## **Why Board Certification Matters** 
Titles like ‘cosmetic surgeon,’ ‘plastic surgeon,’ and ‘cosmetic dermatologist’ have seemingly become interchangeable as of late, but the qualifications of such doctors can vary greatly. 

Because there is very little regulation governing how physicians refer to themselves, understanding your provider's board certification(s) can ensure you are consulting with a doctor or surgeon who is formally trained in plastic and reconstructive surgery, otolaryngology, ophthalmology, or dermatology (i.e. the specialties that most relate to aesthetic medicine).

Unlike a medical license, board certification is not necessary to practice medicine in the United States, but it is an important tool in distinguishing whether or not a provider has specialized – and continues to specialize – in a particular area. Many medical specialty boards require recertification every decade or so, ensuring doctors keep on top of their skills.

## **How Doctors Become Board Certified**
A licensed doctor is not the same as a board certified one. A medical license is required for all doctors who wish to legally practice medicine in the U.S. Medical licensure is handled at the state level, and physicians must receive individual licenses in each state they wish to practice in. Board certification, meanwhile, signifies that the doctor has successfully completed a residency in their chosen specialty. 

In order to become board certified, a physician must have completed his or her undergraduate education, medical school, one-year internship, residency in their chosen specialty, and passed the residency board exam. 

## **The ‘Core Four’ Board Certifications at AEDIT**
The AEDIT Provider Network is comprised of medical doctors (MDs) and doctors of osteopathic medicine (DOs) who are board certified by specific member boards of the American Board of Medical Specialties (ABMS) and American Osteopathic Association (AOA) or a very select group of unaffiliated boards for specialties like facial plastic and reconstructive surgery and oculoplastic surgery.

Here is closer look of the specialties that make up the AEDIT Provider Network:

### **Plastic and Reconstructive Surgery**
General plastic and reconstructive surgeons complete three to five years of general surgery training followed by a two to three year cosmetic and reconstructive plastic surgery residency. Plastic and reconstructive surgeons listed with AEDIT can pursue board certification via the American Board of Plastic Surgery or American Osteopathic Board of Surgery - Plastic and Reconstructive Surgery. In the case of the former, they may be dual-certified by the American Board of Surgery. While these physicians are board certified to perform cosmetic and reconstructive surgeries from head to toe, their work may focus on the breasts and body. 

### **Facial Plastic and Reconstructive Surgery**
After completing a five-year residency in otolaryngology (a.k.a. ears, nose, and throat), facial plastic and reconstructive surgeons pursue year-long fellowships in cosmetic and reconstructive procedures specifically related to the face and neck (think: rhinoplasty, facelift, chin and jaw surgeries, etc.). 

There is no ABMS Member Board specifically for facial plastic and reconstructive surgery. Instead, facial plastic surgeons in the AEDIT Provider Network are board certified by the American Board of Otolaryngology – Head and Neck Surgery or the Otolaryngology/Facial Plastic Surgery certification within the American Osteopathic Board of Ophthalmology and Otolaryngology - Head and Neck Surgery. The American Board of Facial Plastic and Reconstructive Surgery is also an accreditation that many facial plastic surgeons seek.

### **Dermatology**
Medical dermatologists diagnose, treat, and prevent conditions affecting the skin, hair, and nails. All dermatologists affiliated with AEDIT are board certified by the American Board of Dermatology or American Osteopathic Board of Dermatology, which means hey have undergone non-surgical, cosmetic dermatology training during their residency program. Dermatologic surgeons refer to physicians who are qualified to perform surgical procedures like liposuction, fat transfer, and Mohs surgery, in addition to non-invasive treatments. They are often fellows of the American Society for Dermatologic Surgery.

### **Oculoplastic Surgery**
While a facial plastic surgeon is able to perform eyelid surgeries like blepharoplasty, oculoplastic or oculofacial surgeons are experts in procedures that reconstruct the eyelids, tear ducts, and orbital area. At AEDIT, they are board certified by the American Board of Ophthalmology or have an Ophthalmology certification within the American Osteopathic Board of Ophthalmology and Otolaryngology - Head and Neck Surgery. Many also have additional fellowship training with the American Society of Ophthalmic Plastic and Reconstructive Surgery.

## **Fellowships & Societies**
Doctors who wish to further specialize in a particular area of medicine may choose to continue their training and education via a fellowship. Fellowship programs are often highly competitive and last one to three years each. Due to the lack of ABMS Member Boards in certain subspecialties, fellowships may be used to denote expertise. An ophthalmologist who wishes to subspecialize in oculoplastic surgery, for instance, would complete a two-year fellowship through the aforementioned ASOPRS. 

Society memberships, meanwhile, offer another way that is separate from licensing, board certification, and fellowships for providers to continue their education, in addition to offering mentoring and networking opportunities. If you spent any time on TikTok over the summer, you likely saw an endless stream of videos extolling the virtues of #TrapTox. The alliterative hashtag refers to the injection of a neurotoxin into the trapezius muscles to slim and relax the area for both aesthetic and functional benefits. 

While it seems like the ‘trap Botox’ trend came out of nowhere, it’s actually been an established – albeit under-the-radar – off-label use of botulinum toxin type-A injections for some time. So, what are the benefits of injecting Botox® and the like into the trapezius muscles and who is a candidate for treatment? Top injectors break it down.

## **Anatomy 101: The Trapezius Muscle**
Before we dive into trap Botox, let’s start with a quick anatomy lesson. The trapezius is a large, superficial back muscle that gets its name from its shape – it resembles a trapezoid. It starts at the base of the neck and extends across the shoulders and down the middle of the back. 

The main function of the trapezius (a.k.a. ‘the traps’) is stabilizing the scapula (read: shoulder blades) and controlling it during movements of the shoulders and arms (think: throwing, shrugging, raising your hand, sitting up straight, etc.). “Many of us hold tension in our shoulders,” says [Kiran Mian, DO](https://www.drkiranmian.com/), a board certified dermatologist at Hudson Dermatology & Laser Surgery in New York City. “We also hold postures in our day to day that require our shoulders to be hunched over, like working at a computer, sitting at a desk, or even on our phones.” 

The overuse of the trapezius causes hypertrophy or enlargement, Dr. Mian notes, which can lead to both functional (think: physical discomfort) and aesthetic concerns. “Most people have a bulge where the neck meets the shoulder,” says [Nigar N. Ahmedli, MD](https://www.montefiore.org/doctors/profiles/1790164341/nigar-n-ahmedli), a board certified otolaryngologist and director of facial plastic and reconstructive surgery at Montefiore Medical Center in NYC. “It can interrupt the flow of the neck, thus shortening the appearance of the neck.” This is where trap tox comes in.

## **What Is ‘Trap Tox’?**
Whether you call it ‘Trap Tox’ or ‘Barbie Botox’ (yes, the movie mania infiltrated aesthetic medicine, too), the treatment is the same. Neurotoxins like Botox®, Dysport®, Jeuveau®, and Xeomin® are injected into the trapezius muscle to address physical and/or cosmetic concerns. “Physically, Botox injections into the trapezius muscle can make the two sides more even, less bulky, and/or more proportional to the client’s overall frame,” says [Kristina Kitsos, RN](https://www.instagram.com/kristinakitsos/?hl=en), a registered aesthetic nurse in Beverly Hills. “It can also make the neck appear longer and shoulders thinner, which is why many people call it ‘Barbie Botox.’”

Much like any other botulinum toxin type-A injection, the treatment is entirely customizable. “Injections of Botox into the trapezius muscle are targeted toward specific regions within the muscle to create the desired effect – ranging from enhancing the shape of the muscle, promoting symmetry, and refining proportions or slimming the muscle,” Kitsos says. “Based on the patient’s individual goals, placement and dosage is modified.”

### **Aesthetic Benefits of Trap Tox**
Much like Botox is injected into the jaw to slim the lower face, it can be injected into the trapezius to improve the contour of upper body. “Neurotoxin injections in the trapezius can have a slimming effect on the neck,” says [Margarita Lolis, MD](https://www.schweigerderm.com/providers/margarita-lolis-m-d/), a board certified dermatologist and Mohs surgeon in New Jersey. “This is because the neurotoxin itself lowers the trapezius muscle, making the neck appear longer.” 

The injections “elongate the neck, creating a more feminine contour between the neck and shoulders,” Dr. Mian explains. The swan-like effect creates the appearance of a more delicate upper body that is likened to a Barbie doll.

### **Functional Benefits of Trap Tox**
We may associate Botox with wrinkle reduction, but BoNT-A injections are also used to treat muscle spasms associated with TMJ, lazy eye, vaginismus, hyperhidrosis (read: excessive sweating), and, yes, neck and shoulder pain. “Trap tox can relax tension or spasm in the trapezius muscle,” says[ Jennifer Levine, MD](https://www.drjenniferlevine.com/), a double board certified facial plastic and reconstructive surgeon in NYC. 

