About Amy R. Kulak, MD
Dr. Kulak is an ophthalmologist who is fellowship trained in ophthalmic and facial plastic surgery practicing in Jacksonville FL. Dr. Kulak specializes in reconstructive and cosmetic eyelid surgery, medical and surgical management of tearing disorders, eyelid tumor reconstruction as well as comprehensive eye care. She is dedicated to individualizing patient care to help achieve personalized, natural results and utilizes the most current non-invasive technologies and products for facial rejuvenation including Botox®, facial fillers and fractional CO2 laser resurfacing. Dr. Kulak graduated from Brown University with research distinctions and then pursued clinical research interest at Weil Cornell Medical Center in New York City. She attended the University of Miami Miller School of Medicine where she graduated at the top of her class and received her medical degree. She stayed in Miami to complete an internship in Internal Medicine at Jackson Memorial Hospital prior to moving back to New York for residency in ophthalmology at SUNY Downstate Medical Center. She completed her oculoplastic fellowship training at Beaumont Medical Center, The Kresge Eye Institute/Detroit Medical Center, and at the Consultants in Ophthalmic and Facial Plastic Surgery in Southfield Michigan before settling in Jacksonville, FL . In her free time, Dr. Kulak enjoys playing tennis, playing on the beach with her two dogs and spending time with her daughter and husband.
