scars](https://aedit.com/concern/acne-scars).

## **Facelifts for Men vs. Facelifts for Women**
All facelifts, regardless of the gender of the patient, target muscle, fat, tissue, and excess skin. For the most part, the steps involved in facelifts for both men and women are the same, but the incision placement is different. “There are some minor technical differences between male and female facelifts,” Dr. Friedman shares. “But, in general, the goal is natural-looking results without changing the identity of a person and minimal downtime.”

As we touched on, there are also differences in what men are usually looking to address with their surgery. The lower face and neck obviously make the list, but they may also choose to [pair the facelift](https://aedit.com/aedition/best-procedures-to-combine-with-a-facelift) with additional procedures to maximize results:

- **Turkey necks & neck laxity affect men**: Men are usually concerned with the fat that dangles from the front of their neck that may cause them to need a bigger shirt size. “Getting rid of the turkey waddle and naturally tightening the neck is what most men want to achieve with a facelift,” Dr. Friedman shares. 
- **Fat under the chin can be problematic**: Extra fat accumulates under the chin and between the jawline and neck, making the chin appear less defined. Dr. Cabin says men have more blood vessels and a stronger blood supply in their faces than women. “The body requires additional blood supply for [facial hair](https://aedit.com/aedition/facial-hair-treating-razor-burn-ingrown-hairs-beard-dandruff), and, because of this, there is a slightly higher risk of blood collection (a hematoma) post-operatively,” he explains. “It is usually minor and easy to treat, but I am extra careful during surgery to control bleeding and any potentially problematic blood vessels.”

## **Achieving a Natural Result**
One thing that men stress that they don’t want is a tight, pulled look. “They don’t want to appear like they’ve had surgery, nor do they want to look feminized,” Dr. Cabin says. He says achieving a natural result starts with good surgical technique. “It's about using a combination of a deep plane lift, tension-free skin closure with carefully hidden incisions around the ear and hairline, and repositioning the skin and soft tissue at a natural angle,” Dr. Cabin explains. “Otherwise, the face can look windswept or unnatural.” 

Most men don’t realize that it’s possible to achieve such a natural  — and masculine — result. “By taking all tension off of the skin and lifting the deeper layers of the face, [deep plane facelifts](https://aedit.com/procedure/deep-plane-facelift) achieve more natural results with a reduction in scarring risk,” he adds.

Not all men care for their skin the same way that women do. “Sure, there are different groups of men who have different skincare habits, but, from my experience, most are not big followers of skincare unless they’ve always been like that,” Dr. Abramson shares. 

Just because a [skincare routine](https://aedit.com/aedition/best-mens-skincare-routine-products-ingredients-for-20s) doesn't sit at the top of a man's daily to-do list, however, doesn't mean the surgery results will be affected. Environmental and lifestyle choices, like sunbathing and [smoking](https://aedit.com/aedition/impact-of-cigarette-smoking-vaping-on-skin-hair-teeth-health), impact the results more than forgetting to apply moisturizer or bypassing [vitamin C](https://aedit.com/aedition/guide-to-topical-vitamin-c-in-skincare). For patients with poor skin quality, your surgeon may suggest a resurfacing laser during the facelift for more comprehensive rejuvenation.

## **A Clean Cut**
Most (though not all) men who get a facelift wear their hair shorter, so, of course, visibility of scars is always a worry. Dr. Friedman says facelift incisions are hidden differently in men than women because of bearded facial skin. A good plastic surgeon will place the incisions (later scars) in front of and behind the ears and in the hairline. Once healed, the scars should be a barely detectable fine, white line or even look like a tiny wrinkle.

## **The Results**
Nothing is permanent — not even a facelift. “It’s important for facelift patients to understand that the procedure is like a reset button, and you will continue to age,” Dr. Abramson advises.