At their core, neuromodulators like Botox®, Dysport®, Jeuveau®, and Xeomin® relax muscles. In the forehead, for example, that improves the appearance of wrinkles. In the upper back, that can ease stress and discomfort. “The muscles of this region – including the paired trapezius muscles – can tighten and spasm, leading to neck and shoulder pain as well as tension headaches,” Dr. Ahmedli explains. “Injecting the trapezius muscles with neurotoxin relaxes the muscles, allowing the tightness to improve and, therefore, helping decrease pain and headaches.”

## **The Ideal Candidate for Trap Tox**
A candidate for trap tox could be someone looking to relieve tension in the neck and shoulders, someone looking to slim the area, or someone looking for a bit of both. “The ideal patient is someone with stiffness or pain in the trapezius muscles, people with overdeveloped trapezius muscles, and anyone who has neck and shoulder pain from overuse of their trapezius muscles,” Kitsos shares. “A good candidate would be anyone who would like to create a more svelte, longer-looking neck and less bulky shoulders.”

Injectors will study a prospective patient a few ways to determine candidacy. “The ideal patient for trap tox has palpable trapezius muscles – the provider can feel the muscle with their hands – with a noticeable bulge,” Dr. Ahmedli says. “The neurotoxin injection will diminish the bulge and create a sleek neckline.” Body language can also be a sign. “The ideal patient for trap tox is someone who subconsciously hunches their shoulders, causing increased tension in the muscle that builds over time as well as hypertrophy,” Dr. Mian adds. 

The treatment, however, is not for everyone. Generally speaking, neurotoxins are not recommended for those under 18, women who are pregnant or nursing, anyone with an allergy to the components of the injectables, or those with certain chronic medical conditions. When it comes to trap tox specifically, “one should avoid this treatment if they have neuromuscular conditions, nerve damage in that area, or any injury or generally weak musculature in that area,” Dr. Mian cautions. 

## **What to Expect from a Trap Tox Treatment**
If you’ve ever had neurotoxin injections on the face, Dr. Levine says you can expect pretty much the same experience when it comes to trap tox preparation, in-office experience, and recovery. “Much like facial Botox injections, trapezius muscle injections have little to no pain and take just a couple of minutes to administer,” Kitsos notes.

On the pain point, facial Botox injections rarely feel like more than a pinprick. Trap tox can have a slightly stronger – though still manageable – sensation. “A longer needle is used to get to the muscle, as the trapezius lies deeper than facial muscles,” Dr. Mian shares. “It can cause a little bit more discomfort during the procedure.” If pain is a concern, Dr. Lolis says patients may choose to numb for 15 to 30 minutes prior to treatment. 

What is different between trap tox and facial Botox is the amount of product needed. “We are talking about much higher doses of toxin for the trapezius because it is a much more dynamic and stronger muscle compared to your facial muscles,” Dr. Lolis notes. While the actual dosage will depend on the patient and product, there are some general rules of thumb. “Because the trapezius muscles are much larger than the muscles in the face, the dosage in the trapezius is often two to three times that of popular areas like crow’s feet and frown lines,” Kitsos says.

Each brand of neurotoxin requires slightly different dosing, but Dr. Ahmedli shares that if you are used to getting 20 to 35 units of Botox® in the forehead, the trapezius may need 40 to 75 “depending on the muscle size and tightness.” Dr. Levine offers a similar guesstimate. “The trapezius is a very large muscle, so it takes many units to get an effect,” she shares. “If it is 10 to 20 units for the forehead, the trapezius may need 50 or more per side.” Neurotoxins tend to be priced per unit based on the provider and location, but you can expect to pay around $1,000 for a trap tox treatment.

For the best experience, Dr. Lolis recommends avoiding exercising and lifting heavy weights on the day of the injection to ensure the traps are “as relaxed and tension free as possible.” Post-injection, avoid alcohol, strenuous activity, and massaging the treated area for the first 24 hours. “Patients mainly need to be careful not to apply any pressure on the shoulders/trapezius muscles for a few hours after injections,” Dr. Lolis adds. 

## **When to Expect Results from Trap Tox**
If you’ve ever had a neurotoxin treatment, you know the results are not instant. In the case of trap tox, the results are often staggered. “Patients will start to notice a difference a few days after injection as the muscle begins to feel less tight,” Dr. Ahmedli says. “Maximal effect is seen at the two-week mark.”

Depending on the product used and the patient’s lifestyle and metabolism, the effect can last anywhere from two to six months. In some cases, more than one treatment may be needed to enjoy the full aesthetic and functional benefits. “Some patients will see results after the first treatment, while others may take more sessions,” Dr. Mian notes. 

While “the physical relief of tension will be felt after the first treatment,” she says, the aesthetic results “of elongated neck-shoulder curvature” may take a second treatment because “alleviated hypertrophy of the muscle can take some time.” In other words, “the muscle needs time to shrink now that it’s not being recruited constantly,” Dr. Mian adds.

### **A Note About Muscle Movement**
Neuromodulators temporarily paralyze the muscles they are injected into until the toxin wears off, so are you basically opting for partial neck paralysis when you get trap tox? Not exactly. “Ideally you want to obtain the effects of tension release and aesthetic results without affecting functional movement,” Dr. Mian shares. “The ideal dosing lies in that balance between relaxing the muscle enough to achieve those results, while still being able to have movement like shrugging your shoulders.”

That’s not to say you won’t notice a difference. “At first, the muscle may be a bit weaker than normal,” Kitsos says. “After the neurotoxin wears off, patients will find that their trapezius muscles will go back to normal movement and they will once again be able to easily shrug their shoulders.” In Dr. Ahmedli’s experience, it’s mostly about tension. “The main difference patients notice is a decrease in the tightness in the shoulders,” she says. “As the neurotoxin wears off, the tightness will return.”

These are all things to talk to your injector about prior to treatment. “There is definitely a potential for muscle weakness in the treated area, which can affect muscle function,” Dr. Lolis shares. “You can potentially lose muscle since it is not being worked, and the other potential downside is that the two sides might be asymmetric.”

## **The Takeaway**
Yes, trap tox can give you a longer neck and more contoured upper body. But it is really a game changer for those who experience chronic neck and shoulder pain as a result of stress or tension. You’ve likely seen the before and after pictures on social media of people who seemingly had all of their under eye concerns erased thanks to a bit of strategically placed filler in the tear trough region. For the right patient, under eye filler can have a transformative contouring, smoothing, and brightening effect. But depending on the cause of your dark circles, hollowing, and skin quality concerns, under eye filler can actually make matters worse.

So, how can you tell if you are a candidate for under eye filler? We’re talking to top dermatologists and plastic surgeons about the pros and cons of the minimally invasive procedure and the factors that go into determining whether or not it’s right for you.
## **The Benefits of Under Eye Filler**
When we talk about under eye concerns, it's often in general terms. How many times have you heard someone say their eyes look 'tired'? Or that they want them to appear more ‘awake’? The reality is that there are a number of smaller factors that contribute to the eye area looking a little sleepy. 

One of the biggest culprits of 'tired' under eyes is a lack of volume in the under eye area either due to anatomy or age. “In some patients, aging leads to the loss of fat under the eyes, giving a hollow or skeletal appearance,” says [Nigar N. Ahmedli, MD,](https://www.montefiore.org/doctors/profiles/1790164341/nigar-n.-ahmedli) a board certified otolaryngologist in New York City who specializes in facial plastic and reconstructive surgery. Those hollows can then create the illusion of darkness in the region. "They worsen the appearance of dark circles under the eyes by creating a shadow that accentuates the darkness," she notes.

Additionally, the volume loss draws attention to the tear trough. “The ‘tear trough’ area refers to the separation between the lower eyelid and the upper cheek,” says [Elizabeth Callahan, MD,](https://sarasotadermatologist.com/) board certified dermatologic surgeon in Sarasota, FL. Coupled with changes in skin quality, the result is fine lines, dark circles, a general loss of contour, and, yes, an overall 'tired' appearance. 

Injecting a hyaluronic acid (HA)-based filler into the under eye area can be an effective treatment for this. “Under eye filler is best for providing volume to the hollows beneath your eyes,” says [Melissa Doft, MD,](https://aedit.com/provider/melissa-doft-md-63) a double board certified plastic and reconstructive surgeon in NYC. "Adding filler to the under eye area will help with shadows and also act as a wonderful moisturizer to help with dehydrated and crepey skin."

It can also be used to improve the contour of the upper face. "Under eye fillers can address the hollows created beneath the eyelid and at the junction of the cheek and lower eyelid," says [Kenrick Spence, MD,](https://www.mynewbeauty.com) a board certified plastic and reconstructive surgeon in Orlando, FL. "Asymmetry can be corrected and dark circles can be softened with the help of under eye fillers."

## **The Misconceptions About Under Eye Filler**

Here’s the thing, under eye filler “can address aesthetic concerns like hollowness, volume loss, and shadows,” says [Kami Parsa, MD,](https://www.oculoplastic.info/) a board certified oculoplastic surgeon in Beverly Hills. And here comes the ‘but.’ “A misconception is that filler can help everyone,” Dr. Callahan notes. This is because “under eye filler is not a blanket intervention that addresses all under eye-related concerns,” Dr. Ahmedli adds.

With that in mind, we asked our pros to break down some of the most common misconceptions about under eye filler:
### **1. Under Eye Filler Treats Eye Bags**
For starters, under eye filler does not treat what we colloquially refer to as ‘eye bags’ and puffiness. “Puffiness is due to excess fat or lack of proper lymphatic drainage,” Dr. Doft says. “Both of which will not improve with fillers.” Regardless of the cause, the appearance of puffiness is “the opposite of having hollows,” Dr. Ahmedli notes. 

In the case of puffiness caused by excess fat, “the under-eye fat protrudes forward instead of retracting into the eye socket,” Dr. Ahmedli explains. Filler will only exacerbate this. “Adding filler to an area that’s already puffy just makes it worse,” Dr. Parsa says. This can be true even if you simultaneously have both depressions and puffiness as a result of displaced fat. “In some people, there are under eye bags, which create a relative bulge followed by a hollow,” says [Lesley Rabach, MD,](https://lmmedicalnyc.com/dr-lesley-rabach/) a double board certified facial plastic and reconstructive surgeon in NYC.
### **2. Under Eye Filler Erases Dark Circles**
You’ll be hard pressed to find someone who hasn’t dealt with dark circles under the eye. And while under eye filler can help brighten the area by lessening the shadowing caused by hollowing, it does not directly treat discoloration. “Filler will not improve dark circles or discoloration of the skin,” says [Oren Tepper, MD,](https://orenteppermd.com/) a board certified plastic and reconstructive surgeon in NYC. 


The reason? Dark circles are not one size fits all. “Dark circles and hollows under the eyes can be caused by various factors, including lack of sleep, genetics, aging, allergies, sun exposure, dehydration, various lifestyle factors, and other medical conditions,” Dr. Callahan shares. And remember: “Everyone has some degree of darkness under the eye,” Dr. Rabach emphasizes.
### **3. Under Eye Filler Is Risk Free**
Getting HA filler in any part of the face can seem like a ‘quick fix’ that offers immediate results with little to no downtime. But the reality is that injecting filler – especially to the complex under eye area – requires a great deal of training and understanding of facial anatomy. “There is a misconception that placing filler under the eye is completely safe,” Dr. Doft cautions. 

As she explains, under eye filler can have a tyndall effect when it’s injected too superficially, leading to a blue tint under the skin. It can also cause “scarring of the lymphatics leading to malar bags and possible blindness,” she says. For these reasons, “this is a procedure I would only have a board certified plastic surgeon or dermatologist perform,” Dr. Doft shares. 

Technique is also vital to ensure a safe, effective, and aesthetically pleasing result. “Many providers place tear trough or under eye filler in the wrong spot,” Dr. Rabach notes. “It is a very delicate area, but it is most of the time placed too low – almost over the orbital rim – instead of high into the tear trough.” Seeing a board certified provider who regularly performs the procedure can help mitigate this risk.
### **4. Under Eye Filler Is Like a Filter**
Even in the best hands, it’s also important to keep your expectations in check. “The most common misconceptions about under eye filler are that it is a simple intervention that improves under eye hollows, bags, dark circles, and skin texture,” Dr. Ahmedli explains. “It is crucial to understand that dark circles are due to the deposition of pigment into the thin skin of the eyelid, and lower eyelid skin tends to become like crepe paper – crinkled with fine lines – with aging.” 

And no, you are not going to look perpetually Facetuned. “People think under eye filler will completely make under eye hollows or darkness go away, and that you will look like a heavily filtered Instagram model after treatment,” Dr. Rabach says. “No one looks like this.”
## **Are You a Candidate for Under Eye Filler?**
Now that you know the benefits and limitations of under eye filler, it’s time to determine if you are a candidate. During a consultation with a board certified dermatologist or plastic surgeon, the provider will be studying the under eye region to determine the root cause of your aesthetic concerns. "There are so many factors when we discuss the under eye hollows because they happen for so many reasons," Dr. Rabach notes.

As she explains, it is important to look at skin tone and what is behind any darkness or discoloration. “The skin is so thin, so we can see tiny blood vessels under the skin,” she shares. “The skin is also more prone to pigmentation due to the hollow and being so delicate.” Additionally, darkening under the eyes can be caused by “light reflecting off the concavity making a shadow,” she notes. Only the latter can be improved with filler.

And then there is the contour of the under eye region. “In some people, there are under eye ‘bags’ that create a relative bulge followed by a hollow,” she says. “We also need to consider the relationship and general anatomy of where the orbital rim is related to the eyelashes and the direction of the hollow.” These factors and more, she notes, “need to be weighed for candidacy for under eye filler.”

Here are some general guidelines for what makes someone an ideal candidate for under eye filler:
### **Ideal Candidates for Under Eye Filler**
The ideal candidate for under eye filler “is a patient who has hollows under the eyes without puffiness and without heavy dark circles,” Dr. Ahmedli says. “This allows for filler to be placed in the hollow to correct the depression and to restore the natural slope of the under eye-to-cheek transition found in a youthful face.”

When it comes to volume loss and puffiness, there is a fine line between how much is too much. “An ideal candidate for under eye fillers is someone that has a modest loss of volume, mild bulging of fat pads, and modest skin laxity under the eye region,” Dr. Spence says. This means age will be a factor. Dr. Rabach says ideal candidates for under eye filler are generally “younger” – meaning twenties, thirties, and forties. 

Skin quality matters as well, Dr. Tepper notes. “Thicker skin,” Dr. Rabach says, is better suited for filler because it usually blocks any visible under eye vessels that can cause discoloration. Additionally, she said the region should have little to no pigmentation (another common cause of dark circles). 

Last but not least, under eye filler can be an option for “people who are not ready for surgery,” Dr. Parsa says. In such cases, however, it’s important to know what you are signing up for. “There is definitely a limitation on how much they work,” he notes. “And the key here is to understand that fillers are not a permanent solution.” Though they may hang around longer than expected. “Fillers last many years, even though we are told they last only six months,” he cautions. 
### **Non-Candidates for Under Eye Filler**
Generally speaking, “someone who is older with larger bags, lots of excess skin, pigmentation, and/or very thin skin” would not be a good candidate for under eye filler, Dr. Rabach says.

As it relates to most dark circles, filler is not a fix. “Filler cannot correct issues such as genetic hyperpigmentation or blood vessels shown under the eyes,” Dr. Callahan shares. Similarly, it can’t necessarily contour away puffiness. “Some patients lack under eye hollows and instead have more fullness or ‘bags,’” Dr. Ahmedli says. “This creates a bubbling look in which the under-eye bulges forward and accentuates the tear trough groove underneath leading to a harsher separation between the under eye and cheek.” Filling the area can exacerbate this.

It’s also important to consider the anatomy of your entire face – not just the under eye. “If the patient lacks cheek fat, then filling the tear trough groove becomes tricky,” Dr. Ahmedli cautions. “A provider cannot fill the groove to maximal correction because the lower lid will appear even puffier, creating the dreaded ‘chipmunk’ look.” Dr. Spence agrees that those with “severe volume loss from the cheek are not going to be the ideal candidates for under eye filler.” Ultimately, Dr. Callahan says you need to identify the actual culprit of your aesthetic concerns. “Filler will not help if the real problem is lack of volume in the cheeks just below the dark circles,” she notes.

Another thing to keep in mind? Your health. “A bad filler candidate is someone who suffers from chronic/seasonal allergies (itchy swollen eyes), puffy lower eye bags, fluid retention syndrome (eye bags that swell in the morning or based on what you eat), and those who have a significant amount of loose skin under the eyes,” Dr. Parsa shares. “Adding volume with HA filler to the undereyes will make things look worse in these situations.”
### **Do You Actually Need a Blepharoplasty?**
We’ve talked a lot about why you might not be a good candidate for under eye filler if you have under eye bags, puffiness, and skin laxity. What may be a solution, however, is a surgical blepharoplasty. “Blepharoplasty is a surgical procedure aimed at rejuvenating the eyelids,” Dr. Callahan explains. “It involves the removal of excess skin, fat, and sometimes muscle from the upper and/or lower eyelids.” 

When it comes to the lower lid, the eyelid surgery can address eye bags and skin laxity concerns. “Lower eyelid blepharoplasty, or lower eyelid lift, allows for excess skin to be removed and for the fat pockets to be contoured and repositioned to improve the appearance of the puffiness and the tear trough groove,” Dr. Ahmedli shares. As Dr. Tepper explains, a blepharoplasty is the preferred treatment if “tear trough deformity is moderate to severe” and “there is excess fat protruding in the lower eyelids.” While “filler will not correct or address this,” he says, “the fat can be removed or transposed” surgically.

For some patients, under eye filler is a gateway to blepharoplasty. “It is more common for younger patients to begin with under eye filler given the lack of excess skin,” Dr. Ahmedli notes. But that is not always the case. “A lot of our patients think the best way to treat their under eye concerns is by adding volume with filler,” Dr. Parsa says. “We have had countless patients come to us to remove their under eye filler and proceed with a blepharoplasty for long-term results.” In such cases, the under eye filler must be completely dissolved before proceeding with surgery, he adds.

For others, surgery is the best and only option. “If the real problem is fat pad herniation (a.k.a. eye bags) then surgery is the best choice,” Dr. Callahan says. Additionally, “if a patient has excess lower eyelid skin that is bunching, then blepharoplasty is a better option than filler,” Dr. Ahmedli shares. Ultimately, “a patient with significant skin laxity and bulging fat pads with volume loss to the cheeks is better served with a surgical approach,” Dr. Spence says. “The procedure actually removes excess skin and fat rather than filling in any hollowness.”

Are You a Candidate for Under Eye Filler? Here’s How to Tell

It is said that summer bodies are built in the winter. That logic can extend to any time of the year and is true of both at-home nutrition and fitness plans and in-office [body contouring](https://www.aedit.com/procedure/body-contouring-solutions) treatments. “There is no such thing as an overnight sensation,” says [Amy Wechsler, MD](https://www.dramywechsler.com/), a board certified dermatologist and psychiatrist in New York City. But what if a special event or trip popped up and your timeline is tighter than, say, the six months between the winter and summer solstices – are there any toning and tightening treatments that can get the job done sooner? 

The answer is yes, if your expectations are properly managed. “It takes three to six months for any invasive procedure to have optimal results,” says [Joseph Zakhary, MD](https://breastreconstructionaz.com/about/practitioners/dr-joseph-zakhary/), a board certified plastic and reconstructive surgeon and co-founder of Southwest Breast & Aesthetics in Arizona. “Less invasive procedures may have speedier recoveries, but expectations should be tailored as the result is usually less dramatic when compared to a surgery.”

So, while there may be, as Dr. Wechsler notes, no substitute for “eating healthfully, exercising, sleeping enough,” and budgeting a proper amount of time, there are some body contouring procedures that can sculpt key areas of the body with minimal downtime and – relatively – quick results. Here, we’re breaking them down (and level-setting your expectations) with the help of expert dermatologists and plastic surgeons.

## **The Most Popular Areas for Body Contouring**

Before we dive into the procedures that can tone, tighten, and sculpt, let’s talk about the areas of the body that are most often treated. “Common areas of body contouring for women are the lower abdomen, outer hips, and flanks (love handles), as well as the outer and inner thighs,” says [Lyle Leipziger, MD](https://www.drleipziger.com/), a board certified plastic and reconstructive surgeon and chief of plastic surgery at North Shore University Hospital and LIJ Medical Center on Long Island. “The distribution is different for men, so these areas would be more limited to the trunk, upper and lower abdomen, and hips and flanks.” In some cases, guys may also be interested in sculpting the chest, he adds.

For men and women alike, the waistline is “by far” the most popular area to address, says [Max Polo, MD](https://miamiplasticsurgery.com/), a board certified plastic and reconstructive surgeon at Miami Plastic Surgery. “That includes the abdomen in front and the flanks on the side and in back,” he notes. Speaking of the back, women may seek to slim the mid-back or so-called “bra-fat” area, and the “buttocks is, of course, very popular these days for contouring,” Dr. Polo shares. 

Another way to think about this is in relation to the concerns you are targeting. “The most common areas for patients seeking fat reduction are the abdomen, love handles, arms, and thighs,” says [Jessie Cheung, MD](https://drjessiecheungnyc.com/), a board certified dermatologist who practices in NYC and Chicago. “Besides the face, the buttocks remain the most popular area for filler augmentation.” When it comes to strengthening and toning, “the most common body areas treated for muscle building are the buttocks, abdomen, arms, and calves,” she adds. And let’s not forget cellulite in the trunk area – especially heading into the summer months.

## **Managing Expectations**

Here’s the good news. “There are some treatments that offer some immediate change,” says [Jennifer Levine, MD](https://www.drjenniferlevine.com/), a double board certified facial plastic and reconstructive surgeon in NYC. And here comes the ‘but’ – “most need more than one treatment and have visible results in four weeks with maximal effects at 12 weeks,” she adds.

Downtime is one part of the process where you can save some time. “Many body contouring procedures available have minimal downtime as compared to surgery,” says [Elizabeth Callahan, MD](https://sarasotadermatologist.com/about/dr-callahan/), a board certified dermatologist at SkinSmart Dermatology in Sarasota, FL. But that doesn’t necessarily have any bearing on when you see results. “Even when you do a procedure like CoolSculpting®, you still have to wait for the body to eliminate the treated fat and that can take up to three months,” she notes. “This is one of the main reasons we recommend patients pursue treatments in the winter – so they can be feeling great about their results by summer.”

If time is not on your side, you’ll want to have a conversation with your provider about what’s possible given your circumstances and aesthetic goals. “It's important to educate patients that results from body contouring aren't always immediate – even liposuction takes months to see the final results due to swelling and delayed skin tightening,” Dr. Cheung cautions. “Non-invasive fat reduction, skin tightening, and muscle-building procedures will typically need a series of treatments to see dramatic results, while buttock augmentation with fillers is an instant gratification procedure with minimal downtime.”

The takeaway: “Preparation, planning, and setting expectations is key,” Dr. Zakhary says.

## **The Best Minimally Invasive Body Contouring Procedures**

Now that you have a better sense of the most common treatment areas and what is actually possible to achieve with in-office treatments, it’s time to take a closer look at the minimally invasive non-surgical and surgical body contouring procedures to tone, tighten, and sculpt.

### **For Non-Surgical Augmentation**: Dermal Fillers

**Downtime**: None, though bruising, swelling, soreness possible
**Results**: 1 to 12 weeks

If you are looking for an instant result, hyaluronic acid-based fillers sculpt the face and body upon injection. Biostimulatory injectables, like hyper-diluted [Radiesse®](https://www.aedit.com/procedure/radiesse) and [Sculptra®](https://www.aedit.com/procedure/sculptra), require a bit more time to set in but are still pretty speedy and can improve skin quality and add volume. “Radiesse®, in particular, will have an immediate effect,” Dr. Levine notes, though you’ll see more of the benefits at the four-week mark and even more at 12 weeks. She recommends one to four treatments spaced four weeks apart for best results. 

To boost your backside, Dr. Cheung says “the best buttock augmentation is filler injections.” She says they can be completed within one to three initial sessions depending on your desired outcome. “These need to be continued once every six to eight weeks, and minimal downtime includes possible bruising, swelling, and soreness,” she notes.

### **For Non-Surgical Fat Reduction**: CoolSculpting®

**Downtime**: None, but expect mild swelling and redness
**Results**: 4 to 12 weeks

While there has been a renewed focus on rare but serious adverse effects like [paradoxical adipose hyperplasia (PAH)](https://aedit.com/aedition/paradoxical-adipose-hyperplasia-after-cryolipolysis), the majority of our experts recommend the treatment. “Despite recent bad press, this is still very effective technology with very rare complications,” Dr. Polo shares. “It is best for small areas of fat.” Because of its targeted nature, the treatment is highly customizable. “CoolSculpting® can treat almost any area on the body and usually takes two treatments spaced a month apart for optimal results,” Dr. Wechsler explains.

Since the fat cells need to be removed from the body, it takes several weeks to enjoy the full effect. “Patients see results from CoolSculpting® in three months,” Dr. Callahan shares. “This is due to the biology of fat reduction in the body, which takes some time.” Even so, Dr. Levine says she has noticed the updated CoolSculpting® Elite gives at least a sign of what’s to come much sooner. “It can have effects seen as early as two weeks, she notes.

### **For Non-Surgical Muscle Toning**: Emsculpt® Neo

**Downtime**: None, but soreness and puffiness are common
**Results**: 4 to 12 weeks

**Results**: 4 to 12 weeks
If you are looking to non-surgically target both skin laxity and excess fat concerns, radiofrequency (RF)-based treatments can do both. Think of it as microneedling on steroids. “The needles emit radiofrequency energy into the skin layers, causing the skin to heat and tighten,” Dr. Ostad explains. “This added heat promotes the production of collagen at a much greater level than traditional microneedling.”

It is used to treat the face, neck, abdomen, arms, and legs, and three treatment sessions spaced four weeks apart are needed for the full effect. Redness and swelling are common for a few days after treatment, and you can resume wearing makeup after 48 to 72 hours. “Results are seen from a few to several weeks after each treatment and are maintained for several months to years after completing three sessions,” Dr. Polo says.

For a non-surgical alternative to liposuction, Dr. Polo is a fan of [BodyTite™](https://aedit.com/procedure/bodytite) or radiofrequency assisted lipolysis (RFAL). The “internal probe treatment,” is similar to “a small liposuction procedure,” he says. We’re mostly talking about the body, we’d be remiss not to mention a couple of the minimally invasive sculpting options that exist for the neck and jawline. Depending on your anatomy and aesthetic goals, two to six treatments spaced at least a month apart are needed. Patients see results about four to six weeks after each injection. Swelling and soreness are common, but there is no downtime.

If submental and facial skin tightening is what you are after, Dr. Callahan recommends [Ultherapy®](https://aedit.com/procedure/ultrasound-skin-tighteningultherapy). The non-invasive treatment employs microfocused ultrasound technology to the face and neck to kickstart the body’s own collagen production processes. There is no downtime, but “results take about six months due to the nature of collagen stimulation and growth,” she notes.

### **For Smoothing Cellulite**

**Downtime**: None, save for some minor swelling and bruising
**Results**: 4 to 12 weeks
If smoother skin is what you’re after, there are a few minimally invasive treatments for cellulite that will start showing results in four weeks or less.

- **Avéli™**: Performed under local anesthesia, the procedure identifies and releases the septa that cause cellulite dimples on the buttocks and thighs in just one treatment. Dr. Levine says some results are immediate (once bruising and swelling subside), but they continue to improve for up to 12 weeks. 
- **Cellfina®**: The one-time treatment targets the structural cause of cellulite by releasing tension on the connective bands that are woven into the fat in the thighs and buttocks. Bruising is common, but there is no downtime. The initial results of [Cellfina®](https://aedit.com/procedure/cellfina) are quick (think: days not weeks), and “patients can be bathing suit-ready as soon as one month after,” Dr. Polo shares.
- **Sentient Sculpt™**: This energy-based treatment can address a myriad of concerns – including cellulite. It uses 2.45 gigahertz of energy, 80 percent of which is sent directly into the fat tissue and cellulite fibers. In addition to the smoothing benefits, Dr. Cheung says “it’s the best fat reduction with skin tightening treatment.” She recommends four sessions once a month, with visible results within the first month. 

### **For Surgical Fat Reduction**: Liposuction

**Downtime**: 5 to 7 days, plus up to 6 weeks of swelling
**Results**: 3 to 6 months

The word ‘surgery’ may have you envisioning being holed up in the house for weeks, but [liposuction](https://aedit.com/procedure/liposuction) is one of the least invasive cosmetic surgeries and yields some of the most transformative results for patients looking to reduce fat. “Liposuction is the ‘minimally invasive’ procedure that possesses the opportunity for significant change with the least downtime and scarring,” Dr. Zakhary explains. “It is still surgery, but the incisions are one to two centimeters in length and positioned in areas to address the treatment zones.”

There are a number of types of liposuction that achieve the same goal – albeit with slightly different techniques. “I would recommend [tumescent liposuction](https://aedit.com/procedure/tumescent-liposuction) because it is more effective, safer, and less painful,” Dr. Ostad explains. “We utilize microcannulas, which are blunt instruments that are able to extract the fat through tiny entry points on the body.” Those incisions are so small, he says, there is no need for sutures.

He performs the surgery under local anesthesia and customizes the procedure based on areas of concern, which can include the abdomen, waist, back, arms, outer and inner thighs, and chin. “The procedure time varies with each patient, but you are able to return home at the end of the day and may return to work at least five days post procedure,” he shares.

If a more chiseled look is what you are after, Dr. Leipziger offers [liposculpture](https://aedit.com/procedure/liposculpture) as part of a liposuction procedure. “This involves using very fine cannulas to carefully sculpt problem areas to provide maximum results,” he explains. “The healing time to see full results is usually about six weeks, however, people may return to their normal activities within two weeks of the procedure.”

Regardless of the type of lipo you opt for, the post-op pain is “extremely manageable and leads to minimal downtime,” Dr. Zakhary says. “Patients will spend four to six weeks in a [spandex compression garment](https://aedit.com/aedition/benefits-of-compression-garments-after-cosmetic-surgery).” There will be about two weeks of moderate swelling and bruising that resolves over time from there. One way to move things along? “Aggressive [post-op lymphatic massage](https://aedit.com/aedition/benefits-of-lymphatic-drainage-massages-after-cosmetic-surgery) can accelerate this process and speed recovery,” Dr. Polo notes.

In terms of results, Dr. Polo says you can start to see them “as early as a few weeks after treatment,” but “usually four to six weeks are required before most swelling has resolved and the surface contour is smooth and natural again.” All told, budget three to six months for the full effect.

## **How to Maximize Body Contouring Results**

Here’s the thing, in-office body contouring procedures – while highly effective for the right candidates – are not silver bullets. Achieving sustained results takes commitment. “Just like in surgery, there are no good short cuts for achieving great results,” Dr. Zakhary says. “A healthy diet and physical fitness regimen are important, and it is also important to minimize things that cause skin damage – as in, excess sun exposure or smoking cigarettes.”

Working with a personal trainer or nutritionist can help you find a regimen that works best for your aesthetic goals, but it’s all about developing a routine that works for you. “Diet and exercise are the biggest tips I have for keeping your body toned and tight,” Dr. Ostad says. “Keeping up a healthy lifestyle is very beneficial for many different aspects of your life – including how you feel mentally and physically.”

Body contouring procedures can complement these efforts, but, ultimately, they are more of a catalyst than an end-all solution. “Don't think of these procedures as a quick fix because if you don't change your lifestyle and mindset, you will gain the weight back,” Dr. Cheung concludes. “Think of these procedures as a tool that helps you get to your desired shape when you are disciplined and put in the work of eating healthy and exercising.”


11 Body Contouring Procedures That Offer Quick Results and Minimal Downtime

Whether it’s that person you see in your hot yoga class who seemed to magically drop 15 pounds or the coworker who is on a more significant weight loss journey, the tool everyone seems to be buzzing about these days is semaglutide injections. Better known by the brand names Ozempic® and Wegovy®, the medications are helping people lose weight via both on- and off-label uses.

The successful slimming is not without side effects, however, and the aesthetic changes some patients see are sending them to their plastic surgeons and dermatologists for help. ‘Ozempic Face’ and ‘Ozempic Body’ are the buzzwords being used to describe everything from the loss of volume to skin laxity that may accompany the use of these drugs, and we spoke to top plastic surgeons to get a better understanding of why it happens and how to treat it.

## **What is Ozempic?**
Ozempic® and Wegovy® are two of the best known semaglutide injections on the market today. Ozempic® was approved by the U.S. Food and Drug Administration (FDA) in 2017 to treat type II diabetes in adults. Wegovy®, meanwhile, was FDA approved in 2021 for chronic weight management in overweight or obese adults. More recently, there has been an uptick in people using semaglutide off label to lose smaller amounts of weight.

Both Ozempic® and Wegovy® involve a once-weekly injection of a specific dose under the skin of the thigh, upper arm, or abdomen. Semaglutide is part of a class of medications that ensure the pancreas releases sufficient insulin when blood glucose levels are high. Additionally, the glucagon-like peptide 1 (GLP-1) drugs make a person feel fuller longer and delay gastric emptying. The result? Fewer calories consumed. When coupled with diet and exercise, the effects can be dramatic.

Aesthetic providers are increasingly treating patients who are on the drugs for various reasons. “There are several new weight loss peptides and one of the most popular is Ozempic® or semaglutide,” says [David Shafer, MD](https://www.aedit.com/provider/david-shafer-md-153), a double board certified plastic and reconstructive surgeon and founder of Shafer Clinic in New York City. “We were ahead of the curve on this trend, as I started our metabolic aesthetic center at Shafer Clinic several years ago.”

Patients taking semaglutide injections for significant weight loss may incorporate aesthetic medicine into their treatment plan. “We see patients who use them in our practice and actually promote use of this and other semaglutides for weight loss, when indicated, under the care of a primary care provider to optimize overweight patients for flap surgery,” says [Joseph Zakhary, MD](https://breastreconstructionaz.com/about/practitioners/dr-joseph-zakhary/), board certified plastic and reconstructive surgeon and co-founder of Southwest Breast & Aesthetics in Arizona.

## **Why Everyone’s Talking About Ozempic**
If semaglutide has been on the market for a few years now, you may be wondering what all the recent fuss is about. “Ozempic® has been around for a while,” says [Jennifer Levine, MD](https://www.drjenniferlevine.com/), a double board certified facial plastic and reconstructive surgeon in NYC. “The weight loss aspect of the medication has been more prevalent for the past year.”

For one thing, it started gaining attention on social media due to celebrities and influencers subtly or not-so-subtly talking about it as a weight loss trick of sorts. “I don't think ‘trick’ is the right word to describe this given that Ozempic® is an effective drug used for type II diabetes and/or obesity,” notes [Oren Tepper, MD](https://orenteppermd.com/), a board certified plastic and reconstructive surgeon and director of aesthetic surgery Montefiore Medical Center in NYC. “What has happened over recent years is that the criterion for its use is becoming somewhat less strict.” 

As he explains, semaglutide was initially intended for people with a body mass index (BMI) over 30 or for those with a slightly lower BMI who had other significant medical issues related to being overweight. While that is still the FDA-approved use of Wegovy® (which is a weekly 2.4 milligram dose), smaller doses of semaglutide are being prescribed off-label for more moderate weight loss.


At Dr. Shafer’s practice, patient’s receive a comprehensive examination and evaluation to see if they are candidates. “Ozempic® should not be taken as a standalone medication, but rather as a customized treatment plan,” he shares. “This is not something that you just call your doctor or find online and ask for a prescription without being seen or evaluated.” 

It takes time to establish the right dosing that minimizes side effects and maximizes results, but Dr. Shafer says there is no secret as to why semaglutide injections have become popular. “First, it is very effective and people see quantifiable and visible results,” he explains. “Second, the side effect profile is very mild, and it is well tolerated when it is dosed correctly.” Another plus: “It is an easy once a week dosing,” he adds.

When it comes to treating patients who are obese, Dr. Zakhary says he has noticed the positive effects of semaglutide to “optimize weight loss” more and more in the last year or so. “This leads to patients who see us with a wide spectrum of weight loss… not dissimilar to the patients who had surgical procedures such as a gastric sleeve for weight loss,” he explains. 

## **What Is So-Called ‘Ozempic Face’ and ‘Ozempic Body’?**
If you are familiar with the old ass versus face dilemma, then you have a pretty good sense of what ‘Ozempic Face’ and ‘Ozempic Body’ refer to. “With any weight loss, there are positives and negatives,” Dr. Shafer explains. “When losing weight, we feel better, our clothes fit nicer, conditions like diabetes and hypertension and hypercholesterolemia can all improve.” Here comes the but. “However, with quick weight loss by any method – other weight loss medications, gastric bypass, lap band, etc. – we lose volume in our face and our skin can become looser,” he adds.


For many patients losing smaller amounts of weight, the face is where you notice it. “When we start getting up to 15 to 20 pounds of weight loss, you'll definitely start to see the changes,” explains [Amir Karam, MD](https://www.drkaram.com/), a double board certified facial plastic and reconstructive surgeon and founder Carmel Valley Facial Plastic Surgery and Aesthetic Center in San Diego, CA. “But even as little as five to 10 pounds can have a noticeable effect on the face.”

The most common impact is on facial volume, which can lead patients to feel like they look tired or older, Dr. Karam shares. This is especially true, he says, in people over 40. “Anytime somebody loses weight in their forties and fifties – with Ozempic® or without – their face is going to lose weight in a more accelerated way than their body,” he notes. “Therefore, even small amounts of weight loss will affect one’s face more than their handles.” 

Facial skin laxity is also a concern, particularly in patients losing large amounts of weight in a relatively short period of time. “What is often surprising to patients is the impact that this may have on the face,” Dr. Tepper notes. “The impact of semaglutide to the face includes loss of volume in key structural areas like the cheek, or temples, as well as sagging skin along the jawline and neck.” 

But it’s not just the face. Semaglutide injections can lead to rapid weight loss, which means the effects are not dissimilar to what is seen after a bariatric surgery. “These procedures, much like Ozempic®, can lead to weight loss that occurs very quickly,” Dr. Tepper explains. “With rapid weight loss, it is often difficult for the overlying skin to contract at the same rate, thus resulting in the skin being quite loose.” This most commonly affects the abdomen, breasts, upper arms, or upper thighs. 

## **The Best Procedures for Treating ‘Ozempic Face’**
As we’ve discussed, ‘Ozempic Face’ refers to the loss of facial volume and skin laxity that may accompany weight loss. “When it comes to the face, changes caused by rapid weight loss are best treated with a two-pronged approach,” Dr. Tepper shares. The first is removal and tightening of the skin and the second is the replacement of volume. 

The best way to address these concerns is with “procedures that help with lifting and laxity and promote collagen and elastin production,” Dr. Levine notes. Below are the best surgical and non-surgical solutions for ‘Ozempic Face’:

### **For Restoring Volume**
If your weight loss has resulted in a more gaunt appearance in the cheeks, temples, or other areas of the face, volume can be restored with dermal fillers and fat grafting. 

**Dermal Fillers**: Depending on your aesthetic goals, either hyaluronic acid (HA)-based fillers (think: [Juvederm®](https://www.aedit.com/procedure/juvederm)) or biostimulatory fillers (i.e. [Sculptra®](https://www.aedit.com/procedure/sculptra) and [Radiesse®](https://www.aedit.com/procedure/radiesse)) can effectively sculpt the face. “Biolstimulatory fillers such as Sculptra® or hyperdiluted Radiesse® also help to produce collagen, improving laxity, skin quality and texture,” Dr. Levine shares. The results take longer to see but are more lasting than HA fillers.

**Fat Grafting**: For patients who have unwanted fat on the body, [fat transfer](https://www.aedit.com/procedure/facial-fat-grafting) can be an alternative to synthetic fillers. “In my opinion, fat is a much better option for this because it is long-lasting and can more safely be replaced in larger volumes without additional risks,” Dr. Tepper explains. Fat is harvested via liposuction from a donor site like the flanks or thighs and transferred to areas of the face that have lost volume. Dr. Tepper’s ‘Boomerang Lift’ restores upper cheek volume in – you guessed it – a boomerang-like shape. “This technique works very well for Ozempic-type weight loss because it targets the sagging of the cheek and loss of volume that is caused by this medication,” he says. 

It’s important to remember that fillers address volume, not laxity. Depending on your age and skin quality, fillers or fat transfer might be enough to address your concerns. For others, skin tightening may also be needed. “If somebody is in their thirties and they lose some weight, it's unlikely they're going to have significant sagging,” Dr. Karam says. “They might look a bit more gaunt, and, in that case, they would only require some volume from either a fat transfer or some fillers in very select areas.”

### **For Non-Surgical Skin Tightening**
There are many modalities on the market that have the ability to firm the skin without cutting. They include, radiofrequency, microneedling, ultrasound, energy, and laser to name a few. “Renuvion® skin tightening is a great alternative to help tighten skin without large incisions,” Dr. Shafer says. “For non-invasive treatments, Ultherapy® helps tighten deeper tissues, while helps tighten more superficial skin and is great for lower eyelids.”

Dr. Levine likes Ultherapy® because it “uses microfocused ultrasound to treat the SMAS – which is the layer lifted during a facelift – and helps the body produce new and healthy collagen and elastin.” EmFace® (from the makers of [EmSculpt®](https://www.aedit.com/procedure/emsculpt)) is another one of her go-tos. “It combines radiofrequency with electrical stimulation of the facial muscles,” she explains. “It works on the muscles of elevation to tone and increases the density of the facial muscles for natural volume.” She says it also helps with wrinkles and skin quality.”

### **For Surgical Skin Tightening**
Non-surgical skin tightening can only go so far, and patients experiencing more significant skin laxity concerns would benefit from a facelift and/or neck lift. “A neck lift or facelift can help with the sagging skin around and below the and jowling associated with loss of facial volume,” Dr. Zakhary says. “Some, but not all, of these procedures can be performed at the same time.”

Surgical intervention is usually the most effective course of treatment for patients over 50, Dr. Karam shares. “If someone is in their late forties to fifties, it is likely that their weight loss will lead to sagging or unveil the level of sagging that the person already had due to aging,” he explains. “When someone’s face is fuller due to fat, it actually camouflages and hides facial sagging. But when the weight comes off, the underlying sagging is revealed.” In such cases, a facelift and/or neck lift is the only treatment option, he adds. 

For patients who also have concerns about volume loss, fillers can be coupled with surgery – but fillers alone will not necessarily be enough. “In my practice, I would suggest a vertical restore because fillers in this age group will not improve sagging,” Dr. Karam says. “The important point to make here as far as treatment is: If you have laxity, the treatment is going to be surgical; if you simply have volume loss, but your jawline and neck are pretty taut, the treatment is going to be fillers.”

### **For Improving Skin Quality**
All of these surgical and non-surgical procedures are for naught, if you are not taking care of your skin. “The best thing you can do now or at any point along the aging continuum is to get serious about sun protection and be consistent with cleansing, vitamin C, retinol, as well as additional active ingredients,” Dr. Karam shares. “This was where the concept of the KaramMD Trifecta came from.” The most important rule of skincare: “Consistency is key,” he emphasizes.

Dr. Levine is also a proponent of skincare and is a fan of SkinMedica TNS Advanced+ Serum or Neocutis Bioserum for the face.

## **The Best Procedures for Treating ‘Ozempic Body’**
Much like the face, the treatments for ‘Ozempic Body’ depend on the amount of pounds shed and the aesthetic concerns of the patients. “There are no reliable non-surgical options to address significant skin laxity after massive weight loss,” Dr. Zakhary notes. Excess skin surgery is the only treatment option for such cases, and he says patients must be weight stable for six months at their final goal. “For ideal results, we should agree in consultation that the patient's current weight is where we need it to be to achieve the best postoperative outcome for the patient,” he adds.

There are, however, some non-surgical modalities that can improve tone and firmness following less significant weight loss. Below is an overview of both surgical and non-surgical treatment options for ‘Ozempic Body’:     

### **For Non-Surgical Skin Tightening**
Energy, laser, and ultrasound-based treatments can be used on the body to tone and firm the skin and, in some cases, aid in additional fat reduction. “A great non-invasive treatment is TruBody® by Cutera which uses radiofrequency technology which can actually help tighten skin, reduce fat, and strengthen muscle,” Dr. Shafer says. He is also a fan of NuEra® by Lumenis, which is another RF technology. 


While Dr. Levine likes EmFace® for the face, she is a proponent of EmSculpt® Neo combined with Emtone® for below the neck because it “really helps for laxity and toning of the body,” she says. “Ultherapy® can also be used on the body for laxity and crepiness.”


### **For Surgical Skin Tightening**

There’s a wealth of information on the internet - some good, some bad (ok, a lot bad) - so when we saw a TikTok video about how mouth breathing can dramatically alter the structure of our faces, we couldn’t help but wonder if it had merit. The medical cosmetics industry is a [multi-billion dollar industry](https://www.plasticsurgery.org/documents/News/Statistics/2020/plastic-surgery-statistics-full-report-2020.pdf), so is it really possible that instead of getting masseter [Botox](https://www.aedit.com/procedure/botox) or [Kybella](https://www.aedit.com/procedure/kybella) you can get a more defined jawline by improving your breathing habits?

We interviewed Maryland-based board certified facial plastic and reconstructive surgeon [Samuel Hahn, MD](https://www.aedit.com/provider/samuel-hahn-md-20403) and cosmetic dentists [Anjali Rajpal, DMD,](https://www.aedit.com/provider/anjali-rajpal-428) of Beverly Hills Dental Arts and [Stacy Spizuoco, DDS](https://true-dermatology.com/dermatology-new-york?gclid=CjwKCAjwl6OiBhA2EiwAuUwWZZ30g6zy9uCbVGilK1riNcpKrUM_mdTdedrgBB9mBW7mCwNShh2XQRoC3NoQAvD_BwE), of True Dental NYC to determine whether these TikTok videos had any validity. ## **WHY IS MOUTH BREATHING BAD?** 


The negative effects associated with mouth breathing aren’t anything new. In fact, the topic actually dates back to the 1800’s when author George Catlin warned about the dangers of mouth breathing in his book titled [Shut Your Mouth](https://www.barnesandnoble.com/w/shut-your-mouth-and-save-your-life-george-catlin/1137929412). While it wasn’t backed by science then, there is ample research today that confirms the benefits of nasal breathing. The biggest advantage? The hundreds of nose hairs that filter out any toxins and control temperature. Mouth breathing, on the other hand, is a less efficient way to get oxygen and has many health risks.   

Most of us know whether we’re mouth breathers or not. Common tell-tale signs are a dry mouth, bad breath, snoring, or being a loud breather. What’s less obvious is determining why you may be a mouth breather. Generally, mouth breathing is caused by structural issues, like a deviated septum, nasal valve collapse, turbinate hypertrophy, adenoid hypertrophy, or nasal polyps. But, environmental factors like anxiety, allergens, or viruses can also result in poor breathing habits. 

The most obvious side effect of chronic mouth breathing is poor dental health. As Dr. Spizuoco explains, the mouth needs saliva to cover the teeth and gums to prevent bacteria and decay. This becomes problematic when the mouth is constantly open and, as a result, dry. 

Training yourself to become a nasal breather isn’t an overnight task — especially if there’s a medical reason why you breathe through your mouth. A quick way to determine whether or not you’re an accidental mouth breather (vs a mouth breather out of necessity) is to do a simple test: close your mouth and inhale through your nostrils. If there’s any type of obstruction, you may benefit from meeting with an ENT. Depending on the structure of your nose, an ENT might recommend a [functional rhinoplasty](https://aedit.com/aedition/difference-between-cosmetic-rhinoplasty-vs-functional-rhinoplasty) (a medically necessary nose job) or prescribe medication, like antihistamines.   


## **MOUTH BREATHING AND FACIAL APPEARANCE** 


So, does mouth breathing really change your facial structure? The short answer: yes… but only if chronic mouth breathing is present at a young age.  

According to Dr. Hahn, if mouth breathing starts at an early age, it can affect the development of the upper jaw (maxilla) and palate, which translates to a narrower midface, nose, and more sunken appearance of the cheeks. 

When we breathe through our mouth, [Dr. Rajpal](https://www.aedit.com/provider/anjali-rajpal-428) explains that the tongue is positioned downward instead of resting on the roof of the mouth. In this position, the tongue can misalign the teeth rather than facilitate the growth of a wide palate and formation of a broad nasal cavity. 

Both specialists agree that these changes need to be addressed immediately, as not to cause permanent damage. “The key to treating the symptoms associated with mouth breathing is to diagnose it at an early stage while jaw development is still easily adaptable,” [Dr. Rajpal](https://www.aedit.com/provider/anjali-rajpal-428) advises. “The early recognition of these facial patterns by a joint effort of a multidisciplinary team that includes a pediatrician, orthodontist, and otorhinolaryngologist can help reduce the detrimental effects of breathing impairments on facial characteristics before impaired growth has proceeded irreversibly.”

As [Dr. Hahn](https://www.aedit.com/provider/samuel-hahn-md-20403) and [Dr. Rajpal](https://www.aedit.com/provider/anjali-rajpal-428) explain, you can’t actually reverse chronic mouth breathing’s effects on adult facial structure, despite what social media says. There may be some very subtle changes to facial structure when undergoing treatment for chronic mouth breathing but [Dr. Hahn](https://www.aedit.com/provider/samuel-hahn-md-20403) wouldn't consider them dramatic. 

“Any positive changes are probably mostly due to improved quality of life (e.g. exercise capacity, decreased fatigue) and improved sleep quality rather than any structural changes to their face,” [Dr. Hah](https://www.aedit.com/provider/samuel-hahn-md-20403)n says. Proper breathing can help improve skin quality and appearance, decrease dark circles and bags under the eyes, and increase exercise capacity. 


## **HOW TO IMPROVE BREATHING**  


Regardless of age, there’s still opportunity to change breathing habits. If mouth breathing is present at a young age, it’s best to correct it immediately — especially since it can impact the face’s development. 

“While a patient is still young and within the teenage years, it is an ideal time to correct the malformations with palatal expansion devices and myofunctional therapy to retrain the muscles to perform correctly as well as incorporating breathing exercises. Also, any potential tongue ties need to be released for free movement of the tongue,” [Dr. Rajpal](https://www.aedit.com/provider/anjali-rajpal-428) says.

To combat mouth breathing later in life, Dr. Spizuoco explains that dentists approach the issue by expanding the narrow [dental] arch through orthodontics and sometimes surgery (e.g. jaw or adenotonsil surgery). “The chronic mouth breather will also need to retrain the muscles of their face. That includes how to close their mouth properly, swallow, breath through their nose, speech therapy and sleeping positions,” Dr. Spizuoco adds. 

For nonsurgical remedies, [Dr. Hahn](https://www.aedit.com/provider/samuel-hahn-md-20403) recommends oral antihistamines, nasal sprays, mechanical dilators (e.g. [Breathe Right strips](https://www.breatheright.com/products/breathe-right-original-tan/) or [nasal cones](https://www.amazon.com/Breathing-Snoring-Different-Relief-Dilator/dp/B086YL8LSQ/ref=asc_df_B086YL8LSQ/?tag=hyprod-20&linkCode=df0&hvadid=507467652717&hvpos=&hvnetw=g&hvrand=11192728546694207170&hvpone=&hvptwo=&hvqmt=&hvdev=c&hvdvcmdl=&hvlocint=&hvlocphy=9003447&hvtargid=pla-1252944783999&psc=1)). These solutions are not corrective, but can be used to improve nasal airflow as directed by your board-certified doctor. As for the viral mouth taping trend, [Dr. Hahn](https://www.aedit.com/provider/samuel-hahn-md-20403) strongly urges against it. Not only can it restrict breathing but also it can lead to anxiety and panic attacks during sleep. 

If medically necessary, there are a number of corrective surgeries that can help improve nasal breathing. According to [Dr. Hahn](https://www.aedit.com/provider/samuel-hahn-md-20403), this could include septoplasty, rhinoplasty, nasal valve repair, inferior turbinate reduction, tonsillectomy/adenoidectomy, or maxillomandibular advancement surgery.

The negative side effects of mouth breathing are widely accepted, but its impact on facial structure really only applies if it bagan in adolescence. Becoming a nasal breather has many benefits, like improving oral health and increasing exercise capacity.

Depending on several lifestyle factors and your metabolism, DAXXIFY® can produce results that last six-to-nine months, compared to two-to-three months for other products. The injectable also uses a novel peptide stabilizer, making it suitable for vegans. 

__WHICH AGING CONCERNS CAN DAXXIFY® ADDRESS?__

Qualified injectors can use the product on the upper area of the face to treat concerns such as horizontal frown lines, crow's feet, and of course, those pesky 11s between your brows. It works similarly to Botox® by delivering a botulinum toxin to the injection site. Over a short period, the toxin blocks specific nerve signals to prevent muscles from moving, leaving the area looking smoother. 

__HOW DOES DAXXIFY® COMPARE TO BOTOX? __

The neuromodulator in DAXXIFY® is the same as the one used in Botox, but the real difference comes into play with the stabilizer we mentioned above. The age-old ingredient has been a favorite not just because it helps address whiteheads by fighting off the acne-causing bacteria that live on the skin, but it can be a preventative tool, as well. “When used at the maximum over-the-counter level, sulfur clears up acne and blackheads and helps prevent new breakouts.,” explains a representative from Peter Thomas Roth. So, how does it really work? Keep reading.  
 
## **The Benefits of Sulfur**

“Sulfur helps the skin as it works to absorb excess oil and also has antibacterial and anti-inflammatory properties. Additionally, it may have some exfoliating benefits as well,” Dr. Garshick adds. To break it down further, ZO Skin Health’s representative adds that “it dries out the surface of your skin to help absorb excess oil that may cause congestion, promoting the shedding of dead skin cells and helping to unclog your pores.” And because of its gentle nature, it can be added to the routines of those with sensitive skin and sensitive skin conditions. 

## **The Best Candidates for Sulfur**

We’ve mentioned that sulfur is beneficial for fighting acne but let’s break down the type of acne sulfur can work with, specifically. Because it’s not a harsh exfoliant, its abilities to unclog and reduce oil production are mild and it will primarily help address those pesky whiteheads and blackheads. “As a result of its anti-inflammatory properties, it can also help with conditions like seborrheic dermatitis as well as rosacea,” elaborates Dr. Garshick. And in a time when many exfoliants are deemed harsh for those with sensitive skin, sulfur can come in the clutch to provide some relief. However, it’s not a universal guarantee that the ingredient will fit any user's routine as an allergy is still possible to sulfur Dr. Garshick adds. 

## **How To Find The Right Sulfur Product** 
 
Finding a product that works for you can be like dating – if it’s not a match, it’s not your fault. What makes a sulfur-powered product likely to be a fit for anyone is that it’s available in many iterations. Because the ingredient is drying, it can cause irritation for those with sensitive skin but because it’s gentler than the standard exfoliant, it is manageable to introduce slowly. 

If you’re new to chemical exfoliants, sulfur is a great entry-ingredient but is not recommended if your routine is already filled with retinoids, alpha hydroxy acids, or beta hydroxy acids as the over-exfoliation can lead to a weakened skin barrier.

As a beauty editor, being non-committal to skincare products and tools is a part of the job. While there are lotions, potions, products, ingredients, and devices we know are tried and true, it’s also our duty to test out the latest launches to stay abreast of the innovations on the market. Consider it research. Therefore, by keeping my vanity open to new products, maintaining a curated routine has been a challenge. Yes, I am mindful of the ingredients I use and how they play together, but, I am not as selective on which cleanser or moisturizer I will use from week to week. So, it comes as no surprise that most of my [twenties](https://aedit.com/aedition/best-skincare-routine-products-ingredients-for-20s) were spent experimenting and prioritizing product testing over tailoring a personal [routine](https://aedit.com/aedition/how-to-maintain-youthful-skin-in-your-20s) – until I hit [30](https://aedit.com/aedition/best-skincare-routine-products-ingredients-for-30s).  

Knowing that my skin wont be smooth forever as fine lines, dullness, and pigmentation are a part of life, I knew it was time to do what I’ve been dreading: sticking to a routine with tretinoin. Because adding a prescription-strength vitamin A into the regimen requires a methodic approach (and patience), my hesitancy prevented me from diving into the deep end. Three months ago, I decided to finally incorporate the holy grail of youth-preserving ingredients into my routine, and here’s how that journey went. 

## **What is Tretinoin?** 

For the newbies, tretinoin is “[a vitamin A](https://aedit.com/aedition/guide-to-vitamin-a-retinol-retinoids-in-skincare) derivative, a type of retinoid that is available as a prescription,” explains [Marisa Garshick, MD](https://aedit.com/provider/marisa-garshick-md-313), a board certified dermatologist based in New York. The workhorse ingredient works by regulating skin cell turnover and can help prevent and treat acne, explains Dr. Garshick. But, the fun doesn’t stop there. “Additionally, because it can also help boost collagen production, it can help to improve the appearance of fine lines and wrinkles, in addition to acne scarring,” she adds. To simply put it, the prescription-strength ingredient “can help to improve overall skin tone and texture.” 

The ingredient requires a prescription, and while Dr. Garshick was not my personal dermatologist, her insight is invaluable when it comes to all things skincare.  

## **How To Add Tretinoin To Your Skincare Routine** 

What kept me from starting sooner was the barrier to entry, it is a process that requires more than a prescription to truly make it work. Below are some of Dr. Garshick's tips and recommendations for starting tretinoin.  

### Do
- **Start Slow**: When adding any new product into your routine, especially one as potent as tretinoin, modesty is key. A pea-sized drop of the ivory cream is recommended once a week to build up a tolerance, and with time, the amount can be adjusted. As we always say, let your skin guide you and tell you what it needs. “It is best to incorporate tretinoin into a nighttime routine and, to start off using it just a few times per week and then increase the frequency as tolerated.” 
-  **Limit Exfoliants**: Over-exfoliating the skin can cause a weakened skin barrier, loss of moisture, and therefore, more breakouts to arise. “It is best to avoid using any ingredients that can be harsh on the skin at the same time as tretinoin, such as exfoliating acids, including salicylic acid, and benzoyl peroxide – unless directed to do so.”
- **Be Patient**: From purging to causing irritation, tretinoin is fickle as the barrier of entry is almost like an initiation process. Because the skin’s experiencing increased cell turnover, 

### Don’t 
- **Skip the SPF**: Sun protection should not only be the first line of defense in any skincare routine, it is vital when using a chemical exfoliant. “It is important to always be good about sun protection when incorporating a retinoid into your routine and wearing daily sunscreen,” adds Dr. Garshick.  
- **Worry About A Purge**: Purging, as it is lovingly known, is the process of the skin breaking out which may seem paradoxical considering the ingredient's efficacy when treating acne. However, this is perfectly normal and to be expected. If the acne persists, speak with a trusted dermatologist to adjust the routine or switch to a retinol alternative (hello, [bakuchiol](https://aedit.com/aedition/guide-to-bakuchiol-in-skincare)).  
- **Complicate Things**: It’s natural to want to rush the process, the benefits are numerous, after all. During the adjustment period, focus on the basics and how your skin adjusts before incorporating additional products. “I recommend applying tretinoin after cleansing and before moisturizing, but for those with dry or sensitive skin, it may be used after applying moisturizer which may help reduce the potential for irritation in some individuals,” Dr. Gashick advises. 

## **My Experience With Tretinoin** 

We often discuss the timing of starting a product, the importance of doing a patch test, and the mindful approach required to ensure all ingredients work in harmony, so, to do this right, I overhauled my routine. My first step was to start tret during the winter months due to the decreased sun exposure I would have as my skin acclimates. My second step was cutting out other physical exfoliants. Due to tret being a heavy-duty chemical exfoliant, I opted to shelve my favorite [AHAs](https://aedit.com/aedition/guide-to-topical-alpha-hydroxy-acids-in-skincare) and [BHAs](https://aedit.com/aedition/guide-to-beta-hydroxy-acids-salicylic-acid-in-skincare) for the time being. Because of my hesitancy to stick to a set routine, I wasn’t going to take any risks with over-exfoliating. Here is a breakdown of my AM and PM routines.

Now, to provide clarity on my skin, it’s normal, not prone to acne, and has very little pigmentation. My reasoning for starting tretinoin was to address any fine lines and wrinkles that may arise with time, as my belief has always been that it’s better to prevent than correct. 

During the first few weeks of my journey, the [irritation](https://aedit.com/aedition/skincare-ingredient-alternatives-for-retinol-retinoids) was minimal, and though I was prepared for a purge (cue dramatic music), it never came. Rather than a noticeable breakout, my skin went through a few weeks of whiteheads popping up at inopportune times, slight redness here and there, and more dryness than I usually experience during the colder season. Overall, my fear of the unexpected proved to be just that, a fear.   

I have to admit that due to my being a regular person, my routine did waiver as I would try a new product on days when I wasn’t using tret. My goal was to be litigious throughout the process so as to not throw any curveballs, but I did experiment sporadically with a new gentle mask or two. I maintained my rule to avoid all other chemical exfoliants and was consistent with my SPF application (and re-application). 

After three months, I can confidently say that my skin looks better than I expected. My primary concerns (fine lines forming on my forehead and marionettes) were not as prominent as my skin adjusted to tret. 

## **The Takeaway** 

After carrying what felt like a dirty little secret of not incorporating vitamin A in my routine for quite some time, I am happy to report that the ingredient is all it’s cracked up to be. Offering a noticeable change to my skin in a matter of months, tretinoin has provided me with clearer skin, a smoother complexion, and fewer breakouts. Though the adjustment period felt like a waiting game, trusting the process proved to be worth it.
